OBJECTIVE: To assess the association between exposure to adverse psychosocial working conditions and poor self-rated health among bank employees. METHODS: A cross-sectional study including a sample of 2,054 employees of a government bank was conducted in 2008. Self-rated health was assessed by a single question: "In general, would you say your health is (...)." Exposure to adverse psychosocial working conditions was evaluated by the effort-reward imbalance model and the demand-control model. Information on other independent variables was obtained through a self-administered semi-structured questionnaire. A multiple logistic regression analysis was performed and odds ratio calculated to assess independent associations between adverse psychosocial working conditions and poor self-rated health. RESULTS: The overall prevalence of poor self-rated health was 9%, with no significant gender difference. Exposure to high demand and low control environment at work was associated with poor self-rated health. Employees with high effort-reward imbalance and overcommitment also reported poor self-rated health, with a dose-response relationship. Social support at work was inversely related to poor self-rated health, with a dose-response relationship. CONCLUSIONS: Exposure to adverse psychosocial work factors assessed based on the effort-reward imbalance model and the demand-control model is independently associated with poor self-rated health among the workers studied.

The prevalence of anti-human parvovirus B19 IgG antibodies was determined in sera from 165 chronic hemolytic anemia patients, receiving medical care at Instituto Estadual de Hematologia (IEHE), Rio de Janeiro, during the year of 1994. This sample represents around 10% of the chronic hemolytic anemia patients attending at IEHE. Most of these patients (140) have sickle cell disease. Anti-B19 IgG antibodies were detected in 32.1% of patients. No statistically significant difference (p > 0.05) was seen between IgG antibody prevalence in male (27.8%) and female (35.5%) patients. Anti-B19 IgG antibodies were more frequent in older (37.6%) than younger (28.2%) than 20 years old patients, although this difference had no statistical significance (p > 0.05). Anti-B19 IgG antibody prevalence showed that 67.9% of patients enrolled in the study were susceptible to B19 acute infection. With the aim to detect acute B19 infection, patients follow up continued until February 1996. During this period four patients presented transient aplastic crisis due to human parvovirus B19 as confirmed by the detection of specific IgM antibodies. All four patients were younger than 20 years old, and 3 were younger than 10 years old. Three of them were sickle cell disease patients. Three of the four acute B19 infection occurred during 1994 springtime.

This paper records for the first time the presence of Corbicula fluminea (Philipi, 1844) in the Brazilian Amazon Basin. This exotic bivalve was found in localities on the Amazonas, Pará and Tocantins rivers. Density and population size structure were measured in some localities. Mean density is between 6.66 and 7.3 individuals m-2. Population size structure and the dates of the first records suggest that the introductions may have occurred between 1997 and 1998. The introductions may have been mediated by ocean-going vessels visiting the ports of Manaus and Belém. The potential impact of the invasion on native freshwater bivalves is discussed along with the need for monitoring and prevention of further introductions of non-indigenous bivalves in Brazil.

Discutir as crises ou os eventos paroxísticos que simulem crises epilépticas, enfatizando as diferenças semiológicas entre elas e as perspectivas terapêuticas. Realizamos uma revisão da literatura, selecionando artigos nas bases de dados Medline e Bireme, a partir dos unitermos: "non-epileptic seizures", "psychogenic seizures". As crises não-epilépticas (CNE) podem ser classificadas em fisiológicas (síncope, migrânea, ataque isquêmico transitório) e em psicogênicas (voluntárias ou não). O padrão-ouro para a diferenciação entre as crises epilépticas e as CNE é o videoeletroencefalograma, mas vários dados semiológicos podem auxiliar esse processo. O tratamento das CNE baseia-se em psicoterapia e em farmacoterapia direcionadas aos transtornos psiquiátricos comórbidos. Apesar de a alta prevalência das CNE e de sua elevada morbidade, são escassos os estudos na literatura nacional. São muitos os desafios diagnósticos e terapêuticos. Assim, o psiquiatra atentar-se à sua ocorrência, evitando iatrogenia, como o uso desnecessário de drogas antiepilépticas.

FUNDAMENTO: Pressão arterial elevada é motivo freqüente de procura por pronto-socorro, sendo possível que muitos pacientes recebam erroneamente um diagnóstico de crise hipertensiva e, conseqüentemente, um tratamento inapropriado. OBJETIVO: Analisar os casos de pacientes com pressão arterial elevada atendidos em um pronto-socorro geral, quanto ao preenchimento de critérios para o diagnóstico de crise hipertensiva e a adequação da conduta médica. MÉTODOS: Dos 1.012 pacientes atendidos consecutivamente em um pronto-socorro geral privado de referência, em São Luís, Maranhão, entre agosto e novembro de 2003, 198 (19,56%) tiveram como diagnóstico principal do atendimento pressão arterial elevada. Destes, de apenas 169 pacientes foi possível obter informação adequada nos boletins de atendimento, sendo 54,4% do sexo feminino, com média de idade de 53,3 ± 15,2 anos. Coletaram-se dados referentes aos pacientes e aos médicos atendentes, a fim de classificar cada caso como urgência, emergência ou pseudocrise hipertensiva, e a conduta médica como adequada ou inadequada. Procurou-se ainda identificar os fatores associados à conduta e ao uso de medicação anti-hipertensiva. RESULTADOS: Em apenas 27 pacientes (16%) houve critérios para caracterização de uma crise hipertensiva, sendo todos estes classificados como urgências. A conduta médica foi considerada adequada em 72 casos (42,6%), não sofrendo influência da especialidade (p = 0,5) nem da experiência do médico (p = 0,9). Níveis tensionais, e não a presença ou ausência de sintomas, foram preditivos do uso de medicação anti-hipertensiva (p < 0,001). CONCLUSÃO: Na população analisada, menos de 1/5 dos pacientes atendidos em um pronto-socorro com suposta crise hipertensiva apresentou critérios definidos para tal diagnóstico. A conduta médica foi considerada adequada em menos da metade dos atendimentos.

It is hypothesized that Asian monkeys were the original hosts of Trypanosoma conorhini- because they have been found naturally infected, the vector among rats is a tropicopolitan triatomine bug that belongs to a complex of Asian species, and primates were shown to be more susceptible than rats.

The financial impact of the first outbreak of Trypanosoma vivax in the Brazilian Pantanal wetland is estimated. Results are extended to include outbreaks in the Bolivian lowlands providing a notion of the potential influence of the disease and an analytical basis. More than 11 million head of cattle, valued at more than US$3 billion are found in the Brazilian Pantanal and Bolivian lowlands. The total estimated cost of the 1995 outbreak of T. vivax is the sum of the present values of mortality, abortion, and productivity losses and treatment costs, or about 4% of total brood cow value on affected ranches. Had the outbreak gone untreated, the estimated losses would have exceeded 17% of total brood cow value.

This is the first study describing the genetic polymorphism of Mycobacterium tuberculosis strains in the Indian Ocean Region. Using IS6110 RFLP analysis, 475 M. tuberculosis isolates from Madagascar, Comoros, Mauritius, Mozambique and La Reunion were compared. Of the 332 IS6110 profiles found, 43 were shared by clusters containing 2-65 strains. Six clusters were common to at least two countries. Of 52 families of strains with similar IS6110 profiles, 10 were common to at least two countries. Interestingly, another characteristic was the frequency (16.8%) of IS6110 single-copy strains. These strains could be distinguished using the DR marker. This preliminary evaluation suggests genetic similarity between the strains of the Indian Ocean Region. However, additional markers would be useful for epidemiological studies and to assess the ancient transmission of strains between countries of this region.

In this study, Ascaris DNA was extracted and sequenced from a medieval archaeological sample in Korea. While Ascaris eggs were confirmed to be of human origin by archaeological evidence, it was not possible to pinpoint the exact species due to close genetic relationships among them. Despite this shortcoming, this is the first Ascaris ancient DNA (aDNA) report from a medieval Asian country and thus will expand the scope of Ascaris aDNA research.

This work analyzes whether the relationship between risk and returns predicted by the Capital Asset Pricing Model (CAPM) is valid in the Brazilian stock market. The analysis is based on discrete wavelet decomposition on different time scales. This technique allows to analyze the relationship between different time horizons, since the short-term ones (2 to 4 days) up to the long-term ones (64 to 128 days). The results indicate that there is a negative or null relationship between systemic risk and returns for Brazil from 2004 to 2007. As the average excess return of a market portfolio in relation to a risk-free asset during that period was positive, it would be expected this relationship to be positive. That is, higher systematic risk should result in higher excess returns, which did not occur. Therefore, during that period, appropriate compensation for systemic risk was not observed in the Brazilian market. The scales that proved to be most significant to the risk-return relation were the first three, which corresponded to short-term time horizons. When treating differently, year-by-year, and consequently separating positive and negative premiums, some relevance is found, during some years, in the risk/return relation predicted by the CAPM. However, this pattern did not persist throughout the years. Therefore, there is not any evidence strong enough confirming that the asset pricing follows the model.

ABSTRACTThe Copula Theory was used to analyze contagion among the BRIC (Brazil, Russia, India and China) and European Union stock markets with the U.S. Equity Market. The market indexes used for the period between January 01, 2005 and February 27, 2010 are: MXBRIC (BRIC), MXEU (European Union) and MXUS (United States). This article evaluated the adequacy of the main copulas found in the financial literature using log-likelihood, Akaike information and Bayesian information criteria. This article provides a groundbreaking study in the area of contagion due to the use of conditional copulas, allowing to calculate the correlation increase between indexes with non-parametric approach. The conditional Symmetrized Joe-Clayton copula was the one that fitted better to the considered pairs of returns. Results indicate evidence of contagion effect in both markets, European Union and BRIC members, with a 5% significance level. Furthermore, there is also evidence that the contagion of U.S. financial crisis was more pronounced in the European Union than in the BRIC markets, with a 5% significance level. Therefore, stock portfolios formed by equities from the BRIC countries were able to offer greater protection during the subprime crisis. The results are aligned with recent papers that present an increase in correlation between stock markets, especially in bear markets.

O estudo aborda as situações vivenciadas pelos alunos dos vários semestres de Enfermagem no contexto universitário que, muitas vezes, podem propiciar sentimentos negativos nos alunos, as quais são apontadas como crises, bem como as ações desenvolvidas no enfrentamento das dificuldades. O método de coleta baseou-se em questões abertas para que os mesmos respondessem livremente. A análise dos depoimentos foi realizada categorizando-os em forma de temáticas. O estudo mostrou as dificuldades dos alunos relacionadas consigo próprio, com os colegas, de ordem estrutural e financeira, entre outras.

O presente estudo avaliou as características sociodemográficas e de adesão terapêutica de 27 portadores de hipertensão arterial em tratamento ambulatorial que apresentaram crises de urgências ou emergências hipertensivas e haviam sido atendidos em uma unidade de leito-dia e em uma unidade de emergência da cidade de Fortaleza-Ceará, no período de outubro de 2002 a maio de 2003. A maioria era mulher, com idade de 50 a 60 anos, pouca escolaridade, tempo de tratamento inferior a cinco anos e tempo de diagnóstico entre cinco e dez anos. O uso dos remédios foi o tratamento mais referido, seguido pela redução do consumo de sal e comparecimento às consultas. No entanto, o fato de comparecerem às consultas e receberem orientação parece não modificar o comportamento, uma vez que a maioria dos entrevistados não praticava exercícios físicos e demonstrava deficiência no conhecimento sobre a doença, atribuindo a elevação da pressão arterial a fatores emocionais.
