Este trabalho está associado ao projeto de pesquisa As transformações do cuidado de saúde e enfermagem em tempo de AIDS: representações sociais e memórias de enfermeiros e profissionais de saúde no Brasil, coordenado pelo Programa de Pós-Graduação em Enfermagem da UERJ. Neste trabalho, foi feito um recorte do projeto original, tendo como objetivo fazer um levantamento das práticas desenvolvidas pelos psicólogos que atuam no Programa Nacional DST/AIDS, além de descrever e analisar o conteúdo das representações sociais dos psicólogos sobre o HIV/AIDS e sobre seus pacientes. A fundamentação teórica do trabalho consiste na Teoria das Representações Sociais, inaugurada por Serge Moscovici em 1961, e desenvolvida por outros autores desde então. Os sujeitos da pesquisa foram doze psicólogos que trabalham em serviços de saúde que desenvolvem as ações do Programa Nacional DST/AIDS na cidade do Rio de Janeiro, em SAES e CTAS. A coleta de dados consistiu em duas etapas: a aplicação de questionários aos profissionais que trabalham no Programa Nacional DST/AIDS e realização de entrevista com alguns dos profissionais de saúde que responderam anteriormente ao questionário. Além das questões presentes no roteiro de entrevista comum a todos os profissionais de saúde, foram inseridas algumas questões relativas às práticas e à especificidade do trabalho do psicólogo nas equipes. Para a análise dos dados brutos obtidos através dos dois roteiros de entrevista, foi utilizada a técnica de análise de conteúdo temático-categorial, operacionalizada pelo software QRS NVivo. O resultado da análise do material discursivo ficou concentrado em seis categorias: 1) A memória do HIV/AIDS: o início da epidemia e sua construção social ao longo do tempo; 2) Mudanças e avanços em relação ao HIV/AIDS: conhecimento, cuidado, mentalidade e perfil dos pacientes; 3) O sistema de atendimento a pacientes com HIV/AIDS: práticas comuns e dificuldades vividas pelos profissionais de saúde; 4) A atuação dos psicólogos com pacientes HIV/AIDS: suas práticas, dificuldades e percepções; 5) Os desafios de se viver com o HIV/AIDS: adesão ao tratamento e discriminação; 6) Políticas governamentais, sociais e institucionais para o HIV/AIDS: desafios e questões epidemiológicas. As representações das psicólogas a respeito do HIV/AIDS contêm alguns elementos compartilhados com a população geral no passado, como a ideia de grupos de risco, mortalidade e culpabilização dos pacientes, mas também apresentam elementos que foram incorporados por conta de sua prática profissional, como a melhoria na qualidade de vida dos pacientes, a AIDS como uma doença crônica e a mudança de perfil dos pacientes. Em suas práticas, embora reconheçam algumas particularidades nos pacientes com HIV/AIDS, as psicólogas não reconhecem nestas particularidades uma necessidade de atendimento diferenciado a esses pacientes.

To evaluate the immunogenicity and safety of a 23-valent pneumococcal vaccine in human immunodeficiency virus (HIV)-seropositive patients, 80 men and 18 women received 1 dose of the vaccine (Pneumo 23; Pasteur Mérieux MSD, Brussels). The total IgG antibody response against all 23 Streptococcus pneumoniae capsular antigens was measured. Antibody levels were expressed in arbitrary units per microliter, referring to a standard curve. Geometric mean titers of the total IgG capsular antibodies on the day of vaccination and 30-45 days later were compared. The ratios of titers after and before vaccination in patients with > 500, 200-500, and < 200 CD4 lymphocytes/microL were 10, 10, and 12.6, respectively. Nonresponse (ratio < 4) occurred in 17% of patients and was unrelated to CD4 cell count. The vaccine was well tolerated; no serious side effects occurred. In 83% of the patients with HIV infection, the total antipneumococcal IgG level was higher after vaccination.

Bacterial lipopolysaccharide (endotoxin) is a frequent contaminant of biological specimens and is also known to be a potent inducer of beta-chemokines and other soluble factors that inhibit HIV-1 infection in vitro. Though lipopolysaccharide (LPS) has been shown to stimulate the production of soluble HIV-1 inhibitors in cultures of monocyte-derived macrophages, the ability of LPS to induce similar inhibitors in other cell types is poorly characterized. Here we show that LPS exhibits potent anti-HIV activity in phytohemagglutinin-stimulated peripheral blood mononuclear cells (PBMCs) but has no detectable anti-HIV-1 activity in TZM-bl cells. The anti-HIV-1 activity of LPS in PBMCs was strongly associated with the production of beta-chemokines from CD14-positive monocytes. Culture supernatants from LPS-stimulated PBMCs exhibited potent anti-HIV-1 activity when added to TZM-bl cells but, in this case, the antiviral activity appeared to be related to IFN-gamma rather than to beta-chemokines. These observations indicate that LPS stimulates PBMCs to produce a complex array of soluble HIV-1 inhibitors, including beta-chemokines and IFN-gamma, that differentially inhibit HIV-1 depending on the target cell type. The results also highlight the need to use endotoxin-free specimens to avoid artifacts when assessing HIV-1-specific neutralizing antibodies in PBMC-based assays.

Consensus HIV-1 genes can decrease the genetic distances between candidate immunogens and field virus strains. To ensure the functionality and optimal presentation of immunologic epitopes, we generated two group-M consensus env genes that contain variable regions either from a wild-type B/C recombinant virus isolate (CON6) or minimal consensus elements (CON-S) in the V1, V2, V4, and V5 regions. C57BL/6 and BALB/c mice were primed twice with CON6, CON-S, and subtype control (92UG37_A and HXB2/Bal_B) DNA and boosted with recombinant vaccinia virus (rVV). Mean antibody titers against 92UG37_A, 89.6_B, 96ZM651_C, CON6, and CON-S Env protein were determined. Both CON6 and CON-S induced higher mean antibody titers against several of the proteins, as compared with the subtype controls. However, no significant differences were found in mean antibody titers in animals immunized with CON6 or CON-S. Cellular immune responses were measured by using five complete Env overlapping peptide sets: subtype A (92UG37_A), subtype B (MN_B, 89.6_B and SF162_B), and subtype C (Chn19_C). The intensity of the induced cellular responses was measured by using pooled Env peptides; T-cell epitopes were identified by using matrix peptide pools and individual peptides. No significant differences in T-cell immune-response intensities were noted between CON6 and CON-S immunized BALB/c and C57BL/6 mice. In BALB/c mice, 10 and eight nonoverlapping T-cell epitopes were identified in CON6 and CON-S, whereas eight epitopes were identified in 92UG37_A and HXB2/BAL_B. In C57BL/6 mice, nine and six nonoverlapping T-cell epitopes were identified after immunization with CON6 and CON-S, respectively, whereas only four and three were identified in 92UG37_A and HXB2/BAL_B, respectively. When combined together from both mouse strains, 18 epitopes were identified. The group M artificial consensus env genes, CON6 and CON-S, were equally immunogenic in breadth and intensity for inducing humoral and cellular immune responses.

Microbicides are women-controlled prophylactics for sexually transmitted infections. The most important class of microbicides target HIV-1 and contain antiviral agents formulated for topical vaginal delivery. Identification of new viral entry inhibitors that target the HIV-1 envelope is important because they can inactivate HIV-1 in the vaginal lumen before virions can come in contact with CD4+ cells in the vaginal mucosa. Carbohydrate binding agents (CBAs) demonstrate the ability to act as entry inhibitors due to their ability to bind to glycans and prevent gp120 binding to CD4+ cells. However, as proteins they present significant challenges in regard to economical production and formulation for resource-poor environments. We have synthesized water-soluble polymer CBAs that contain multiple benzoboroxole moieties. A benzoboroxole-functionalized monomer was synthesized and incorporated into linear oligomers with 2-hydroxypropylmethacrylamide (HPMAm) at different feed ratios using free radical polymerization. The benzoboroxole small molecule analogue demonstrated weak affinity for HIV-1BaL gp120 by SPR; however, the 25 mol % functionalized benzoboroxole oligomer demonstrated a 10-fold decrease in the K(D) for gp120, suggesting an increased avidity for the multivalent polymer construct. High molecular weight polymers functionalized with 25, 50, and 75 mol % benzoboroxole were synthesized and tested for their ability to neutralize HIV-1 entry for two HIV-1 clades and both R5 and X4 coreceptor tropism. All three polymers demonstrated activity against all viral strains tested with EC(50)s that decrease from 15000 nM (1500 microg mL(-1)) for the 25 mol % functionalized polymers to 11 nM (1 microg mL(-1)) for the 75 mol % benzoboroxole-functionalized polymers. These polymers exhibited minimal cytotoxicity after 24 h exposure to a human vaginal cell line.

BACKGROUND: With the global expansion of clinical trials and the expectations of the rise of the emerging economies known as BRICs (Brazil, Russia, India and China), the understanding of factors that affect the willingness to participate in clinical trials of patients from those countries assumes a central role in the future of health research. METHODS: We conducted a systematic review and meta-analysis (SRMA) of willingness to participate in clinical trials among Brazilian patients and then we compared it with Indian patients (with results of another SRMA previously conducted by our group) through a system dynamics model. RESULTS: Five studies were included in the SRMA of Brazilian patients. Our main findings are 1) the major motivation for Brazilian patients to participate in clinical trials is altruism, 2) monetary reimbursement is the least important factor motivating Brazilian patients, 3) the major barrier for Brazilian patients to not participate in clinical trials is the fear of side effects, and 4) Brazilian patients are more likely willing to participate in clinical trials than Indians. CONCLUSION: Our study provides important insights for investigators and sponsors for planning trials in Brazil (and India) in the future. Ignoring these results may lead to unnecessary fund/time spending. More studies are needed to validate our results and for better understanding of this poorly studied theme.

Clearance of anogenital and oropharyngeal HPV infections is attributed primarily to a successful adaptive immune response. To date, little attention has been paid to the potential role of stochastic cell dynamics in the time it takes to clear an HPV infection. In this study, we combine mechanistic mathematical models at the cellular level with epidemiological data at the population level to disentangle the respective roles of immune capacity and cell dynamics in the clearing mechanism. Our results suggest that chance-in form of the stochastic dynamics of basal stem cells-plays a critical role in the elimination of HPV-infected cell clones. In particular, we find that in immunocompetent adolescents with cervical HPV infections, the immune response may contribute less than 20% to virus clearance-the rest is taken care of by the stochastic proliferation dynamics in the basal layer. In HIV-negative individuals, the contribution of the immune response may be negligible.

TB-IRIS could be associated with an exaggerated immune response to TB-antigens. We compared the recovery of IFNγ responses to recall and TB-antigens and explored in vitro innate cytokine production in TB-IRIS patients.

RESUMO: Com o presente estudo pretendemos identificar a sobrecarga resultante do envolvimento familiar com os doentes portadores de VIH/SIDA. Numa breve introdução teórica, procedemos à revisão dos conceitos sobrecarga familiar e dos sentimentos/emoções vivenciados pelos prestadores de cuidados. Metodologia: Estudo do tipo descritivo e exploratório, com uma amostra de 51 indivíduos, cuja finalidade consiste na caracterização dos prestadores de cuidados familiares a doentes com VIH/SIDA. Objectivos: Identificar quem o doente com VIH/SIDA, considera ser a pessoa significativa nos cuidados informais. Caracterizar, do ponto de vista sócio-demográfico, os doentes e os prestadores de cuidados familiares. Identificar sentimentos e emoções de vivências, que justifiquem o sofrimento emocional e as repercussões na sobrecarga familiar nos prestadores de cuidados informais. Instrumentos: Na avaliação da sobrecarga familiar, utilizámos o Questionário de Problemas Familiares”- FPQ (Family Problemas Questionnaire). Para identificação dos Acontecimentos de Vida, adoptámos a escala de Holmes e Rahe (Life Events); Para identificação do estrato social escolhemos escala de Graffar. Finalmente, para a caracterização sócio-demografica concebemos dois questionários: um dirigido aos doentes e o outro aos prestadores de cuidados informais. Conclusões: A sobrecarga da doença VIH/SIDA, nos prestadores de cuidados familiares, não é uniforme nas diferentes dimensões. A dimensão sobrecarga subjectiva é superior à objectiva. O suporte social revela-se fraco, relacionado com as perdas familiares, devidas a morte, pelas relações familiares disfuncionais, entre os membros da família, pela falta de apoio e informação dos técnicos de saúde. O sexo feminino é predominante nos cuidadores. As mães e esposas são o grau de parentesco dominante. Os solteiros são o grupo mais afectado pelo VIH/SIDA. Os cuidadores apresentam idade superior à dos doentes. O estrato social preponderante é o médio baixo e o baixo. Os familiares, apesar da atitude negativa dos doentes perante os cuidadores, mantêm-se envolvidos. Segundo a avaliação multiaxial proposta pelo DM-IV, constatámos, ao nível do eixo I, sintomatologia clínica do tipo das perturbações depressivas e perturbações da ansiedade. No eixo IV, os cuidadores evidenciam problemas psicossociais e ambientais, nomeadamente nas categorias problemas com o grupo de apoio primário, problemas relacionados como grupo social, problemas educacionais, problemas de alojamento, problemas económicos. Os problemas relacionados com o grupo de apoio primário, são os que mais parecem contribuir para os problemas psicossociais e ambientais.---------------------------------------ABSTRACT: This study wants to describe several problems as a result of the family’s relationship with HIV/AIDS patients, like overload. In a brief theoric introduction, we made a small revision about the concepts of family’s overload, and feelings or emotions that have been lived by the people who provide cares to the patients with this chronic disease. Methodology: This is a describing and exploratory study, with a sample with 51 individuals, with the aim to characterize the people inside the family who give care HIV/AIDS patients. Aim: To identify who are the most important people in informal cares from the patient perspective. To characterize, in a social-demographic point of view, patients and the people who take care of them. To identify feelings and emotions that could explain an emotional suffer, and some causes in the family burden. Means: to evaluate the family’s overload we used the Family Problems Questionnaire (FPQ). To identify life events we adopted the Holmes and Rahe scale. To identify the social stratum we used the Graffer scale. Finally to do a socio-economic characterization we did two kinds of questionnaire, the first one was directed for the patients, and the second one was chosen for the people who give care. Conclusions: The HIV/AIDS disease burden on the people who takes familiar cares isn’t uniform on several areas that we studied. The subjective overload it is superior to the objective. The social support is weak and poor, and related with family losses by dead, dysfunctional family relationships, and the lack of support and information by the medical staff. Mothers and wives are the dominant relative degree. And the singles are the major group with HIV/AIDS disease. The people who take care are usually older than the sick. The major social status is low or medium-low. The relatives keep evolved though the negative attitude of the sick. According with the evaluation multiaxial proposed by the DM-IV, in axle 1 we note clinic sintomatologic belonging to the type depressive perturbations and perturbations of the anxiety. Regarding with axle IV the caretakers show up psycho-social and environmental problems, namely on the categories: problems with the primary support group and problems related as social group, educational problems, accommodation problems and.

Introduction : Depuis les années 2000, la distribution du traitement antirétroviral hautement actif (TARVHA) dans les pays à bas et moyens revenus (PBMR) s’est accélérée. Dans ces milieux, l’épidémie du VIH s’est superposée à des conditions préexistantes de malnutrition. Le VIH et la malnutrition forment un cycle vicieux qui contribue à la détérioration des fonctions immunitaires et de l’état nutritionnel. Malgré l’accès au traitement, la malnutrition persiste comme déterminant de la progression de la maladie et de la mortalité chez les séropositifs. Par ailleurs, peu de données existent concernant les trajectoires et déterminants des statuts nutritionnels et immunologiques chez les patients débutant le TARVHA. Objectifs : Documenter les défis méthodologiques associés à la recherche nutritionnelle chez les personnes vivants avec le VIH (PVVIH) dans les PBMR. Investiguer les déterminants et trajectoires du statut nutritionnel à l’initiation et en cours du TARVHA. Investiguer l’association entre les fonctions immunologiques à l’initiation et au cours de la première année de TARVHA selon le statut nutritionnel. Méthodologie: Dans le cadre de la cohorte ATARAO (Mali), 250 PVVIH adultes naïfs au TARVHA ont été recrutés et suivis pendant 12 mois suite à l’initiation du traitement. Les données obtenues ont été jumelées à celles d’une étude prospective similaire provenant du Sénégal (n=372). Des analyses transversales et longitudinales ont été entreprises pour évaluer les covariables de la malnutrition à l’initiation et en cours de traitement. Parallèlement, la fiabilité des mesures anthropométriques collectées dans le cadre de la cohorte ATARAO a été documentée lors de deux sous-études. Finalement, une revue systématique de la littérature a été complétée pour investiguer la réponse immunologique selon le statut nutritionnel à l’initiation et en cours de traitement chez les PVVIH traités. Résultats: Seules les mesures de la taille et du poids se sont révélées fiables et ont pu être utilisées dans nos analyses prospectives. Le traitement a permis une amélioration initiale de l’état nutritionnel chez une majorité de patients. Par contre, ses effets ont atteint un plateau après 6 à 9 mois. Un an plus tard, la malnutrition persistait chez plusieurs patients vivants et actifs (entre 7 et 29 % selon la population et l’indicateur nutritionnel). Comme observé dans nos cohortes et revue systématique, l’attrition était fortement associée à la malnutrition. Par ailleurs, il n’existe pas de relation claire entre le statut immunologique et la malnutrition pré- ou post-HAART. La qualité et la quantité des évidences recensées ne nous permettent pas de tirer de conclusions définitives. Conclusions : Les évidences collectées suggèrent l’existence d’une association transversale et longitudinale entre les statuts nutritionnels et immunologiques chez les patients traités. Cependant, cette association ne fait pas consensus. L’état de la littérature ne nous permet pas de nous prononcer quant à l’impact de la malnutrition sur les fonctions immunologiques à l’initiation et eu cours de la première année de TARVHA. Les difficultés méthodologiques associées à la mesure de l’état nutritionnel ainsi que la forte attrition des patients malnutris suite à la mise sous traitement compliquent l’investigation de cette relation.

The primary aim of these investigations was to probe the spectroscopic, electrochemical, biological and single crystal X-ray diffraction studies of some selected transition metal complexes of 4N-monosubstituted thiosemicarbazones. Transition metal complexes with thiosemicarbazones exhibit a wide range of stereochemistries and possess potential biological activity. Metal complexes of thiosemicarbazones are proved to have improved pharmacological and therapeutic effects. The studies are conducted to bring about a fair understanding of the structure activity relationship and to develop certain effective and economical metal-based antimicrobial agents. Study showed that the thiosemicarbazones have antibacterial, antiviral and antiproliferative properties and hence used against tuberculosis, leprosy, psoriasis, rheumatism, trypanosomiasis and coccidiosis. Certain thiosemicarbazones showed a selective inhibition of HSV and HIV infections. The insolubility of most thiosemicarbazones in water causes difficulty in the oral administration in clinical practice. Transition metal complexes are found to have more activity than uncombined thiosemicarbazones. They exhibit a variety of denticity and can be varied by proper substitution. The stereochemistry assumed by the thiosemicarbazones during the coordination with transition metal ions depends on the factors such as preparative conditions and availability of additional bonding site in the ligand moiety and charge of the ligand. The resulting complexes exhibited a wide range of stereochemistries and have biomimic activity and potential application as sensors.
