We present a comprehensive study of the one-dimensional modulation instability of broad optical beams in biased photo refractive-photovoltaic crystals under steady-state conditions. We obtain the one-dimensional modulation instability growth rate by globally treating the space-charge field and by considering distinction between values of Eo in nonlocal effects and local effects in the space-charge field, where Eo is the field constant correlated with terms in the space-charge field, which depends on the external bias field, the bulk photovoltaic effect, and the ratio of the optical beam's intensity to that of the dark irradiance. The one-dimensional modulation instability growth rate in local effects can be determined from that in nonlocal effects. When the bulk photovoltaic effect is neglectable, irrespective of distinction between values of Eo in nonlocal effects and local effects in the space-charge field, the one-dimensional modulation instability growth rates in nonlocal effects and local effects are those of broad optical beams studied previously in biased photorefractive-nonphotovoltaic crystals. When the external bias field is absent, the one-dimensional modulation instability growth rates in nonlocal effects and local effects predict those of broad optical beams in open- and closed-circuit photorefractive-photovoltaic crystals. We investigate the modulation instability of quasi-plane-wave optical beams in biased photorefractive-photovoltaic crystals by globally treating the space-charge field. The modulation instability growth rate is obtained, which depends on the external bias field, on the bulk photovoltaic effect, and on the ratio of the optical beam's intensity to that of the dark irradiance. Our analysis indicates that this modulation instability growth rate is identical to the modulation instability growth rate studied previously in biased photorefractive-nonphotovoltaic crystals when the bulk photovoltaic effect is negligible for shorted circuits, and predicts the modulation instability growth rate in open- and closed-circuit photorefractive-photovoltaic crystals when the external bias field is absent.

In order to resist lateral loads, modern methods of timber construction are reliant on the in-plane shear strength of the walls orientated parallel to the applied action. In closed panel systems, the shear stresses are transferred to the foundations by the sole plate through the sheathing board, which is usually mechanically jointed to the timber frame. Since closed panels are delivered to site as single units, access to the internal bottom rail is rather restricted and novel, efficient solutions to secure the panel to the substrate are required. Sole plate fixing components for open and closed panel systems were tested in isolation and combination in order to validate a simplistic version of the weakest link theory. As a result, findings were embedded into a software database with a direct link to a previously developed sole plate and racking design application. This integrated process facilitates the structural optimization of the sole plate detail.

In this paper we present some early work concerned with the development of a simple solid fuel combustion model incorporated within a Computational Fluid Dynamics (CFD) framework. The model is intended for use in engineering applications of fire field modelling and represents an extension of this technique to situations involving the combustion of solid cellulosic hels A simple solid &el combustion model consisting of a thermal pyrolysis model, a six flux radiation model and an eddydissipation model for gaseous combustion have been developed and implemented within the CFD code CFDS-FLOW3D The model is briefly described and demonstrated through two applications involving fire spread in a compartment with a plywood lined ceiling. The two scenarios considered involve a fire in an open and closed compartment The model is shown to be able to qualitatively predict behaviours similar to flashover - in the case of the open room - and backdrafl - in the case of the initially closed room.

We propose a general framework to effectively `open' a high-Q resonator, that is, to release the quantum state initially prepared in it in the form of a traveling electromagnetic wave. This is achieved by employing a mediating mode that scatters coherently the radiation from the resonator into a one-dimensional continuum of modes such as a waveguide. The same mechanism may be used to `feed' a desired quantum field to an initially empty cavity. Switching between an `open' and `closed' resonator may then be obtained by controlling either the detuning of the scatterer or the amount of time it spends in the resonator. First, we introduce the model in its general form, identifying (i) the traveling mode that optimally retains the full quantum information of the resonator field and (ii) a suitable figure of merit that we study analytically in terms of the system parameters. Then, we discuss two feasible implementations based on ensembles of two-level atoms interacting with cavity fields. In addition, we discuss how to integrate traditional cavity QED in our proposal using three-level atoms.

Objectives: The Objective Structured Clinical Exam (OSCE) is a widely accepted assessment method in undergraduate dental education. It aims to test higher order skills, attitudes and aspects of professionalism which other summative assessments such as MCQs and other written examinations are less able to do. The aim of this study was to evaluate the perceptions of 4th year undergraduate dental students of an OSCE undertaken in the Conservation Department.
Methods: On completion of the OSCE examination 51 fourth year undergraduate students were asked to complete an anonymised questionnaire. The questionnaire was made 22 questions, and requiring the students to provide both open and closed responses.
Results: A lot of positive aspects to the OSCE were observed in responses, students felt that the OSCE was a meaningful way for assessing their clinical skills (85%), it reflected real life conditions (79%) and that it was a fair method of assessment (75%).
A number of negative aspects were also noted. Most students felt the OSCE was stressful (72%) and they felt nervous during the examination (77%). Of the undergraduates asked 42% did not feel confident doing the OSCE.
A number of students felt it would be helpful to have additional information given to them on the OSCE prior to the assessment process.
Conclusion: In general the students found the OSCE a fair, meaningful form of assessment which reflected real life clinical situations, providing them with an opportunity to show their clinical knowledge and practical skills. A number study cohort did not feel confident during the OSCE and felt nervous and stressed by the experience. The information gained from the reflective nature of the feedback questionnaire has proved invaluable in the design of subsequent diets of the OSCE examination.

Objective: To determine the frequency of falls and identify risk factors in the homes of the elderly under the Home Care Service of a village in Alentejo (Portugal). Method: Exploratory, descriptive study. The target group were elderly persons under the Home Care Service (23). The questionnaire consists of open and closed questions, and was based on the Jefferson Area Board for Aging Safety in the Home Assessment; Instrument to Assess the Risk of Falls and Adaptations to Prevent Falls at Home. Results: Of the 23 seniors, 13 were men; the mean age was 85; 10 widowers; 11 live alone; 12 cannot read or write; 17 have experienced falls, loss of balance being the main cause. They report changes in vision (21), hearing (14) and rheumatic diseases (14); hypertension (19); they use 4 or more drugs on a daily basis (16). Conclusion: The physiological changes associated with ageing may increase the risk of falls. Due to the consequences, it is a priority field in community intervention.

RESUMO - Introdução: O presente trabalho, desenvolvido ao longo dos últimos meses, teve como objetivo analisar comparativamente o impacto das Unidades Locais de Saúde e dos Agrupamentos de Centros de Saúde no processo de articulação entre cuidados de saúde primários e hospitais. Para tal, foram avaliadas as seguintes variáveis: frequência de contacto entre médicos de família e especialistas; percentagem de informação de retorno recebida pelos médicos de família; percentagem de recusas recebida pelos médicos de família; e tempo de espera entre o pedido das consultas hospitalares e a efetivação das mesmas para as especialidades mais referenciadas. As instituições escolhidas para o estudo foram a Unidade Local de Saúde de Castelo Branco e o ACeS Cova da Beira. Metodologia: O instrumento de medida utilizado para este estudo foi um questionário, com questões de resposta aberta e fechada, dirigido a médicos de família da Unidade Local de Saúde de Castelo Branco e do ACeS Cova da Beira, pretendendo assim averiguar a perceção que os mesmos têm em relação às variáveis descritas no tópico da Introdução. Resultados: Segundo dados estatísticos, meramente descritivos, os médicos de família da ULSCB apresentaram uma frequência de contacto inferior aos médicos de família do ACeS com os médicos hospitalares, e a percentagem de informação de retorno recebida pelos médicos de família da ULSCB revelou ser também inferior à recebida pelos médicos de família do ACeS. No entanto, as diferenças encontradas não puderam ser confirmadas para a amostra existente, uma vez que o teste Qui-quadrado foi inconclusivo. Quanto à percentagem de recusas recebida pelos médicos de família de ambas as instituições, e aos tempos de espera para a realização das consultas das especialidades mais referenciadas pelo ACeS Cova da Beira e pela ULSCB, a ULSCB não mostrou desvantagem significativa, mas também não revelou superioridade. Conclusão: As principais conclusões extraídas deste estudo vão no sentido de questionar a eficácia do modelo de organização institucional das ULS | Unidades Locais de Saúde no que diz respeito à articulação entre cuidados de saúde primários e cuidados de saúde hospitalares, em particular, no que se refere à partilha de informação clínica e à eficiência do processo de referenciação para consultas hospitalares.

Abstract This descriptive study sought to identify the similarities and differences between the various educational institutions offering therapeutic recreation curriculum across Canada. The study utilized mixed methods, including open and closed-ended questions on a survey and document analysis. The research participants were from 14 educational institutions located across the nation. Results from this study identify similarities and differences in the curriculum used to prepare students pursuing a career in the TR field. Core competencies and standards of practice for the field of therapeutic recreation were defined and discussed. Accreditation and the accrediting bodies in the field of TR are reviewed because of their significant impact on curriculum. Implications regarding certification and regulation pertaining to the education for therapeutic recreation practitioners were discussed along with suggestions for future research.

L’atteinte sensorimotrice découlant d’une lésion médullaire traumatique affecte la capacité à se tenir debout de façon sécuritaire. Chez les individus ayant une lésion médullaire incomplète, les chutes lors des tâches locomotrices sont fréquentes après la réadaptation, entraînant des blessures qui affectent la participation sociale et la qualité de vie. Une meilleure compréhension du contrôle postural en clinique aiderait à cibler des interventions efficaces à ce niveau. L’objectif général de cette thèse était donc d’étudier le contrôle postural debout lors de tâches variées chez les personnes avec lésion médullaire traumatique en utilisant une approche biomécanique. Les objectifs spécifiques étaient d’étudier divers aspects du contrôle postural en lien avec chacune des tâches et d’identifier les variables prédictives de la stabilité. Vingt-cinq (25) personnes ayant une lésion médullaire traumatique incomplète ont été recrutées. Elles ont été évaluées au laboratoire d’analyse de mouvement lors du maintien de la station debout quasi-statique yeux ouverts et fermés, de l’exécution d’un test des limites de stabilité multidirectionnelles, de la marche naturelle et de l’initiation et l’arrêt de la marche. Des mesures biomécaniques caractérisant le déplacement du centre de pression (COP) ainsi que le modèle du contrôle postural dynamique des forces stabilisantes et déstabilisantes ont été utilisés pour comparer le contrôle postural des patients à celui d’un groupe formé de 33 personnes en santé. Les résultats ont montré une diminution du contrôle postural quasi-statique et dynamique chez les personnes ayant une lésion médullaire comparativement aux personnes en santé. Cette diminution s’accompagnait d’une contribution élevée des informations visuelles à la station debout quasi-statique qui était associée au score du mini BESTest. Le déplacement du COP lors du test des limites de stabilité multidirectionnelle se caractérisait par une difficulté à suivre la direction indiquée. Ce manque de précision causait une augmentation du trajet nécessaire pour atteindre la distance maximale dans chacune des directions. Les résultats au maintien de la station debout quasi-statique et au test des limites de stabilité multidirectionnelle n’étaient pas corrélés. La phase unipodale de la marche des personnes ayant une lésion médullaire différait de celle des personnes en santé par une force stabilisante maximale moindre et une force déstabilisante plus grande alors que l’arrêt de la marche se révélait plus instable que l’initiation de la marche chez les personnes ayant une lésion médullaire. Pour l’ensemble des tâches de marche, la vitesse du COM et la distance entre le COP et la base de support étaient les facteurs explicatifs principaux des forces stabilisantes et déstabilisantes. En somme, les résultats confirment l’impression clinique d’une atteinte générale du contrôle postural debout des personnes ayant une lésion médullaire; ils en précisent les caractéristiques et ciblent les paramètres à considérer dans la rééducation.

Les anomalies du tube neural (ATN) sont des anomalies développementales où le tube neural reste ouvert (1-2/1000 naissances). Afin de prévenir cette maladie, une connaissance accrue des processus moléculaires est nécessaire. L’étiologie des ATN est complexe et implique des facteurs génétiques et environnementaux. La supplémentation en acide folique est reconnue pour diminuer les risques de développer une ATN de 50-70% et cette diminution varie en fonction du début de la supplémentation et de l’origine démographique. Les gènes impliqués dans les ATN sont largement inconnus. Les études génétiques sur les ATN chez l’humain se sont concentrées sur les gènes de la voie métabolique des folates du à leur rôle protecteur dans les ATN et les gènes candidats inférés des souris modèles. Ces derniers ont montré une forte association entre la voie non-canonique Wnt/polarité cellulaire planaire (PCP) et les ATN. Le gène Protein Tyrosine Kinase 7 est un membre de cette voie qui cause l’ATN sévère de la craniorachischisis chez les souris mutantes. Ptk7 interagit génétiquement avec Vangl2 (un autre gène de la voie PCP), où les doubles hétérozygotes montrent une spina bifida. Ces données font de PTK7 comme un excellent candidat pour les ATN chez l’humain. Nous avons re-séquencé la région codante et les jonctions intron-exon de ce gène dans une cohorte de 473 patients atteints de plusieurs types d’ATN. Nous avons identifié 6 mutations rares (fréquence allélique <1%) faux-sens présentes chez 1.1% de notre cohorte, dont 3 sont absentes dans les bases de données publiques. Une variante, p.Gly348Ser, a agi comme un allèle hypermorphique lorsqu'elle est surexprimée dans le modèle de poisson zèbre. Nos résultats impliquent la mutation de PTK7 comme un facteur de risque pour les ATN et supporte l'idée d'un rôle pathogène de la signalisation PCP dans ces malformations.

Los traumatismos por accidentes de tránsito, constituyen un problema de salud pública, a nivel mundial. Las lesiones más frecuentes son las fracturas de extremidades (84.3%). Las fracturas tienen un elevado riesgo de presentar infecciones, secuelas e incapacidades permanentes. Objetivo : Determinar si los factores asociados con la patología (lugar de fractura, clasificación de fractura, comorbilidades del paciente) y/o los factores relacionados con la atención médica (uso de profilaxis antibiótica diferente al protocolo institucional, tiempo prolongado para remisión, demoras en manejo quirúrgico) se asocian a mayor probabilidad de presentar infección de fracturas abiertas, en población mayor a 15 años, atendidos por accidente de tránsito, en una clínica de Bogotá de tercer nivel especializada en atención de SOAT, durante el período Octubre de 2012 a Octubre de 2013. Metodología: Estudio de casos y controles no apareado, relación 1:3, conformado por 43 casos (fracturas abiertas infectadas) y 129 controles (fracturas abiertas no infectadas). Resultados: La edad media de los casos fue de 39.42 +/- 16.82 años (med=36 años) y la edad media de los controles fue de 33.15 +/- 11.78 años (med=30 años). El 83.7% de los casos y el 78.3% de los controles corresponden al sexo masculino. Predominaron los accidentes en motocicleta en el 81.4% de los casos y el 86% de los controles. En el análisis bivariado se encuentra que la edad mayor a 50 años (p=0.042), una clasificación de la fractura grado IIIB o IIIC (p=0.02), cumplimiento del protocolo antibiótico institucional según el grado de fractura (p=0.014) y un tiempo mayor a 24 horas desde el momento del accidente al centro especializado en trauma (p=0.035) se asociaron significativamente con infección de la fractura abierta. En el análisis multivariado se encuentra únicamente que la clasificación de la fractura grado IIIB o IIIC se asocia con infección de la fractura OR 2.6 IC95% (1.187 – 5.781) (p=0.017). La duración de hospitalización fue mayor en los casos (32.37+/- 22.92 días, med=26 días) que en los controles (8.81 +/- 7.52 días, med=6 días) (p<0.001). El promedio de lavados quirúrgicos fue mayor en los casos (4.85±4.1, med=4.0) que en el grupo control (1.94±1.26, med=2) (p<0.001). Conclusiones: La infección posterior a una fractura abierta, implica costos elevados de atención con hospitalizaciones prolongadas y mayor frecuencia de intervenciones quirúrgicas como se evidencia en el presente estudio. Se debe fortalecer el sistema de remisión y contra remisión para acortar los tiempos de inicio de manejo especializado de los pacientes con fracturas abiertas. Se debe incentivar dentro de las instituciones, el cumplimiento de protocolos de profilaxis antibiótica según el grado de la fractura para disminuir el riesgo de complicación infecciosa.
