OBJETIVO: Calcular los índices infestación por triatominos en comunidades indígenas en Colombia. MÉTODOS: Se realizó estudio descriptivo en 19 comunidades indígenas del municipio de Valledupar Departamento de Cesar, Colombia. Durante junio a diciembre de 2007 se recolectaron triatominos por búsqueda activa en las viviendas de los indígenas. Los insectos luego fueron identificados por las claves de Lent & Wygodzinsky. Se desarrolló estudio del proceso infectivo en modelo animal y análisis enzimático de cepas de Trypanosoma cruzi, detectadas en heces de triatominos. RESULTADOS: Rhodnius prolixus presentó índice de densidad en las viviendas de 154,7%, Triatoma dimidiata de 102,45%, Triatoma maculata de 109,25% y Panstrogylus geniculatus de 0,3%. El índice promedio de infestación de las cuatro especies fue de 40,54% y, el de infección con T. cruzi de 9,4%. De cinco hemocultivos positivos para T. cruzi, tres se caracterizaron por isoenzimas, clasificándose en T. cruzi grupo I. El estudio de las biopsias reveló pocas características patológicas durante el proceso de infección con las cepas de T. cruzi aisladas de triatominos domiciliados. CONCLUSIÓN: Los altos índices de infestación por triatominos en las viviendas y el índice de infección por T. cruzi, evidencian la transmisión activa de la enfermedad de Chagas, situación que amerita la aplicación de medidas de control vectorial y el estudio seroepidemilógico de la población en riesgo. La identificación de las cepas de T. cruzi como grupo I concuerda con otros estudios realizados en esta región colombiana.

OBJECTIVE: To assess the effect of a health promotion program on cardiometabolic risk profile in Japanese-Brazilians. METHODS: A total of 466 subjects from a study on diabetes prevalence conducted in the city of Bauru, southeastern Brazil, in 2000 completed a 1-year intervention program (2005-2006) based on healthy diet counseling and physical activity. Changes in blood pressure and metabolic parameters in the 2005-2006 period were compared with annual changes in these same variables in the 2000-2005 period. RESULTS: During the intervention, there were greater annual reductions in mean (SD) waist circumference [-0.5(3.8) vs. 1.2(1.2) cm per year, p<0.001], systolic blood pressure [-4.6(17.9) vs. 1.8(4.3) mmHg per year, p<0.001], 2-hour plasma glucose [-1.2(2.1) vs. -0.2(0.6) mmol/L per year, p<0.001], LDL-cholesterol [-0.3(0.9) vs. -0.1(0.2) mmol/L per year, p<0.001] and Framingham coronary heart disease risk score [-0.25(3.03) vs. 0.11(0.66) per year, p=0.02] but not in triglycerides [0.2(1.6) vs. 0.1(0.42) mmol/L per year, p<0.001], and fasting insulin level [1.2(5.8) vs. -0.7(2.2) IU/mL per year, p<0.001] compared with the pre-intervention period. Significant reductions in the prevalence of impaired fasting glucose/impaired glucose tolerance and diabetes were seen during the intervention (from 58.4% to 35.4%, p<0.001; and from 30.1% to 21.7%, p= 0.004, respectively). CONCLUSIONS: A one-year community-based health promotion program brings cardiometabolic benefits in a high-risk population of Japanese-Brazilians.

OBJETIVO: Evaluar la asociación entre frecuencia de ver televisión, sobrepeso y obesidad en una muestra representativa de mujeres peruanas a nivel nacional. MÉTODOS: Análisis secundario de la Encuesta Nacional Demográfica y de Salud Familiar incluyendo mujeres de 15 a 49 años. Las variables resultados fueron obesidad (índice de masa corporal >30 kg/m²) y sobrepeso (>25 peso <30 kg/m²); mientras que la exposición fue la frecuencia de ver televisión (nunca, ocasionalmente, casi todos los días). Se usó regresión logística para análisis de muestras complejas según el diseño de la encuesta ajustando por potenciales confusores. Los resultados fueron presentados como Odds Ratio ajustados (ORa) con sus respectivos intervalos de confianza al 95% (IC95%). RESULTADOS: Un total de 21.712 mujeres fueron incluidas en el análisis. La prevalencia de sobrepeso fue 34,7% (IC95% 33,8;35,7) y de obesidad fue 14,3% (IC95% 13,6;15,1). Las mujeres que veían televisión ocasionalmente y casi todos los días tuvieron mayor probabilidad de tener obesidad: ORa 1,7 (IC95% 1,3;2,3) y ORa 2,6 (IC95% 2,0;3,5), respectivamente comparado con aquellas que nunca veían televisión. La magnitud de la asociación fue menor para el caso de sobrepeso: ORa 1,2 (IC95% 1,3;2,3) y ORa 1,6 (IC95% 1,1;1,4), respectivamente. La fuerza de la asociación fue mayor en el área urbana. CONCLUSIONES: En mujeres peruanas la frecuencia de ver televisión estuvo asociada con presentar obesidad y sobrepeso, y la fuerza de asociación varió de acuerdo al área de residencia. Estos hallazgos deberían orientar las estrategias de prevención de la obesidad en el contexto peruano.

OBJECTIVE: To analyze the strengths and limitations of the Family Health Strategy from the perspective of health care professionals and the community. METHODS: Between June-August 2009, in the city of Vespasiano, Minas Gerais State, Southeastern Brazil, a questionnaire was used to evaluate the Family Health Strategy (ESF) with 77 healthcare professionals and 293 caregivers of children under five. Health care professional training, community access to health care, communication with patients and delivery of health education and pediatric care were the main points of interest in the evaluation. Logistic regression analysis was used to obtain odds ratios and 95% confidence intervals as well as to assess the statistical significance of the variables studied. RESULTS: The majority of health care professionals reported their program training was insufficient in quantity, content and method of delivery. Caregivers and professionals identified similar weaknesses (services not accessible to the community, lack of healthcare professionals, poor training for professionals) and strengths (community health worker-patient communications, provision of educational information, and pediatric care). Recommendations for improvement included: more doctors and specialists, more and better training, and scheduling improvements. Caregiver satisfaction with the ESF was found to be related to perceived benefits such as community health agent household visits (OR 5.8, 95%CI 2.8;12.1), good professional-patient relationships (OR 4.8, 95%CI 2.5;9.3), and family-focused health (OR 4.1, 95%CI 1.6;10.2); and perceived problems such as lack of personnel (OR 0.3, 95%CI 0.2;0.6), difficulty with access (OR 0.2, 95%CI 0.1;0.4), and poor quality of care (OR 0.3, 95%CI 0.1;0.6). Overall, 62% of caregivers reported being generally satisfied with the ESF services. CONCLUSIONS: Identifying the limitations and strengths of the Family Health Strategy from the healthcare professional and caregiver perspective may serve to advance primary community healthcare in Brazil.

O objetivo dessa comunicação foi analisar a variação do perfil dos benefícios previdenciários por transtornos mentais e comportamentais e sua relação com o trabalho. Foram utilizados dados secundários da Previdência Social brasileira de 2008 a 2011. Foram calculadas taxas de variação média anual no período acerca da população economicamente ativa, número de segurados, benefícios auxílio-doença concedidos em geral e, especificamente, os relativos a transtornos mentais e comportamentais. Os transtornos mentais mantêm-se como a terceira causa das concessões. Houve aumento médio anual de 0,3% de novas concessões, com queda de 2,5% da incidência média anual. Foram considerados relacionados ao trabalho 6,2% dos casos, na média, principalmente decorrentes de transtornos de humor.As autoridades governamentais devem usar os dados da Previdência Social para auxiliar no debate sobre políticas públicas de saúde mental.

ABSTRACT OBJECTIVE To describe the spatial distribution of avoidable hospitalizations due to tuberculosis in the municipality of Ribeirao Preto, SP, Brazil, and to identify spatial and space-time clusters for the risk of occurrence of these events. METHODS This is a descriptive, ecological study that considered the hospitalizations records of the Hospital Information System of residents of Ribeirao Preto, SP, Southeastern Brazil, from 2006 to 2012. Only the cases with recorded addresses were considered for the spatial analyses, and they were also geocoded. We resorted to Kernel density estimation to identify the densest areas, local empirical Bayes rate as the method for smoothing the incidence rates of hospital admissions, and scan statistic for identifying clusters of risk. Softwares ArcGis 10.2, TerraView 4.2.2, and SaTScanTM were used in the analysis. RESULTS We identified 169 hospitalizations due to tuberculosis. Most were of men (n = 134; 79.2%), averagely aged 48 years (SD = 16.2). The predominant clinical form was the pulmonary one, which was confirmed through a microscopic examination of expectorated sputum (n = 66; 39.0%). We geocoded 159 cases (94.0%). We observed a non-random spatial distribution of avoidable hospitalizations due to tuberculosis concentrated in the northern and western regions of the municipality. Through the scan statistic, three spatial clusters for risk of hospitalizations due to tuberculosis were identified, one of them in the northern region of the municipality (relative risk [RR] = 3.4; 95%CI 2.7–4,4); the second in the central region, where there is a prison unit (RR = 28.6; 95%CI 22.4–36.6); and the last one in the southern region, and area of protection for hospitalizations (RR = 0.2; 95%CI 0.2–0.3). We did not identify any space-time clusters. CONCLUSIONS The investigation showed priority areas for the control and surveillance of tuberculosis, as well as the profile of the affected population, which shows important aspects to be considered in terms of management and organization of health care services targeting effectiveness in primary health care.

De 3464 migrantes em trânsito por São Paulo, de dezembro de 1970 a dezembro de 1971, foram obtidas informações sobre dados demográficos e econômico-sociais, tendo sido titulados os níveis de antitoxina tetânica dos soros pela Reação de Hemaglutinação Indireta. Em 25 soros selecionados, com diferentes níveis de antitoxina tetânica pela reação de hemaglutinação indireta, foram dosados os títulos de antitoxina tetânica pela reação de neutralização "in vivo". A correlação entre os títulos dos soros por ambos os métodos, feita pela análise de regressão, mostrou que a reação de hemaglutinação indireta apresenta baixo poder preditivo (47%), mas corresponde a uma técnica adequada nos estudos de inquéritos populacionais, considerando-se sua alta sensibilidade, fácil execução e baixo custo. A ausência de antitoxina circulante em 55,5% do grupo em estudo, associada à pequena proporção de títulos considerados protetores em 1% da população total e em 0,3% da população de mulheres de 15 a 44 anos "grupo de risco especial" explicam os altos índices de morbidade e mortalidade devidos ao tétano em nosso meio, revelando a precariedade da organização dos serviços de Saúde Pública.

In twenty five patients who presented the cutaneous form of loxoscelism, serum haptoglobin and lactic dehydrogenase, erythrocyte glucose-6-phosphate dehydrogenase, glutathione reductase, glutathione peroxidase, methemoglobin, bilirubin and reticulocytes were investigated after bite. No hemolysis was detected but an increase in methemoglobin was found in 54% of the cases; in 7% it was between 1.1% and 2%, in 27% it ranged from 2.1% to 4%, and in 20% from 4.1% to 8%. Blood samples of a normal, blood group 0 individual and of a patient who exhibited methemoglobinemia after Loxosceles bite were incubated separately with antisera against Loxosceles gaucho, Crotalus terrificus, Bothrops jararaca, with Loxosceles gaucho venom and 0.3% phenol. No methemoglobin was found after 1, 4,8 and 15 days in both sets of samples. At the 25th day all the samples, including the controls, exhibited similar methemoglobin reductase decrease. The data suggest that the methemoglobinemia which occurs in 50% of the patients probably arises from in vivo venom metabolism, inasmuch as the crude venom does not induce methemoglobinemia.

Padronizou-se método de fluorescência (solução de diacetato de fluoresceína DF e brometo de etídio BE) para análise de viabilidade de células fúngicas, em 40 amostras de liquor, provenientes de casos comprovados de neurocriptococose. A utilização de solução aquosa de saponina a 0,3% eliminou fluorescências interferentes emitidas por hemácias e leucócitos. Após o processamento dos materiais biológicos, foram retiradas alíquotas de 0,1 ml das supensões obtidas e misturadas a volumes iguais da solução DF-BE preparada pouco antes do uso. O tempo de coloração ideal foi de 30 minutos, resultando perfeita diferenciação entre microrganismos viáveis (fluorescência verde) e não viáveis (fluorescência vermelha).

Between October 1988 and April 1989 a cross-sectional survey was carried out in six out of eight blood banks of Goiânia, Central Brazil. Subjects attending for first-time blood donation in the mornings of the study period (n = 1358) were interviewed and screened for T. cruzi infection as a part of a major study among blood donors. Tests to anti-T. cruzi antibodies were performed, simultaneously, by indirect hem agglutination test (IHA) and complement fixation test (CFT). A subject was considered seropositive when any one of the two tests showed a positive result. Information on age, sex, place of birth, migration and socio-economic level was recorded. Results from this survey were compared with seroprevalence rates obtained in previous studies in an attempt to analyse trend of T. cruzi infection in an endemic urban area. The overall seroprevalence of T. cruzi infection among first-time donors was found to be 3.5% (95% confidence interval 2.5%-4.5% ). The seroprevalence rate increased with age up to 45 years and then decreased. Migrants from rural areas had higher seroprevalence rates than subjects from urban counties (1.8%-16.2% vs. 0%-3.6%). A four fold decrease in prevalence rates was observed when these rates were compared with those of fifteen years ago. Two possible hypotheses to explain this difference were suggested: 1. a cohort effect related with the decrease of transmission in rural areas and/or 2. a differential proportion of people of rural origin among blood donors between the two periods. The potential usefulness of blood banks as a source of epidemiological information to monitor trends of T. cruzi infection in an urban adult population was stressed.

The fungicidal action of sodium hypochlorite (0.3, 1, 2.5, 5 and 10%); formaldehyde (2, 5, and 10%); and ethyl alcohol (70%) on yeast forms of Paracoccidioides brasiliensis Pb 18 and a newly-isolated Goiana strain was described. Contact between the fungus and the disinfectants was maintained for 1, 2, 24, 48 and 72 hours at room temperature. Viability was evaluated by the fluorescein diacetate-ethidium bromide treatment, culture in solid and liquid media (36ºC and 26ºC); yeast to mycelial germination at room temperature; and radiometric study of metabolic activity. All concentrations of disinfectants were found to be effective in inactivating Pb 18 and Goiana strains, except for the 1-hour contact with 2% formaldehyde, in which fluorescein diacetate-ethidium bromide treatment was found to reveal 40 and 27% of viable cells, respectively. The yeast to mycelial germination method was considered to reveal faster and similar results as compared to culture in solid and liquid media.

Dez lotes de vacina contra a Raiva, tipo Fuenzalida & Palacios, líquida, uso humano, produzidos no Instituto Butantan, foram estocados às temperaturas de 45, 37, 28 e 2-8ºC. Amostras de cada lote, colhidas a intervalos de tempo que variavam com a temperatura de estocagem, tiveram sua potência (valor antigênico) determinada pelo método do N.I.H. Foram consideradas satisfatórias vacinas portadoras de um valor antigênico (VA) >0,3. Foi verificado que apenas um dos lotes apresentou VA inferior ao mínimo requerido, após 2 h de exposição a 45ºC. Todos os lotes mantiveram VA satisfatório até 3 dias de armazenamento de 37ºC, enquanto que, a 28 e 2-8ºC, o mesmo ocorreu com 10 e 360 dias, respectivamente. À temperatura ideal de armazenamento (2-8ºC), 100% dos lotes retiveram a potência mínima por período de tempo mais longo (16 meses) do que o prazo de validade (12 meses), o que permite sugerir que a validade da vacina contra a Raiva, que nos países da América Latina e Caribe é de 6 a 12 meses, poderia ser ampliada para 16 meses, de modo a evitar o descarte de produto em condições de uso.

The objective of the present study was to estimate the prevalence of herpes simplex virus type 2 (HSV 2) antibodies in child bearing women of 2 Brazilian populations with different socioeconomic status and to determine the risk of neonatal HSV exposure by means of maternal cultures at the onset of labor. The study was conducted at 2 hospitals: A, serving very low income patients and B, serving middle socioeconomic class. 173 participants from group A and 127 from B answered a questionnaire which showed that the patients had similar ages (27.7 and 26.8 years, respectively) but differed with regard to socioeconomic status, age at first intercourse (18.6 vs 20.6 years), number of sex partners (1.5 vs 1.2) and previous sexually transmitted diseases (15% vs. 1.5%). History of genital herpes was given by 11% of group A participants and by a similar number, 7%, of patients from group B. In addition, 200 serum samples from population A and 455 from B were tested by ELISA for and HSV antibodies and 92% and 86%, respectively, were found to be positive. Sixty seropositive samples from group A and 90 from B were further analyzed by Western blot, which showed the presence of type 2 specific antibodies in 46% and 36%, respectively, suggesting an overall HSV 2 prevalence of 42% in group A and 31% in B. Cervical specimens were obtained for culture from 299 asymptomatic patients of population A and 313 of B. HSV was isolated from one specimen in each group, indicating a 0.3% incidence of asymptomatic viral excretion in both populations. In conclusion, the prevalence of type 2 antibodies in childbearing women was very high, but it did not differ with the socioeconomic status. The risk of HSV perinatal transmission was also similar in the 2 study populations and it was comparable with the data from developed countries. Our findings do not indicate the need of special screening programs for asymptomatic HSV excretion in Brazilian pregnant women.

El método recomendado por la Organización Mundial de la Salud (OMS) para la prueba de potencia de vacunas antirrábicas como producto final es la prueba NIH. Algunas técnicas in vitro se han propuesto para el control durante el proceso de produción y complementan el ensayo in vivo antes mencionado. Este trabajo presenta los resultados obtenidos cuando se utilizó la técnica de contrainmunoelectroforésis (CIE) para determinar el contenido de antígenos en muestras de 84 y 40 lotes de vacunas antirrábicas producidas en tejido nervioso de cerebro de ratón lactante mediante cultivo de tejidos, respectivamente. La evaluación de las muestras en, y en torno de, las 0.3 UI por ambos métodos muestran que, en la práctica, un título CIE de 1:4 cumpliría con un mínimo de potencia de la prueba NIH. Un bajo grado de variabilidad de la prueba CIE fue observada en nuestro laboratorio cuando dos lotes de vacunas de referencia de trabajo y 7 lotes de vacunas antirrábicas, de diferente origen y actividad, fueron ensayadas en cinco pruebas independientes. Todos los títulos se ubicaron dentro de una dilución doble, lo que es indicativo de su reproducibilidad. Se observó buena sensibilidad para detectar el deterioro del antígeno en el ensayo de degradación térmica, cuando muestras de 3 lotes de vacuna líquida de cerebro de ratón lactante fueron mantenidas a4 y 37ºC cada una, por 28 días. Se evaluaron semanalmente por los ensayos de CIE y NIH. Finalmente, se observó que el ensayo de CIE podría ser utilizado por los productores para estimar el punto final de los procesos de concentración para que se corresponda con un valor antigénico deseado en la prueba de potencia NIH.

Cerebrospinal fluid (CSF) samples from 2083 patients with acquired immunodeficiency syndrome (AIDS) and neurological complications were bacteriologically examined during a period of 7 years (1984-1990). The percentage of patients who had at least one bacterial agent cultured from the CSF was 6.2%. Mycobacterium tuberculosis was the most frequently isolated agent (4.3%), followed by Mycobacterium avium complex or MAC (0.7%), Pseudomonas spp (0.5%), Enterobacter spp (0.4%), and Staphylococcus aureus (0.3%). Among 130 culture positive patients, 89 (68.5%) had M. tuberculosis and 15 (11.6%) had MAC. The frequency of bacterial isolations increased from 1988 (5.2%) to 1990 (7.2%), partly due to the increase in MAC isolations. Bacterial agents were more frequently isolated from patients in the age group 21-30 years and from women (p<0.05).
