Objective: To describe the prevalence and demographic, clinical and functional correlates of childhood trauma in patients attending early psychosis clinics. Method: Participants were recruited from outpatients attending four early psychosis services. Exposure to childhood trauma was assessed using the Childhood Trauma Questionnaire (CTQ). Psychopathology was measured using the Positive and Negative Syndrome Scale and the Depression, Anxiety and Stress Scale. Social and vocational functioning and substance use were also assessed. Results: Over three-quarters of the 100 patients reported exposure to any childhood trauma. Emotional, physical and sexual abuse were reported by 54%, 23% and 28% of patients, respectively, while 49% and 42% of patients reported emotional and physical neglect, respectively. Female participants were significantly more likely to be exposed to emotional and sexual abuse. Exposure to childhood trauma was correlated with positive psychotic symptoms and higher levels of depressive, anxiety and stress symptoms; however, it had no impact on social or vocational functioning or recent substance use. Conclusion: Exposure to childhood trauma was common in patients with early psychosis, and associated with increased symptomatology. Existing recommendations that standard clinical assessment of patients with early psychosis should include inquiry into exposure to childhood trauma are supported.

La schizophrénie découle d’une modification du fonctionnement du cerveau et entraîne divers symptômes qui ont pour conséquence une altération de la perception de la réalité, des troubles cognitifs, et des dysfonctionnements sociaux et comportementaux. En plus des observations cliniques de jour, la schizophrénie montre également des signes nocturnes qui peuvent se manifester durant le rêve. Cette thèse vise à caractériser les rêves dans la schizophrénie et cherche à explorer les relations qui existent entre les caractéristiques du contenu onirique des personnes atteintes de schizophrénie et les symptômes de cette maladie. Pour ce faire, nous avons comparé diverses mesures de l’activité onirique recueillies auprès de 14 participants atteints de schizophrénie traités sous antipsychotiques atypiques et 15 participants témoins par le biais de questionnaires et de collectes de rêves en laboratoire à la suite d’éveils provoqués en sommeil paradoxal (SP) et en sommeil lent (SL). Les résultats obtenus au questionnaire révèlent que les participants atteints de schizophrénie rapportent un nombre de cauchemars plus élevé comparativement aux participants témoins. Les collectes en laboratoire démontrent une fréquence de rappel de rêves équivalente au sein des deux groupes de participants, indépendamment du stade de sommeil durant lequel elles sont effectuées. Les récits de rêves du SL des deux groupes de participants sont généralement plus courts et comprennent un nombre moins élevé d’items quantifiables comparativement à ceux du SP. Les récits de rêves recueillis en SP et en SL chez les participants atteints de schizophrénie s’avèrent plus courts que ceux des participants témoins et, lorsque le nombre de mots est pondéré, la plupart des différences observées dans le contenu de rêve entre les deux groupes tendent à disparaître. En comparaison aux participants témoins, ceux atteints de schizophrénie évaluent leurs rêves comme étant moins bizarres, en dépit d’un nombre équivalent d’éléments bizarres dans leurs récits. Finalement, bien qu’il n’y ait pas de différence dans la densité des mouvements oculaires rapides (MORs) entre les deux groupes de participants, seuls les participants témoins montrent une corrélation positive entre les MORs et certaines variables du contenu onirique. Les résultats de la présente thèse suggèrent que les caractéristiques du contenu onirique des personnes atteintes de schizophrénie peuvent refléter certaines des manifestations psychopathologiques de cette maladie.

De nos jours, l’idée selon laquelle bon nombre d’individus atteints de psychose peuvent se rétablir sensiblement au fil du temps est de plus en plus répandue. Alors que le milieu médical associe le rétablissement à la rémission symptomatique, les usagers de services en santé mentale le voient plutôt comme un processus où l’absence de symptômes n’est pas une condition sine qua non. Ne s’opposant à ni l’un ni l’autre de ces points de vue, le traitement actuel des psychoses vise le retour à un fonctionnement social approprié. Chez les jeunes atteints d’un premier épisode psychotique, une diminution marquée des symptômes après un traitement pharmacologique se traduit rarement en une amélioration significative du fonctionnement, ce qui justifie la mise en place d’interventions psychosociales. On connait aujourd’hui quelques variables associées au fonctionnement social chez cette population, mais celui-ci a peu été étudié de façon spécifique. De même, on connait mal la manière dont ces jeunes vivent leur rétablissement en tant que processus. Cette thèse porte donc sur le rétablissement et le fonctionnement social de jeunes en début de psychose. La première partie consiste en une introduction aux concepts de rétablissement et de réadaptation, et en une revue de la littérature des interventions psychosociales pouvant favoriser leurs rétablissement et fonctionnement. Plus spécifiquement, nous illustrons comment ces interventions peuvent s’appliquer aux adolescents, clientèle sur laquelle peu de littérature existe actuellement. La deuxième partie présente les résultats d’une étude sur la valeur prédictive de variables sur trois domaines du fonctionnement social de 88 jeunes adultes en début de psychose. Des équations de régression multiple ont révélé qu’un niveau plus élevé de dépression était associé à une vie sociale moins active, qu’un moins bon apprentissage verbal à court terme était associé à un plus bas fonctionnement vocationnel, que le fait d’être de sexe masculin était négativement associé aux habiletés de vie autonome, et qu’un niveau plus élevé de symptômes négatifs prédisait de moins bonnes performances dans les trois sphères du fonctionnement. Finalement, la troisième partie s’intéresse au processus de rétablissement en début de psychose par l’analyse de récits narratifs selon un modèle en cinq stades. Nous avons découvert que l’ensemble de notre échantillon de 47 participants se situaient dans les deux premiers stades de rétablissement lors du premier temps de mesure, et que le fait de se trouver au deuxième stade était associé à de meilleurs scores d’engagement social et de fonctionnement occupationnel, à un meilleur développement narratif, à moins de symptômes négatifs et positifs, et à plus d’années de scolarité. Par ailleurs, l’examen des stades à deux autres temps de mesure a révélé des profils relativement stables sur une période de neuf mois. En somme, les résultats démontrent la nécessité d’évaluer le fonctionnement social de façon plus spécifique et l’importance d’offrir des interventions psychosociales en début de psychose visant notamment le développement des relations et l’intégration scolaire/professionnelle, ou visant à améliorer le faible développement narratif. Cette thèse est, de plus, un premier pas dans l’étude du processus de rétablissement des jeunes atteints de psychose.

Various trials have been conducted evaluating depression management programs for patients with Coronary Heart Disease (CHD). However, to date, the most effective way to manage this co-morbidity in the real world setting remains unclear. To better understand the past successes and failures of previous trials and subsequently develop suitable interventions that target key components of health related quality of life (HRQOL) such as mental, physical and vocational functioning, we first need to understand the mechanisms underpinning the relationship between the two conditions. This paper will draw on the key literature in this field as identified by psychiatric, medical and social sciences databases (Cochrane Central Register of Controlled Trials, PubMed, OVID, Medline) available up to January 2012, with the aim to conduct a narrative review which explores: the aetiological relationship between depression and CHD; its association with HRQOL; the relationship between CHD, depression and vocational functioning; and the impact of depression treatment on these outcomes. Key recommendations are made regarding the management of this prevalent co-morbidity in clinical settings.

We surveyed 506 Australian high school students on career development (exploration, planning, job-knowledge, decision-making, indecision), personal functioning (well-being, self-esteem, life satisfaction, school satisfaction) and control variables (parents’ education, school achievement), and tested differences among work-bound, college-bound and university-bound students. The work-bound students had the poorest career development and personal functioning, the university-bound students the highest, with the college-bound students falling in-between the other two groups. Work-bound students did poorest, even after controlling for parental education and school achievement. The results suggest a relationship between career development and personal functioning in high school students.

An increased emphasis on community-based care has not ensured that people recovering from psychiatric disorders return to active and valued roles in their local communities. Although clinical recovery remains a priority for mental health services there is increasing recognition of the need for functional recovery to be attained and demonstrated in roles valued by the wider community. With this need in mind, a method for classifying socially-valued role functioning among people with schizophrenia or schizoaffective disorder was developed and trialed. Participants (n = 104) were recruited via mental health, psychosocial rehabilitation, and other community support services. Socially-valued roles were investigated via participation in five categories: (1) self-care and home duties; (2) caring for others; (3) self-development, voluntary work or rehabilitation; (4) formal education or training; and (5) employment. Activities were classified by primary role type and role status level at baseline, six, and 12 months. Current role status was assessed along with highest and lowest status in the previous year. Preliminary psychometric results were favorable. Research applications are now recommended for monitoring socially-valued role functioning in community settings.

This paper focuses on the determinants of the labor market situation of young people in developed countries and the developing world, with a particular emphasis on the role of vocational training and education policies. We highlight the role of demographic factors, economic growth and labor market institutions in explaining young people's transition into work. Subsequently, we assess differences between the setup and functioning of the vocational education and training policies across major world regions as an important driver of differential labor market situation of youth. Based on our analysis, we argue in favor of vocational education and training systems combining work experience and general education and provide some policy recommendations regarding the implementation of education and training systems adapted to a country's economic and institutional context.

As editors of the recently published Vocational psychological and organisational perspectives on career: Towards a multidisciplinary dialogue (Collin & Patton, 2009), we have considerable interest in this particular issue of the Australian Journal of Career Development. This short piece will first present the purpose and thesis of that book and, in the light of them, will then comment on the four papers. The book suggests that to understand the multidimensional and multilayered nature of career, “it has to be studied in a similarly multilayered and multi-perspectival way, and, indeed, it has been” (p. 3). Scholars have pointed out that there is a wide array of disciplines including economics, sociology, anthropology, geography, political science, various branches of psychology (e.g. industrial/organisational (I/O), vocational, counselling), psychiatry, education, organisation studies, organisational behaviour, personnel/human resource management, industrial relations, and more, all of which have something to say about career. Of these, the most influential, according to Peiperl and Arthur (2000), have been psychology, sociology, education and management. These many disciplinary perspectives on career constitute the rich field of career studies.

The course evaluation process used by a large VET provider was evaluated using guidelines suggested by the course evaluation literature and feedback obtained from multiple stakeholders. A modified model is presented as an exemplar for course evaluation in the VET sector.

It has long been lamented that, although several disciplines contribute to career scholarship, they work in isolation from one another, thus denying career theory, research, and practice the benefits that multidisciplinary collaboration would bring. This constitutes a lost opportunity at a time when new understandings and approaches are needed in order to respond effectively to global changes in society and work. This book takes a major step towards remedying this situation by bringing together two key perspectives on career, the vocational psychological and the organisational (interpreted broadly to include organisation behaviour and human resource management). Written by international experts, the book opens by identifying some of the “tributaries” that flow into the “great delta of careers scholarship”, and noting the need to link what are at present separate “islands” of scholarship. It is structured to allow comparison between the ways in which the two perspectives address career development and career management theory, research and interventions. It concludes by pointing to the possibilities for dialogue, and even collaboration, between these perspectives, and suggesting ways in which these could be brought about. The book will be essential reading for career scholars because, with its potential to stimulate new thinking and developments in theory and research and also, importantly, in practice (with beneficial spin-offs for policy-makers), this dialogue could open a new phase in career scholarship. With its overviews of the history, theory, research and practice of both perspectives, the book will also be a valuable resource for students of both perspectives.

Vocational education and training for the library and information services (LIS) sector in Australia offers students the career pathway to become library technicians. Library technicians play a valuable role in drawing on sound practical knowledge and skills to support the delivery of library and information services that meet client needs. Over the past forty years, the Australian Library and Information Association (ALIA) has monitored the quality of library technician courses. Since 2005, ALIA has run national professional development days for library technician educators with the goal of establishing an alternative model for course recognition focusing on the process of peer review to benchmark good practice and stimulate continuous improvement in library technician education. This initial developmental work has culminated in 2009 with site visits to all library technician courses in Australia. The paper presents a whole-of-industry case study to critically review the work undertaken to date.

Behavioral and cognitive interventions for people with psychosis have a long and distinguished history, although the evidence for their application to young people remains limited. We anticipate that the next decades will show substantial research into psychological intervention for this population. Important targets will include the management of environmental stressors, reduction of substance misuse, and promotion of early treatment. Psychological management of positive symptoms, depression, and suicidal behavior will continue to be critical objectives. Important secondary prevention goals will be the retention of cognitive functioning, vocational options, social skills, and social network support, including appropriate family support. We expect primary prevention to include both universal programs and interventions for adolescents at particularly high risk. Technical innovations will include increasing use of Internet-based intervention and behavior cueing devices. Pressures for intervention brevity will continue, as will problems with the systematic delivery of effective procedures.
