This thesis proposes the implementation of a space efficient Prolog implementation based on the work of David H. D. Warren and Hassan Aït-Kaci. The Common Lisp is the framework used to the construction of the Prolog system, it was chosen both to provide a space efficient environment and a rich programming language in the sense that it supply the user with abstractions and new ways of thinking. The resulting system is a new syntax to the initial language that runs on top of the SBCL Common Lisp implementation and can abstract away or exploit the underlying system.

Background Delirium is highly prevalent, especially in older patients. It independently leads to adverse outcomes, but remains under-detected, particularly hypoactive forms. Although early identification and intervention is important, delirium prevention is key to improving outcomes. The delirium prodrome concept has been mooted for decades, but remains poorly characterised. Greater understanding of this prodrome would promote prompt identification of delirium-prone patients, and facilitate improved strategies for delirium prevention and management. Methods Medical inpatients of ≥70 years were screened for prevalent delirium using the Revised Delirium Rating Scale (DRS--‐R98). Those without prevalent delirium were assessed daily for delirium development, prodromal features and motor subtype. Survival analysis models identified which prodromal features predicted the emergence of incident delirium in the cohort in the first week of admission. The Delirium Motor Subtype Scale-4 was used to ascertain motor subtype. Results Of 555 patients approached, 191 patients were included in the prospective study. The median age was 80 (IQR 10) and 101 (52.9%) were male. Sixty-one patients developed incident delirium within a week of admission. Several prodromal features predicted delirium emergence in the cohort. Firstly, using a novel Prodromal Checklist based on the existing literature, and controlling for confounders, seven predictive behavioural features were identified in the prodromal period (for example, increasing confusion; and being easily distractible). Additionally, using serial cognitive tests and the DRS-R98 daily, multiple cognitive and other core delirium features were detected in the prodrome (for example inattention; and sleep-wake cycle disturbance). Examining longitudinal motor subtypes in delirium cases, subtypes were found to be predominantly stable over time, the most prevalent being hypoactive subtype (62.3%). Discussion This thesis explored multiple aspects of delirium in older medical inpatients, with particular focus on the characterisation of the delirium prodrome. These findings should help to inform future delirium educational programmes, and detection and prevention strategies.

Cette thèse propose une redfinition de la notion de frontière dans le contexte américain, avec pour point de dpart les romans de trois voix littéraires issues de trois minorités ethniques : Sandra Cisneros (Caramelo), Cristina Garcia (The Agüero Sisters) et David Plante (The Family et The Native). Je conceptualise la frontière comme fluctuation entre mouvement et immobilité, entre porosité et imperméabilité. Dans le premier chapitre, je fournis des repères sur la théorie des frontières et j'analyse les avancées de ce champ d'étude, du concept de terre frontalière (“Borderland Theories”) jusqu'aux récits d'immigration. Je propose un cadre conceptuel que j'appelle « Écrire la frontière à partir de la perspective de la frontière », lequel permet une lecture neuve des récits de frontière, et une redfinition de la notion elle-même. Prise comme perspective, la frontière est une dynamique vivante, ce qui la rend plurielle et impossible à fixer dfinitivement; aussi les récits de frontière présentent-ils une grande variété dexpériences, toutes liées à des moments et à des points de vue uniques. Dans le second chapitre, j’analyse la porosité des frontières dans le contexte géopolitique contemporain, en mettant en lumière comment la colonisation, la mondialisation économique et l’immigration sont autant de mécanismes de transgression des frontières qui suivent des orientations transnationales, dnationales et postnationales. Dans le troisième chapitre, j’étudie la résurgence des frontières dans la vie des immigrants qui habitent aux États-Unis. J’identifie l’insécurité capitaliste ainsi que la marchandisation de l’espace et de l’ethnicité comme étant à l'origine du renforcement des frontières dlimitant les quartiers ethniques; génératrices de stéréotypes négatifs, ces divisions physiques deviennent une technologie dexclusion et dinjustice sociale. Le dernier chapitre présente une lecture des aspects esthétiques de la frontière, voyant comment ils peuvent contribuer à écrire la frontière à partir de la perspective de la frontière. Dans les textes à l'étude, j'examine de près la problématisation du concept de représentation, la multiplicité des points de vue narratifs, l’inaccessibilité du réel, et la partialité de la médiation. Pemphigus foliaceus is a life threatening skin disease that is associated with autoimmunity to desmoglein, a skin protein involved in the adhesion of keratinocytes. This disease is endemic in certain areas of South America, suggesting the mediation of environmental factors triggering autoimmunity. Among the possible environmental factors, exposure to bites of black flies, in particular Simulittm nigrimanum has been suggested. In this work, we describe the sialotranscriptome of adult female S. nigrimanum flies. It reveals the complexity of the salivary potion of this insect, comprised by over 70 distinct genes within over 30 protein families, including several novel families, even when compared with the previously described sialotranscriptome of the autogenous black fly, S. vitiation. The uncovering of this sialotranscriptome provides a platform for testing pemphigus patient sera against recombinant salivary proteins from S. nigrimanum and for the discovery of novel pharmacologically active compounds.

The recent identification of several additional members of the family of sugar transport facilitators (gene symbol SLC2A, protein symbol GLUT) has created a heterogeneous and, in part, confusing nomenclature. Therefore, this letter provides a summary of the family members and suggests a systematic nomenclature for SLC2A and GLUT symbols.

We undertook annual surveys of flavivirus virus activity in the community of Billiluna of Western Australia in the southeast Kimberley region between 1989 and 2001. Culex annulirostris was the dominant mosquito species, particularly in years of above average rains and flooding. Murray Valley encephalitis (MVE) virus was isolated in 8 of the 13 years of the study from seven mosquito species, but more than 90% of the isolates were from Cx. annulirostris. The results suggest that MVE virus is epizootic in the region, with activity only apparent in years with average or above average rainfall and increased numbers of Cx. annulirostris. High levels of MVE virus activity and associated human cases were detected only once (in 1993) during the survey period. Activity of MVE virus could only be partially correlated with wet season rainfall and flooding, suggesting that a number of other factors must also be considered to accurately predict MVE virus activity at such communities.

Abnormalities of calcium and vitamin D metabolism in cystic fibrosis (CF) are well documented. We tested the hypothesis that alterations in calcium metabolism are related to vitamin D deficiency, and that bone resorption is increased relative to accretion in patients with CF. Calcitropic hormones, electrolytes, osteocalcin (OC) and bone alkaline phosphatase (BAP), (markers of bone mineralisation), urinary deoxypyridinoline [total (t) Dpd, a marker of bone resorption] and lumbar spine bone mineral density (LS BMD), expressed as a z-score, were measured in 149 (81 M) CF and 141 (61 M) control children aged 5.3-10.99 years, adolescents aged 11-17.99 years and adults aged 18-55.9 years. Data were analysed by multiple regression to adjust for age. In patients, FEV1% predicted and CRP (as disease severity markers), genotype and pancreatic status (PS) were recorded.
