We investigated the effect of photodynamic therapy (PDT) and of an anti-vascular cell adhesion molecule-1 (VCAM-1) monoclonal antibody on the in vivo growth of C6 glioma. Seven days after inoculation with C6 cells, adult male Wistar rats weighing 280-300 g with MRI-confirmed glioma were randomly assigned to 4 groups (N = 15 per group): PDT + VCAM-1 antibody group; PDT group; VCAM-1 antibody group; control group. Eight days after inoculation, hematoporphyrin monomethyl ether (HMME) was administered as a photosensitizer and PDT was performed at 630 nm (illumination intensity: 360 J/cm²) for 10 min. VCAM-1 antibody (50 µg/mL) was then administered (0.5 mL) through the tail vein every other day from day 8 to day 16. At day 21, 5 rats in each group were sacrificed and cancers were harvested for immunohistochemistry and Western blot assay for the detection of VCAM-1, and TUNEL assay was used to detect apoptosis. Survival and tumor volume were recorded in the remaining 10 rats in each group. In the PDT group, tumor growth was significantly suppressed (67.2%) and survival prolonged (89.3%), accompanied by an increase in apoptosis (369.5%), when compared to control. Furthermore, these changes were more pronounced in the PDT + VCAM-1 antibody group. After PDT, VCAM-1 expression was markedly increased (121.8%) and after VCAM-1 monoclonal antibody treatment, VCAM-1 expression was significantly reduced (58.2%). PDT in combination with VCAM-1 antibody can significantly inhibit the growth of C6 glioma and prolong survival. This approach may represent a promising strategy in the treatment of glioma.

Os principais problemas das organizações de saúde são má alocação de recursos, ineficiência, custos crescentes e desigualdade nas condições de acesso dos usuários. A rentabilidade expressa resultados por recursos aplicados. A avaliação custo-eficácia das intervenções das organizações de saúde busca soluções de menor despesa para obter resultado esperado e de melhor resultado com determinada despesa. O ABC - activity-based costing- é o método de custeio que melhor subsidia a decisão sobre composição de atividades, custo de recursos e consumo de recursos. A avaliação custo-eficácia mostra-se, portanto, uma ferramenta útil para a melhoria da qualidade das organizações de saúde.

Este artigo apresenta a experiência de implantação da metodologia de custos baseada em atividades (ABC) do Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto da Universidade de São Paulo, por meio de um estudo de caso, num período de cinco anos. As características dos hospitais universitários de alta tecnologia, tais como atividades docente-assistenciais e serviços básicos de saúde, fazem com que seus custos sejam mais elevados do que os de hospitais não universitários. Na sua trajetória institucional, o HCFMRP-USP não possuía uma metodologia consistente de mensuração e avaliação dos custos de sua atividade assistencial. No entanto, dificuldades de financiamento de suas atividades, associadas a uma completa reestruturação em seu modelo de atendimento vinculado ao Sistema Único de Saúde, impulsionaram a adoção do mapeamento de custos por atividades, para demonstrar, de forma adequada, como pode ser mensurado o impacto do ensino nos custos assistenciais, visto que a agenda democrática nacional tem reivindicado transparência, accountability e, fundamentalmente, eficiência na gestão dos recursos públicos

Realizou-se inquérito sorológico para pesquisar anticorpos neutralizantes contra o vesiculovírus Piry, na cidade de Catolândia-Bahia-Brasil. Duas técnicas de vírus-neutralização foram comparadas em cultura das células C6/36, com revelação pelo método imunoenzimático ( TN-C6/36) e em camundongos recém-nascidos (TN-camundongos), que é considerada a prova maior. Em 204 soros, dos 1 .274 colhidos, a concordância das duas técnicas foi de 98,7% (K= 0,9853). Com este resultado do TN-C6/36, que também é mais exeqüível, decidiu-se desenvolver o estudo soro-epidemiológico do vesiculovírus Piry, em Catolândia, baseado nesta nova técnica.

Pela primeira vez é registrada a presença do Aedes (Stg) albopictus no Estado do Pará, Brasil, em área urbana no município de Medicilândia distante cerca de 90 km de Altamira, onde foram capturados por meio de isca humana 42 exemplares de mosquitos adultos. Estes foram inoculados em C6/36 e em camundongos recém-nascidos na tentativa de isolamento viral, não tendo sido isolado nenhum vírus. A presença de Aedes albopictus em áreas da Amazônia onde circulam os vírus de dengue e de febre amarela é preocupante e representa um risco potencial desta espécie de mosquito se tornar infectada com tais vírus.

Uma epidemia de dengue tipo 1 se iniciou em Novembro de 1990 na Região de Ribeirão Preto, Norte do Estado de São Paulo. Foram confirmados por exames laboratoriais cerca de 3.500 casos até fevereiro de 1991. A Unidade de Pesquisa em Virologia da Faculdade de Medicina de Ribeirão Preto - USP, estudou soros de 502 pessoas suspeitas de apresentarem dengue. Fez-se o diagnóstico sorológico através do método da inibição da hemaglutinação (HAI) para dengue tipo 1 em 19% dos analisados. Passou-se a utlilizar um teste imuno-enzimático para dengue em culturas celulares infectadas (EIA-ICC), que permite identificação simultânea de IgG e IgM. O EIA-ICC embora menos sensível quando comparado ao HAI (89%), mostrou-se mais eficiente, porque: dispensou a obtenção de segundas amostras séricas para o diagnóstico; trata-se de técnica simples, podendo ser efetuada em apenas 5 horas. O vírus dengue tipo 1 foi isolado do sangue de 21 pacientes, por inoculação em células de mosquitos C6/36. Fez-se a identificação dos vírus isolados por método de imunofluorescência indireta, utilizando anti-soro contra todos os flavivirus e anticorpos monoclonais tipo-específicos de dengue. Os sintomas mais freqüentemente observados em 71 indivíduos com diagnóstico de dengue confirmado foram febre (90% dos casos), mialgias (57%) e artralgias (41%)

Registramos a ocorrência de epidemia de dengue causada pelo sorotipo 2 (DEN 2) na cidade de Araguaina, estado do Tocantins (TO) situado no Brasil central. Quatrocentos indíviduos de 74 famílias, residentes nos bairros S. João, Araguaina Sul e Neblina foram entrevistados e sangrados, independentemente de terem adoecido ou não. Os soros tanto de adultos quanto de crianças de ambos os sexos foram usados para pesquisa de anticorpos inibidores da hemaglutinação (IH) e IgM através de ensaio imunoenzimático (MAC ELISA). Nas casas onde haviam doentes no momento do inquérito, sangue total também foi colhido para tentativa de isolamento de vírus. O quadro clínico apresentado pelos pacientes foi caracterizado por febre, cefaléia, mialgias, artralgias e exantema do tipo máculo-papular não pruriginoso. A infecção foi mais frequente em mulheres (33.9%) do que nos homens (23.8%), ocorrendo em todas as faixas etárias, inclusive em crianças com menos de um ano de idade, bem como em maiores de 70 anos. Um total de 1105 mosquitos (56 fêmeas e 45 machos de Culex quinquefasciatus e 567 fêmeas e 437 machos de Aedes aegypti) foram obtidos a partir de larvas coletadas em Araguaina. As fêmeas de Ae. aegypti obtidas das larvas fizeram repasto sangüíneo em 8 pacientes febris. O diagnóstico laboratorial foi feito por isolamento de vírus (cultura de células de Aedes albopictus, clone C6/36) e por sorologia (IH e MAC ELISA). Foram isoladas 5 amostras de DEN 2 de pacientes febris e tipadas por imunofluorescência indireta usando anticorpos monoclonais de dengue. Nenhuma amostra viral foi isolada de mosquitos. Outrossim, comprovou-se infecção em 111 pessoas sangradas, o que revelou um índice de positividade de 27.75% (111 em 400), sendo que 66.2% das famílias estudadas apresentaram pelo menos um indivíduo positivo. Ocorreram ainda, 26.1% de infecções assintomáti-cas. Por outro lado, a correlação de positividade entre os dois testes usados (IH e MAC ELISA) foi de 94.6%. Estimamos que ocorreram aproximadamente 83.250 casos da doença, entre 15 de março a 31 de maio de 1991. Esta é a primeira epidemia de DEN 2 em um estado da Amazônia Brasileira, portanto em área endêmica de febre amarela, e a primeira evidência da interiorização do DEN 2, até então restrito ao Rio de Janeiro.

We show here a simplified RT-PCR for identification of dengue virus types 1 and 2. Five dengue virus strains, isolated from Brazilian patients, and yellow fever vaccine 17DD as a negative control, were used in this study. C6/36 cells were infected and supernatants were collected after 7 days. The RT-PCR, done in a single reaction vessel, was carried out following a 1/10 dilution of virus in distilled water or in a detergent mixture containing Nonidet P40. The 50 µl assay reaction mixture included 50 pmol of specific primers amplifying a 482 base pair sequence for dengue type 1 and 210 base pair sequence for dengue type 2. In other assays, we used dengue virus consensus primers having maximum sequence similarity to the four serotypes, amplifying a 511 base pair sequence. The reaction mixture also contained 0.1 mM of the four deoxynucleoside triphosphates, 7.5 U of reverse transcriptase, 1U of thermostable Taq DNA polymerase. The mixture was incubated for 5 minutes at 37ºC for reverse transcription followed by 30 cycles of two-step PCR amplification (92ºC for 60 seconds, 53ºC for 60 seconds) with slow temperature increment. The PCR products were subjected to 1.7% agarose gel electrophoresis and visualized by UV light after staining with ethidium bromide solution. Low virus titer around 10 3, 6 TCID50/ml was detected by RT-PCR for dengue type 1. Specific DNA amplification was observed with all the Brazilian dengue strains by using dengue virus consensus primers. As compared to other RT-PCRs, this assay is less laborious, done in a shorter time, and has reduced risk of contamination

Three cases of dengue fever involving the central nervous system (CNS) are reported. All occurred in 1994 during a dengue (DEN) epidemic caused by serotypes DEN-1 and DEN-2. The first case examined was a 17-year-old girl who complained of fever, nuchal rigidity and genital bleeding. Three blood samples were positive by anti-dengue IgM ELISA and showed hemagglutination-inhibition (HI) test titers ³ 1,280. The second case concerned a 86-year-old woman with fever, muscle and joint pains, altered consciousness, syncope, nuchal rigidity and meningismus. Her blood sample showed an HI titer of 1:320 for flaviviruses, and an IgM ELISA positive for dengue. The third case was a 67-year-old woman with fever, abnormal behaviour, seizures, tremor of extremities, thrombocytopenia, increased hematocrit and leukopenia. The patient suffered a typical case of dengue hemorrhagic fever with ensuing shock and a fatal outcome. A single blood sample showed HI antibodies of ³ 1,280 and an IgM ELISA positive for dengue. No virus could be isolated from any patient by inoculation of blood into C6/36 cells and suckling mice. No other agent of disease was encountered in the patient.

The authors report the isolation of dengue 3 virus for the first time in Brazil. The patient, resident in Limeira-SP, traveled to Nicaragua on May 16th, 1998, where he stayed for two months. Starting on August 14th he had fever, headache, myalgia, arthralgia, retro-orbital pain and diarrhea. He returned to Brazil on August 16th and was hospitalized in the next day. The patient had full recovery and was discharged on August 20th. The virus was isolated in C6/36 cell culture inoculated with serum collected on the 6th day after the onset of the symptoms. The serotype 3 was identified by indirect immunofluorescence assays performed with type-specific monoclonal antibodies. This serotype was further confirmed by polymerase chain reaction analysis. The introduction of a new dengue serotype in a susceptible population is a real threat for the occurrence of severe forms of the disease. The isolation and identification of dengue virus are important in order to monitoring the serotypes circulating in Brazil and to take the measures necessary to prevent and control an epidemic.

In 2001, an autochthonous case of dual viremia, resulting from naturally acquired dengue virus DEN-1 and DEN-2 infections was detected during the dengue outbreak that occurred in Barretos, a city with about 105,000 inhabitants in the North region of São Paulo State. Serotype identification was based on virus isolation to C6/36 mosquito cells culture and immunofluorescence assays using type-specific monoclonal antibodies. The double infection was also confirmed by reverse transcriptase polymerase chain reaction (RT-PCR). Comparative analysis of the 240-nucleotide sequences of E/NS1 gene junction region between the genome of DEN-1 and DEN-2 isolates of the corresponding reference Nauru and PR 159S1 strains, respectively, showed some nucleotide differences, mainly silent mutations in the third codon position. Results of maximum likelihood phylogenetic analysis of E/NS1 gene sequences indicated that both genotypes of DEN-1 and DEN-2 viruses recovered from double infection in Barretos belonged to genotypes I and III, respectively.

This paper reports the isolation of St. Louis encephalitis virus (SLEV) from a febrile human case suspected to be dengue, in São Pedro, São Paulo State. A MAC-ELISA done on the patient's acute and convalescent sera was inconclusive and hemagglutination inhibition test detected IgG antibody for flaviviruses. An indirect immunofluorescent assay done on the C6/36 cell culture inoculated with the acute serum was positive for flaviviruses but negative when tested with dengue monoclonal antibodies. RNA extracted from the infected cell culture supernatant was amplified by RT-PCR in the presence of NS5 universal flavivirus primers and directly sequenced. Results of BLAST search indicated that this sequence shares 93% nucleotide similarity with the sequence of SLEV (strain-MSI.7), confirmed by RT-PCR performed with SLEV specific primers. Since SLEV was identified as the cause of human disease, it is necessary to improve surveillance in order to achieve early detection of this agent in the state of São Paulo and in Brazil. This finding is also an alert to health professionals about the need for more complete clinical and epidemiological investigations of febrile illnesses as in the reported case. SLEV infections can be unrecognized or confused with other ones caused by an arbovirus, such as dengue.

Mayaro virus (MAYV) is an arbovirus (Togaviridae: Alphavirus) enzootic in tropical South America and maintained in a sylvan cycle involving wild vertebrates and Haemagogus mosquitoes. MAYV cases occur sporadically in persons with a history of recent activities inside or around forests. This paper reports three cases of MAYV fever detected in men infected in Camapuã, MS, Brazil. Serum samples collected at four days and two months after the onset of the symptoms and examined by hemagglutination inhibition test, revealed monotypic seroconversion to MAYV. Isolation of the virus was obtained from one of the samples by inoculation of the first blood samples into newborn mice. A suspension of the infected mouse brain was inoculated into C6/36 cells culture and the virus was identified by indirect immunofluorescent assay with alphavirus polyclonal antibodies. RT-PCR, performed with RNA extracted from the supernatant of C6/36 infected cells in the presence of alphavirus generic primers as well as specific MAYV primers, confirmed these results. The reported cases illustrate the importance of laboratory confirmation in establishing a correct diagnosis. Clinical symptoms are not always indicative of a disease caused by an arbovirus. Also MAYV causes febrile illness, which may be mistaken for dengue.

After detecting the death of Howlers monkeys (genus Alouatta) and isolation of yellow fever virus (YFV) in Buri county, São Paulo, Brazil, an entomological research study in the field was started. A YFV strain was isolated from newborn Swiss mice and cultured cells of Aedes albopictus - C6/36, from a pool of six Haemagogus (Conopostegus) leucocelaenus (Hg. leucocelaenus) mosquitoes (Dyar & Shannon) collected at the study site. Virus RNA fragment was amplified by RT-PCR and sequenced. The MCC Tree generated showed that the isolated strain is related to the South American I genotype, in a monophyletic clade containing isolates from recent 2008-2010 epidemics and epizootics in Brazil. Statistical analysis commonly used were calculated to characterize the sample in relation to diversity and dominance and indicated a pattern of dominance of one or a few species. Hg. leucocelaenus was found infected in Rio Grande do Sul State as well. In São Paulo State, this is the first detection of YFV in Hg. leucocelaenus.

We report the first isolation of Dengue virus 4 (DENV-4) in the state of São Paulo, from two patients - one living in São José do Rio Preto and the other one in Paulo de Faria, both cities located in the Northwest region of the state. The virus isolations were accomplished in the clone C6/36 Aedes albopictus cell line, followed by indirect immunofluorescence assays, performed with type-specific monoclonal antibodies that showed positive reactions for DENV-4. The results were confirmed by Nested RT-PCR and Real-Time RT-PCR assays. The introduction of DENV-4 in a country that already has to deal with the transmission of three other serotypes increases the possibility of the occurrence of more severe cases of the disease. The importance of early detection of dengue cases, before the virus spreads and major outbreaks occur, should be emphasized.
