SUMMARY Combination Antiretroviral Therapy (cART) aims to inhibit viral replication, delay immunodeficiency progression and improve survival in AIDS patients. The objective of this study was to compare two different schemes of cART, based on plasma viral load (VL) and CD4+ T lymphocyte count, during 48 weeks of treatment. For this purpose, 472 medical charts of a Specialized Outpatient Service were reviewed from 1998 to 2005. Out of these, 58 AIDS patients who had received a triple drug scheme as the initial treatment were included in the study and two groups were formed: Group 1 (G1): 47 individuals treated with two nucleoside reverse-transcriptase inhibitors (NRTI) and one non-nucleoside reverse-transcriptase inhibitor; Group 2 (G2): 11 patients treated with two NRTI and one protease inhibitor. In G1 and G2, 53.2% and 81.8% respectively were patients with an AIDS-defining disease. The T CD4+ lymphocyte count increased progressively up until the 24th week of treatment in all patients, while VL became undetectable in 68.1% of G1 and in 63.6% of G2. The study concluded that the evolutions of laboratory tests were similar in the two treatment groups and that both presented a favorable clinical evolution.

The in vitro activity of thiabendazole on Ascaris lumbricoides eggs, which were recovered from uteri of worm excreted after chemotherapeutic treatment, was studied. Four concentrations of the drug were used: 1 -- 2.5 -- 5 -- and 10 ppm during 24, 48 and 72 hours of exposure. Subsequently, the eggs were centrifuged, washed three times and H2SO4 0.1N was added. The eggs were maintained in an incubator for 20 days at 28°C. Finally, the percentage of embryonated eggs was determined under a lightmicroscope at a 100X magnification. After 48 and 72 hours of thiabendazole exposure, at a concentration of 10ppm, the drug showed complete inhibition of egg embryonation.

Os escorpiões (Arachnida, Scorpiones), especialmente na área urbana, devido à grande densidade demográfica e possibilidade de confronto, representam risco à saúde pública. A espécie Tityus serrulatus é a mais importante, do ponto de vista médico, causando o maior número de acidentes. Objetivou-se levantar dados epidemiológicos e a ocorrência de acidentes escorpiônicos em Belo Horizonte, entre 1990 e 1997. Os dados foram obtidos no Hospital de Pronto Socorro João XXIII. Ocorreram 3265 acidentes, sendo a maioria em 1996. Aconteceram 6 acidentes fatais. Com maior incidência nos meses de janeiro, o sexo masculino, os membros superiores e a faixa etária entre 25 e 65 anos, foram os mais atingidos. Os resultados constituem importante subsídio para o controle do escorpionismo, pois delimitam as áreas mais atingidas e o perfil dos acidentados, permitindo campanhas educativas e de prevenção mais eficientes.

Foi realizado um levantamento das helmintoses intestinais em 18.973 escolares do primeiro grau (7 a 14 anos), da rede pública do Estado de Minas Gerais, utilizando o método de Kato-Katz de exame de fezes (duas lâminas por amostra). Foram estudadas 3 mesorregiões: Triângulo Mineiro/Alto Paranaíba (60 municípios), Noroeste de Minas (13) e Sul/Sudoeste (144). Entre os escolares examinados, 15.545 (82%) estavam negativos, 2.863 (15%) monoparasitados e 565 (3%) poliparasitados. A prevalência de A. lumbricoides foi de 10,3%, de T. trichiura 4,7%, de ancilostomídeos 2,9%, de E. vermicularis 1,2%, de H. nana 0,4% e de Taenia sp 0,2%. As maiores prevalências de helmintos/mesorregião foram de 24,2% para T. trichiura e 18,7% para A. lumbricoides (Sul/Sudoeste) e 12,1% para ancilostomídeos e 0,7% para Taenia sp (Noroeste de Minas). As helmintoses intestinais continuam sendo um grave problema de saúde pública, inclusive em áreas onde as condições socioeconômicas são mais favoráveis.

A survey was done to determine the most common hospital accidents with biologically contaminated material among students at the Medical College of the Federal University of Minas Gerais. Six hundred and ninety-four students (between fifth and twelfth semesters of the college course) answered the questionnaire individually. Three-hundred and forty-nine accidents were reported. The accident rate was found to be 33.9% in the third semester of the course, and increased over time, reaching 52.3% in the last semester. Sixty-three percent of the accidents were needlestick or sharp object injuries; 18.3% mucous membrane exposure; 16.6% were on the skin, and 1.7% were simultaneously on the skin and mucous membrane exposure. The contaminating substances were: blood (88.3%), vaginal secretion (1.7%), and others (9.1%). The parts of the body most frequently affected were: hands (67%), eyes (18.9%), mouth (1.7%), and others (6.3%). The procedures being performed when the accidents occurred were: suture (34.1%), applying anesthesia (16.6%), assisting surgery (8.9%), disposing of needles (8.6%), assisting delivery (6.3%), and others (25.9%). Forty-nine percent of those involved reported the accident to the accident control department. Of these 29.2% did not receive adequate medical assistance. Eight percent of those involved used antiretroviral drugs and of these 86% discontinued the treatment on receiving the Elisa method applied to the patient (HIV-negative); 6.4% discontinued the treatment due to its side-effects; and 16% completed the treatment.

Descreve-se a epidemiologia da raiva animal na região de Araçatuba, noroeste do estado de São Paulo, durante o período de 1993 a 2007, com base nos resultados dos diagnósticos realizados em laboratórios da região, utilizando as técnicas de imunofluorescência direta e inoculação intracerebral em camundongos. De 10.579 amostras analisadas, 4,9% foram positivas (518/10.579). Os casos em cães corresponderam a 67% (346/518) do total e ocorreram entre 1993 a 1997. Dentre as demais amostras positivas, 16% do total (84/518) foi detectado em bovinos e 9,7% (50/518) em morcegos. Dos 42 municípios da região, 23 (55%) apresentaram pelo menos um caso positivo da doença, sendo que 13 deles registraram casos em morcegos. Foram identificados três ciclos distintos da raiva na região Noroeste do Estado de São Paulo, o ciclo urbano caracterizado predominantemente pela raiva canina (1993 a 1997) e os ciclos aéreo e rural, a partir de 1998 com predominância de casos em quirópteros nas áreas urbanas e em herbívoros.

Introduction Chagasic megaesophagus (CM) is the most common digestive manifestation of Chagas disease in Brazil, and the State of Goiás is one of the most affected regions. In recent decades, the Hospital das Clínicas (HC)/Universidade Federal de Goiás (UFG) has been a reference center for the study and treatment of CM. The objective of this study was to characterize the current epidemiological profile of patients with CM observed at the HC of the UFG from 1998 to 2010. Methods In total, 939 patient records were analyzed, and age, gender, place of birth, serology, symptoms and radiological classification according to Rezende et al. were analyzed. Results The median patient age was 55 years. Male patients were more (54%) prevalent than female patients. The prevalence of younger patients (less than 31 years of age) was 4.2%, but 82.1% of the younger patients were from State of Bahia. Patients older than 40 years were the majority (85.5%). The radiological groups were distributed as follows: Group I (35.9%), Group II (32.9%), Group III (17%) and Group IV (14.2%). Conclusions Compared with previous studies by the same group in 1975, 1994 and 1995, the number of younger patients decreased, and the frequency curve has shifted to older patients.

Introduction Rabies is an important zoonosis that occurs in mammals, with bats acting as Lyssavirus reservoirs in urban, rural and natural areas. Rabies cases in bats have been recorded primarily in urban areas in Northwestern State of São Paulo since 1998. This study investigated the circulation of rabies virus by seeking to identify the virus in the brain in several species of bats in this region and by measuring rabies-virus neutralizing antibody levels in the hematophagous bat Desmodus rotundus. Methods From 2008 to 2012, 1,490 bat brain samples were sent to the Universidade Estadual Paulista (UNESP) Rabies Laboratory in Araçatuba, and 125 serum samples from vampire bats that were captured in this geographical region were analyzed. Results Rabies virus was detected in the brains of 26 (2%) of 1,314 non-hematophagous bats using the fluorescent antibody test (FAT) and the mouse inoculation test (MIT). None of the 176 hematophagous bat samples were positive for rabies virus when a virus detection test was utilized. Out of 125 vampire bat serum samples, 9 (7%) had levels of rabies virus neutralization antibodies (RVNAs) that were higher than 0.5IU/mL; 65% (81/125) had titers between 0.10IU/mL and 0.5IU/mL; and 28% (35/125) were negative for RVNAs using the simplified fluorescent inhibition microtest (SFIMT) in BHK21 cells. The observed positivity rate (1.7%) was higher than the average positivity rate of 1.3% that was previously found in this region. Conclusions The high percentage of vampire bats with neutralizing antibodies suggests that recent rabies virus exposure has occurred, indicating the necessity of surveillance measures in nearby regions that are at risk to avoid diffusion of the rabies virus and possible rabies occurrences.

Abstract: INTRODUCTION: Due to the importance that Howler monkeys have on the yellow fever (YF) epidemiological sylvatic cycle in Brazil, more accurate morphological diagnostic criteria needs to be established, especially considering the differences that may exist between the genera of Brazilian non-human primates (NHPs) involved in yellow fever virus (YFV) epizootics. METHODS: Records of YF epizootics in NHPs in Brazil between 2007 and 2009 were obtained from the Brazilian Ministry of Health database to select YF positive (n=98) Howler monkeys (Alouatta sp.) for this study. The changes described in the histopathological reports were categorized by organ and their frequencies calculated. RESULTS: The most frequent lesions observed in the animals with YF were hepatocyte apoptosis (Councilman body formation), midzonal hepatocyte necrosis, steatosis, liver hemorrhage, inflammatory mononuclear cell infiltration of the liver, renal acute tubular necrosis and interstitial nephritis. Midzonal hepatocyte necrosis, steatosis and hemorrhage presented positive correlations with apoptosis of hepatocytes, suggesting strong YFV pathogenic effect association; they were also the main histopathological changes in the Alouatta sp. A pronounced negative correlation between apoptosis of hepatocytes and hepatic mononuclear cell infiltration pointed to significant histopathological differences between YFV infection in Howler monkeys and humans. CONCLUSIONS: The results warn that NHPs may exhibit different response patterns following YFV infection and require a more careful diagnosis. Presumptive diagnosis based on primate histopathological lesions may contribute to public health service control.

The overactive bladder is characterized by symptoms of frequency, urgency, and urge incontinence, substantially affecting the quality of life of millions of people throughout the world. The symptoms are associated with significant social, psychological, occupational, domestic, physical, and sexual problems. Despite the considerable impact of this condition on quality of life, sufferers are often unwilling to discuss their problem with family members or health care professionals. This situation is unfortunate, for much can be done to alleviate the symptoms of this distressing condition. It is therefore of utmost importance that medical education about symptoms of the overactive bladder and other related problems be improved to help health care professionals identify and treat patients who will benefit from therapy. This article reviews current thinking regarding definition, epidemiology, quality of life effects, evaluation, and management of the overactive bladder.

Lower urinary tract dysfunction is a major cause of morbidity and decreased quality of life in elderly men and women. With the progressive aging of the population, it is important to understand common micturitional disorders that may occur in this population. Most urinary problems in the elderly are multifactorial in origin, demanding a comprehensive assessment of the lower urinary tract organs, functional impairments, and concurrent medical diseases. Urodynamics is a highly valuable tool in the investigation of elderly patients with lower urinary tract symptoms. Urodynamic tests are not always necessary, being indicated after excluding potentially reversible conditions outside the urinary tract that may be causing or contributing to the symptoms. Although urodynamic tests may reveal common diagnoses such as bladder outlet obstruction and stress urinary incontinence in the elderly population, findings such as detrusor overactivity and impaired detrusor contractility are common and have important prognostic and therapeutic implications. The purpose of this article is to describe common urologic problems in the elderly and review the indications for and clinical aspects of urodynamic studies in these conditions.

No ramo da preservação de madeira, a eficiência de compostos naturais há muito tem sido avaliada, visando encontrar substâncias que não só aumentem a vida útil de madeiras de baixa durabilidade, mas também causem menor dano ao meio ambiente. Nesse contexto, o potencial de extrativos do lenho e da casca das espécies madeireiras Pouteria guianensis,Buchenavia parviflora e Dinizia excelsa em repelir o ataque de cupins Nasutitermes sp. foi avaliado, utilizando-se como substrato a espécie Simarouba amara, de baixa durabilidade. Os extrativos foram obtidos em etanol 95%, diluídos em solução hidroalcoólica (1 e 0,1%), e impregnados em blocos de S. amara, segundo ASTM D1413-76. Os blocos foram montados em quadrado latino em uma plataforma e submetidos ao ataque dos cupins. O efeito das soluções foi classificado pela perda de peso dos blocos de S. amara. Os resultados mostraram que a maioria dos tratamentos conferiu ótima resistência à madeira de S. amara. O melhor tratamento foi aquele efetuado com extrativos da casca de P. guianensis, onde a perda foi somente de 10% do peso inicial dos blocos, enquanto que o menos efetivo foi aquele com extrativos da casca de D. excelsa, com perda de 70% de peso do bloco. Assim, pode-se concluir que os extrativos do lenho e da casca de P. guianensis e B. parviflora e do lenho de D. excelsa apresentam, a princípio, compostos com propriedades de repelência a Nasutitermes sp., e os extrativos da casca de D. excelsa não apresentam estes componentes.

A atividade comercial pesqueira na Amazônia Central é predominantemente direcionada para Manaus, porém o perfil das atividades pesqueiras efetuadas nos demais centros também é fundamental para o planejamento do setor. Neste contexto, o presente trabalho visa caracterizar o perfil da produção pesqueira que abastece a cidade de Manacapuru, um dos principais centros urbanos da Amazônia Central. Os desembarques ocorreram a partir de canoas a remo, canoas motorizadas, barcos de pesca e recreios. A média mensal de pescado desembarcado foi de 175,36 ± 39,50 t em 2001 e de 172,13 ± 18,88 t em 2002, não apresentando diferença significativa entre anos (P>0,05). Dos 35 nomes específicos comuns registrados, observa-se que curimatã (Prochilodus nigricans), jaraquis (Semaprochilodus spp.), cubiu (Anodus spp.), mapará (Hypophthalmus spp.), e tambaqui (Colossoma macropomum) foram os itens mais importantes nos dois anos, e piramutaba (Brachyplatystoma vaillantii) em 2002. Sete sub-regiões foram visitadas pela frota pesqueira, destacando-se Baixo-Solimões e o rio Purus.

OBJETIVO: Avaliar os diferentes métodos de quantificação de insuficiência mitral (IM) ao ecocardiograma transesofágico (ETE) em pacientes com suspeita clínica de disfunção de prótese mitral. MÉTODOS: Foram estudados 15 pacientes, divididos em dois grupos, conforme a presença ou não de IM expressiva (grau importante) ao cateterismo cardíaco (Cate). A IM foi quantificada ao ETE pelos seguintes métodos, habitualmente empregados para IM de valvas nativas: avaliação subjetiva do jato regurgitante ao mapeamento de fluxo a cores, avaliação objetiva com base na área absoluta do jato regurgitante e na sua área relativa (área do jato/área do átrio esquerdo (AE)), e avaliação baseada na presença de fluxo sistólico reverso em veia pulmonar. RESULTADOS: A IM foi predominantemente de origem transprotética (14 pacientes) e de distribuição excêntrica (11 pacientes). Observou-se concordância estatisticamente significante (p<0,05) entre IM expressiva ao Cate (8 pacientes) e ao ETE pela avaliação subjetiva e presença de fluxo sistólico reverso em veia pulmonar. As avaliações pelas áreas absoluta (área de jato >7cm²) e relativa (área de jato >35% da área do AE) não mostraram concordância significante com o Cate, com nítida subestimação ao ETE pela área relativa. Houve, porém, concordância significante, quando considerado como IM expressiva, jato cuja área relativa foi >30% da área do AE. CONCLUSÃO: O ETE identificou adequadamente as IM protéticas angiograficamente expressivas, particularmente pelos métodos subjetivo e de fluxo sistólico reverso em veia pulmonar. É necessário cautela na utilização de critérios baseados na área do jato regurgitante, em virtude da subestimação da área na presença de jato excêntrico, freqüente em disfunção de prótese mitral.
