American tegumentary leishmaniasis presents as two major clinical forms: localized cutaneous leishmaniasis (LCL) and mucocutaneous leishmaniasis (MCL). The immune response in leishmaniasis is efficiently evaluated by the response to Leishmania antigen through the Montenegro skin test (MST). Both LCL and MCL present positive response to MST, indicating that the patients present cell-mediated immunity against the parasite - Leishmania. In spite of the presence of immunity in MCL, this is not sufficient to stop disease progression and prevent resistance to treatment. In this study we demonstrated interleukin (IL) 2, 4, 5 and interferon (IFN) gamma expression in biopsies of MST of ten patients with American tegumentary leishmaniasis. The obtained results were compared between LCL (n = 5) and MCL (n = 5) patients. The MST of MCL patients displayed a higher expression of IL-2, IL-4 and IL-5, in comparison to LCL. There was no significant difference in IFN-gamma expression between groups. The obtained results suggest the role of IL-4 and IL-5 in the maintenance of the immunopathogenic mechanism of the destructive lesions that characterize MCL.

The Flaviviridae family, Flavivirus genus includes viruses that are transmitted to vertebrates by infected mosquitoes or ticks. The genus Flavivirus includes a variety of viruses that cause diseases such as acute febrile illness, encephalitis, and hemorrhagic fever. Flaviviruses primarily infect blood monocytes and tissue macrophages, which have been shown to be permissive, supporting viral replication and serving as virus reservoirs. On the other hand, these cells may have an important antiviral activity related to modulation by cytokine production and by the capacity of these cells to synthesize reactive free radicals such as nitric oxide (NO) which can have a microbicidal effect. The present study was performed in order to determine the production of cytokines interleukin-1beta (IL-1β), tumor necrosis factor -alpha (TNF-α), transforming growth factor- beta (TGF-β) and interferon -alpha (IFN-α) and NO by macrophages infected with one of four Brazilian flaviviruses, Bussuquara virus (BUSV), Yellow Fever virus (YFV), Rocio virus (ROCV) and Encephalitis Saint Louis virus (SLEV), and to verify the possible antiviral effect of NO during macrophage infection with ROCV. Moreover, we asked if the different viruses were able to regulate bacterial lipopolysaccharide (LPS) induced cytokine production. Our results showed that YFV and SLEV reduced the production of IL-1β and TGF-β by LPS-stimulated macrophages, while ROCV only diminished LPS-stimulated TGF-β synthesis. On the other hand, BUSV more likely favored an enhancement of the LPS-induced production of IL-1β by macrophages. Additionally, while most of the viruses stimulated the production of IFN-α, none of them altered the production of TNF-α by murine macrophages. Interestingly, all viruses induced synthesis of NO that was not correlated with antiviral activity for ROCV.

The complex interaction between hepatitis C virus infection, iron homeostasis and the response to antiviral treatment remains controversial. The aim of this study was to evaluate the influence of hepatic iron concentration (HIC) on the sustained virological response (SVR) to antiviral therapy in patients with chronic hepatitis C. A total of 50 patients who underwent pretreatment liver biopsy with assessment of HIC by graphite furnace atomic absorption spectroscopy and were subsequently submitted to antiviral treatment with interferon/peginterferon and ribavirin were included in the study. Patients with alcoholism, history of multiple blood transfusion, chronic kidney disease, hemolytic anemia and parenteral iron therapy were excluded. The iron related markers and HIC were compared between those who achieved an SVR and non-responders (NR) patients. The mean age was 45.7 years and the proportion of patients' gender was not different between SVR and NR patients. The median serum iron was 138 and 134 µg/dL (p = 0.9), the median serum ferritin was 152.5 and 179.5 ng/mL (p = 0.87) and the median HIC was 9.9 and 8.2 µmol/g dry tissue (p = 0.51), for SVR and NR patients, respectively. Thus, hepatic iron concentration, determined by a reliable quantitative method, was not a negative predictive factor of SVR in patients with chronic hepatitis C presenting mild to moderate hepatic iron accumulation.

Formalin-killed promastigotes (FKP) of Leishmania major, in combination with Montanide ISA 720 (MISA), BCG or alum were used in vaccination of an inbred murine model against cutaneous leishmaniasis (CL). Significant and specific increases in anti-FKP IgG responses were detected for both alum-FKP and BCG-FKP compared to MISA-FKP (p < 0.001). Significant increases in splenic lymphocyte recall proliferation was obtained in the MISA-FKP vaccinated mice compared to alum-FKP or BCG-FKP vaccinated groups (p < 0.01). The highest interferon-γ responses were observed in the BCG-FKP group followed by the MISA-FKP while the alum-FKP gave the least responses. Significantly reduced lesion sizes were obtained in the MISA-FKP group compared to the BCG/alum adjuvants-FKP vaccinated groups. Although the BCG-FKP group showed the highest IFN-γ responses, it failed to control cutaneous lesions. Significant reductions in parasite numbers were observed in the MISA-FKP and BCG-FKP vaccinated groups (p < 0.001). There was a good correlation between parasite burden and IFN-γ level indicating IFN-γ response as a sensitive parameter of the immune status. In conclusion, MISA-FKP is the most efficacious vaccine formulation against murine cutaneous leishmaniasis.

In this study we investigated the efficacy of hyperbaric oxygen (HBO) therapy, alone or combined with the pentavalent antimonial glucantime on Leishmania amazonensis infection. In parallel, the effect of Brazilian red propolis gel (propain) alone or combined with glucantime on L. amazonensis infection was evaluated. The inhibition of the infection in macrophages treated with glucantime in combination with HBO exposition was greater than that of macrophages treated with glucantime alone or HBO alone. The susceptible mouse strain BALB/c infected in the shaved rump with L. amazonensis treated with glucantime and exposed to HBO showed: time points in the course of the disease in which lesions were smaller than those of mice treated with glucantime alone and revascularization of the skin in the lesion site; interferon-gamma (IFN-g) levels were not elevated in lymph node cells from these animals. Propain alone was not efficient against lesions, although less exudative lesions were observed in animals treated with propain alone or combined with glucantime. These results reveal the potential value of HBO and red propolis in combination with glucantime for treating cutaneous leishmaniasis and encourage further studies on the effect of more aggressive HBO, propolis and glucantime therapies on different mouse models of leishmaniasis.

In this study, we report on the safety and skin delayed-type hypersensitivity (DTH), responses of the Leishmania donovani whole cell sonicate antigen delivered in conjunction with alum-BCG (AlBCG), Montanide ISA 720 (MISA) or Monophosphoryl lipid A (MPLA) in groups of vervet monkeys. Following three intradermal injections of the inoculums on days 0, 28 and 42, safety and DTH responses were assessed. Preliminary tumor necrosis factor alpha (TNF-α) and interferon gamma (IFN-γ) levels were also measured and these were compared with DTH. Only those animals immunized with alum-BCG reacted adversely to the inoculum by producing ulcerative erythematous skin indurations. Non-parametric analysis of variance followed by a post-test showed significantly higher DTH responses in the MISA+Ag group compared with other immunized groups (p < 0.001). The MPLA+Ag group indicated significantly lower DTH responses to the sonicate antigen compared with the AlBCG+Ag group. There was a significant correlation between the DTH and cytokine responses (p < 0.0001). Based on this study we conclude that Leishmania donovani sonicate antigen containing MISA 720 is safe and is associated with a strong DTH reaction following immunization.

Chronic infection by hepatitis C virus (HCV) is one of the main risk factors for the development of liver cirrhosis and hepatocellular carcinoma. However, the emergence of hepatocellular carcinoma (HCC) in non-cirrhotic HCV patients, especially after sustained virological response (SVR) is an unusual event. Recently, it has been suggested that HCV genotype 3 may have a particular oncogenic mechanism, but the factors involved in these cases as well as the profile of these patients are still not fully understood. Thus, we present the case of a non-cirrhotic fifty-year-old male with HCV infection, genotype 3a, who developed HCC two years after treatment with pegylated-interferon and ribavirin, with SVR, in Brazil.

The authors report a case of a male patient from Bacabal, MA with diffuse cutaneous leishmaniasis (DCL), for at least nine years, with 168 lesions on his body. These were tumour-like nodules with some ulceratmi. He usedpentavalent antimonial (glucantime®) and an association of gamma interferon plus glucantime with improvement of the lesions but relapsed later. Recently, pentamidine isethionate (pentacarinat®) was given a dosage of 4mg/kg/weight/day on alternate days for 20 applications. After 3 months a similar course of 10 application was given 2 times. Later he developed diabetic signs with weight loss of 10kg, polydypsia, polyuria and xerostomia. The lower limbs lesions showed signs of activity. Blood glucose levels normalised and remain like this at moment. Attention is drawn to the fact that pentamidine isethionate should be used as a therapy option with care, obeyng rigorous laboratory controls including a glucose tolerance test.

O vírus rábico RV194-2, uma variante avirulenta da cepa CVS (Challenge Vírus Standard), produz uma infecção inaparente quando inoculado intracerebralmente em camundongos adultos. Sugerindo que a resposta imunológica do hospedeiro permite a eliminação do vírus do sistema nervoso central. Por esta razão foram estudadas a indução de interferon e a resposta imune humoral em camundongos BALB/c inoculados com vírus RV194-2. Durante a infecção, estes camundongos apresentaram elevados níveis de interferon no plasma e no cérebro com altos títulos de anticorpos neutralizantes anti-rábicos. A 2-5A sintetase. um marcador da ação dos interferons,foi também analisada no cérebro destes animais. Sua atividade, aumentou, paralelamente, á produção de interferon, demonstrando que este interferon é bioquímicamente ativo. O vírus RV194-2 também induziu, 45 dias após sua inoculação, proteção aos animais quando desafiados com a cepa virulenta CVS. Estes resultados demonstram que a cepa RV194-2possui um alto nível imunogênico.

A hepatite B constitui grave problema de saúde pública. Estima-se que 350 milhões de pessoas, ou seja, 5% da população mundial sejam portadores dessa virose. Admite-se que a infecção evolui para a cura em 90% a 95% dos casos e para o estado de portador crônico nos restantes 5% a 10%; a infecção persistente pode resultar também em cirrose, insuficiência hepática e carcinoma hepatocelular. O diagnóstico de qualquer das formas clínicas da hepatite B realiza-se através de técnicas sorológicas. Os médicos, hoje, possuem acesso a modernas técnicas laboratoriais capazes de avaliar a carga viral, o índice de replicação do agente infeccioso e a eficácia das novas medicações utilizadas. Vários agentes antivirais têm sido usados no tratamento dos indivíduos com hepatite crônica, como o intérferon alfa, a lamivudina, o famciclovir, e o adefovir dipivoxil, entre outros. A imunização ativa utilizando as modernas vacinas recombinantes constitui, na atualidade, a arma mais importante no combate à infecção pelo vírus da hepatite B.

Estima-se que cerca de 3% da população mundial esteja infectada pelo vírus da hepatite C. Todos os que receberam transfusão de sangue ou seus componentes e os usuários de drogas podem estar infectados. Procedimentos odontológicos, médicos, tatuagem ou acupuntura também constituem fatores de risco. A infecção se cronifica em até 85% dos indivíduos, com evolução assintomática durante anos ou décadas e apresentação clínica variada. Para o diagnóstico, a determinação do anti-VHC revela-se muito sensível e a confirmação se faz pela determinação do RNA-VHC no sangue; o estadiamento da doença e a avaliação da atividade inflamatória pela biópsia hepática. O tratamento objetiva deter a progressão da doença hepática através da inibição da replicação viral. Devido à baixa eficácia terapêutica aliada a importantes efeitos colaterais do interferon e da ribavirina, esses medicamentos encontram indicações e contra-indicações específicas. Vários fatores preditivos de resposta ao tratamento, principalmente a carga viral e o genótipo do VHC, mostram-se úteis na avaliação dos pacientes.

Antigenic extracts from five Leishmania stocks were used to vaccinate C57BL/10 mice. The Leishvacin® and PH8 monovalent vaccine yielded the highest IFN-gamma levels in the supernatants of spleen cell culture from vaccinated animals. Each single strain immunized group showed evidence of protective immunity six months after the challenge with promastigotes of Leishmania (Leishmania) amazonensis. No differences were detected between the vaccinated groups. It can be concluded that vaccines composed of single Leishmania stocks can provide protection to C57BL/10 mice against L. (L.) amazonensis infection.

O vírus da hepatite D (VHD), também chamado de vírus delta, é um pequeno vírus contendo RNA circular. O VHD causa infecção, quando há coinfecção com o vírus da hepatite B (VHB) em indivíduos normais ou superinfecção em portadores crônicos do VHB. Três genótipos já foram clonados e seqüenciados. A infecção apresenta distribuição mundial, sendo a região ocidental da Amazônia brasileira considerada área de alta endemicidade. Estima-se que 18 milhões de pessoas encontram-se infectadas pelo vírus entre os 350 milhões de portadores crônicos do VHB no mundo. As vias de transmissão do VHD e os fatores de risco mostram-se similares aos da infecção pelo VHB. O diagnóstico se faz pela identificação imuno-histológica do HDAg no fígado e pelo encontro das frações IgM e IgG anti-HD no soro por radioimunoensaio ou ELISA. O curso clínico da infecção pelo VHD mostra-se variável. Os pacientes podem apresentar formas fulminantes de hepatite. As formas crônicas associam-se a achados histopatológicos graves no fígado, com curso rápido e progressivo, evoluindo para cirrose, insuficiência hepática e morte. O interferon alfa constitui a única opção terapêutica com algum efeito benéfico no tratamento da hepatite. O transplante hepático encontra indicação nos casos terminais de cirrose. A profilaxia indireta da infecção pelo VHD tornou-se possível com o advento da vacina contra o vírus da hepatite B.

A mielopatia associada ao HTLV-1 (HAM), também conhecida como paraparesia espástica tropical (TSP), é uma doença desmielinizante crônica progressiva que afeta a medula espinal e a substância branca do cérebro. Menos de 5% dos portadores crônicos do HTLV-1 desenvolverão essa complicação. As primeiras manifestações da doença ocorrem na quarta década da vida e observa-se relação mulher/homem de 2:1. Os distúrbios da marcha, a fraqueza e o enrijecimento dos membros inferiores constituem os principais sinais e sintomas de apresentação da mielopatia. As extremidades inferiores são afetadas com maior intensidade do que as extremidades superiores. A espasticidade pode variar de moderada a intensa e a dor lombar baixa revela-se comum. Com a progressão da doença há, com freqüência, disfunção vesical e intestinal. O envolvimento sensitivo mostra-se discreto e manifesta-se com graus variados de perdas sensitivas e sensação de disestesia. A ressonância nuclear magnética do sistema nervoso pode resultar normal ou revelar atrofia da medula espinal e alterações inespecíficas no cérebro. Há evidências de envolvimento imunológico na gênese da lesão medular. Não há tratamento eficaz para a mielopatia. Os corticoesteróides e o interferon-a produziram benefícios transitórios no tratamento da doença. Não houve melhora da marcha e da disfunção vesical em pacientes que usaram o danazol, um esteróide anabolizante. O valor da zidovudina (anti-retroviral) no tratamento da mielopatia ainda não se encontra definido.

This study assessed the number of CD4 T lymphocytes, the parasitemia and serum levels of interferon gamma (IFN-gamma), tumor necrosis factor alpha (TNF-alpha), interleukin-1 (IL-1), IL-4 and IL-10 of patients infected by human immunodeficiency virus (HIV) and human immunodeficiency virus/Chagas' disease coinfection. CD4 T lymphocytes were low in the two groups of patients, although significantly lower in patients without Chagas' disease. Serum levels of IFN-gamma, IL-4 and TNF-alpha were significantly higher in patients with HIV/Chagas' disease. IL-4/IFN-gamma ratios were higher in patients with HIV/Chagas' disease, which showed a clear balance in favor of Th2-like cytokines in this group of patients. This Th2 balance was higher in patients with detectable parasitemia. We conclude that, although immunosuppression was observed, with CD4 T lymphocytes bellow 200/µm³, these patients did not display reactivation of T. cruzi infection and that a balance favorable to Th2 was associated with the presence of parasitemia.
