BACKGROUND Prisoners represent a vulnerable population for blood-borne and sexually transmitted infections which can potentially lead to liver fibrosis and ultimately cirrhosis. However, little is known about the prevalence of liver fibrosis and associated risk factors among inmates in sub-Saharan Africa. METHODS Screening of liver fibrosis was undertaken in a randomly selected sample of male inmates incarcerated in Lome, Togo and in Dakar, Senegal using transient elastography. A liver stiffness measurement ≥9.5 KPa was retained to define the presence of a severe liver fibrosis. All included inmates were also screened for HIV, Hepatitis B Virus (HBV) and Hepatitis C Virus (HCV) infection. Substances abuse including alcohol, tobacco and cannabis use were assessed during face-to-face interviews. Odds Ratio (OR) estimates were computed with their 95 % Confidence Interval (CI) to identify factors associated with severe liver fibrosis. RESULTS Overall, 680 inmates were included with a median age of 30 years [interquartile range: 24-35]. The prevalence of severe fibrosis was 3.1 % (4.9 % in Lome and 1.2 % in Dakar). Infections with HIV, HBV and HCV were identified in 2.6 %, 12.5 % and 0.5 % of inmates, respectively. Factors associated with a severe liver fibrosis were HIV infection (OR = 7.6; CI 1.8-32.1), HBV infection (OR = 4.8; CI 1.8-12.8), HCV infection (OR = 52.6; CI 4.1-673.8), use of traditional medicines (OR = 3.7; CI 1.4-10.1) and being incarcerated in Lome (OR = 3.3; CI 1.1-9.8) compared to Dakar. CONCLUSIONS HIV infection and viral hepatitis infections were identified as important and independent determinants of severe liver fibrosis. While access to active antiviral therapies against HIV and viral hepatitis expands in Africa, adapted strategies for the monitoring of liver disease need to be explored, especially in vulnerable populations such as inmates.

The molecular pathogenesis of various categories of breast cancer (BC) has been well described, but surprisingly few reports have appeared on analysis of somatic mutations in bilateral BC. We have performed a polymerase chain reaction (PCR)-driven investigation of chromosomal regions showing common loss of heterozygosity (LOH) in 23 cases (46 rumors) from patients diagnosed with bilateral BC, LOH was observed in 15/46 (33%) informative tumors for chromosome 1p, 5/32 (16%) for 5q, 12/44 (27%) for 11q, 15/40 (38%) for 13q and 4/24 (17%) for 17p. These values are within the range of interlaboratory variations reported fur unilateral BC, There was no strong evidence for concordance of LOH within the same patient for any of the chromosomal loci tested. Atypical for breast carcinomas, 7/46 (15%) turners accumulated a high frequency (ranging from 11 to 29%) of shortened dinucleotide CA repeats, implying microsatellite instability (MI). Further analysis with the highly informative BAT-26 marker allowed for the classification of two of these tumors as having a replication error positive (RER+/MSI-H) phenotype, whereas the remaining five carcinomas harbored so-called borderline MI. Thus an involvement of both RER+ and borderline MI appears to be a distinct feature of bilateral breast carcinomas compared to unilateral lesions. (C) 2000 Elsevier Science Ireland Ltd. This dissertation was an analysis of the root and proximate causes of the September 2002 civil war in Côte d’Ivoire. The central question of this study was: Why did Côte d’Ivoire, which was relatively stable under President Houphouët-Boigny, suddenly begin to experience political violence in the 1990s and an explosion in 2002? Côte d’Ivoire was an interesting case because it was stable for a long period of time, apparently making it an infertile ground for conflict. It was also interesting for comparative purposes because of the fact that several states in West Africa (for instance, Benin, Togo, and Ghana) have experienced military coups but not have civil wars. Finally, this case was an opportunity to revisit the debate on the causes of civil wars in the African context. Chapter one has outlined the entire dissertation project and contextualized the analysis that follows in the subsequent chapters. Chapter two has reviewed the literature on civil wars in general, identified the different types of civil wars, and the type the Ivoiran war is. Chapter three has examined the domestic and international political economy as a source of the civil violence in Côte d’Ivoire. Chapter four has examined the role of ethnicity and region as identities of the war, while chapter five has analyzed the role of the foreign relations in the civil war, as well as the regional political context. Chapter six has distinguished between the root and proximate causes of the Ivoirian civil war, made judgments about the relative weight of the various causes, and the extent to which the weight of the causes can be measured. The study found that the “Ivoirité” was the most important trigger of the civil war in Côte d’Ivoire. The overall conclusion of my dissertation was that the September 2002 crisis in that country was a political crisis which occured in the context of a political reform. It first started with succession problems in 1993 followed by the controversial elections in 1995 and 2000. Later, this electoral politics spread beyond electoral issues, namely citizenship matters.

Le travail examine le rôle des prix dans le processus d’ajustement de la balance commerciale des pays de la zone CFA et vérifie la pertinence de la condition Marshall Lerner. Nous avons dérivé des fonctions d'importation et d'exportation à partir d'un problème d'optimisation dynamique puisé dans l'article de Reinhart (1994). Nous nous sommes servis des tests de cointégration. L'étude est calibrée sur le Cameroun, la Côte d'Ivoire, le Gabon et le Togo. Les résultats laissent croire qu'il n'existe pas de relation de long terme entre les prix, les exportations et les importations. La condition Marshall Lerner s'est révélée non valable à court terme. La révision des hypothèses sous-jacentes à cette condition a mis en exergue les élasticités des importations dans les possibilités de réussite de cette dévaluation.

Objetivo: Establecer la relación entre la exposición ocupacional a altas temperaturas o sobrecarga térmica con el comportamiento fisiológico, metabólico y electrocardiográfico. Métodos: estudio de corte transversal, donde se incluyeron dos grupos (expuesto y no expuesto a altas temperaturas) en una empresa minera, en el departamento de Boyacá, Colombia, en el año 2016. El número de participantes fue de 160 trabajadores del género masculino, grupo expuesto (n=86) y grupo no expuesto (n=74). La exposición ocupacional a sobrecarga térmica se evaluó con el índice de temperatura de globo y bulbo húmedo (TGBH), el comportamiento fisiológico con el índice de costo cardiaco relativo (ICCR) con mediciones de frecuencia cardiaca (FC), el comportamiento metabólico con la determinación del colesterol total (CT), colesterol de alta densidad (C-HDL), colesterol de baja densidad (C-LDL), triglicéridos (TG) y glicemia basal (GL). Las alteraciones electrocardiográficas con la toma de Electrocardiograma de 12 derivaciones. También fueron evaluadas variables antropométricas, tensión arterial, hábitos y antecedentes de enfermedad cardiovascular en ambos grupos. Resultados: incrementos significativos del ICCR (p<0.001) y la carga física (p<0.001) fueron encontrados en los trabajadores expuestos a altas temperaturas. Los índices lipídicos y glicemia, así como los antecedentes personales cardiovasculares, IMC, consumo de cigarrillo y consumo de alcohol, no mostraron significancia. El antecedente familiar de ACV (p=0.043) y el EKG alterado (p=0.011) mostraron una asociación significativa con la exposición a altas temperaturas. El modelo de regresión lineal múltiple explicó la relación entre el incremento del ICCR y la exposición a altas temperaturas (β=4,213, IC 95%: 1.57,6.85) ajustado por variables fisiológicas y electrocardiográficas. Conclusiones: La exposición ocupacional a altas temperaturas, presenta asociación con las alteraciones cardiovasculares a nivel fisiológico y electrocardiográfico, aumentando el ICCR y la carga física de trabajo (GE trabajo).

El presente estudio de caso tiene como objetivo analizar la influencia de la gobernación de Tokio en la formulación de la política exterior de Japón durante la disputa territorial por las islas Senkaku/Diaoyu. Para ello, se identifican los puntos más relevantes de la política exterior de seguridad de Japón después de la Segunda Guerra Mundial. Se hace un énfasis en la política bilateral de seguridad sino-japonesa, con el fin de ubicar el conflicto territorial por las islas Senkaku/Diaoyu como un punto importante en la agenda internacional de seguridad de ambos países. Se estudia y analiza el concepto de paradiplomacia; articulado, a su vez, por los conceptos de identidad y rol en política exterior de la perspectiva teórica del Constructivismo de las Relaciones Internacionales, para así analizar la influencia de Tokio en el manejo de la política exterior de Japón en el marco del conflicto territorial por las islas Senkaku/Diaoyu.

Introducción: Se considera el trabajo rotativo nocturno como aquel que se lleva a cabo fuera de las horas laborales diurnas. Se ha planteado que el trabajo en horas nocturnas podría estar relacionado con diversos desordenes, dentro de ellos un incremento en el índice de masa corporal con el consecuente desarrollo de sobrepeso y obesidad. El objetivo de este estudio es realizar una revisión de la literatura existente, acerca del trabajo rotativo nocturno y su relación con el índice de masa corporal (IMC) en trabajadores de la salud. Métodos: Se realizó una revisión sistemática de información publicada en los últimos 10 años en la base de datos PUBMED y se seleccionaron únicamente los artículos de texto completo relacionados con este tema en los idiomas inglés, español e italiano. Resultados: A través de la búsqueda se encontraron 10 artículos que cumplieron los criterios de inclusión definidos. Predominantemente se obtuvieron estudios de diseño transversal. Tres de estos estudios encontraron una asociación entre trabajo rotativo nocturno actual e incremento en el IMC y por el contrario dos estudios no hallaron esta asociación. Por otro lado, seis estudios coinciden en una asociación positiva entre mayor tiempo acumulado en trabajo rotativo nocturno y un incremento en el IMC. Conclusiones: Los hallazgos encontrados en los estudios analizados apuntan a la existencia de una estrecha relación entre el aumento del Índice de Masa Corporal (IMC) con los turnos nocturnos (o rotativos que incluyen este horario) llevados a cabo de manera prolongada, en los trabajadores de la salud, en comparación con los trabajadores que realizan turnos diurnos regulares.
