Os profissionais de saúde mental são o principal instrumento de intervenção nesta área considerada como prioridade de Saúde Pública e estão sujeitos a desgaste emocional e stress, que pode afetar negativamente a sua qualidade de vida. Com este estudo pretende-se avaliar a perceção do estado de saúde e da qualidade de vida relacionada com a saúde nos profissionais de saúde mental. Para responder ao objetivo traçado optou-se por realizar um estudo observacional analítico transversal, com abordagem quantitativa. Foi utilizado o SF36v2 como instrumento genérico de avaliação da Qualidade de Vida, que já se encontra validado para a população portuguesa, complementado por um questionário socioprofissional. A recolha de dados decorreu online, de 28 de Janeiro a 30 de Abril de 2013. A amostra foi constituída por 201 profissionais de saúde mental de Portugal Continental. Os profissionais de saúde mental que integraram a amostra revelaram pior perceção do estado de saúde e qualidade de vida, quando comparados com os valores de referência para a população portuguesa com nível de instrução alto. Em termos de domínios do instrumento, piores resultados foram encontrados para todos eles com exceção do domínio Saúde Mental. A avaliação da qualidade de vida dos profissionais de saúde mental possibilita a implementação alterações no funcionamento dos serviços de saúde mental, nomeadamente de programas de gestão de stress, de estratégias de coping e de competências de interação e comunicação entre os profissionais, podendo contribuir para uma melhoria na prestação dos cuidados de saúde aos utentes.

OBJECTIVE: To examine the effectiveness of motivational interviewing (MI) training among medical students. METHODS: All students (n=131) (year 5) at Lausanne Medical School, Switzerland were randomized into an experimental or a control group. After a training in basic communication skills (control condition), an 8-h MI training was completed by 84.8% students in the exprimental group. One week later, students in both groups were invited to meet with two standardized patients. MI skills were coded by blinded research assistants using the Motivational Interviewing Treatment Integrity 3.0. RESULTS: Superior MI performance was shown for trained versus control students, as demonstrated by higher scores for "Empathy" [p<0.001] and "MI Spirit" [p<0.001]. Scores were similar between groups for "Direction", indicating that students in both groups invited the patient to talk about behavior change. Behavior counts assessment demonstrated better performance in MI in trained versus untrained students regarding occurences of MI-adherent behavior [p<0.001], MI non-adherent behavior [p<0.001], Closed questions [p<0.001], Open questions [p=0.001], simple reflections [p=0.03], and Complex reflections [p<0.001]. Occurrences were similar between groups regarding "Giving information". CONCLUSION: An 8-h training workshop was associated with improved MI performance. PRACTICE IMPLICATIONS: These findings lend support for the implementation of MI training in medical schools.

Although interpersonal continuity is commonly assumed to be essential for care, some patients prefer to attend a university outpatient clinic where physicians change regularly and interpersonal continuity of care is not ensured. The aim of this exploratory study was to evaluate the differences between patients attending a university outpatient clinic and patients frequenting a private practice, explore their patterns of care-seeking and their understanding of continued care. We conducted a cross-sectional study of patients attending the university medical outpatient clinic (OC) in Lausanne, Switzerland and ten randomly selected private general practices (PP). Eligible patients were >30 years, Swiss nationals or long term residents, with one or more chronic conditions and attending the same practice for >3 years. They were asked to complete a questionnaire on sociodemographic data, use of medical resources and reasons for choosing and remaining at the same practice. Semi-structured interviews were conducted with a randomly selected subset of 26 patients to further explore their preferences. 329 patient questionnaires were completed, 219 by PP and 110 by OC patients. OC patients tended to be of lower socioeconomic status than PP patients. The main reason for choosing a PP were personal recommendation, while a higher percentage of patients chose the OC because they could obtain a first appointment quickly. A higher percentage of PP patients accorded importance to physician communication skills and trust, whereas a higher percentage of OC patients favoured investigation facilities. Qualitative data suggested that although OC and PP patients reported different reasons for consulting, their expectations on the medical and relationship level were similar. Our study suggests that the two groups of patients belong to different social backgrounds, have different patterns of care-seeking and attach importance to different aspects of care continuity. However, patients' expectations and perceptions of the physician-patient relationship are similar.

PROBLEM: Truth-telling is an important component of respect for patients' self-determination, but in the context of breaking bad news, it is also a distressing and difficult task. INTERVENTION: We investigated the long-term influence of a simulated patient-based teaching intervention, integrating learning objectives in communication skills and ethics into students' attitudes and concerns regarding truth-telling. We followed two cohorts of medical students from the preclinical third year to their clinical rotations (fifth year). Open-ended responses were analysed to explore medical students' reported difficulties in breaking bad news. CONTEXT: This intervention was implemented during the last preclinical year of a problem-based medical curriculum, in collaboration between the doctor-patient communication and ethics programs. OUTCOME: Over time, concerns such as empathy and truthfulness shifted from a personal to a relational focus. Whereas 'truthfulness' was a concern for the content of the message, 'truth-telling' included concerns on how information was communicated and how realistically it was received. Truth-telling required empathy, adaptation to the patient, and appropriate management of emotions, both for the patient's welfare and for a realistic understanding of the situation. LESSONS LEARNED: Our study confirms that an intervention confronting students with a realistic situation succeeds in making them more aware of the real issues of truth-telling. Medical students deepened their reflection over time, acquiring a deeper understanding of the relational dimension of values such as truth-telling, and honing their view of empathy.

This study examined the attitudes of South Korean teachers of English in Jeollanamdo toward Konglish, particularly in relation to English education. The literature search shows that Konglish is a typical local variety, evolved from the borrowing and redefining of English words that became part of everyday South Korean speech. Konglish is not unique in this regard. Japlish in Japan and Chinglish in China developed for similar reasons and display the distinctive characteristics of those languages. However, Konglish is usually defined as poor and incorrect. Teachers in the study expressed embarrassment, shyness, guilt, and anger about Konglish. On the other hand, they also valued it as something uniquely theirs. Teachers believed that students should not be taught that Konglish is bad English. However, students should be taught that it is poor or incorrect. With few exceptions, they correct Konglish in their classes. Teachers exhibited considerable inner conflict. They defined Konglish as valid when used in Korea with Koreans. However, some preferred that their students not use it, even with their friends. This may cause students to judge Konglish as unacceptable or inferior. The teachers believed that students should learn to distinguish between Konglish and "Standard English," and that they should learn about the contexts in which each is appropriate or preferred. The conclusion, therefore, is that South Korean teachers see the value of teaching about varieties of English. The recommendations are that intelligibility, broader communication skills, and information about International English be included in the curriculum in South Korea.

The new Physiotherapy and Occupational Therapy programmes, based in the Faculty of Health Sciences, McMaster University (Hamilton, Ontario) are unique. The teaching and learning philosophies utilized are based on learner-centred and selfdirected learning theories. The 1991 admissions process of these programmes attempted to select individuals who would make highly qualified professionals and who would have the necessary skills to complete such unique programmes. In order to: 1 . learn more about the concept of self-directed learning and its related characteristics in health care professionals; 2. examine the relationship between various student characteristics - personal, learner and those assessed during the admissions process - and final course grades, and 3. determine which, if any, smdent characteristics could be considered predictors for success in learner-centred programmes requiring self-directed learning skills, a correlational research design was developed and carried out. Thirty Occupational Therapy and thirty Physiotherapy smdents were asked to complete 2 instruments - a questionnaire developed by the author and the Oddi Continuing Learning Inventory (Oddi, 1986). Course grades and ratings of students during the admissions process were also obtained. Both questionnaires were examined for reliability, and factor analyses were conducted to determine construct validity. Data obtained from the questionnaires, course grades and student ratings (from the admissions process) were analyzed and compared using the Contingency Co-efficient, the Pearson's product-moment correlation co-efficient, and the multiple regression analysis model. The research findings demonstrated a positive relationship (as identified by Contingency Coefficient or Pearson r values) between various course grades and the following personal and learner characteristics: field of smdy of highest level of education achieved, level of education achieved, sex, marital stams, motivation for completing the programmes, reasons for eru-oling in the programmes, decision to enrol in the programmes, employment history, preferred learning style, strong selfconcept and the identification of various components of the concept of self-directed learning. In most cases, the relationships were significant to the 0.01 or 0.(X)1 levels. Results of the multiple regression analyses demonstrated that several learner and admissions characteristic variables had R^ values that accounted for the largest proportion of the variance in several dependent variables. Thus, these variables could be considered predictors for success. The learner characteristics included: level of education and strong self-concept. The admissions characteristics included: ability to evaluate strengths, ability to give feedback, curiosity and creativity, and communication skills. It is recommended that research continue to be conducted to substantiate the relationships found between course grades and characteristic variables in more diverse populations. "Success in self-directed programmes" from the learner's perspective should also be investigated. The Oddi Continuing Learning Inventory should continue to be researched. Further research may lead to refinement or further development of the instrument, and may provide further insight into self-directed learner attributes. The concept of self-directed learning continues to be incorporated into educational programmes, and thus should continue to be explored.

The purpose of this study was to identify elite triathlon coaches’ beliefs and practices as they pertain to motivating world-class triathletes. Semi-structured interviews were conducted with four elite triathlon coaches operating out of three different training groups. Furthermore, the athletes within these groups completed questionnaires assessing their motivational profiles, basic psychological needs, and perceived autonomy support. The interviews were analyzed deductively according to the Motivational Model of the Coach-Athlete Relationship (Mageau & Vallerland, 2003). The data revealed that coaches predominantly used an autonomy-supportive coaching style while also providing structure and involvement. The coaches emphasized an individualized approach for each athlete to best meet their needs. Athletes’ responses to the questionnaires provide support for the coaches’ philosophies and perceived behaviours. The findings of this study provide valuable direction for elite and developing coaches looking to enhance their communication skills in order to optimize athlete needs and motivation.

Ce mémoire porte sur la perception des acteurs multisectoriels de la petite enfance concernant le développement langagier des tout-petits et les interventions qu’ils proposent pour le soutenir. L’origine de cette question vient de l’Enquête sur la maturité scolaire des enfants montréalais. En route pour l'école! (DSPM, 2008a) qui révélait que les tout-petits montréalais étaient plus vulnérables dans les domaines du développement cognitif et langagier et des habiletés de communication et connaissances générales que ceux de l’échantillon normatif canadien, jumelée à la préoccupation des acteurs eux-mêmes relativement à cette problématique. Les objectifs sont de décrire le portrait qu’ont ces acteurs du développement langagier des enfants de leur milieu et analyser ces représentations sur le plan de leur articulation problème – solution. Pour y répondre, un cadre conceptuel écologique mettant en relation quatre niveaux d’influence (enfant, famille, voisinage, environnement général) et cinq axes d’intervention a été conçu. La méthodologie adoptée fut une étude de cas qualitative à base d’analyses documentaires et d’entrevues (n=10) auprès des six secteurs engagés dans l’intervention intersectorielle en petite enfance dans un CSSS montréalais. Les résultats montrent que ces acteurs connaissent les facteurs influençant le développement langagier des tout-petits. Les interventions actuelles ou souhaitées sont cohérentes avec les écrits scientifiques et partiellement stratégiques par rapport aux facteurs de risques identifiés. Des actions doivent toutefois être posées pour soutenir davantage les jeunes familles, dont celles visant à hausser la qualité du voisinage ou encore, l’accès à des services publics de qualité.

Ce mémoire de maîtrise porte sur la gestion des risques informationnels dans l’entreprise privée. Plus précisément, nous avons cherché à comprendre, à partir de l’expérience et du point de vue des gestionnaires de la sécurité, comment s’élaborait une prise de décisions relativement à la protection des actifs informationnels d’une entreprise, de l’identification des risques à la mise en place de mesures visant à les réduire. Pour ce faire, nous devions dégager les éléments du contexte organisationnel qui contribuent à façonner les décisions du gestionnaire de la sécurité en cette matière en considérant deux principales dimensions : la dynamique relationnelle à l’œuvre de même que les enjeux, les contraintes et les opportunités susceptibles d’influence la prise de décisions. Nous voulions également connaitre le rôle et les responsabilités du gestionnaire de la sécurité au sein de ce processus décisionnel et préciser, le cas échéant, sa participation au modèle de gouvernance de gestion des risques. Pour rendre compte de la complexité de ce processus, il semblait approprié de concevoir un cadre théorique combinant deux approches: l’approche multidimensionnelle du risque et l’approche de la transaction sociale. Si la première considère que la définition du risque doit être contextualiser, l’autre admet que la dynamique relationnelle n’est pas le simple fait de jeux entres acteurs stratégiques. L’analyse en fonction de ses deux approches a révélé que la gestion des risques informationnels dans une entreprise est largement tributaire des caractéristiques personnelles du gestionnaire de la sécurité. Certes, le contexte organisationnel et la multiplication des enjeux sécuritaires exercent une influence considérable sur le processus décisionnel mais l’expérience, les connaissances et les capacités communicationnelles du gestionnaire contribuent directement à la réussite de chaque phase du processus de gestion des risques informationnels.

Se revisan diferentes formas en que la influencia social puede incidir sobre los comportamientos heterosexuales de prevención de la transmisión del VIH de los jóvenes y se presentan los resultados de algunos trabajos de las autoras, así como de otros investigadores, en que se analizan dichas relaciones. Se concluye resaltando: 1) la utilidad clínica de la evaluación de las expectativas de autoeficacia para poder intervenir específicamente en aquellas áreas en que los jóvenes se perciban con menores capacidades para ser preventivos, 2) la relación observada entre el uso de preservativo autoinformado y la creencia en su aceptación por parte de los referentes sociales más cercanos y 3) la conveniencia de que los jóvenes posean suficientes habilidades de comunicación que les permitan negociar con éxito el uso del preservativo y les ayuden a compensar posibles influencias sociales en contra de su empleo
