O consumo de verduras cruas desempenha importante papel na transmissão de várias doenças infecciosas pela freqüente prática de irrigação de hortas com água contaminada. O objetivo deste estudo é a avaliação das condições higiênico-sanitárias de todas as hortas produtoras de verduras de Ribeirão Preto, SP com implantação de um sistema de fiscalização. A análise laboratorial de 129 hortas revelou irregularidades em 20,1% delas, destacando-se elevada concentração de coliformes fecais em 17%, presença de Salmonella em 3,1% e de vários enteroparasitas (Ascaris sp, Ancylostomidae, Strongyloides sp, Hymenolepis nana e Giardia sp ) em 13,1%. A repetição da análise das hortas irregulares determinou a interdição definitiva de uma delas; todas as demais foram aprovadas, comprovando a eficácia do sistema de fiscalização, particularmente com a implantação, inédita no país, do certificado de vistoria sanitária.

O presente trabalho tem como objetivo determinar a prevalência e a viabilidade de ovos e larvas de helmintos e cistos de protozoários presentes em biossólido e em esgoto submetido ao tratamento anaeróbio em estações de tratamento de esgoto onde são empregados reatores anaeróbios de lodo fluidizado (RALF), na região metropolitana de Curitiba, Paraná. Os parasitos presentes no esgoto e no lodo foram helmintos: Ascaris sp (85%), Toxocara sp (5,5%), Trichuris sp (4,5%), Hymenolepis diminuta (3,7%), H. nana (1%) e Taenia sp (0,4%), Protozoários: Isospora sp, Entamoeba coli, Entamoeba histolytica, Giardia lamblia, Endolimax nana. Houve diferença significativa quanto ao número de ovos viáveis de helmintos presentes no material de quatro estações estudadas. A redução da viabilidade dos ovos de helmintos variou de 59,7 a 93%. No tratamento biológico baseado na digestão anaeróbia a eficácia depende do tempo e da temperatura. Novos tratamentos higienizantes são necessários para a utilização do lodo produzido por digestão anaeróbia na reciclagem em agricultura ou para outros objetivos visando reduzir o risco para saúde humana e animal.

It is still controversial whether intestinal parasitic infections can influence the nutritional status of children. The relationship between protein-energy malnutrition, vitamin A and parasitic infections was evaluated in 124 children. The food intake estimated by recall method was generally low and poor. Seventy five percent of the children were infected with intestinal parasites. The mean±SD weight-for-age and height-for-age Z-score were skewed one standard deviation to the left, when compared to normal standards. An association was found between protein-energy malnutrition and Giardia lamblia, but not with Ascaris lumbricoides or Hymenolepis nana infection. Only Giardia-infected children had a decreased weight-for-age and weight-for-height Z-score. Hypovitaminosis A was a major nutritional problem, but no relationship between this deficiency and parasitic infection was found. Our data indicate that low and poor food intake were the major cause of protein-energy malnutrition among the children, and except for Giardia, this was not influenced by parasitic infections.

Foi realizado um levantamento das helmintoses intestinais em 18.973 escolares do primeiro grau (7 a 14 anos), da rede pública do Estado de Minas Gerais, utilizando o método de Kato-Katz de exame de fezes (duas lâminas por amostra). Foram estudadas 3 mesorregiões: Triângulo Mineiro/Alto Paranaíba (60 municípios), Noroeste de Minas (13) e Sul/Sudoeste (144). Entre os escolares examinados, 15.545 (82%) estavam negativos, 2.863 (15%) monoparasitados e 565 (3%) poliparasitados. A prevalência de A. lumbricoides foi de 10,3%, de T. trichiura 4,7%, de ancilostomídeos 2,9%, de E. vermicularis 1,2%, de H. nana 0,4% e de Taenia sp 0,2%. As maiores prevalências de helmintos/mesorregião foram de 24,2% para T. trichiura e 18,7% para A. lumbricoides (Sul/Sudoeste) e 12,1% para ancilostomídeos e 0,7% para Taenia sp (Noroeste de Minas). As helmintoses intestinais continuam sendo um grave problema de saúde pública, inclusive em áreas onde as condições socioeconômicas são mais favoráveis.

The objective of the present study was to estimate the frequency of infection by Cryptosporidium spp and other intestinal parasites in dehydrated children with gastroenteritis who were admitted to a pediatric hospital. Stool examinations from 218 children were performed. Cryptosporidium spp was identified in eighteen out of 193 stool samples (9.3%) subjected to safranin-methylene blue staining. Giardia lamblia was detected in ten out of 213 (4.7%) samples examined via the direct or Ritchie methods. Other parasites identified were Ascaris lumbricoides (4.2%), Blastocystis hominis (1.4%), Entamoeba coli (0.9%), Entamoeba histolytica/Entamoeba dispar (0.5%), Endolimax nana (0.5%), Trichuris trichiura (0.5%) and Enterobius vermicularis (0.5%).

The aim of this study was to determine the occurrence of intestinal parasites and commensals among children in four peripheral districts located in the northern, southern, eastern and western sectors of Uberlândia, Minas Gerais, using the Baermann methods as modified by Moraes and Lutz. Out of 160 individuals studied, 93 (58.1% CI: 50.4-65.7) were infected, distributed among the sectors as follows: northern (72.5%), southern (47.5%), eastern (57.5%) and western (55%). The positive findings according to age groups were: 0-5 years (26.9%), 5-10 years (21.2%) and 10-15 years (10%). Male children presented 2.7 times higher risk of infection than females did (OR: 2.7; CI: 1052-7001). The parasites and commensals identified were: Giardia lamblia (27.5%), Entamoeba coli (20.6%), Ascaris lumbricoides (14.4%), Enterobius vermicularis (8.8%), Hymenolepis nana (7.5%), Hymenolepis diminuta (5%), hookworms (3.1%), Trichuris trichiura (2.5%), Endolimax nana (2.5%), Entamoeba hartmanni (2.5%), Strongyloides stercoralis (1.3%), Iodamoeba butschlii (1.3%) and Capillaria hepatica (0.6%). The infection rate in these children was high and showed the need to implement prophylactic education programs in the community.

INTRODUCTION: Children are an important high-risk group for helminth and protozoa infections. Daycare centers are environments where children have proven to be more susceptible to acquiring intestinal parasites. Thus, the purpose of this study was to verify the prevalence of intestinal parasites in children who attended the two daycare centers maintained by the local government of Uberlândia, State of Minas Gerais, Brazil. METHODS: Fecal samples were collected from 133 children (73 children at the Public Preschool for Early Childhood Education, PPECE A, and 60 at the PPECE B) following identification according to sex and age and agreement to participate by parents or guardians who signed the free, informed consent form. The samples were examined by the Lutz method. RESULTS: Coproparasitological tests performed on 133 children showed that 29.3% of them were parasitized for enteroparasites or commensals, 6.7% of the children presented polyparasitism. Among the protozoa, Giardia lamblia were the most prevalent and Hymenolepis nana were the most frequent among the helminths. CONCLUSIONS: Thus, analysis of the results showed that intestinal parasites still represent a public health problem, especially among children and in areas where the socioeconomic and educational conditions are less favorable.

INTRODUCTION: Studies strongly indicate Dientamoeba fragilis as one of the causes of diarrhea in human immunodeficiency virus (HIV) patients. METHODS: The objective of the present study was to evaluate the prevalence of D. fragilis associated with the causes of diarrhea in 82 HIV/ AIDS patients hospitalized at the Instituto de Infectologia Emílio Ribas from September 2006 to November 2008. RESULTS: In total, 105 samples were collected from 82 patients. Unprotected sex was the most frequent cause of HIV infection (46.3%), followed by the use of injectable or non-injectable drugs (14.6%). Patients presented with viral loads of 49-750,000 copies/ mL (average: 73,849 ± 124,850 copies/mL) and CD4 counts ranging of 2-1,306 cells/mm³ (average: 159 ± 250 cells/mm³). On an average, the odds of obtaining a positive result by using the other techniques (Hoffman, Pons and Janer or Lutz; Ritchie) were 2.7 times higher than the chance of obtaining a positive result by using the simplified iron hematoxylin method. Significant differences were found between the methods (p = 0.003). CONCLUSIONS: The other techniques can detect a significantly greater amount of parasites than the simplified iron hematoxylin method, especially with respect to Isospora belli, Cryptosporidium sp., Schistosoma mansoni, and Strongyloides stercoralis, which were not detected using hematoxylin. Endolimax nana and D. fragilis were detected more frequently on using hematoxylin, and the only parasite not found by the other methods was D. fragilis.

Introduction This study evaluated the presence of pathogenic human parasites on field-grown strawberries in the Federal District of Brazil. Methods A total of 48 samples of strawberries and 48 soil samples from 16 properties were analyzed. Results Contaminated strawberries were detected in 56% of the properties. Schistosoma mansoni, Ascaris lumbricoides or Ascaris suum, Balantidium coli, Endolimax nana, and Entamoeba spp. were detected. Soil was contaminated with Entamoeba spp., Entamoeba coli, Strongyloides spp., Ancylostomatidae, and Hymenolepis nana. Conclusions Producers should be instructed on the safe handling of strawberries in order to reduce the incidence of strawberries that are contaminated with enteroparasites.

Estudo taxonômico das espécies neotropicais de Melittomma é apresentado, enfatizando as espécies distribuídas no território brasileiro. Discute-se brevemente os caracteres diagnósticos de M. brasiliense com dados de sua distribuição geográfica; descrições e ilustrações de três novas espécies, Melittomma nana sp.n. distribuída pela região Neotropical, Μ. brunnea sp.n. da Amazônia Central e M. panamensis sp.n. do Panamá, são apresentadas.

OBJETIVO: Comparar o grau de repercussão, fatores responsáveis e época de aparecimento dos sintomas do defeito do septo atrioventricular (DSAV), em pacientes com e sem síndrome de Down. MÉTODOS: Foram estudados 80 pacientes com idade <2 anos, sendo 55 (69%) com síndrome de Down - grupo I (GI) e 25 (31%) sem síndrome de Down - grupo II (GII). Avaliaram-se a idade de manifestação dos sintomas, sua intensidade, classe funcional (CF), repercussão clínica, tipo anatômico e grau de malformação da valva atrioventricular (VAV). RESULTADOS: A idade média de manifestação dos sintomas foi de 50 (±75) dias nos dois grupos. A CF II (NYHA) predominou no GI (31 casos - 56,5%) e a CF III-IV no GII (19 - 76%) p<0,005. O quadro de insuficiência cardíaca esteve presente em 34 (62%) pacientes do GI e em 21 (84%) do GII e, o de hipertensão pulmonar em 21 (38%) do GI e em 4 (16%) do GII, p<0,04. Pressão média da artéria pulmonar >50mmHg ocorreu em 56% dos casos do GI em relação a 28% do GII p<0,019. A evolução até a cirurgia foi instável (agravamentos da insuficiência cardíaca) em 33 (60%) do GI e em 21 (84%) do GII p<0,03. O tipo A de Rastelli foi o mais encontrado nos dois grupos, 35 (67%) pacientes no GI e nos 25 (100%) do GII. A alteração anatômica da VAV foi importante em 8% no GI e em 38% no GII. CONCLUSÃO: Há sugestão de predominância de hiper-reatividade vascular pulmonar nas crianças com síndrome de Down e de manifestações de insuficiência cardíaca nas geneticamente normais.

Ebstein's anomaly with coarctation of the aorta is an extremely unusual condition. In this report, the clinical and surgical features of 3 male patients, aged 7 months, 4 years and 14 years, are discussed. All patients were in situs solitus. The first 2 patients had atrioventricular and ventriculoarterial discordance and progressed to heart failure in the neonatal period. The third had atrioventricular and ventriculoarterial concordance, as well as Wolf-Parkinson-White syndrome, with frequent episodes of paroxysmal tachycardia. The 3 patients underwent surgery for correction of the coarctation of the aorta. The patient with atrioventricular and ventriculoarterial concordance underwent tricuspid valvuloplasty using a DeVega-like technique. In addition, ablation of 2 anomalous pathways (Kent bundle), which were detected by the electrophysiologic study, was also subsequently performed. The 3 patients showed a good postoperative outcome for 2 years, although, in those with discordance, the surgical procedure did not influence the dysplasia of the tricuspid valve, because this valve showed light to moderate dysfunction.

We report the case of an 8-month-old female infant with Uhl's anomaly, who underwent successful cardiac transplantation. The clinical findings, complementary laboratory tests, anatomic findings, and differential diagnosis of the anomaly are discussed.

OBJECTIVE: To compare immediate and late results in patients with or without fenestration who underwent cavopulmonary anastomosis so that we could assess the efficiency of the technique. METHODS: Sixty-two patients underwent surgery between 1988 and 1999, 41 with fenestration (group I -G I) and 21 without fenestration (group II -G II). Tricuspid atresia was prevalent in group I (23-56%) and single ventricle was prevalent in group II (14-66%). Mean ages at the time of operation were 7.3 years in group I and 7.6 in group II. At late follow-up, mean ages were 10.6 years in group I and 12.8 years in group II. RESULTS: Immediate and late mortality were 7.3% in G-I and 4.7% in G-II. Significant pleural effusion occurred in 41.4% of G-I patients and in 23.8% of G-II patients. Significant pericardial effusion occurred in 29.2% and 14.2%, respectively, in groups I and II. Central venous pressure was greater in G-II, 17.7 cm in H2O, as opposed to 15 cm in G-I. Hospital stay was similar between the groups, 26.3 and 21.8 days, respectively. Cyanosis and arterial insaturation occurred in 5 patients, and 4 patients were in functional class II, all from G-I. At late follow-up, 58 (93.5%) were in functional class I. Sinus rhythm was present in 94%, and pulmonary perfusion was similar in both groups. Eleven patients who underwent spirometry had good tolerance to physical effort. CONCLUSION: Atrial fenestration did not improve the immediate or late follow-up of patients who underwent cavopulmonary anastomosis, and is, therefore, dispensable.
