Introdução. Durante a gravidez e amamentação, algumasmães alteram a sua dieta com o propósito de aumentar a quantidade e/ou qualidade do seu leite. Objectivos. Caracterizar as alterações dietéticas maternas durante a gravidez e amamentação, identificar factores associados a essas alterações e a sua relação com a manutenção do aleitamento materno aos três meses de vida. Metodologia. Estudo observacional, descritivo, transversal bifásico de uma amostra de 122 díades mãe-filho, realizado num hospital de apoio perinatal diferenciado. Primeira fase: auto-preenchimento de um inquérito nas 48h após o parto, avaliando variáveis sócio-demográficas, história prévia de amamentação, conhecimentos e atitudes face à amamentação e a dieta materna. Segunda fase: inquérito telefónico três meses após o parto, avaliando a duração da amamentação, alterações dietéticas realizadas e eventuais associações a sintomas no lactente. Resultados. A idade materna média foi 30 anos; 43% frequentou o ensino superior; 81% referiu ter alterado a dieta durante a gravidez. Noventa e nove por cento pretendia amamentar e 98% planeava alterar a dieta durante a amamentação: café, chocolate, refrigerantes com cafeína, enchidos, conservas e feijão seriam os alimentos a evitar/eliminar. Aos três meses, 67% das díades mantinha o aleitamento materno; 89% das mães alterou a sua dieta neste período, com um padrão de alimentos evitados/eliminados semelhante ao referido na primeira fase; em 39% dos casos foi associada sintomatologia do lactente, sobretudo cólicas, a alimentos da dieta materna. Não foi encontrada associação entre o número de alimentos evitados/eliminados durante a gravidez/amamentação e a manutenção da amamentação aos três meses de vida. Conclusões. Este estudo evidencia as restrições dietéticas efectuadas pelas mães durante a gravidez e amamentação. A limitação da dieta materna na gravidez e amamentação reduz a experiência de sabores a que é exposto o feto e o lactentecom potencial prejuízo na diversificação alimentar. É necessário implementar medidas de ensino dietético para superar preconceitos.

Introduction: The 2D:4D digit ratio is sexually-dimorphic, probably due to testosterone action through the perinatal period. We characterize the 2D:4D ratio in newborn (NB) infants, in between the pre- and postnatal surges of testosterone, and relate it to the mother's 2D:4D and to testosterone levels in the amniotic fluid (AF). Subjects and methods: Testosterone was assayed in samples of maternal plasma and AF collected at amniocentesis. Shortly after birth, 106 NBs and their mothers were measured for 2D:4D ratio. Results: NB males had lower mean 2D:4D ratios than females but this dimorphism was significant only for the left hand (males: 0.927; females: 0.950; p=0.004). Mothers who had sons had lower 2D:4D ratios than those who had daughters and the mother's 2D:4D were higher than those of NBs regardless of sex. Both hands of NB females were negatively correlated with AF testosterone and positively correlated with the mother's 2D:4D, but males showed no significant associations. Maternal plasma testosterone also showed a negative weak correlation with NB's digit ratio in both sexes. Conclusions: Sexual dimorphism at birth was only significant for the left hand, in contrast with reports of greater right hand dimorphism, suggesting that postnatal testosterone is determinant for 2D:4D stabilization. The lower 2D:4D ratios in mothers who had sons support claims that hormone levels in parents are influential for determining their children's sex. NB female's digit ratio, but not males', was associated to the level of AF testosterone. The mother's 2D:4D ratios were positively correlated with their daughters' 2D:4D, but the same was not observed for male NBs, suggesting that prenatal testosterone levels in male fetus lead their 2D:4D ratios to stray from their mothers' with high individual variability.

In this paper we present a study of members of 265 nuclear families, aged six or more. This study is based of family heredograms, and takes into account the clinical form ofschistosomiasis observed before treatment with oxamniquine. The probability of occurrence of two or more cases ofhepatosplenomegaly is low, notwithstanding the fact that it was observed in 38 families. Even less frequent is the occurrence of three or more cases observed in 17 families (P=0.002). The concentration of the hepatosplenic form was higher among siblings than it was among mothers and children, or fathers and children. It was found to be not significant between husband (father) and wife (mother). These observations reinforce the evidence for the presence of a genetic component in susceptibility to the hepatosplenic form of the disease. In cases in which the mother was hepatosplenic there was a higher incidence of hepatosplenic children; the relative risk was a least five times higher than in those in which the father was the affected member (the maternal effect). In cases where both members were affected by the hepatointestinal form, the risk to the filial generation was similar to that of the population in general. Thus, in the process towards severe forms of Schistosomiasis mansoni, pre and post natal factors might be involved.

Avaliamos a transmissão maternal do Trypanosoma cruzi em 278 filhos de 145 mães com a doença crônica causada por esse protozoário, nascidos após o estabelecimento do diagnóstico materno. Usamos, nas mães e nos filhos, provas sorológicas como base do objetivo do estudo. Verificamos que ocorreram apenas duas veiculações pela modalidade citada (2/278 = 0,7%). Como houve amamentação, não foi possível diferenciar, nesses casos, a transmissão congênita da adquirida pelo leite, mas, de qualquer forma, processou-se transmissão maternal, que constitui uma das modalidades alternativas de infecção pelo Trypanosoma cruzi. A metodologia empregada é mais uma dentre as diversas propostas para aquilatar a prevalência desse tipo de propagação do Trypanosoma cruzi. Ao lado da constatação fundamental, registramos fatos relacionados com a gestação, parto, puerpério, abortamento, prematuridade, nati-neomortalidade e aleitamento materno, que podem representar subsídios para melhores interpretações sobre o assunto.

We studied three pregnant women with acute chagasic infection. Two patients, infected in the third trimester of pregnancy, had uninfected children. The third patient, infected earlier, had an infected newborn. These results encourage research on risk factors of transmission and on medical decisions concerning pregnant women with acute Chagas' disease.

Malaria is one of the most devastating diseases in the world. In Plasmodium endemic regions, pregnant women are among the most vulnerable groups. Pregnancy Associated Malaria (PAM) threatens both maternal and foetal lives. Despite differences between human and mouse placentas PAM mouse models recapitulate key pathological features of human PAM. RESUMO - É recomendado o aleitamento materno exclusivo desde o nascimento até aos 6 meses. Angola apresenta uma situação bastante preocupante, pois é um dos países do mundo com maior taxa de mortalidade infantil: em 2010 era de 114,9 mortes por cada 1000 nascimentos. O desmame precoce, a contaminação dos alimentos e da água potável e a desnutrição criam condições para aparecimento de doenças que causam elevadas mortes em crianças com menos de um ano. Em muitas regiões de África, o parto ainda é um acontecimento marcado por práticas culturais ancestrais que podem prejudicar a saúde e sobrevivência dos recém-nascidos, além de pôr em perigo a vida das mães. Algumas dessas práticas incluem: (i) deitar fora o colostro; e (ii) alimentar os bebés com outros alimentos que não o leite materno. Este estudo é quantitativo com metodologia transversal e pretende identificar, descrever e analisar os fatores que contribuem para o abandono do aleitamento materno exclusivo antes dos 6 meses de idade, na província do Uíge, em Angola, bem como indicar formas de intervenção para que os índices de prevalência do aleitamento materno exclusivo possam aumentar. A população em estudo são mães com filhos até um ano de idade e utilizadoras do centro de saúde materno infantil do município sede da província de Uíge em Angola, da qual será considerada uma amostra de 418 mães. Os dados obtidos através de questionário serão registados em quadros e gráficos para posterior análise estatística através de tabelas de frequências, cálculo de percentagens e taxas de incidência.

INTRODUCTION: ​This study aimed to describe the epidemiology of congenital and maternal syphilis in the Brazilian Federal District in 2010. ​ METHODS: ​A retrospective descriptive study was conducted on the basis of the cases recorded in the System of Notifiable Disease Information. RESULTS: ​The study population comprised 133 cases of congenital syphilis; of these, 116 (52.6%) mothers received prenatal care, and 70 (60.4%) were diagnosed with syphilis during pregnancy. Only 1 mother was adequately treated, and 100 (75.2%) of the pregnant women's partners did not undergo treatment for syphilis. ​ CONCLUSIONS: Although mothers attended prenatal care, not all were diagnosed during pregnancy or received adequate treatment for syphilis, as their partners did not undergo treatment for syphilis.

Breastfeeding is the natural and safe way of feeding small infants, providing nutritional, immunological, psychological and economic recognized and unquestionable advantages. These qualities are especially important in premature infants, because of their vulnerability. Despite highly desirable, there is, in general, little success in breastfeeding preterm infants, especially in special care neonatal units. There are evidences that a high supportive hospital environment, with an interdisciplinary team, makes possible to these infants to be breastfed. In this article, the authors present an up-to-date review about the components of human milk and its unique characteristics, as well as describes aspects that make the breast milk particularly suitable for feeding the premature newborn.

Profound modifications in the profile of patients are currently being observed within the epidemic context of AIDS, especially with respect to pauperization and feminization of the disease. The population most frequently affected is in the reproductive age, and among adults aged 18 to 24 years, the ratio is 1 man to 1 woman, a phenomenon occurring uniformly all over the world. One of the main challenges for HIV-1-infected pregnant women and their doctors is the effect of the interaction between HIV infection and pregnancy. The present article is a review of the literature; and its objective is to assess the influence of HIV-1 infection seen from the maternal perspective, with a discussion of immunologic function, maternal prognosis, and the HIV-abortion interface. At present, we cannot conclude that pregnancy has a short-term effect on the evolution of HIV infection, but the concomitance of HIV and pregnancy may adversely affect the prognosis of gestation, especially in view of its frequent association with increased abortion and puerperal morbidity rates.

PURPOSE: To evaluate the evolution of glycemic levels in newborns of hypertensive mothers according to maternal treatment. METHODS: Prospective randomized study, including 93 newborns of mothers treated with isradipine (n = 39), atenolol (n = 40), or low sodium diet (control group - n=14). Glycemia was determined at birth (mother and newborn by the oxidase glucose method) and in the 1st, 3rd, 6th, 12th, and 24th hours after birth (newborn by a test strip method). The evolution of glycemia was analyzed in each group (Friedman test). The groups were compared regarding glycemia (Kruskall-Wallis test), and linear regression models were constructed for the analyses (independent variable = maternal glycemia; dependent variables = umbilical cord, 3rd, and 6th hour glycemia). RESULTS: There were no statistically significant differences among the mean blood glucose levels of the 3 groups in any of the assessments. There was a correlation between maternal and umbilical cord blood glucose in the isradipine (r = 0.61; P <.05) and control (r = 0.84; P <.05) groups. Regarding glycemia levels of the mothers and newborns in the third and sixthhours postpartum, this correlation was present only in the control group (maternal x third hour: r = 0.65; P <.05; maternal x sixth hour: r = 0.68; P <.05). There were no correlations in the atenolol group. Hypoglycemia was detected in 51.3% of the isradipine group, 60% of the atenolol group, and 35.7% of the control group, and it was more frequent in the first hour postpartum in all groups. CONCLUSIONS: The results suggest a similar effect of the 3 types of treatment upon newborn glycemia. The correlation analysis suggests that isradipine could have effects upon newborn glycemia only after birth (correlation only in umbilical cord blood), whereas atenolol could act earlier (there was no correlation at any moment). The results also point to the need for glycemic control from the first hour postpartum of newborns of hypertensive mothers whether they have or have not undergone treatment with antihypertensive drugs.

The Great Lakes lie within a region of East Africa with very high human genetic diversity, home of many ethno-linguistic groups usually assumed to be the product of a small number of major dispersals. However, our knowledge of these dispersals relies primarily on the inferences of historical, linguistics and oral traditions, with attempts to match up the archaeological evidence where possible. This is an obvious area to which archaeogenetics can contribute, yet Uganda, at the heart of these developments, has not been studied for mitochondrial DNA (mtDNA) variation. Here, we compare mtDNA lineages at this putative genetic crossroads across 409 representatives of the major language groups: Bantu speakers and Eastern and Western Nilotic speakers. We show that Uganda harbours one of the highest mtDNA diversities within and between linguistic groups, with the various groups significantly differentiated from each other. Despite an inferred linguistic origin in South Sudan, the data from the two Nilotic-speaking groups point to a much more complex history, involving not only possible dispersals from Sudan and the Horn but also large-scale assimilation of autochthonous lineages within East Africa and even Uganda itself. The Eastern Nilotic group also carries signals characteristic of West-Central Africa, primarily due to Bantu influence, whereas a much stronger signal in the Western Nilotic group suggests direct West-Central African ancestry. Bantu speakers share lineages with both Nilotic groups, and also harbour East African lineages not found in Western Nilotic speakers, likely due to assimilating indigenous populations since arriving in the region ~3000 years ago.

Background: Research has separately indicated associations between pregnancy depression and breastfeeding, breastfeeding and postpartum depression, and pregnancy and postpartum depression. This paper aimed to provide a systematic literature review on breastfeeding and depression, considering both pregnancy and postpartum depression. Methods: An electronic search in three databases was performed using the keywords: “breast feeding”, “bottle feeding”, “depression”, “pregnancy”, and “postpartum”. Two investigators independently evaluated the titles and abstracts in a first stage and the full-text in a second stage review. Papers not addressing the association among breastfeeding and pregnancy or postpartum depression, non-original research and research focused on the effect of antidepressants were excluded. 48 studies were selected and included. Data were independently extracted. Results: Pregnancy depression predicts a shorter breastfeeding duration, but not breastfeeding intention or initiation. Breastfeeding duration is associated with postpartum depression in almost all studies. Postpartum depression predicts and is predicted by breastfeeding cessation in several studies. Pregnancy and postpartum depression are associated with shorter breastfeeding duration. Breastfeeding may mediate the association between pregnancy and postpartum depression. Pregnancy depression predicts shorter breastfeeding duration and that may increase depressive symptoms during postpartum. Limitations: The selected keywords may have led to the exclusion of relevant references. Conclusions: Although strong empirical evidence regarding the associations among breastfeeding and pregnancy or postpartum depression was separately provided, further research, such as prospective studies, is needed to clarify the association among these three variables. Help for depressed pregnant women should be delivered to enhance both breastfeeding and postpartum psychological adjustment.
