INTRODUÇÃO: Visando atualizar suas práticas pedagógicas, atender as exigências da comunidade, da reestruturação do sistema de saúde e os avanços tecnológicos, a Faculdade de Ciências Médicas da Universidade Estadual de Campinas implementou uma grande reforma curricular para alunos ingressantes de 2001. OBJETIVO: Descrever uma experiência de ensino voltada à integração dos conhecimentos para atenção aos indivíduos nas diversas fases da vida, dentro da realidade de assistência primária à saúde, com ênfase no conhecimento, nas habilidades clínicas, na responsabilização e nas atitudes humanísticas e éticas. MÉTODOS: No novo currículo, a integração intra, inter e transdisciplinar foi estruturada em módulos interdepartamentais, inserção progressiva das disciplinas clínicas, contato mais cedo e progressivo do aluno com a sistema de saúde, preservando módulos integradores horizontais e verticais. A iniciação da prática clínica em Centros de Saúde tem, no quarto ano, 432 horas destinada a atendimentos clínico-ambulatoriais de assistência à criança, à mulher, ao adulto e ao idoso num contexto de saúde da família. A supervisão é realizada por professores, médicos assistentes da Faculdade e tutores selecionados entre os profissionais da rede primária de saúde. O Programa Nacional de Reorientação da Formação Profissional em Saúde Pró-Saúde facilitou a inserção e a parceria do curso de medicina com as UBS. O conteúdo teórico é integrado em seminários ministrados em dois períodos semanais e avaliado por meio de provas teóricas (conhecimento cognitivo). As habilidades e competências nas atividades clínicas são avaliadas por meio de discussões teórico-práticas quinzenais ao longo do estágio, avaliações clínicas estruturadas de atendimentos à criança, mulher e adulto, além da composição de portfólio com planilha de atendimentos totais, casos selecionados para revisão e auto-crítica de aprendizado. RESULTADOS: O módulo foi avaliado na forma de fóruns semestrais de discussão, com participação de discentes, docentes, tutores e gestores. Os grupos foram unânimes em considerar plenamente atingidos os objetivos de responsabilização, vínculo e ética, e parcialmente atingida a integração dos conteúdos teórico-práticos e trabalho em equipe. CONCLUSÃO: O currículo integrado propiciou uma visão clínica abrangente da família. Permitiu que o estudante se responsabilizasse e criasse vínculo com o paciente, entendendo a resolutividade e demandas da atenção básica à saúde por meio de sua vivência.

Among the building materials used in rural facilities, roofs are noteworthy for being largely responsible for thermal comfort, influencing the thermal balance within the shelter. This study aimed to evaluate the influence of roof on the Enthalpy (H), Thermal Load of Radiation (TLR), and Black Globe Temperature and Humidity Index (BGHI) in individual shelters for dairy calves. The design was completely randomized with three treatments: Z - zinc tile, AC - asbestos-cement tile and ACW - asbestos-cement tile painted white on the upper side. The averages were compared by the Scott Knott test at 1% probability. The results showed no statistical difference between treatments (P<0.01) and the external environment for H. For TLR, there was statistical difference among all treatments, where ACW showed the lowest TLR, 489.28 W m-2, followed by AC with 506.72 W m-2 and Z with the highest TLR, 523.55 W m-2. For BGHI, the lowest values were observed for ACW (76.8) and AC (77.4), differing significantly from Z, which obtained the highest value (81.6). The tiles with white paint on the upper side promoted the lowest TLR and the lowest BGHI, favoring the thermal environment in the shelter.

In Tropical regions, the animal performance is often affected by climate conditions. This study aimed to evaluate covering materials in individual shelters, normally used to house dairy calves, and its influence on the calves physiology and performance. The design used was completely randomized, with a 2x3 factorial arrangement to compare the averages of 5% through the Tukey's test, i.e., both genders- and three types of covering in the shelters (Z - zinc; AC - asbestos cement; and WPAC - white-painted asbestos cement). Parameters evaluated included daily weight gain (DWG), dry matter intake (DMI), feed conversion (FC), rectal temperature (RT), and respiratory frequency (RF). Results showed significant differences (P < 0.05) among males (1.04kg/day) and females (0.74kg/day) for DWG and interaction between gender and treatment (P < 0.05) for zinc covering (0.562kg/day for females and 1.120kg/day for males). Significant differences were also observed in FI of animals housed under shelters with the covering of zinc (48.35kgDM/day for females and 96.91 kgDM/day for males). There were no significant differences (P > 0.05) in the FC and the RT, and there were significant differences (P < 0.05) for RF in the Z treatments (56.9 mov.min-1), WPAC (62.2 mov.min-1) and FC (70.25 mov.min-1). It was concluded that different covering materials did not affect performance and dry matter intake of dairy calves. However, the animals' physiology of thermoregulation was altered by the different covering materials used in individual shelters.

Over the last few years, some of our experiments in which mycobacterial antigens were presented to the immune system as if they were viral antigens have had a significant impact on our understanding of protective immunity against tuberculosis. They have also markedly enhanced the prospects for new vaccines. We now know that individual mycobacterial protein antigens can confer protection equal to that from live BCG vaccine in mice. A critical determinant of the outcome of immunization appears to be the degree to which antigen-specific cytotoxic T cells are generated by the immune response. Our most recent studies indicate that DNA vaccination is an effective way to establish long-lasting cytotoxic T cell memory and protection against tuberculosis.

Erythrocyte membrane proteins from 44 representative mammals were studied. Protein 4.2 was not detected in guinea pigs (Cavia porcellus) (N = 14), Southern Brazilian swamp large rats (Myocastor coypus) (N = 2), cutias (Dasyprocta sp) (N = 4), and horses (Equus caballus) (N = 13). These animals also presented high ankyrin concentrations except for the horse which did not exhibit a sharp band, although minor components located between proteins 2 and 3 could account for the ankyrin family. The rodents studied did present band 6, which was not detectable in other common rodents such as white rats (Rattus norvegicus) (N = 9) and mice (Mus musculus) (N = 12). Since the absence of protein 4.2 does not disrupt the cytoskeleton membrane, we suggest that it is not an essential protein. Its absence may be compensated physiologically by the higher ankyrin concentration observed.

Xenobiotic metabolism is influenced by a variety of physiological and environmental factors including pregnancy and nutritional status of the individual. Pregnancy has generally been reported to cause a depression of hepatic monooxygenase activities. Low-protein diets and protein-energy malnutrition have also been associated with a reduced activity of monooxygenases in nonpregnant animals. We investigated the combined effects of pregnancy and protein-energy malnutrition on liver monooxygenase O-dealkylation activity. On pregnancy day 0 rats were assigned at random to a group fed ad libitum (well-nourished, WN) or to a malnourished group (MN) which received half of the WN food intake (12 g/day). WN and MN rats were killed on days 0 (nonpregnant), 11 or 20 of pregnancy and ethoxy- (EROD), methoxy- (MROD) and penthoxy- (PROD) resorufin O-dealkylation activities were measured in liver microsomes. Only minor changes in enzyme activities were observed on pregnancy day 11, but a clear-cut reduction of monooxygenase activities (pmol resorufin min-1 mg protein-1) was noted near term (day 0 vs 20, means ± SD, Student t-test, P<0.05) in WN (EROD: 78.9 ± 15.1 vs 54.6 ± 10.2; MROD: 67.8 ± 10.0 vs 40.9 ± 7.2; PROD: 6.6 ± 0.9 vs 4.3 ± 0.8) and in MN (EROD: 89.2 ± 23.9 vs 46.9 ± 15.0; MROD: 66.8 ± 13.8 vs 27.9 ± 4.4; PROD: 6.3 ± 1.0 vs 4.1 ± 0.6) dams. On pregnancy day 20 MROD was lower in MN than in WN dams. Malnutrition did not increase the pregnancy-induced reduction of EROD and PROD activities. Thus, the present results suggest that the activities of liver monooxygenases are reduced in near-term pregnancy and that protein-energy malnutrition does not alter EROD or PROD in pregnant rats.

To identify early metabolic abnormalities in type 2 diabetes mellitus, we measured insulin secretion, sensitivity to insulin, and hepatic insulin extraction in 48 healthy normal glucose-tolerant Brazilians, first-degree relatives of type 2 diabetic patients (FH+). Each individual was matched for sex, age, weight, and body fat distribution with a person without history of type 2 diabetes (FH-). Both groups were submitted to a hyperglycemic clamp procedure (180 mg/dl). Insulin release was evaluated in its two phases. The first was calculated as the sum of plasma insulin at 2.5, 5.0, 7.5, and 10.0 min after the beginning of glucose infusion, and the second as the mean plasma insulin level in the third hour of the clamp procedure. Insulin sensitivity index (ISI) was the mean glucose infusion rate in the third hour of the clamp experiment divided by the mean plasma insulin concentration during the same period of time. Hepatic insulin extraction was determined under fasting conditions and in the third hour of the clamp procedure as the ratio between C-peptide and plasma insulin levels. FH+ individuals did not differ from FH- individuals in terms of the following parameters [median (range)]: a) first-phase insulin secretion, 174 (116-221) vs 207 (108-277) µU/ml, b) second-phase insulin secretion, 64 (41-86) vs 53 (37-83) µU/ml, and c) ISI, 14.8 (9.0-20.8) vs 16.8 (9.0-27.0) mg kg-1 min-1/µU ml-1. Hepatic insulin extraction in FH+ subjects was similar to that of FH- ones at basal conditions (median, 0.27 vs 0.27 ng/µU) and during glucose infusion (0.15 vs 0.15 ng/µU). Normal glucose-tolerant Brazilian FH+ individuals well-matched with FH- ones did not show defects of insulin secretion, insulin sensitivity, or hepatic insulin extraction as tested by hyperglycemic clamp procedures.

Recent data from our laboratory have shown that patients with the indeterminate form of Chagas' disease can have impairment of left ventricular contractility, as evaluated by the slope of the left ventricle end-systolic pressure-dimension relationship. We also showed that Chagas' disease patients with minimal baseline wall motion abnormalities detected by two-dimensional echocardiography have more intense contractility impairment when compared to patients with the indeterminate form of the disease without this abnormality. The prognostic implications of these findings have not been established. We evaluated 59 patients (37-76 years, mean = 55 years) with different clinical forms of Chagas' disease, who had normal left ventricular global systolic function at baseline (57.6 ± 6.9%) and who had at least one additional echo during clinical follow-up (0.4-17.6; mean 4.6 years). Group 1 consisted of 14 patients with minor baseline left ventricle wall motion abnormalities and group 2 consisted of 45 patients without these abnormalities. During follow-up, global left ventricle systolic function deterioration was observed in 10 group 1 patients (71.4%) and in only 10 group 2 patients (22.2%; P < 0.005). Age and duration of follow-up were not independent determinants of left ventricular function deterioration in these patients. The present data indicate that mild segmental left ventricular wall motion abnormalities are associated with worsening of systolic function in Chagas' disease patients who have normal baseline global systolic performance.

Common variants of the transcription factor 7-like 2 (TCF7L2) gene have been found to be associated with type 2 diabetes in different ethnic groups. The Japanese-Brazilian population has one of the highest prevalence rates of diabetes. Therefore, the aim of the present study was to assess whether two single-nucleotide polymorphisms (SNPs) of TCF7L2, rs7903146 and rs12255372, could predict the development of glucose intolerance in Japanese-Brazilians. In a population-based 7-year prospective study, we genotyped 222 individuals (72 males and 150 females, aged 56.2 ± 10.5 years) with normal glucose tolerance at baseline. In the study population, we found that the minor allele frequency was 0.05 for SNP rs7903146 and 0.03 for SNP rs12255372. No significant allele or genotype association with glucose intolerance incidence was found for either SNP. Haplotypes were constructed with these two SNPs and three haplotypes were defined: CG (frequency: 0.94), TT (frequency = 0.027) and TG (frequency = 0.026). None of the haplotypes provided evidence for association with the incidence of glucose intolerance. Despite no associations between incidence of glucose intolerance and SNPs of the TCF7L2 gene in Japanese-Brazilians, we found that carriers of the CT genotype for rs7903146 had significantly lower insulin levels 2 h after a 75-g glucose load than carriers of the CC genotype. In conclusion, in Japanese-Brazilians, a population with a high prevalence of type 2 diabetes, common TCF7L2 variants did not make major contributions to the incidence of glucose tolerance abnormalities.

Com o objetivo de avaliar o comportamento individual de plantas originadas de sementes de alto e baixo vigor em populações de arroz irrigado (Oryza sativa L.), constituídas de diferentes combinações de distribuição ao longo da linha de semeadura, foi conduzido este estudo. O experimento constou de diferentes combinações de distribuição de sementes de arroz, cv. BR-IRGA 410, oriundas de lotes de alto e baixo vigor, semeadas em ambiente controlado e, após, transplantadas ao campo para garantir o estande de plantas e o arranjo da semeadura. Para tal, foram avaliados altura, matéria seca da planta e número de perfilhos aos 60, 102 e 134 dias após transplante ao campo e número de panículas por planta, rendimento de grãos e índice de colheita, ao final do ciclo. As plantas originadas de sementes de alto vigor apresentaram desempenho superior para todas as características avaliadas, independentemente do sistema de distribuição das plantas ao longo da linha de semeadura. As plantas originadas de sementes de alto vigor apresentaram rendimento médio de grãos superior ao das plantas originadas de sementes de baixo vigor. Plantas de arroz originadas de sementes de alto vigor não apresentam desempenho dominante sobre as plantas adjacentes de baixo vigor na linha de semeadura.

O objetivo deste artigo é discutir a aproximação teórica entre duas vertentes distintas de pensamento que, inicialmente, pode parecer improvável: a psicanálise e o marxismo. O texto discorre sobre a reflexão da gênesis do problema político, interpretando sociedade e indivíduo como inter-relação da natureza humana, conectando, a partir desse princípio, o problema político e o problema psicológico, este se situando como base original daquele. Portanto, no entrecruzamento de sociedade, poder político e natureza humana, expande-se um campo de investigação que se abre a diversas possibilidades, inclusive à aproximação teórica entre as ideias de Marx e Freud. Dentre diversos autores que discutem o freudomarxismo e o posicionamento de Sigmund Freud, diante das ideias de Karl Marx, será dada exclusividade a Wilhelm Reich, Herbert Marcuse e a Ludwig Marcuse. Para uma crítica às ideias de Herbert Marcuse e análise da inter-relação entre marxismo e psicanálise, por sua vez, recorre-se ao filósofo marxista Louis Althusser e a outros analistas e críticos dessas mesmas ideias.
