INTRODUCTION: A decline in hepatitis D virus (HDV) occurrence was described in Europe and Asia. We estimated HDV prevalence in the Brazilian Amazon following hepatitis B vaccination. METHODS: This is a cross-sectional survey of HDV measured by total antibodies to HDV (anti-HD T). RESULTS: HDV prevalence was 41.9% whiting HBsAg carries and was associated with age (PR = 1.96; 95% CI 1.12-3.42; p = 0.01), hepatitis B virus (HBV) infection (PR = 4.38; 95% CI 3.12-6.13; p < 0.001), and clinical hepatitis (PR =1.44; 95% CI 1.03-2.00; p = 0.03). Risk factors were related to HDV biology, clinical or demographic aspects such as underlying HBV infection, clinical hepatitis and age. CONCLUSIONS: Our study demonstrated that HDV infection continues to be an important health issue in the Brazilian Amazon and that the implementation of the HBV vaccination in rural Lábrea had little or no impact on the spread of HDV. This shows that HDV has not yet disappeared from HBV hyperendemic areas and reminding that it is far from being a vanishing disease in the Amazon basin.

The introduction, implementation, successes and failures of multidrug therapy (MDT) in all Hansen's disease endemic countries are discussed in this paper. The high efficacy of leprosy treatment with MDT and the global reduction of prevalence led the World Health Organization, in 1991, to establish the goal of elimination of Hansen's disease (less than 1 patient per 10,000 inhabitants) to be accomplished by the year 2000. Brazil, Nepal and East Timor are among the few countries that didn't reach the elimination goal by the year 2000 or even 2005. The implications of these aspects are highlighted in this paper. Current data from endemic and previously endemic countries that carry a regular leprosy control programme show that the important fall in prevalence was not followed by the reduction of the incidence. This means that transmission of Mycobacterium leprae is still an issue. It is reasonable to conclude that we are still far from the most important goal of Hansen's disease control: the interruption of transmission and reduction of incidence. It is necessary to emphasize to health managers the need of keeping Hansen's disease control activities to better develop control programmes in the future. The recent international proposal to interrupt the transmission of leprosy by the year 2020 seems to unrealistic and it is discussed in this paper. The possibility of epidemiological impact related to the human immunodeficiency virus/Hansen's disease coinfection is also considered.

Abstract Postharvest lettuce often lose water, thus affecting both its market value and consumer acceptance. However, the mechanism of the water-loss is still waiting well exploration. The aim of the present study was to investigate the effect of a foliar application of ABA on the fresh weight-loss and the chlorophyll content of postharvest lettuce as well as its association with the regulation of stomata. The present data demonstrated that exogenously application of ABA, in a concentration range of 0 to 100 µM, significantly lowered the fresh weight-loss of postharvest lettuce. ABA also delayed chlorophyll reduction during ambient storage, but this protective effect was ABA concentration-dependent. Among the tested ABA concentrations, 50 µM or lower ABA produced an inhibition effect on chlorophyll degradation in postharvest lettuce leaves. The results demonstrated that the exogenous ABA treatment can obviously reduce the transpiration rate of lettuce leaves by promoting the stomatal closure of postharvest lettuce, therefore eventually delay fresh weight-loss. The present study primarily showed that the application of exogenous ABA, which originated from a naturally-produced phytohormone, has a great potential in retaining the freshness of postharvest lettuce that is stored in an ambient condition, although possible practical application still need to be further evaluated.

Considerando-se o possível aproveitamento dos resíduos, provenientes do beneficiamento do camarão, realizouse, neste trabalho, a determinação da concentração de carotenoides totais dos resíduos do camarão Litopenaeus vannamei e da farinha obtida após secagem. As análises foram realizadas nos resíduos in natura e na farinha recémprocessada (dia 0) e aos 60, 120 e 180 dias de armazenamento sob congelamento. Os resíduos frescos apresentaram, no dia 0, teores de 42,74 µg/g de carotenoides totais e, a farinha recém-processada, de 98,51 µg/g. Após 180 dias de armazenamento, sob congelamento, os teores de carotenoides totais diminuíram significativamente, quando comparados com os do dia 0 (p < 0,05).

OBJETIVO: Descrever as características da mortalidade materna segundo Sistema de Informação sobre Mortalidade em relação a dados correspondentes a esses registros em outros sistemas. MÉTODOS: Estudo descritivo, com utilização dos dois sistemas de informações de dados vitais e do sistema hospitalar, para as 26 capitais estaduais e o Distrito Federal do Brasil, em 2002. Inicialmente foram calculadas as razões de mortalidade materna e obtidas informações das mortes maternas declaradas. A partir dessas mortes relacionou-se probabilisticamente o Sistema de Informação sobre Mortalidade com o Sistema de Informações sobre Nascidos Vivos e com o Sistema de Informações Hospitalares, utilizando-se o programa "Reclink II", com estratégia de blocagem em múltiplos passos. Para os registros pareados, foram detalhados os diagnósticos e procedimentos hospitalares aproximados pelos critérios mais conhecidos de morbidade materna grave. RESULTADOS: Foram registradas 339 mortes maternas em 2002, com razões de mortalidade materna oficial e ajustada, respectivamente, de 46,4 e 64,9 (mortes por 100.000 nascidos vivos). No relacionamento com os dados do sistema de nascidos vivos, foi possível localizar 46,5% das mortes maternas e, com o de informações hospitalares, localizaram-se 55,2% das mortes. O diagnóstico de internação mais freqüente foi o de infecção (13,9%), e o procedimento com maior percentagem (39,0%) foi o de admissão à UTI. CONCLUSÕES: Foi baixa a percentagem de relacionamento entre os registros das três fontes estudadas. Nenhuma das possíveis falhas e/ou impossibilidade de relacionamento apontadas, isoladamente ou em conjunto, podem explicar esse baixo percentual.

OBJECTIVE To compare the effectiveness of two speech therapy interventions, vocal warm-up and breathing training, focusing on teachers’ voice quality.METHODS A single-blind, randomized, parallel clinical trial was conducted. The research included 31 20 to 60-year old teachers from a public school in Salvador, BA, Northeasatern Brazil, with minimum workloads of 20 hours a week, who have or have not reported having vocal alterations. The exclusion criteria were the following: being a smoker, excessive alcohol consumption, receiving additional speech therapy assistance while taking part in the study, being affected by upper respiratory tract infections, professional use of the voice in another activity, neurological disorders, and history of cardiopulmonary pathologies. The subjects were distributed through simple randomization in groups vocal warm-up (n = 14) and breathing training (n = 17). The teachers’ voice quality was subjectively evaluated through the Voice Handicap Index (Índice de Desvantagem Vocal, in the Brazilian version) and computerized voice analysis (average fundamental frequency, jitter, shimmer, noise, and glottal-to-noise excitation ratio) by speech therapists.RESULTS Before the interventions, the groups were similar regarding sociodemographic characteristics, teaching activities, and vocal quality. The variations before and after the intervention in self-assessment and acoustic voice indicators have not significantly differed between the groups. In the comparison between groups before and after the six-week interventions, significant reductions in the Voice Handicap Index of subjects in both groups were observed, as wells as reduced average fundamental frequencies in the vocal warm-up group and increased shimmer in the breathing training group. Subjects from the vocal warm-up group reported speaking more easily and having their voices more improved in a general way as compared to the breathing training group.CONCLUSIONS Both interventions were similar regarding their effects on the teachers’ voice quality. However, each contribution has individually contributed to improve the teachers’ voice quality, especially the vocal warm-up.TRIAL RECORD NCT02102399, “Vocal Warm-up and Respiratory Muscle Training in Teachers”.

Systematic examination of the upper respiratory and digestive tracts (URDT) was performed in a group of 80 paracoccidioidomycosis (PCM) patients submitted to post-treatment follow-up ranging from 8 months to 17 years. Mucosae of the URDT had been involved prior to specific treatment in 74 patients, distributed as follows: oropharynx, 50 (41 alone, 7 in association with the larynx, and 2 with the nasal mucosa); larynx, 30 (23 alone and 7 in association); and nasal mucosa, 3(1 alone and 2 in association). Inactive lesions were observed in all the 50 patients with lesions of the oropharynx, 3 of whom with deforming scars (1 with retraction of the tongue and 2 with narrowing of the oral orifice). One case presented a destructive lesion, with perfuration of the palate. Of the other 46 cases, examination showed nacreous white striated scars which were nearly imperceptible in some cases and in others displayed partial retraction of anatomical structures without any alteration of their features. Patients presented a high rate of missing teeth. In 3 patients with involvement of the nasal mucosa, none of whom presented active PCM lesions, 2 still had nasal voices. In 30 patients with lesions of the larynx, 1 suffered a relapse of PCM and 2 developed epidermoid carcinoma. Of the other 27 cases, none of whom had active PCM lesions, 15 presented dysphonia, 3 were tracheotomized, and 9 were asymptomatic.

Com critérios previamente definidos de inclusão e prévio consentimento, 26 pacientes consecutivos (19 a 64 anos), com queixas referentes ao aparelho digestivo superior, foram submetidos à endoscopia digestiva, com biópsia, constando de 8 fragmentos da região antropilórica (4 da parede anterior e 4 da posterior). Dois fragmentos destinados â cultura; dois a teste da urease livre; dois para esfregaço corado, todos colhidos em meio de transporte adequado sob refrigeração; dois fragmentos foram imersos em formalina a 10 % para exame histopatológico. Dos 26pacientes, 25 (96%) apresentaram infecção pelo Helicobacter pylori por um ou mais dos métodos empregados. Em 16 (61%), dos 26, foram observadas alterações pela endoscopia (gastrite em 11, úlcera péptica em dois e cicatriz de úlcera em três casos). Dos pacientes com gastrite endoscópica, 10/11 (91%) apresentaram-se positivos, bem como todos (100%>) os portadores de cicatriz ou úlcera péptica. Foi observada estreita relação entre a presença de H. pylori e gastrite crônica em 24/25 (96%). Corte histológico corado pela hematoxilina-eosina foi o teste de maior sensibilidade diagnostica: 24/25 (96%), seguido pelo teste da urease 23/25 (92%), esfregaço corado 19/25 (76%) e cultura 18/25 (72%). Conclui-se que a prevalência de infecção gástrica por H. pylori em portadores de sintomas é elevada, correlacionando-se com gastrite crônica e úlcera. Exame histológico corado pela hematoxilina-eosina e o teste da urease são os mais sensíveis no diagnóstico da infecção. Os estudos devem prosseguir para elucidação de mais questões relacionadas á infecção, incluindo-se grupo controle de sintomáticos, por sexo e idade.

Introduction We hypothesized that nutritional deficiency would be common in a cohort of postpartum, human immunodeficiency virus (HIV)-infected women and their infants. Methods Weight and height, as well as blood concentrations of retinol, α-tocopherol, ferritin, hemoglobin, and zinc, were measured in mothers after delivery and in their infants at birth and at 6-12 weeks and six months of age. Retinol and α-tocopherol levels were quantified by high performance liquid chromatography, and zinc levels were measured by atomic absorption spectrophotometry. The maternal body mass index during pregnancy was adjusted for gestational age (adjBMI). Results Among the 97 women 19.6% were underweight. Laboratory abnormalities were most frequently observed for the hemoglobin (46.4%), zinc (41.1%), retinol (12.5%) and ferritin (6.5%) levels. Five percent of the women had mean corpuscular hemoglobin concentrations < 31g/dL. The most common deficiency in the infants was α-tocopherol (81%) at birth; however, only 18.5% of infants had deficient levels at six months of age. Large percentages of infants had zinc (36.8%) and retinol (29.5%) deficiencies at birth; however, these percentages decreased to 17.5% and 18.5%, respectively, by six months of age. No associations between infant micronutrient deficiencies and either the maternal adjBMI category or maternal micronutrient deficiencies were found. Conclusions Micronutrient deficiencies were common in HIV-infected women and their infants. Micronutrient deficiencies were less prevalent in the infants at six months of age. Neither underweight women nor their infants at birth were at increased risk for micronutrient deficiencies.

Este trabalho discute os efeitos das mudanças do uso do solo na biogequímica dos rios da bacia de drenagem do rio Ji-Paraná (Rondônia). Nesta região, a distribuição espacial do desmatamento e das propriedades do solo resultam em sinais diferentes, possibilitando a divisão dos sistemas fluviais em três grupos: rios com águas pobres em íons e baixo impacto; rios com conteúdo iônico intermediário e impacto médio e rios com elevados conteúdo iônico e impacto antropogênico. As características biogeoquímicas dos rios têm relação significativa com a área de pasto, melhor parâmetro para prever a condutividade elétrica (r² = 0,87) e as concentrações de sódio (r² = 0,75), cloreto (r² = 0,69), potássio (r² = 0,63), fosfato (r² = 0.78), nitrogênio inorgânico (r² = 0.52), carbono inorgânico (r² = 0.81) e carbono orgânico (rain ² = 0.51) dissolvidos. Cálcio e magnésio tiveram sua variância explicada pelas características do solo e pastagem. Nossos resultados indicam que as mudanças observadas na micro-escala constituem "sinais biogeoquímicos" gerados pelo processamento do material nas margens dos rios. A medida em que os rios evoluem para ordens superiores, os sinais persistentes nos canais fluviais estão mais associdados às características da bacia de drenagem (solos e uso da terra). Apesar dos efeitos das mudanças observadas no uso do solo não serem ainda detectáveis na macro-escala (bacia amazônica), a disrupção da estrutura e funcionamento dos ecossistemas é detectável nas micro e meso escalas, com alterações significativas na ciclagem de nutrientes nos ecossistemas fluviais.

This is the first study describing the genetic polymorphism of Mycobacterium tuberculosis strains in the Indian Ocean Region. Using IS6110 RFLP analysis, 475 M. tuberculosis isolates from Madagascar, Comoros, Mauritius, Mozambique and La Reunion were compared. Of the 332 IS6110 profiles found, 43 were shared by clusters containing 2-65 strains. Six clusters were common to at least two countries. Of 52 families of strains with similar IS6110 profiles, 10 were common to at least two countries. Interestingly, another characteristic was the frequency (16.8%) of IS6110 single-copy strains. These strains could be distinguished using the DR marker. This preliminary evaluation suggests genetic similarity between the strains of the Indian Ocean Region. However, additional markers would be useful for epidemiological studies and to assess the ancient transmission of strains between countries of this region.

In this study the effect of eight DNA topoisomerase inhibitors on the growth Trypanosoma rangeli epimastigotes in cell culture was investigated. Among the eight compounds tested, idarubicin was the only compound that displayed promising trypanocidal activity with a half-maximal growth inhibition (GI50) value in the sub-micromolar range. Fluorescence-activated cell sorting analysis showed a reduction in DNA content in T. rangeli epimastigotes when treated with idarubicin. In contrast to T. rangeli, against Trypanosoma cruzi epimastigotes idarubicin was much less effective exhibiting a GI50 value in the mid-micromolar range. This result indicates that idarubicin displays differential toxic effects in T. rangeli and T. cruzi. Compared with African trypanosomes, it seems that American trypanosomes are generally less susceptible to DNA topoisomerase inhibitors.

Objective To explores nursing faculty members’ attitudes towards older people, their thoughts about gerontological nursing education. Method Five focus groups and a survey were used with nursing faculty members 132 at the three nursing schools to explore their attitudes towards the care of older people and the perceived status of gerontological nursing education. The survey was given to 132 faculty members, including 76 clinical instructors, 40 associate professors and 16 professors. The nursing faculty in general had a positive attitude toward older people (M=3.36, SD 0.25), and teachers’ attitudes were higher than those of their nursing students (M=3.18, SD0.29). Results This study results suggests that Saudi nursing curricula should include more extensive gerontology content and clinical experience with older people. Conclusion This is the first time in Saudi Arabia that research has listened to their voices and examined their commitments toward gerontology education.

O nevus composto do colo uterino representa um achado muito raro. Relatos excepcionais de lesões melanocíticas benignas e malignas da endo e exocérvice uterina têm sido publicados. Relatamos o achado ocasional do nevus composto da exocérvice uterina em uma paciente de 47 anos de idade que não apresentava queixas ginecológicas. O diagnóstico foi sugerido pelo exame colposcópico e confirmado à histopatologia. Devido à possibilidade de transformação maligna dessas lesões e à dificuldade de seguimento da paciente, o tratamento foi concluido com a realização de histerectomia total abdominal.
