A szerző az Európai Unióhoz az elmúlt évtizedben csatlakozott országok felzárkózásának összefüggéseit vizsgálja, rámutatva arra, hogy a kelet-közép-európai térség országainak természetes modernizációs központja az Európai Unió. Az európai integrációba történő szerves beépülésnek nem volt és a jövőben sincs reális alternatívája. Annak ellenére, hogy a nemzetközi pénzügyi és gazdasági válság rendkívül kedvezőtlen fordulatot hozott, az új tagországok többségében jelentős ütemű felzárkózási folyamat indult el: egy évtizeden belül relatív pozíciójuk az uniós átlaghoz képest átlagosan 15 százalékpont javult a vásárlóerő-paritáson mért egy főre jutó GDP-t tekintve. A tagság számos előnye közül ki kell emelni, hogy az uniós költségvetésből származó közvetlen transzferek hatására az új tagállamok számottevő külső forráshoz jutottak, aminek révén hosszú távú fejlődésüket és versenyképességüket befolyásoló területeken jelentős beruházásokat hajthattak végre. E transzferek kapcsán a szerző felhívja a figyelmet arra, hogy a szűk látókörűen számított nettó haszonélvezői pozíció valójában kölcsönös előnyöket rejt: az EU fejlettebb országai számára is komoly előnyök származnak a megnövekedett importkeresletből és általában a bővülés kereskedelemteremtő hatásaiból. _____ The author analyses some catch-up aspects of countries joining the EU in 2004 and 2007, pointing out that the EU is an obvious centre of modernization for the countries in East Central Europe. There was no realistic alternative to participating in the European integration process and this applies also to the future. Contrary to the extremely bad general environment caused by the international financial and economic crisis, most new member-states were able to converge on the EU average quite fast: within a decade the relative regional level of development measured in GDP per capita terms and compared with the EU average increased by 15 per cent. It should be stressed that among several advantages of EU membership, direct transfers from the EU-level budget played a crucial role in improving competitiveness through investment. Looking beyond narrow-minded net budgetary positions, the author sees mutual comprehensive benefits: the more developed member-states can enjoy additional demand for their products, and in general benefit from the trade-creating effects of enlargement.

Background:

Knowing the scope of neurosurgical disease at Mbarara Hospital is critical for infrastructure planning, education and training. In this study, we aim to evaluate the neurosurgical outcomes and identify predictors of mortality in order to potentiate platforms for more effective interventions and inform future research efforts at Mbarara Hospital.

Methods:

This is retrospective chart review including patients of all ages with a neurosurgical disease or injury presenting to Mbarara Regional Referral Hospital (MRRH) between January 2012 to September 2015. Descriptive statistics were presented. A univariate analysis was used to obtain the odds ratios of mortality and 95% confidence intervals. Predictors of mortality were determined using multivariate logistic regression model.

Results:

A total of 1876 charts were reviewed. Of these, 1854 (had complete data and were?) were included in the analysis. The overall mortality rate was 12.75%; the mortality rates among all persons who underwent a neurosurgical procedure was 9.72%, and was 13.68% among those who did not undergo a neurosurgical procedure. Over 50% of patients were between 19 and 40 years old and the majority of were males (76.10%). The overall median length of stay was 5 days. Of all neurosurgical admissions, 87% were trauma patients. In comparison to mild head injury, closed head injury and intracranial hematoma patients were 5 (95% CI: 3.77, 8.26) and 2.5 times (95% CI: 1.64,3.98) more likely to die respectively. Procedure and diagnostic imaging were independent negative predictors of mortality (P <0.05). While age, ICU admission, admission GCS were positive predictors of mortality (P <0.05).

Conclusions:

The majority of hospital admissions were TBI patients, with RTIs being the most common mechanism of injury. Age, ICU admission, admission GCS, diagnostic imaging and undergoing surgery were independent predictors of mortality. Going forward, further exploration of patient characteristics is necessary to fully describe mortality outcomes and implement resource appropriate interventions that ultimately improve morbidity and mortality.

Archaeozoological mortality profiles have been used to infer site-specific subsistence strategies. There is however no common agreement on the best way to present these profiles and confidence intervals around age class proportions. In order to deal with these issues, we propose the use of the Dirichlet distribution and present a new approach to perform age-at-death multivariate graphical comparisons. We demonstrate the efficiency of this approach using domestic sheep/goat dental remains from 10 Cardial sites (Early Neolithic) located in South France and the Iberian Peninsula. We show that the Dirichlet distribution in age-at-death analysis can be used: (i) to generate Bayesian credible intervals around each age class of a mortality profile, even when not all age classes are observed; and (ii) to create 95% kernel density contours around each age-at-death frequency distribution when multiple sites are compared using correspondence analysis. The statistical procedure we present is applicable to the analysis of any categorical count data and particularly well-suited to archaeological data (e.g. potsherds, arrow heads) where sample sizes are typically small.

Soil erosion data in El Salvador Republic are scarce and there is no rainfall erosivity map for this region. Considering that rainfall erosivity is an important guide for planning soil erosion control practices, a spatial assessment of indices for characterizing the erosive force of rainfall in El Salvador Republic was carried out. Using pluviometric records from 25 weather stations, we applied two methods: erosivity index equation and the Fournier index. In all study area, the rainiest period is from May to November. Annual values of erosivity index ranged from 7,196 to 17,856 MJ mm ha(-1) h(-1) year(-1) and the Fournier index ranged from 52.9 to 110.0 mm. The erosivity map showed that the study area can be broadly divided into three major erosion risk zones, and the Fournier index map was divided into four zones. Both methods revealed that the erosive force is severe in all study area and presented significant spatial correlation with each other. The erosive force in the country is concentrated mainly from May to November.

Hepatocyte growth factor (HGF) plays a role in the improvement of cardiac function and remodeling. Their serum levels are strongly related with mortality in chronic systolic heart failure (HF). The aim of this study was to study prognostic value of HGF in acute HF, interaction with ejection fraction, renal function, and natriuretic peptides. We included 373 patients (age 76 ± 10 years, left ventricular ejection fraction [LVEF] 46 ± 14%, 48% men) consecutively admitted for acute HF. Blood samples were obtained at admission. All patients were followed up until death or close of study (>1 year, median 371 days). HGF concentrations were determined using a commercial enzyme-linked immunosorbent assay (human HGF immunoassay). The predictive power of HGF was estimated by Cox regression with calculation of Harrell C-statistic. HGF had a median of 1,942 pg/ml (interquartile rank 1,354). According to HGF quartiles, mortality rates (per 1,000 patients/year) were 98, 183, 375, and 393, respectively (p <0.001). In Cox regression analysis, HGF (hazard ratio1SD = 1.5, 95% confidence interval 1.1 to 2.1, p = 0.002) and N-terminal pro b-type natriuretic peptide (NT-proBNP; hazard ratio1SD = 1.8, 95% confidence interval 1.2 to 2.6, p = 0.002) were independent predictors of mortality. Interaction between HGF and LVEF, origin, and renal function was nonsignificant. The addition of HGF improved the predictive ability of the models (C-statistic 0.768 vs 0.741, p = 0.016). HGF showed a complementary value over NT-proBNP (p = 0.001): mortality rate was 490 with both above the median versus 72 with both below. In conclusion, in patients with acute HF, serum HGF concentrations are elevated and identify patients at higher risk of mortality, regardless of LVEF, ischemic origin, or renal function. HGF had independent and additive information over NT-proBNP.

To analyze mortality from all causes in Mexico during the winter months. Methods: Data was extracted and tabulated on monthly all-cause mortality in the general population from 2000 to 2012 from the INEGI database. Coeficients of seasonal variation in mortality were calculated. Results: In Mexico there was an increase of 14 % in mortality in the general population during winter. The more susceptible age groups were older people and children, with increases of 18% and 13% respectively. The months with low levels of mortality were April, October and September for children, adolescents and older people respectively. Conclusions: Important increases in winter mortality occur in Mexico.

The aims of the present study were to determine the effect of firefighter's boots on the vertical component of the ground reaction force (GRF) at heel strike, also known as heel strike transient and to analyze the effect of the viscoelastic insoles placed into the firefighter’s boots on this force during the gait. The magnitude of the impact force (FZI) from the vertical ground reaction force, the time to the production of this force (TZI) and the loading rate (GC) were registered. 39 firefighters without any pathology during 2 years before the study were recruited. Three different walking conditions were tested: 1) gait with firefighter's boots, 2) gait with firefighter's boots and viscoelastic insoles and 3) gait with sport shoes. The results showed a higher production and magnitude of the impact force during gait with firefighter's boots than during gait with sport shoes (13,1 vs. 2,6 % of occurrence of the impact force and 61,39 ± 35,18 %BW (body weight) vs. 49,38 ± 22,99 %BW, respectively). The gait with viscoelastic insoles placed into the firefighter's boots did not show significant differences in any of the parameters characterizing the impact force compared to the gait without insoles. The results of this study show a lower cushioning of the impact force during the gait with firefighter's boots in comparison to the gait with sport shoes and the inefficiency of the viscoelastic insoles placed inside the firefighter's boots to ameliorate the cushioning of the impact force at natural walking speed.

In this essay, the authors respond to several of the papers included in this special issue. First reflecting on the relation between waters, ‘First law’,1 and settler law, the authors then draw connections between some of the contributions to the issue. Water, the authors contend, is a productive site for thinking through the organs and processes of settler law, though such attention, they argue, also reveals how the ‘constitutional’ question of waters is occluded by the presence and dominance of settler law. The final section turns to Aotearoa/New Zealand as a negative example of this situation, one in which the constituting force of waters is nullified by the incorporation of indigenous politics within the processes and institutions of the settler legal order.

The notion of agency is being used with increasing frequency in early childhood policies, replacing traditional assumptions about young children’s immaturity and their role as mere recipients of adults’ arrangements. Agency is thus both an educational aspiration as well as a signifier of a strong rights-based political commitment to countering views of children as immature and incompetent. This article develops the argument that agency is inherently a sociocultural product that is driven by children’s clear attempts to bond with others and to develop a sense of belonging. Using examples of the everyday experiences of two Chinese immigrant children in an early childhood centre, the article considers ways in which agency was exercised by the children in an unfamiliar sociocultural setting because they wanted to belong. Some crucial issues are highlighted for practice and policy development in the area of immigrant children’s education, arguing that the shaping of early childhood education requires an attention to children’s ‘invisible’ capabilities, needs to belong and ‘small’ everyday life realities.
