Serum samples from 356 HBsAg positive asymptomatic carriers, which were titrated by reverse passive hemagglutination, were analysed for the presence of HBV-DNA, HBsAg and IgM anti-HBc. The samples were divided in three classes, according to the titers of HBsAg and IgM anti-HBc and the distribution of HBV-DNA and HBsAg among these classes was studied. In the high titer class of HBsAg, 65% of samples have one or both markers against only 19% in the low titer class. From the total of 356 samples, 121 gave positive results for IgM anti-HBc (33.9%). From these, 38.9% of HBV-DNA and 47.9% of HBeAg were observed, whereas in samples with absence of IgM anti-HBc, 18.3% and 16.6% were respectively found. A higher frequency of agreement between all these markers was found in the class of high titers of HBsAg; however, HBV-DNA was detected in the low titer class of HBsAg and little or no IgM anti-HBc, showing potential blood infectivity even in HBsAg positive borderline samples.

Se refieren 3 casos autóctonos de rickettsiosis cutáneo ganglionar trasmitidos por garrapatas de perros (Amblyomma maculatum, en uno de ellos) en el Uruguay. Dos de los 3 casos fueron seguramente provocados por Rickettsia conorii de acuerdo a los resultados de la reacción específica de inmunofluorescencia indirecta - IgM, anti R. conorii. Se incluye un tercer paciente no estudiado con tal técnica, por la similitud clínico-epidemiológica, la reactividad del suero frente al Proteus OX 19 y la rápida respuesta a la tetraciclina. La no descripción previa de la rickettsiosis por R. conorii en forma autóctona en el área de las Américas confiere especial interés a la comunicación, recomendándose la búsqueda de la afección en otros países de la región.

Foram estudadas 37 amostras de sangue de pacientes com leptospirose, forma icterohemorrágica, com intervalo de tempo de 2 a 12 dias entre o início dos sintomas e a coleta do material. Isolou-se leptospiras por hemocultura de 5 (13,5%) pacientes e em 4 destes, o agente etiológico pertencia ao sorogrupo Icterohaemorrhagiae sorovar copenhageni. O teste imunoenzimático ELISA-IgM apresentou reatividade em 35 (94,6%) pacientes, incluindo os 4 pacientes dos quais o agente etiológico foi isolado. Este teste demonstrou ser um importante recurso laboratorial para o diagnóstico da letospirose humana, mesmo no início da doença quando ainda na fase de leptospiremia.

Specific anti-B19 IgM was demonstrated in sera from three children showing transient aplastic crisis. A two years-old boy living in Rio de Janeiro suffering from sickle-cell anaemia showed the crisis during August, 1990. Two siblings living in Santa Maria, RS, developed aplastic crisis during May, 1991, when they were also diagnosed for hereditary spherocytosis. For a third child from this same family, who first developed aplastic crisis no IgM anti-B19 was detected in her sera.

Se evaluó la utilidad de la prueba de aglutinación directa (AD) para diagnosticar el Mal de Caderas. Se emplearon cuarenta y cuatro sueros provenientes de dos lotes de equinos naturalmente infectados con el Trypanosoma evansi (Lote 1 y Lote 2). La AD fue positiva (Aglutinación > 1:512) en 13 de 16 equinos (81.2%), de los que se aislaron los parásitos. En doce de estos animales (92%) se detectaron IgM anti T. evansi mediante la AD realizada con 2-mercaptoetanol (AD + 2-ME). La AD fue positiva en 17 de los 28 equinos que resultaron negativos al diagnóstico parasitológico. Un tercer lote de cinco equinos infectados con T. evansi que presentaba elevados títulos de AD, fue tratado con Suramina Sódica (Nagano1-Bayer). La AD + 2-ME evidenció en cuatro animales que, gran parte de estos anticuerpos se ubicaban en la fracción IgM. Posterior al tratamiento farmacológico y, en concordancia de la negativización de la parasitemia, la detección de la IgM anti T. evansi no fue posible, mientras que los anticuerpos IgG continuaron detectándose a los doce meses post-tratamiento en valores de 1:512, en tres animales. Cincuenta sueros de equinos de una zona libre del parásito, que se emplearon como controles, fueron negativos a la AD. Se recomienda la utilización de la AD y AD + 2-ME para el uso de rutina en el diagnóstico del Mal de Caderas de los equinos, en combinación con otros métodos parasitológicos sensibles.

A seroepidemiologic survey about hepatitis A virus (HAV) infection was carried out in a group comprising 310 children, ranging in age from 3 months to 9 years, from day-care centers, in Goiania, a middle sized city in the central region of Brazil. The biomarkers employed in the investigation of previous infection include total IgG and IgM anti-HAV antibodies, and for the detection of more recent infection, IgM anti-HAV antibodies were analyzed. The study was performed in 1991 and 1992. According to the results, 69.7% of the children presented total IgG/IgM anti-HAV antibodies, with 60% of the group in the age range of 1 to 3 years. Among 10 day-care centers analyzed, the prevalence of the biomarker IgM anti-HAV was 3.2%, with an uniform distribution of the cases in the group of children ranging in age from 1 to 4 years. Multi-variate analysis was performed to investigate the sociodemographic factors that could influence the results. It was verified that the risk for the infection increased with the length of the attendance in the day-care centers, i.e., the risk for children with attendance of one year or more was 4.7 times higher, when compared with children with one month attendance (CI 95% 2.3-9.9). According to the results, hepatitis A is an endemic infection in day-care centers in the study area. The length of attendance in the day-care settings was demonstrated to be a risk factor for the HAV infection. Such findings suggest that if hepatits A vaccination becomes available as a routine policy in our region, the target group should be children under one year. Moreover, those children should receive the vaccine before they start to attend the day-care centers.

Twenty-seven mycologically proven cases of paracoccidioidomycosis (PCM) were treated with itraconazole (100-200 mg/day in month 1 and 100 mg/day until month 6-8) and evaluated clinically and serologically, up to 3.5 years post-therapy, using Dot-blot and ELISA for measuring the titers of IgG, IgA and IgM anti-P. brasiliensis antibodies and Western-blot for determining IgG, IgA and IgM antibodies against the antigen components of the fungus. Before treatment, 81.5% (Dot-blot) and 84% (ELISA) of the patients presented elevated IgG anti-P. brasiliensis antibody titers which dropped slightly with treatment. On the other hand, the percentages of pre-treatment high-titered sera for IgA and IgM anti-P.brasiliensis were lower (5l.9% and 5l.8%: Dot-blot; 16.5 and 36%: ELISA, respectively) but the titers tended to become negative more frequently with treatment. Prior to treatment, the percentages of positivity for IgG, IgA and IgM anti-P.brasiliensis antibodies in Western-blot were 96%, 20.8% and 41.6%, respectively. Antigens with molecular weights varying from 16-78 kDa, from 21-76 kDa and from 27-78 kDa were reactive for IgG, IgA and IgM antibodies, respectively. The most frequently reactive antigenic components had molecular weights of 27, 33 and 43 kDa for IgG, and 70 for IgA and IgM antibodies. During the period of study, the patients responded well to treatment. The present data confirm the diversity and complexity of the humoral response in PCM, and the importance of utilizing different serological tests to detect IgG, IgA and IgM anti-P. brasiliensis antibodies

Some viruses of the families Retroviridae, such as Human T Lymphotropic Virus (HTLV); Herpesviridae as the Cytomegalovirus (CMV) and Hepadnaviridae such as the Hepatitis B Virus (HBV) are liable to be co-transmitted with the Human Immunodeficiency Virus (HIV). Since prisoners are exposed to several and important risk factors involved in the transmission of HIV and the above mentioned viruses, male inmates from the penitentiary complex of Campinas, SP, Brazil, including HIV + and HIV - ones, were examined for the presence of HTLV-I and/or II antibodies; IgG and IgM anti-CMV antibodies, and the research of the superficial hepatitis B antigen (HBsAg). The presence of anti-HTLV-I and/or II was determined by the Western Blot (WB) technique, whereas IgG and IgM anti-CMV and the search of HBsAg were carried out by the Microparticle Enzyme Immunoassay (MEIA-Abbott Lab).With regard to anti-HTLV-I and/or II, 58.3% (14/24-Number of positive reactions/number of sera examined) were reactive among the anti-HIV positive sera. Conversely, only 12.5% (3/24) among the HIV- negative sera showed positive reactions to HTLV-I and/or II antibodies. When looking for IgG anti-CMV percentages of 97.7% (43/44) and 95% (38/40) were obtained for anti-HIV positive and negative sera, respectively. As to IgM anti-CMV antibodies 11.36% (5/44) and 2.5% (1/40) of reactive sera were found for anti-HIV positive and negative, respectively. The HBsAg was found in 12.8% (5/39) of the sera which were anti-HIV positive.

This study evaluates the transmission of CMV infection in 120 children aged 1 to 15 years with Down syndrome who attended a day-care center for handicapped children in São Paulo, Brazil. A blood sample was obtained from each children at the beginning of the study for detection of IgG and IgM cytomegalovirus (CMV) antibodies by an immunofluorescence assay. Samples of saliva and urine were obtained every 3 months from the children with CMV antibodies to detect shedding of the virus by culture in human foreskin fibroblasts, by detection of pp65 CMV-antigen and by a nested PCR assay. The prevalence of anti CMV-IgG antibodies was 76.6% (92/120), and IgM anti-CMV antibodies were detected in 13% (12/92) of the seropositive children. During the first viral evaluation, CMV was detected in the urine and/or saliva in 39/90 (43.3%) of the seropositive children. In the second and third evaluations, CMV was detected in 41/89 (46%) and in 35/89 (39.3%) children, respectively. Detection of CMV was shown both in urine and saliva in 28/39 (71.8%), 19/41(46.3%) and 20/35 (57.1%) of the children excreting the virus, respectively. Additionally, in 33/49 (67.4%) of the excreters CMV could be demonstrated in urine or saliva in at least two out of the three virological evaluations carried out sequentially in a six month period. Of the 28 initially seronegative children, 26 were re-examined for anti-CMV IgG antibodies about 18 months after the negative sample; seroconversion was found in 10/26 (38.5%). Taking all 536 samples of urine or saliva examined by virus culture and pp65 antigen detection during the study into account, 159 (29.6%) were positive by virus culture and 59 (11%) gave a positive result with the pp65 assay. These data demonstrate the high prevalence of CMV shedding and the high risk of CMV infection in children with Down syndrome attending a day-care center for mentally handicapped patients. The virus culture was more sensitive than the pp65 CMV antigen assay for CMV detection in both urine and saliva samples.

A thirty three year-old, male patient was admitted at the Hospital of the São Paulo University School of Medicine, at the city of São Paulo, Brazil, with complaint of pains, tingling and decreased sensibility in the right hand for the last four months. This had progressed to the left hand, left foot and right foot, in addition to a difficulty of flexing and stretching in the left foot. Tests were positive for HBeAg, IgM anti-HBc and HBsAg, thus characterizing the condition of acute hepatitis B. The ALT serum level was 15 times above the upper normal limit. Blood glucose, cerebral spinal fluid, antinuclear antibodies (ANA) and anti-HIV and anti-HCV serum tests were either normal or negative. Electroneuromyography disclosed severe peripheral neuropathy with an axon prevalence and signs of denervation; nerve biopsy disclosed intense vasculitis. The diagnosis of multiple confluent mononeuropathy associated to acute hepatitis B was done. This association is not often reported in international literature and its probable cause is the direct action of the hepatitis B virus on the nerves or a vasculitis of the vasa nervorum brought about by deposits of immune complexes.

The aim of the present work was to determine the prevalence of IgG and IgM anti-Toxoplasma gondii antibodies and the factors associated to the infection in pregnant women attended in Basic Health Units in Rolândia, Paraná, Brazil. The sample was divided in two groups: group I (320 pregnant women who were analyzed from July 2007 to February 2008) and group II (287 pregnant women who were analyzed from March to October 2008). In group I, it was found 53.1% of pregnant women with IgG reactive and IgM non-reactive, 1.9% with IgG and IgM reactive, 0.3% with IgG non-reactive and IgM reactive and 44.7% with IgG and IgM non-reactive. In group II, it was found 55.1% with IgG reactive and IgM non-reactive and 44.9% with IgG and IgM non-reactive. The variables associated to the presence of IgG antibodies were: residence in rural areas, pregnant women between 35-40 years old, low educational level, low family income, more than one pregnancy, drinking water which does not originate from the public water supply system and the habit of handling soil or sand. Guidance on primary prevention measures and the quarterly serological monitoring of the pregnant women in the risk group are important measures to prevent congenital toxoplasmosis.

Nonhuman primates are considered as the natural hosts of Hepatitis A virus (HAV), as well as other pathogens, and can serve as natural sentinels to investigate epizootics and endemic diseases that are of public health importance. During this study, blood samples were collected from 112 Neotropical primates (NTPs) (Sapajus nigritus and S. cay, n = 75; Alouatta caraya, n = 37) trap-captured at the Paraná River basin, Brazil, located between the States of Paraná and Mato Grosso do Sul. Anti-HAV IgG antibodies were detected in 4.5% (5/112) of NTPs, specifically in 6.7% (5/75) of Sapajus spp. and 0% (0/37) of A. caraya. In addition, all samples were negative for the presence of IgM anti-HAV antibodies. These results suggest that free-ranging NTPs were exposed to HAV within the geographical regions evaluated.

Em sete pacientes submetidos a transplante de rim e imonodeprimidos reconheceram os Autores a ocorrência de doença de Chagas, em fase aguda. Tais eventos permitiram verificações sem dúvida expressivas e, entre elas, afigurou-se conveniente salientar as seguintes: presença de febre como manifestação proeminente; valor elucidativo da pesquisa do Trypanosoma cruzi no "creme" leucocitário em etapa de realização de diagnóstico diferencial, quando escassos os elementos orientadores; utilidade da evidenciação, por imunofluorescência, de anticorpos IgM antitripanossomo no soro; falta de participação de enfartamento de linfonodos superficiais ao exame clínico e de linfocitose, com linfócitos atípicos, ao hemograma; decurso não grave, sendo que, para tanto, pode ter contribuído a administração de benzonidazol; possibilidade de manutenção do uso de medicamentos imonossupressores; ausência de prejuízo quanto ao transplante. Julgaram os Autores oportuno relatar esses fatos, em virtude de implicações que eles ensejam quando em desenvolvimento trabalhos de natureza assistencial.

Foi padronizado um teste de hemaglutinação para a sorologia da malária humana, com reagente constituído de suspensão de hemácias de camundongos infectadas pelo Plasmodium berghei e preservadas por fixação aldeídica. Em pacientes com parasitemia por P. falciparum ou P. vivax obteve-se uma sensibilidade de 98,9% nos 88 casos estudados, o teste apresentando títulos entre 40 e 640. Para o grupo de 476 soros de indivíduos não-maláricos, obteve-se uma especificidade de 96,0%. O teste apresentou elevada reprodutibilidade, mesmo para diferentes lotes de antígenos. Nos 200 soros, obtidos ao acaso, de indivíduos de área endêmica, o teste apresentou positividade de 48,5%, contra 88,0% do teste de imunofluorescência-IgG. A baixa positividade pode ser devida a que o teste de hemaglutinação detecta anticorpos IgM. Após tratamento com 2-mercaptoetanol, todos os soros de pacientes com parasitemia tornaram-se não reagentes. Em relação ao teste de imunofluorescência-IgG, o teste de hemaglutinação apresentou índice de co-positívidade de 0,989 para os soros de maláricos com parasitemia. Para os soros de não-maláricos o teste de hemaglutinação apresentou índice de co-negatividade de 0,969. Por outro lado, no grupo de soros de área endêmica, o índice de co-positividade foi de 0,528 e o de co-negatividade, de 0,833.

Para avaliar a participação etiológica do Mycoplasma pneumoniae em infecções respiratórias agudas, o isolamento dessa bactéria foi tentado em secreções do aparelho respiratório de 64 pacientes (média 24 anos) com quadro respiratório aguda Foi realizada, também, a pesquisa de anticorpos específicos anti-M. pneumoniae através da reação de fixação do complemento (FC) e da reação de contra-imunoeletroforese (CIE). O M. pneumoniae não foi isolado. O diagnóstico presuntivo de infecção pelo M. pneumoniae foi feito pela FC em 3,1% (2/64) e pela CIE em 1,6% (1/64) dos pacientes. Paralelamente, em 200 indivíduos sadios, os mesmos testes sorológicos foram realizados, sendo o índice de positividade de 4% (8/200) pela CIE e de 1% (2/200) pela FC. Apesar das aiscrepâncias observadas entre os dois métodos sorológicos, a FC parece ser indicada para diagnóstico da infecção, sendo a CIE recomendada nas avaliações soroepidemiológicas. Com base nos dados do nosso estudo, a prevalência das infecções respiratórias pelo M. pneumoniae parece ser baixa em nosso meio.
