O presente trabalho integra um amplo projeto de educação em saúde, que visa desenvolver e avaliar estratégias e materiais para escolas de 1º grau. Nesta parte do estudo, complementar ao artigo anterior (SCHALL et al.), através de entrevistas e exames parasitológicos de fezes realizados em quatro escolas municipais da periferia de Belo Horizonte, buscou-se investigar a prática pedagógica e a prevalência de helmintíases entre os escolares. Os resultados demonstraram que: a) tanto professores quanto alunos apresentaram escassas informações a respeito das helmintoses, assim como noções incorretas e desconhecimento de mecanismos de transmissão; b) altas prevalências nas escolas sendo de 68,0% e 76,0% (Bairro Gorduras) e 46,0% e 24,0% (Vale do Jatobá e Barreiro de Cima) no 1º ano de estudo (1988). Comparando-se as quatro escolas, encontrou-se uma associação das condições habitacionais dos alunos e os índices de prevalência. Considerando o fato de tais helmintíases serem sabidamente prevalentes a longo prazo nessas regiões, percebe-se quão alheia está a escola em relação aos problemas da realidade de seus escolares.

A serological survey, involving indirect immunofluorescence testing of blood sera samples, was carried out on the residents of one in every five dwellings in the town of Barcelos (in the northern part of the State of Amazonas, on the right bank of the Rio Negro, 490 Km from Manaus by river) and on the rural populations of the villages of Piloto and Marará (also on the right bank of the Rio Negro, 30 minutes by boat from Barcelos). A total of 710 sera samples were tested, 628 from the resident population in the town of Barcelos, 35 from Piloto and 47 from Marará. The tests were carried out using human anti-gammaglobulin type IgG (Biolab) and antigen from formolized culture of T.cruzi Y strain. The sera were serially diluted from 1:40 to 1:320 in PBS 7.2. Of the 710 samples examined 89(12.5%) were positive for anti-T.cruzi antibodies: 2 of these (2.2%) at a dilution of 1:320; 12(13.4%) at 1:160; 38 (42.6%) at 1:80; and the remainder at 1:40, giving a median serological dilution of 1:80. The following questions are discussed: the high serological prevalence for Chagas'infection found in our survey; the possibility of serological cross-reactions; the need for confirmatory tests for the positives reactions; and the strong correlation between our results and preliminary epidemiological data (such as the level of human contact with wild triatominae, know locally as "Piacava's lice". We draw attention to the isolation by xenodiagnosis of one strain of T.cruzi from a patient with positive serology for Chagas' infection.

This study reports preliminary results of virus neutralizing antibody (VNA) titers obtained on different days in the course of human anti-rabies immunization with the 2-1-1 schedule (one dose is given in the right arm and one dose in the left arm at day 0, and one dose is apllied on days 7 and 21), recommended by WHO for post-exposure treatment with cell culture vaccines. A variant schedule (double dose on day zero and another on day 14) was also tested, both employing suckling mouse brain vaccine. A complete seroconversion rate was obtained after only 3 vaccine doses, and almost all patients (11 of 12) presented titers higher than 1.0 IU/ml. Both neutralizing response and seroconversion rates were lower in the group receiving only 3 doses, regardless of the sample collecting day. Although our results are lower than those found with cell culture vaccines, the geometry mean of VNA is fully satisfactory, overcoming the lower limit recommended by WHO of 0.5 IU/ml. The 2-1-1 schedule could be an alternative one for pre exposure immunization, shorter than the classical 3+1 regimen (one dose on days 0, 2, 4 and 30) with only three visits to the doctor, instead of four.

The vertical transmission of the human T-cell lymphotropic virus type I (HTLV-I) occurs predominantly through breast-feeding. Since some bottle-fed children born to carrier mothers still remain seropositive with a frequency that varies from 3.3% to 12.8%, an alternative pathway of vertical transmission must be considered. The prevalence rate of vertical transmission observed in Japan varied from 15% to 25% in different surveys. In Brazil there is no evaluation of this form of transmission until now. However, it is known that in Salvador, Bahia, 0.7% to 0.88% of pregnant women of low socio-economic class are HTLV-I carriers. Furthermore the occurrence of many cases of adult T-cell leukemia/lymphoma and of four cases of infective dermatitis in Salvador, diseases directly linked to the vertical transmission of HTLV-I, indicates the importance of this route of infection among us. Through prenatal screening for HTLV-I and the refraining from breast-feeding a reduction of ~ 80% of vertical transmission has been observed in Japan. We suggest that in Brazil serologic screening for HTLV-I infection must be done for selected groups in the prenatal care: pregnant women from endemic areas, Japanese immigrants or Japanese descendents, intravenous drug users (IDU) or women whose partners are IDU, human immunodeficiency virus carriers, pregnant women with promiscuous sexual behavior and pregnant women that have received blood transfusions in areas where blood donors screening is not performed. There are in the literature few reports demonstrating the vertical transmission of HTLV-II.

Serum- and/or- cerebrospinal fluid (CSF) samples obtained from 190 patients suffering from chronic, progressive neurological disease were screened for the presence of human T-cell lymphotropic viruses type I (HTLV-I) and type II (HTLV-II) antibodies over a six-year period (1996 to 2001) in Belém, Pará, Brazil. Patients were of both sexes (male subjects, 52%) with ages ranging from 2 to 79 years (mean, 35.9). Overall, 15 (7.9%) subjects - of whom 12 (80%) were female adults - reacted HTLV-I/II-seropositive when screened by enzyme-linked immunosorbent assay (ELISA). Serum samples from 14 of these patients were also analyzed using a recombinant Western blot (WB) assay that yielded HTLV-I-, HTLV-II-, and HTLV-I/II- reactivities for 10 (71.4%), 3 (21.4%) and 1 (7.2%) of them, respectively. The yearly rates of HTLV-I/II antibodies ranged from 2.6% (2001) to 21.7% (2000), with progressively increasing seropositivities from 1998 to 2000. Altogether, walking difficulty (n = 5 subjects), spasticity (n = 4) and leg weakness (n = 3) accounted for 80% of symptoms recorded among the 15 patients whose sera had antibodies to HTLV-I/II as detected by ELISA. These findings provide evidence that both HTLV-I and HTLV-II play a role in the development of chronic myelopathy in Belém, Pará, Northern Brazil.

Os autores relatam os resultados obtidos no diagnóstico laboratorial de varíola, durante o segundo ano de funcionamento de uma unidade montada no Instituto Presidente Castello Branco, da Fundação Instituto Oswaldo Crus, no Rio de Janeiro. O exame de 105 espécimens de crostas e de 76 de líquido vesicular /pustular, forneceu 39 e 34 amostras de vírus da varíola, respectivamente (Tabela 1). A demora em chegar ao laboratório influencia significativamente a taxa de isolamento de vírus (Tabela 2). Foi encontrada estreita relação entre os diagnósticos clínico e laboratorial (Tabela 3), quando possível compará-los. A inoculação em ovos embrionados após 1 a 2 horas do abaixamento da mebrana cório-alantóica, foi considerada como adequada às condições em que são realizados os exames. O laboratório continua a receber regularmente mais especimens para diagnóstico.

As part of the health survey on the Ilha da Conceição, a 25 percent random sample of households was identified and a health guestionnaire completed at 236 households. These households contained 536 children, of whom 239 were under age six. Prenatal care had been obtained by 70 percent of the mothers during the pregnancies of the youngest children and 83 percent of these children had been born in hospital. The use of available health facilities was reported more frequently for the younger children in comparison to the older children. Over 90 percent of the children had been vaccinated against one or more diseases but only 50 to 60 percent of the children had complete vaccination against pertussis, diphtheria and tetanus. Almost two-thirds of the stool specimens from the children revealed evidence of parasites and were most commonly found in children two to three years of age. Low hemoglobin values were found commonly under age three and hemoglobin leveis ábove 12 grams were not commonly found until age six. Compared with a North American standard for height and weight, proportionately more children on the Ilha da Conceição were found below the 25th and 3rd percentiles. These findings suggest that an improved health status for the children on the Ilha da Conceição would result from a household health record maintained at the island clinic including current information on vaccination status of all children, and a health education program focused on expectant mothers and the well baby clinic program.

The development of integrated measures which involve sterile mate release to supplement the conventional insecticidal techniques used in controlagainst insects of medical importance, raised the question, whether the vectors of Chagas'disease possess the natural mechanisms by manipulation of which they may be controlled. Results of earlier expenments, that had been published previously, were restricted to fragmentary information that raised various questions, the answer to which became available in the study herein described. Interspecific hybrids were produced from reciprocal crosses between T. pseudomaculata and T. sórdida and from unilateral crosses between female T. pseudomaculata and male. T. infestans. These females mated with males, laid less than the normal complement of eggs, but offspring was relatively abundant. When T. pseudomaculata females were paired with T. brasiliensis males, hybridization was more difficult because few of the females mated and those that did had a strongly reduced fertility. Adults emerged from ali crosses but exhibited sex disproportion, females predominating in all populations but one. The two Rhodnius species tested were also found to cross, but only when female R. prolixus were paired with male R. neglectus. These females laid a relatively high complement o f eggs, had a strongly reduced fertility, but 50% of the fertile eggs developed into vigorous adults, males predominating females. Neither type of hybrid male elicited fertilized eggs from either parental type of female, through their vesicula seminal is were found to be packed with spermatozoa, some normal looking and moving, others underdeveloped and motionless. Although, no artificial insemination was performed, the sperm in itself did not appear to be the prime inducer of sterility. Females paired with these hybrids did mate, sperm was transfered, as evidenced by the discharged spermatophores smeared with sperm, but did notcontain spermatozoa in their spermatecae. The failure of the sperm to migrate to the spermatecae indicate prezygotic pos-copulation incompatibility, thus the hybrid male can't be used to suppress populations. The female hybrids mated with parent males of either species had reduced fertility and ther sons were sterile as were those of their fertile daughters. However, continous backcrossing of the hybrid females and their female progeny to parental males partially restored fertility of the males and increased fertility of females, as scored by egg hatchability. Fertility of hybrid females, measured by the yield of adults capable to reproduce, indicated that the reproductive perfomance decreased when hybrid females and their daughters were backcrossed additional generations to parental males. It is tentatively suggested that hybrid females could be used for suppression if they compete efficiently with wild females.

Os Autores descrevem um caso grave de esquistossomose mansoni (forma hepática com hipertensão portal associada a forma pulmonar com hipertensão pulmonar e cor pulmonalej sobre o qual evoluiu uma sindrome toxi-infectuosa grave e de longa duração. Sucessivos exames radiológicos do tórax revelaram acometimento predominantemente arteriolar, ao lado de uma micronodulação pulmonar grosseira e difusa em ambos os hemitóraces e configuração de coração pulmonar. A histopatologia de material de biópsia pulmonar identificou basicamente uma arterite pulmonar característica, inflamação granulomatosa atípica provocada principalmente por vermes e raros ovos de S. mansoni envolvidos por escassa reação inflamatória. Para explicar a origem de constelação clínica toxi-infectuosa, foram afastadas as hipóteses de associação da esquistossomose a concausas infectuosas ou não, mas de curso febril, e a superposição de uma forma toxêmica sobre outra crônica pré-existente. Com base em dados clínicos, particularmente em subsídios da laparoscopia, e anatômicos, concluiu-se tratar o caso de uma forma crônica de esquistossomose reativada, provavelmente em virtude de alterações imunológicas inusitadas do hospedeiro. Ao que tudo parece indicar, o desvio de ovos e de vermes aos pulmões deveu-se à sindrome de hipertensão portal, cujos shunts entre a circulação portal e sistêmica determinaram a rota preferencial da migração.

Em inquérito pela intradermorreação de Montenegro, realizado em 402 pessoas da população da Praia Vermelha, Ilha Grande, município de Angra dos Reis, RJ, os autores observaram uma prevalência de 11,94% de positividade; não ocorreram diferenças estatisticamente significativas entre sexo e faixas etárias. Esse Inquérito ainda revelou que o risco em adquirir a infecção é de 3 vezes maior na população masculina que trabalha na área durante todo o ano, quando comparada à população de pescadores que se ausenta da área por longos períodos; também revelou um risco 8 vezes maior nos moradores dos domicílios onde ocorreu um caso humano da doença.

The clinical records of 182 patients with cutaneous leishmaniasis probably due to Leishmania braziliensis braziliensis are analysed. 68% had a single lesion which was usually an ulceron the lower anterior tibial third. Many had short histories of one to two months and all age groups were represented 13% had closed lesions of a verrucose or plaque like nature. Evolution of these skin lesions after treatment was related to the regularity of antimony therapy. Although healing usually occurred in three months, the time to scarring after commencing treatment was variable and related to the size ofthe lesion (p < 0.01). Usually if sufficient antimony treatment was given the lesion closed. Seven of the ten patients with initially negative leishmanin skin tests converted to positive after treatment. A significant decline of indirect fluorescent antibody titres occurred in patients followed, during and after therapy.

Foi comparada, em laboratório, a atividade moluscicida do extrato hexânico da casca da castanha do caju - Anacardium occidentalel L. (EHCCC), do complexo de cobre (II), docomplexo de chumbo (II) e do ácido anacárdico com objetivo de encontrar entre eles um produto que apresentasse maior estabilidade que o ácido anacárdico. Este foi preparado tratando o EHCCC com hidróxido de chumbo (II) ou com o sulfato de cobre mais hidróxido de sódio ou com hidróxido de cobre (II). Em seguida, o complexo de chumbo (II) ou os complexos de cobre (II)preparados foram tratados com uma solução de ácido sulfúrico diluída. As misturas dos dez produtos obtidos foram testadas sobre caramujos adultos de Biomphalaria glabrata nas concentrações de la 10 ppm. Os mais ativos foram o complexo de cobre (II), obtido com sulfato de cobre mais hidróxido de sódio, e o ácido anacárdico (hidróxido de chumbo) que apresentaram atividade a partir da concentração de 4 ppm. O teor de chumbo do ácido anacárdico (hidróxido de chumbo) foi acima das normas recomendadas pelos Padrões de Saúde Pública dos Estados Unidos.

INTRODUTION: A major concern with the visceral leishmaniasis (VL) is its high lethality rate, even with proper treatment. Low age, prior malnutrition, disease duration prior to diagnosis, severe anemia, fever for more than 60 days, diarrhea and jaundice are known poor prognostic factors. The goals of this study are to describe the clinical and laboratory characteristics of VL among children under 12 years of age and to identify the factors associated with VL poor outcome. METHODS: Two hundred and fifty children under 12 years of age with confirmed VL admitted to Hospital João Paulo II (FHEMIG), Belo Horizonte, Brazil, between January 2001 and December 2005 were evaluated retrospectively. The primary outcome was the poor clinical evolution: sepsis, and/or pneumonia, and/or urinary tract infection, and/or of bleeding (expect epistaxis), and/or severe neutropenia (neutrophil < 500 cells/mm3). Odds ratio (crude and adjusted) and its 95% confidence interval for each variable were calculated. Values less than 0.05 were considered significant. RESULTS: Average age was 3.3 years (3.6 months-11.6 years), 71.2% were younger than 5 years and 47.2% lived in Metropolitan Area of Belo Horizonte. The mean fatality rate was 3.6%. Sixty-six (26.4%) patients presented poor evolution. After a multivariate analysis, age <18 months, abnormal respiratory physical examination on hospital admission, and platelets <85,000/mm3 remained associated with increased chance of poor evolution. CONCLUSIONS: The results suggest that patients aged between 12 and 18 months, with platelet counts bellow 85,000/mm3, and respiratory abnormalities at admission should be considered potentially severe.

Um inquérito nutricional foi realizado na população ribeirinha do rio Negro, no Estado do Amazonas, numa área de baixíssima densidade populacional. Todas as famílias com crianças abaixo de 6 anos, num trecho de 20OKm entre a foz do rio Branco e a cidade de Barcelos foram estudados num total de 60 famílias e 121 crianças. Este número de crianças representa cerca de. 4,8% das crianças da área rural, dos Municípios envolvidos. Todas as crianças foram submetidas a exame clínico, medidas e pesadas e, de 66 crianças, foi colhido sangue por punção digital para determinação de, hematócrito e, hemoglobina. Foram coletadas amostras de fezes de 78 crianças, para exame panasitológico. Todas as mães foram também medidas e pesadas, além de entrevistadas quanto a hábtos de higiene, alimentação da criança e alimentos por ela ingeridos nas últimas 24 horas, De 42 mães, foi colhido sangue pana determinação de hematúcrito e hemoglobina.As condições de higiene da população local são bastante primitivas. A água. é consumida diretamente do rio, e a defecação efetuada ao nível do solo, em torno das casas. A natimortalidade for estimada em 80/1000 nascimentos, abortos espontâneos de 66,7/1000 gestações e mortalidade infantilde 93,2/1000 nascidos vivos. Todos esses índices são elevados para as condições locais. 0 período de amamentação na área é prolongado e o desmame ocorre em torno de 16 meses, com 72% das crianças amamentadas por mais de 1 ano. Metade das mães introduzem outros alimentos na dieta infantil em torno de 6 meses, principalmente papa de farinha de. mandioca. De acordo com os critérios de Gomez, 63,3% das crianças são malnutridas, enquanto que segundo os critérios de Waterlow, 70% apresentavam nanismo nutricional e 18% atrofia nutricional. No primeiro se -mestre de vida, o nanismo nutricional, atingiu somente 15,8% das crianças, enquanto que no segundo semestre esse valor se eleva para 63,6%. Apesar da amamentação prolongada , 25% das crianças, no primeiro ano de vida, apresentaram diarréia no momento do exame. Os picos na prevalência de diarréia foram observados no segundo semestre e. segundo ano de vida, coincidindo com a alta prevalência de desnutrição aguda (atrofia nutricional).A desnutrição crônica (nanismo nutricional) foi significantemente mais prevalente nas crianças com histórias de quadros diarréicos freqüentes. Parasitas intestinais foram encontrados em 84,6% das crianças cujas fezes foram coletadas, sendo que A. lumbricoides, Ancilostomideo e T. trichiura afetando mais de 60%. Os sinais clínicos de deficiências de ferro e possivelmente vitaminas, tais como riboflavina e vitamina A, foram as mais freqüentemente observados.Níveis de hemoglobina abaixo de 11 g% e concentração de Hemoglobina Corpuscular média abaixo de 30% foram observados em 71,2% das crianças, sugerindo que anemia feropriva é altamente prevalente na área.Obesidade foi mais freqüentemente encontrada nas mães do que emagrecimento, sugerindo que o consumo total de energia e alimentos não é limitante nessa população. Peixe foi consumido nas últimas 24 horas por 75% das mães e 50% consumiu de mandioca é altíssimo. O consumo de frutas foi limitado, principalmente bananas, e o uso de vegetais limitado a condimentos. Os níveis de hemoglobina foram baixos em 62% das mães, sugerindo que a deficiência de ferro é um problema na área, apesar da dieta ser alta em proteinas e baixa em fibras e fitato.
