A Doença Renal Crônica é um problema de saúde pública crescente no mundo. A detecção e o tratamento precoces reduziriam as altas taxas de morbimortalidade e os custos associados. Este trabalho buscou identificar o panorama do acesso ao cuidado a partir da conduta dos médicos da Atenção Primária à Saúde na linha de cuidado da doença. Aplicaram-se questionários para 62 médicos de família dos Centros de Saúde da Família do município de Fortaleza. Os achados apontam que a Taxa de Filtração Glomerular foi mensurada por apenas 8.1% dos médicos para pacientes diabéticos e 4.8% para pacientes hipertensos. Mais da metade dos médicos (51.2%) referenciariam o paciente apresentando redução leve/moderada da Taxa de Filtração Glomerular ao nível secundário. Por outro lado, 25.8% dos médicos não referenciariam o paciente com Doença Renal Crônica avançada ao especialista. A lacuna entre esses dois níveis da atenção implica em barreira de acesso ao usuário, podendo comprometer avanços no plano da integralidade. A criação de novos dispositivos no processo de trabalho torna-se urgente e o apoio matricial apresenta-se como proposta viável para a articulação das ações entre os níveis da atenção no cuidado do portador da Doença Renal Crônica ou seus fatores de risco.

Report of an experience of nursing students in providing care to a patient with epidermodysplasia verruciformis, during the internship in the discipline Nursing in Transmittable Diseases, at the dermatology ward of a teaching hospital, located in the state of São Paulo, Brazil, in 2009. This is a rare genodermatosis, susceptible to infection with different types of human papilloma viruses, that cause exacerbated physical changes that impact the psyche of the affected individual. Care delivery was based on the assistance complexity indicated by the application of the Patient Classification System proposed by Perroca. The experience allowed students to learn how to cope with a patient affected by a rare and complex disease, of difficult resolution, and to provide integral care to this individual, perceiving him beyond the lesions. One conclude that the experience has greatly contributed to the growth of the students as future professionals.

Introduction: the relationship between professional and patient oriented by humanization standards is a current issue in health research, since this relationship influences the processes of physical recovery, pain reduction and quality of life of individuals. However, this is not observed in most graduate programs in health, which emphasize knowledge and methods in the traditional Cartesian thinking. Objective: to evaluate the importance of contents and practices with a focus on issues of embodiment and alterity, from the perspective of the humanization of health in Physiotherapy courses. Method: we used a qualitative method and data were collected through semi-structured interviews conducted by five randomly selected physiotherapists, aged between 23 and 36 years, with professional experience between two and thirteen years. The questions were on: physical therapy, physical therapist and patient relationship, work pace, involvement in trade associations, field and insert content and practices of the humanization of health in undergraduate courses. Result: the Physiotherapy graduate courses do not prepare the professional to take a close bond with patients. The application of techniques and protocols are focused on knowledge and do not apply to skills related to living together. Conclusion: the curriculum of graduating courses in health should be revised to expand the capacity of the students to become a professional who take care of patients and act as mediator between techniques and protocols, from the perspective of humanization of health, which requires contents and procedures based on embodiment and alterity.

The burn is among the leading causes of morbidity and mortality in our society. It shows a great complexity and is hard to treat. Beyond the physical suffering, the burned patient is affected by psychological distress, requiring a high level of knowledge for assistance planning. The Professional Practice Law No. 7498 establishes the nurse as in charge of the client, as leader of the nursing team and as responsable for the management of physical and human resources. The nurse has autonomy to design the quantitative and qualitative picture of the nursing staff and should use the methodologies for their suitability to the real levels of assistance needed. Material resources represent 15 to 25% of total expenditures at health organizations. Therefore, to maintain the care level, nurses must determine the needs, considering the quantitative, qualitative and financial aspects. The study aimed the survey of the human and material resources necessary for nursing care to patients in a Burns Treatment Unit and identify its epidemiological profile and its nursing diagnoses. We collected the data from medical records of hospitalized between July and August, and the nursing diagnoses were classified through the Taxonomy II proposed by the North American Nursing Diagnosis Association (NANDA). The design of the picture of nurses followed the parameters of COFEN Resolution nº 293/2004. We apply the Fugulin's Patients Classification System to establish levels of the required care. The institution's Cost Center provided a spreadsheet with the purchased items, subsequently classified into ABC. Most hospitalized patients were men, aged between 20 and 50. There was a predominance of patiences with minor burned and the most common type of burn was due to fire. The average residence time was 28.71 days, and 88% of the patients were discharged... Dramatic changes occur in the lives of patients on peritoneal dialysis and his family life, changing lifestyle, professional and social activities. Considering the important role of family in treatment, is crucial for nurses to know how these families perceive their role as caregivers and their difficulties and needs, in the process of peritoneal dialysis. The approach chosen for the study was the phenomenological method, based on Merleau-Ponty, who proposes to understand the human experience, from the description of those who lived it. The study findings show that in the beginning of treatment, family members, feel impacted with the disease severity and eager to become responsible for maintenance treatment, fearing not match the expectations.Guided by staff, appear confident in taking care and deal with the difficulties and complications of treatment, supported by professionals. However, resent the great changes in their social activities and work, his life turns out to be quite limited, due to the dedication to a sick family member. Many feel overwhelmed because they are not supported by other family members. Some envision a future outlook for renal transplant, others seem skeptical, given the long waiting list, especially when advanced age of the sick family member. These results suggest the need for individualized attention to family caregivers, and to encourage the family to organize itself and develop a joint work. In this sense, the design of health care, taking care with the focus of the family, it seems highly appropriate in preparing the plan of family orientation, this is defined as a dynamic unity, which, working in harmony, can contribute positively in the treatment of health a sick member

Objective: The objective of this work, and review concepts on the physiology of hormones Adrenals to enhance pre, trans and post-operative care to dentists facing surgical, dental care of immunosuppressed patients. Literature review: The corticosteroids are among the most used drugs in medicine in recent times due to its anti-inflammatory and immunosuppressive activities, especially in transplant patients and patients with chronic autoimmune diseases. However, the continuing use of these drugs leads to a decrease in endogenous production of cortisol by the adrenal. Besides depressing the patient’s immune system, it may generate many parallel undesirable effects like blocking the production of the natural hormone of adrenal cortex, or steroid called cortisol, which may be the cause of a malfunctioning of the adrenal gland, causing hypofunction or hyperfunction of the gland. Final considerations: This fact will lead to changes in the planning of dental surgery, where it will be necessary special actions towards the patients who have this condition.

The Araçatuba School of Dentistry along with the Veterinary Medical Course, both from UNESP, develop the Extension Project called “Cão Cidadão UNESP” (Unesp Citizen Dog), which is accomplished in some assistance societies, including the Dental Assistance Center to Persons with Disabilities (“Centro de Assistência Odontológica à Pessoa com Deficiência” - CAOE-UNESP). This project develops a multidisciplinary work involving professionals and students from health sciences. The project aims at developing activities to relieve stress, fear and anxiety of patients with disabilities in the moment that precedes the dental treatment. The team performs procedures such as walking with the dogs and patients, toothbrushing the dogs so the patients can see it, in order to learn and motivate themselves, dressing the dogs with some accessories such as white clothes, mask and cap, to associate with image of the dentist, making this relationship more pleasant and welcoming. It is evidenced, through reports of professionals, that the patients become calmer and more collaborative with the dentists, distracting them from the treatment focus and creating a positive bond. It should be noted that this experience has contributed to motivate the professionals to introduce new techniques for dental approach, such as Animal Assisted Therapy. It also shows that animals can be very helpful in treating people with disabilities, because they offer a lot of love, companionship and provide moments of joy and happiness to the participants.

People with disabilities require special dental care according to the type of disability, degree of involvement of intellectual and motor complications, such individuals may have dental problems that require specific training of the dentist. The objective of this study was a literature review emphasizing the need for early and preventive dental treatment for people with disabilities and training and technical knowledge necessary for quality service performed by the dentist. The literature review was conducted primarily using articles published nationally, to establish the development of dentistry for special patients in Brazil, thus emphasizing some policies and measures adopted in certain areas of the country. It was concluded that preventive care is essential and that the projects of health promotion are important facilitators of social inclusion and provide increased success in dental care. Technical training of professionals is extremely important, therefore, be important to trace an appropriate treatment plan and establish communication with the multidisciplinary team involved in patient care.

This study approached perceptions of mental illness by nineteen professionals who work in a Family Health Unit, by means of graphic representations (drawings). We used qualitative methods. Data was collected by means of a Presentative-Expressive Procedure. Four themes were identified using Thematic Analysis. The professionals associate mental disorders with: health care, medical-centered view, exclusion/inclusion, social environment. These perceptions are related to a biological paradigm, favoring the reproduction of prejudices about mental illness. The analysis also emphasizes the importance of a wide range of cares, while considering how difficult it may be for the professionals. We conclude that it is necessary to invest on professional preparation, in an attempt to transform ideoaffective contents presented by the subjects’ imaginary.

O tabagismo é considerado a maior causa evitável de morbidade e mortalidade. O manuseio farmacológico da síndrome de abstinência de nicotina possibilita melhores taxas de cessação. Desenvolvemos um sistema de coleta de dados em nosso programa de assistência ao fumante, que inclui dois instrumentos novos: um escore para dependência de nicotina em fumantes de < 10 cigarros/dia e uma escala de conforto do paciente durante o tratamento do tabagismo. Descrevemos aqui os dois instrumentos, que estão em processo de validação.

Objetivo: Dente Duplo (“double tooth”) é uma anomalia de forma que ocorre pela fusão de dois ou mais dentes ou geminação de um único dente, geralmente durante a fase de morfodiferenciação do germe dentário. A incidência de dente duplo varia 0,14-5,0% da população mundial, sem predileção pelo gênero. Apresentação unilateral é mais comum do que a bilateral. No geral, as anomalias dentarias em pacientes com fissura labiopalatina ocorrem próximas à região da fissura. O objetivo deste trabalho é relatar o tratamento endodôntico não cirúrgico de um caso de dentes duplos em um paciente com fissura labiopalatina. Relato de Caso: Paciente de gênero feminino de 10 anos com fissura labiopalatina matriculado no Hospital de Reabilitação de Anomalias Craniofaciais (HRAC-USP), encaminhado ao setor de Endodontia do HRAC-USP para realizar o tratamento endodôntico dos dentes 11 e 12 para ser re-anatomizados. Pela superposição dos dentes 11, 12, e presença de uma cúspide acessória (“talon cusp”) além de um dente supranumerário, as radiografia panorâmica e a radiografia periapical não mostram claramente a anatomia externa do dente e a interna dos condutos radiculares, motivo pelo qual foi solicitada uma tomografia computadorizada Cone-Beam para determinar o comprimento de trabalho para preparo biomecânico e obturação dos condutos radiculares. Serão apresentadas as etapas de diagnóstico e tratamento realizados no caso. Conclusão: Pela morfologia anormal da coroa e complexidade dos canais radiculares, o tratamento endodôntico apresentaria dificuldades, por isso o exame clínico e radiográfico cuidadoso é essencial para o sucesso do tratamento endodôntico. Desta forma concluise que a imagem fornecida pela tomografia computadorizada Cone-Beam foi útil no planejamento e tratamento deste caso.

En el ámbito de la relación profesional odontólogo-paciente, se verifica la tendencia hacia la consideración del lugar que ocupa el paciente como sujeto portador de derechos, lo que se complementa con un modelo de teorías y acciones orientadas primero hacia la salud y luego hacia la enfermedad. Proponemos, a modo de introducción, una perspectiva interdisciplinaria basada en la imbricación de factores de orden socio-cultural y una filosofía de trabajo centrada en el reconocimiento del rol de paciente como sujeto que demanda acciones en favor de su salud.

Introdução: A unidade de terapia intensiva (UTI) é conhecida por ser um setor de alta complexidade dos pacientes e por seu alto custo ao sistema hospitalar. A gravidade da doença, o tempo de internação na UTI e a necessidade de ventilação mecânica invasiva (VMI) são fatores conhecidos como influenciadores no custo destas unidades, sendo que aproximadamente 30% dos pacientes internados em UTI necessitam de VMI. Os objetivos deste estudo foram avaliar os custos de internação em UTI comparando unidades com assistência de fisioterapia 24 horas e unidades com assistência de fisioterapia 12 horas e analisar o impacto da fisioterapia nos referidos custos. Método: Este é um estudo observacional, prospectivo, realizado em um hospital geral, público e de grande porte, localizado na cidade de São Paulo. Foram incluídos pacientes clínicos e cirúrgicos com 18 anos de idade ou mais, que estiveram em VMI por um período >= 24 horas e que receberam alta da UTI para a enfermaria. A coleta de dados incluiu diagnóstico de internação hospitalar, diagnóstico de admissão na UTI, gravidade do paciente no momento da admissão na UTI através do Acute Physiology and Chronic Health Disease Classification (APACHE II), tempo de VMI e tempo de internação na UTI; para a análise de custos utilizamos a ferramenta Omega French Score. Um modelo de regressão linear múltipla foi construído para verificar a associação entre o custo de internação em UTI com o turno diário de assistência fisioterapêutica. Resultados: Foram incluídos na amostra 815 pacientes, distribuídos em dois grupos conforme o turno de fisioterapia existente na UTI: 332 pacientes em UTI\'s com 24 horas de assistência fisioterapêutica (PT-24) e 483 pacientes em UTI\'s com 12 horas de assistência fisioterapêutica (PT-12). Os grupos não apresentaram diferença quanto ao APACHE II (p=0,65); comparado ao grupo PT-12 o grupo PT-24 era mais velho (p < 0,001), apresentou menor tempo de VMI (p < 0,001) e de internação na UTI (p=0,013). Quanto a análise de custos o grupo PT-24 apresentou custos menores indicados pela menor pontuação no Omega 3 (p=0,005) e Omega Total (p=0,010), menor custo direto, custo com equipe médica e enfermagem (p=0,010). A análise de regressão linear múltipla indicou associação do custo da internação em UTI com as variáveis APACHE II (p < 0,001), tempo de internação da UTI (p < 0,001) e assistência fisioterapêutica em turnos de 24 horas (p=0,05). Conclusão: O grupo com assistência de fisioterapia em turnos de 24 horas apresentou custos menores sendo que a severidade da doença, o tempo de internação na UTI e a assistência de fisioterapia foram variáveis preditoras para redução de custo de internação na UTI
