Leptospirosis is a zoonotic disease caused by pathogenic spirochetes of theLeptospira genus. Vaccination with bacterins has severe limitations. Here, we evaluated the N-terminal region of the leptospiral immunoglobulin-like B protein (LigBrep) as a vaccine candidate against leptospirosis using immunisation strategies based on DNA prime-protein boost, DNA vaccine, and subunit vaccine. Upon challenge with a virulent strain ofLeptospira interrogans, the prime-boost and DNA vaccine approaches induced significant protection in hamsters, as well as a specific IgG antibody response and sterilising immunity. Although vaccination with recombinant fragment of LigBrep also produced a strong antibody response, it was not immunoprotective. These results highlight the potential of LigBrep as a candidate antigen for an effective vaccine against leptospirosis and emphasise the use of the DNA prime-protein boost as an important strategy for vaccine development.

OBJECTIVE To analyze the regional governance of the health systemin relation to management strategies and disputes.METHODOLOGICAL PROCEDURES A qualitative study with health managers from 19 municipalities in the health region of Bahia, Northeastern Brazil. Data were drawn from 17 semi-structured interviews of state, regional, and municipal health policymakers and managers; a focus group; observations of the regional interagency committee; and documents in 2012. The political-institutional and the organizational components were analyzed in the light of dialectical hermeneutics.RESULTS The regional interagency committee is the chief regional governance strategy/component and functions as a strategic tool for strengthening governance. It brings together a diversity of members responsible for decision making in the healthcare territories, who need to negotiate the allocation of funding and the distribution of facilities for common use in the region. The high turnover of health secretaries, their lack of autonomy from the local executive decisions, inadequate technical training to exercise their function, and the influence of party politics on decision making stand as obstacles to the regional interagency committee’s permeability to social demands. Funding is insufficient to enable the fulfillment of the officially integrated agreed-upon program or to boost public supply by the system, requiring that public managers procure services from the private market at values higher than the national health service price schedule (Brazilian Unified Health System Table). The study determined that “facilitators” under contract to health departments accelerated access to specialized (diagnostic, therapeutic and/or surgical) services in other municipalities by direct payment to physicians for procedure costs already covered by the Brazilian Unified Health System.CONCLUSIONS The characteristics identified a regionalized system with a conflictive pattern of governance and intermediate institutionalism. The regional interagency committee’s managerial routine needs to incorporate more democratic devices for connecting with educational institutions, devices that are more permeable to social demands relating to regional policy making.

OBJECTIVE To analyze the characteristics of health diagnosis according to the ecohealth approach in rural and urban communities in Mexico.METHODS Health diagnosis were conducted in La Nopalera, from December 2007 to October 2008, and in Atlihuayan, from December 2010 to October 2011. The research was based on three principles of the ecohealth approach: transdisciplinarity, community participation, gender and equity. To collect information, a joint methodology and several techniques were used to stimulate the participation of inhabitants. The diagnostic exercise was carried out in five phases that went from collecting information to prioritization of problems.RESULTS The constitution of the transdisciplinary team, as well as the participation of the population and the principle of gender/equity were differentials between the communities. In the rural community, the active participation of inhabitants and authorities was achieved and the principles of transdisciplinarity and gender/equity were incorporated.CONCLUSIONS With all the difficulties that entails the boost in participation, the incorporation of gender/equity and transdisciplinarity in health diagnosis allowed a holistic public health approach closer to the needs of the population.

Vaccination of infants with conjugated Haemophilus influenzae type b (Hib) vaccines has been proven to reduce Hib meningitis by 95% and pneumoniae by 20%. The routine use of Hib vaccine is facilitated by the introduction of combination vaccines into the EPI (Expanded Plan of Immunization). The objective of this study was to compare the immunogenicity and reactogenicity of an extemporaneously mixed DTPw/Hib (diphtheria-tetanus-whole cell pertussis) combination, using the technology of two Brazilian manufacturers, against a licensed DTPw/Hib European combination in 108 infants vaccinated at 2, 4 and 6 months according to the local national schedule. The Brazilian combination was highly immunogenic with Hib seroprotection rates (anti-PRP > 0.15 mg /ml of 98% after 2 doses and 100% after 3). Also for tetanus and pertussis the new Brazilian combination was as immunogenic as the European counterpart, except the diphtheria seroprotection rates and titers were lower. There was also no clinically relevant difference in reactogenicity. If these feasibility results are confirmed, the Brazilian DTPw/Hib combination should help to boost the uptake of Hib vaccination in Brazil.

Realizamos uma revisão com agregação de resultados dos ensaios randomizados que compararam intervenção coronariana percutânea (ICP) com cirurgia de revascularização miocárdica (CRM). Os 25 ensaios selecionados envolveram 12.305 pacientes dos quais 11.103 pertenciam a estudos em multiarteriais e 1.212 pertenciam a estudos em lesão única de descendente anterior (DA). Nos estudos em multiarteriais a ICP mostrou uma tendência a menor mortalidade precoce (1,2% versus 2%) e menor incidência de acidente vascular cerebral (AVC): 0,7% versus 1,65%. Não houve diferença na mortalidade intermediária (3,8% versus 3,8%). Houve tendência à superioridade da CRM na mortalidade tardia (10,5% versus 9,6%). A diferença deveu-se exclusivamente aos estudos da era balão, tendendo a inverter-se na era stent (9,6% versus 9,9%). Nos estudos de lesão única de DA não houve diferença significativa em nenhum desfecho. A agregação de resultados de nove estudos que avaliaram a mortalidade tardia em diabéticos mostrou diferença favorável à cirurgia (21,3% versus 15,9%). Dois estudos que avaliaram lesão de tronco não mostraram diferença significativa na mortalidade em um ano (3,9% versus 4,7%). A incidência de nova revascularização foi consistentemente maior na ICP, apesar de progressiva melhora dos resultados na era stent.

Informações sobre o comportamento dos Kerteszia obtidas nos Estados de São Paulo, Paraná e Santa Catarina, foram comparadas entre si e com dados relativos a outros anofelinos neotrópicos e etiópicos. Das conclusões obtidas destacam-se: 1. Em seguida ao pôr do sol os Kerteszia tornam-se mais ativos nas copas das árvores da mata a ao anoitecer, quando começam a se inverter as diferenças microclimáticas existentes entre a floresta e ao ar livre, passam a predominar nas áreas abertas; 2. nas localidades não dedetizadas não existe grande diferença entre a proporção dos Kerteszia que se alimenta dentro das casas e a que tem sido observada para o A. gambiae, na África. Entretanto, memso antes do aparecimento do DDT, os Kerteszia não eram mosquitos endófilos. Para os anofelinos ditos zoófilos, como o A. strodei, a relação entre o número de mosquitos capturados simultaneamente no peridomicílio e dentro de casa, é pelo menos duas vezes mais elevada; 3. Alguns dados sobre a ação impedidora ('deterrency") do DDT, para os kerteszia, mostram que ela é elevadíssima, outros são da mesma ordem de grandeza dos observados para o A. gambiae. Foi verificado que os Kerteszia evitam entrar mesmo em casas com muito pouco inseticida; 4. Apenas uma pequena proporção dos Kerteszia, que se alimenta dentro das casas, pousa nas paredes; a maioria voa diretamente para as pessoas e depois para fora. O tempo de permanência nas superfícies dedetizadas é o mesmo que tem sido observado para outros anofelinos, geralmente inferior a 10 minutos; 5. Ao contrário da maioria dos transmissores de malária a atividade dos Kerteszia, fora da mata, concentra-se nas primeiras horas da noite, quando há maior probabilidade de existir pessoas fora de casa; 6. o estudo do contato homem-mosquito ("man-bitting rate") mostrou que, mesmo nas casas não dedetizadas, esse contato é quase sempre maior fora dos domicilios. Nas localidades dedetizadas a componente externa, desse contato, pode ser até 10 vezes mais elevada. O que, logicamente, resulta do horário de atividade; 7. Os trabalhos de profilaxia do antigo Serviços Nacional de Malária já haviam mostrado que, mesmo com distribuição de medicamentos, essa diminuição dos contatos homem-mosquito dentro das casas e a ação tóxica do inseticida, são suficientes para impedir a transmissão da malária. Fato que, juntamente com os dados das capturas extradomiciliares, sugere a realização de pesquisas visando encontrar medidas capazes de diminuir a densidade dos kerteszia nas áreas freqüentadas pela população humana.

CD8+ T cells have been implicated as critical effector cells in protection against the pre-erythrocytic stage of malaria in mice and humans following irradiated sporozoite immunization. Immunization experiments in animal models by several investigators have suggested different strategies for vaccination against malaria and many of the targets from liver stage malaria antigens have been shown to be immunogenic and to protect mice from the sporozoite challenge. Several prime/boost protocols with replicating vectors, such as vaccinia/influenza, with non-replicating vectors, such as recombinant particles derived from yeast transposon (Ty-particles) and modified vaccinia virus Ankara, and DNA, significantly enhanced CD8+ T cell immunogenicity and also the protective efficacy against the circumsporosoite protein of Plasmodium berghei and P. yeti. Based on these experimental results the development of a CD8+ T cell inducing vaccine has moved forward from epitope identification to planning stages of safety and immunogenicity trials of candidate vaccines.

The human immunodeficiency virus/acquired immunodeficiency syndrome (HIV/AIDS) epidemic is of unprecedented gravity and is spreading rapidly, notably in the most disadvantaged regions of the world. The search for a preventive vaccine is thus an absolute priority. For over 10 years the French National Agency for AIDS research (ANRS) has been committed to an original program combining basic science and clinical research. The HIV preventive vaccine research program run by the ANRS covers upstream research for the definition of immunogens, animal models, and clinical research to evaluate candidate vaccines. Most researchers in 2004 believe that it should be possible to obtain partial vaccine protection through the induction of a strong and multiepitopic cellular response. Since 1992, the ANRS has set up 15 phases I and II clinical trials in order to evaluate the safety and the capacity of the candidate vaccines for inducing cellular immune responses. The tested candidate vaccines were increasingly complex recombinant canarypox viruses (Alvac) containing sequences coding for certain viral proteins, utilized alone or combined with other immunogens (whole or truncated envelope proteins). ANRS has also been developing an original strategy based on the utilization of lipopeptides. These comprise synthetic fragments of viral proteins associated with lipids that facilitate the induction of a cellular immune response. These approaches promptly allowed the assessment of a prime-boost strategy combining a viral vector and lipopeptides.

The lack of immunogenicity of most malaria antigens and the complex immune responses required for achieving protective immunity against this infectious disease have traditionally hampered the development of an efficient human malaria vaccine. The current boom in development of recombinant viral vectors and their use in prime-boost protocols that result in enhanced immune outcomes have increased the number of malaria vaccine candidates that access pre-clinical and clinical trials. In the frontline, adenoviruses and poxviruses seem to be giving the best immunization results in experimental animals and their mutual combination, or their combination with recombinant proteins (formulated in adjuvants and given in sequence or being given as protein/virus admixtures), has been shown to reach unprecedented levels of anti-malaria immunity that predictably will be somehow reproduced in the human setting. However, all this optimism was previously seen in the malaria vaccine development field without many real applicable results to date. We describe here the current state-of-the-art in the field of recombinant adenovirus research for malaria vaccine development, in particular referring to their use in combination with other immunogens in heterologous prime-boost protocols, while trying to simultaneously show our contributions and point of view on this subject.

A eletroquímica e a concentração de matéria orgânica nos horizontes superficiais de Latossolos correlacionam-se estreitamente. No presente trabalho, avaliaram-se as propriedades eletroquímicas do horizonte superficial de sete Latossolos brasileiros após a remoção das substâncias húmicas em meio alcalino. Os solos foram separados em dois grupos, segundo sua mineralogia, caulinítica e oxídica. O extrator NaOH 0,1 mol L-1 solubilizou substâncias húmicas correspondentes a menos de 3 % da MO total, indicando forte adsorção específica do material orgânico à fração mineral. A extração de ácidos húmicos e fúlvicos foi capaz de inverter o sinal de carga da superfície; no entanto, o efeito mais acentuado da retirada das substâncias húmicas solúveis foi a perda da capacidade-tampão das cargas do solo. A capacidade da dupla camada elétrica, uma medida indireta desse poder-tampão, aumentou de 3 a 15 vezes depois da extração das substâncias húmicas alcalino-solúveis. Solos com mineralogia predominantemente oxídica apresentaram maiores valores do ponto de efeito salino nulo, da capacidade da dupla camada elétrica, além da diminuição na capacidade de troca de cátions do que aqueles de natureza caulinítica.

Abstract Objective: To perform a comparative dosimetric analysis, based on computer simulations, of temporary balloon implants with 99mTc and balloon brachytherapy with high-dose-rate (HDR) 192Ir, as boosts to radiotherapy. We hypothesized that the two techniques would produce equivalent doses under pre-established conditions of activity and exposure time. Materials and Methods: Simulations of implants with 99mTc-filled and HDR 192Ir-filled balloons were performed with the Siscodes/MCNP5, modeling in voxels a magnetic resonance imaging set related to a young female. Spatial dose rate distributions were determined. In the dosimetric analysis of the protocols, the exposure time and the level of activity required were specified. Results: The 99mTc balloon presented a weighted dose rate in the tumor bed of 0.428 cGy.h-1.mCi-1 and 0.190 cGyh-1.mCi-1 at the balloon surface and at 8-10 mm from the surface, respectively, compared with 0.499 and 0.150 cGyh-1.mCi-1, respectively, for the HDR 192Ir balloon. An exposure time of 24 hours was required for the 99mTc balloon to produce a boost of 10.14 Gy with 1.0 Ci, whereas only 24 minutes with 10.0 Ci segments were required for the HDR 192Ir balloon to produce a boost of 5.14 Gy at the same reference point, or 10.28 Gy in two 24-minutes fractions. Conclusion: Temporary 99mTc balloon implantation is an attractive option for adjuvant radiotherapy in breast cancer, because of its availability, economic viability, and similar dosimetry in comparison with the use of HDR 192Ir balloon implantation, which is the current standard in clinical practice.

The natural rubber is a strategic material which can not be replaced by synthetic rubber in many technological applications. Brazil is a rubber importer, but new techniques of cultivation, breeding and diversification of producing species can reverse this situation. One of the best ways to add value to this commodity is nanotechnology. The production of nanocomposites is already a reality and shows that the sustainable use of this natural resource can lead to new products and boost the national agribusiness setting labor-qualified in the field.

Food production and preservation of the environment are among the challenges faced by contemporary society. In Brazil, as in most parts of the world, the possibility of increasing the agricultural area is limited by several factors. Thus, an increase in productivity through the application of innovative technologies is regarded as the best solution to overcome such a problem. For long, chemistry has contributed to agricultural innovations such as synthetic pesticides for pest management. However, due to the well-known adverse effects of these compounds, new "greener" strategies are being explored. Research in chemical ecology, in combination with other emerging sciences, is leading to the development of new technologies such as plant-based pesticides (biopesticides); synthetic pheromones and plant volatile organic compounds, both of them to manipulate insect behavior; chemical elicitors to boost plant resistance; and genetic engineering of plant varieties. In these, chemistry plays an important role in the identification and synthesis of functional compounds. These techniques may be incorporated in integrated pest management programs and may contribute to a sustainable agriculture in the future.

The preparation of enantiomerically pure or enriched substances is of fundamental importance to pharmaceutical, food, agrochemical, and cosmetics industries and involves a growing market of hundreds of billions of dollars. However, most chemical processes used for their production are not environmentally friendly because in most cases, stoichiometric amounts of chiral inductors are used and substantial waste is produced. In this context, asymmetric catalysis has emerged as an efficient tool for the synthesis of enantiomerically enriched compounds using chiral catalysts. More specifically, considering the current scenario in the Brazilian chemical industry, especially that of pharmaceuticals, the immediate prospect for the use of synthetic routes developed in Brazil in an enantioselective fashion or even the discovery of new drugs is practically null. Currently, the industrial production of drugs in Brazil is primarily focused on the production of generic drugs and is basically supported by imports of intermediates from China and India. In order to change this panorama and move forward toward the gradual incorporation of genuinely Brazilian synthetic routes, strong incentive policies, especially those related to continuous funding, will be needed. These incentives could be a breakthrough once we establish several research groups working in the area of organic synthesis and on the development and application of chiral organocatalysts and ligands in asymmetric catalysis, thus contributing to boost the development of the Brazilian chemical industry. Considering these circumstances, Brazil can benefit from this opportunity because we have a wide biodiversity and a large pool of natural resources that can be used as starting materials for the production of new chiral catalysts and are creating competence in asymmetric catalysis and related areas. This may decisively contribute to the growth of chemistry in our country.
