OBJECTIVE To analyze the factors associated with the use of pacifiers and/or bottle feeding in infants aged under one year.METHODS This is a cross-sectional study with 34,366 children and using data from the database of the 2nd Nationwide Survey of Breastfeeding Prevalence performed in the Brazilian capitals and Federal District in 2008. Cluster sampling was used. The questionnaire included questions about the use of artificial nipples in the last 24 hours. The analysis considered three outcomes: exclusive use of pacifier, exclusive use of bottle feeding, and use of artificial nipples (pacifier and bottle feeding). Prevalence ratios were obtained using Poisson regression with robust variance following a hierarchical model.RESULTS The following factors were associated with exclusive use of the pacifier: mother working outside the home, primiparity, child was not breastfed within the first hour, and child had consumed tea on the first day at home. The following factors were associated with exclusive use of bottle feeding: mother working outside the home, primiparity, low birth weight, child not breastfed within the first hour, and child had consumed milk formula and tea on the first day at home. The following factors were associated with use of artificial nipples (pacifier and bottle feeding): mother working outside the home, primiparity, cesarean delivery, the male gender, low birth weight, born in a hospital not accredited as “baby friendly”, required health baby monitoring in the Primary Health Care Unit (PR = 0.91), and child had consumed milk formula, water, or tea on the first day at home.CONCLUSIONS This study identified profiles of exclusive users of pacifiers, bottle feeding, and both. The provided information can guide preventive practices for child health.

ABSTRACT OBJECTIVE To estimate the required number of public beds for adults in intensive care units in the state of Rio de Janeiro to meet the existing demand and compare results with recommendations by the Brazilian Ministry of Health. METHODS The study uses a hybrid model combining time series and queuing theory to predict the demand and estimate the number of required beds. Four patient flow scenarios were considered according to bed requests, percentage of abandonments and average length of stay in intensive care unit beds. The results were plotted against Ministry of Health parameters. Data were obtained from the State Regulation Center from 2010 to 2011. RESULTS There were 33,101 medical requests for 268 regulated intensive care unit beds in Rio de Janeiro. With an average length of stay in regulated ICUs of 11.3 days, there would be a need for 595 active beds to ensure system stability and 628 beds to ensure a maximum waiting time of six hours. Deducting current abandonment rates due to clinical improvement (25.8%), these figures fall to 441 and 417. With an average length of stay of 6.5 days, the number of required beds would be 342 and 366, respectively; deducting abandonment rates, 254 and 275. The Brazilian Ministry of Health establishes a parameter of 118 to 353 beds. Although the number of regulated beds is within the recommended range, an increase in beds of 122.0% is required to guarantee system stability and of 134.0% for a maximum waiting time of six hours. CONCLUSIONS Adequate bed estimation must consider reasons for limited timely access and patient flow management in a scenario that associates prioritization of requests with the lowest average length of stay.

The snail density, levels of infection and the monthly production of Schistosoma mansoni cercariae by Biomphalaria glabrata were determined in a focus of Barreiro de Baixo (Belo Horizonte, MG, Brazil). During a period of 38 months (1984 to 1987) 5,366 snails were collected of which 324 (6.03%) were infected with S. mansoni. The total number of cercariae shed was 5,667,312. Each snail shed an average of 17,422 cercariae during the time that it was under study in the laboratory. The greatest longevity of infected snails was 218 days. Natural cure was observed in 42 (12.9%) of the infected specimens about 130 days after collection. The average snail density in the focus during the period of study was 16.3 snails per scoop. The shedding of cercariae by snails collected from the field was compared with laboratory bred specimens infected in mass with the LE strain of S. mansoni from Belo Horizonte. The laboratory infected snails shed an average of 6,061 cercariae each, a value 2.8 times less than the field specimens due to a shorter life span. The prevalence of schistosomiasis in the focus was 14.3%.

A parasitological survey was carried out on 222 inhabitants of five farms in Holambra, located 30 km north of Campinas, São Paulo, Brazil, on October 1992. Approximately 70% of the inhabitants were found to be infected with at least one species of intestinal parasite. The positive rates of 6 helminths and 7 protozoan species detected are as follows: 5.4% Ascaris lumbricoides; 8.6% Trichuris trichiura; 19.8% Necator americanus; 10.4% Strongyloides stercoralis; 14% Enterobius vermicularis; 0.9% Hymenolepis nana; 3.2% Entamoeba histolytica; 2.7% E. hartmanni; 9.9% E. coli; 14.0% Endolimax nana; 2.3% Iodamoeba butschlii; 10.4% Giardia lamblia; 37.8% Blastocystis hominis. The positive rates of helminth infection were generaly higher in the younger-group under 16 years-old than those in the elder group aged 16 or more, whereas the infection rates of protozoan species were higher in the elder group. The infection rate of Strongyloides was found to be 10.4% by a newly developed sensitive method (an agarplate culture methods).

A cross-sectional study on the prevalence of schistosomiasis mansoni in three sites of the "Baixada Ocidental Maranhense" was carried out in 1993 in: Alegre (in the municipality of São Bento), Aliança (in Cururupu) and Coroatá II (in the municipality of São João Batista). Results were compared to those of another study performed at the same sites and in similar conditions, in 1987. The entire population of the three sites, with few exceptions, was submitted to fecal tests using the Kato-Katz method and immediate intradermal tests for schistosomiasis in both studies. Subjects with positive results in one of these tests were clinically evaluated by a physical examination. In 1993, the total of 827 subjects were submitted to fecal examination and 826 to intradermal test. Schistosoma mansoni eggs were found in the feces of 154 (18.6%) subjects, while 478 (57.9%) subjects presented a positive intradermal test. Stool examination was carried out in 367 subjects in Alegre with a positivity rate of 14.9%; the intradermal test, performed in 366 subjects, was positive in 47.5% of the cases. In Aliança, 277 subjects had their feces examined and were submitted to an intradermal test, with a positivity rate of 34.4% and 70.7%, respectively. Finally in Coroatá II, 183 inhabitants submitted to fecal and intradermal tests had positivity rates of 2.2% and 59.0%, respectively. When the present data were compared to those obtained in the survey performed in 1987, a significant decrease in the prevalence of infection by S. mansoni was observed in Alegre and Coroatá II, and a prevalence increase in Aliança.

Between 1992 and 1997, 790 blood donors with anti-HCV EIA-2 strongly reagent (relationship between the sample optical density/cut-off > 3) detected at the blood bank serological screening, were evaluated in ambulatory environment. They were all negative for Chagas disease, syphilis, hepatitis B (HBsAg) and AIDS. Blood samples were collected at the first ambulatorial evaluation, for hemogram, biochemical tests and new serological tests for HCV (anti-HCV EIA-2). In blood samples of 226 repeatedly reagent anti-HCV EIA-2 blood donors, supplementary "immunoblot" test for HCV (RIBA-2) was used. In 209 donors, the presence of HCV-RNA was investigated by the PCR test. The abdominal ultrasonography was realized in 366 donors. In 269 patients liver biopsy was performed for the histopathological study. The follow-up of blood donors showed that 95.6% were repeatedly EIA-2 reagent, 94% were symptomless and denied any hepatitis history, with only 2% mentioning previous jaundice. In 47% of this population at least one risk factor has been detected for the HCV transmission, the use of intravenous drugs being the main one (27.8%). Blood transfusion was the second factor for HCV transmission (27.2%). Hepatomegaly was detected in 54% of the cases. Splenomegaly and signs of portal hypertension have seldom been found in the physical examination, indicating a low degree of hepatic compromising in HCV. Abdominal ultrasound showed alterations in 65% of the subjects, being the steatosis the most frequent (50%). In 83.5% of the donors submitted to the liver biopsy, the histopathological exam showed the presence of chronic hepatitis, usually classified as active (89%) with mild or moderate grade in most of the cases (99.5%). The histopathological exam of the liver was normal in 1.5% of blood donors. The RIBA-2 test and the HCV-RNA investigation by PCR were positive in respectively 91.6 and 75% of the anti-HCV EIA-2 reagent donors. The HCV-RNA research was positive in 82% of the RIBA-2 positive subjects, in 37.5% of the indeterminate RIBA-2 donors and in 9% of the negative RIBA-2 donors. Chronic hepatitis has also been observed in 50% of the histopathological exams of the anti-HCV EIA-2 reagent donors which were indeterminate RIBA-2. Among 18 blood donors with minimal changes histopathological exam 11 (61%) were HCV-RNA positive. Our blood donors anti-HCV reagent generally had clinical, laboratorial and histopathological features observed in patients with chronic HCV hepatitis and a high proportion could be identified in interviews and medical evaluation realized in blood blanks. Generally, these HCV infected donors are identified and discharged only by the serological tests results.

As patients with chronic Chagas disease exhibit morphological and functional changes of the stomach (hypomotility and hypochlorhydria), malnutrition, immunological deficiency and high prevalence of peptic disease associated to Helicobater pylori infection, the purpose of this study was to evaluate if the prevalence of H. pylori infection in chronic chagasic is higher than in non-chagasic individuals in the urban and rural population from Uberlândia, MG, Brazil. Serological determination of IgG antibodies to H. pylori was performed using a second-generation ELISA. Thus, 598 people were evaluated: 128 chagasic (CG), 222 non-chagasic living in urban area (U-NCG) and 248 non-chagasic living in rural area (R-NCG). Regarding the age range from 21 to 50 years, the prevalence of H. pylori infection in the CG (85.1%) was significantly higher than in the U-NCG (56.3%, p < 0.01) and the R-NCG (67.4%, p < 0.05). In the patients over 50 years, the prevalence in the CG (86.4%) was similar to the U-NCG (78.8%) and R-NCG (86.1%). Similar results were also found between the U-NCG and R-NCG for all age ranges, with prevalence rates of 29.1% and 35.3% for the age range from 5 to 13 years, and 47.2% and 40% for that from 14 to 20 years, respectively. We conclude that chagasic patients showed a higher seroprevalence of H. pylori infection than non-chagasic individuals, in the age range from 21 to 50 years, and that the prevalence of this infection was similar in the studied urban and rural non-chagasic population.

Foi examinado um total de 222 cães, sendo que 197 eram cães de rua apreendidos nas áreas suburbanas e rurais do Rio de Janeiro e 25 eram animais de estimação, mantidos em residências (apartamentos) nas zonas Centro e Sul da cidade. Do total de 197 cães de rua, foram encontrados 49 (24,87%) parasitados por coccidios do gênero Isospora, sendo que 6 deles (3,04%) eram parasitados por I. canis e 43 (21,83%) por I. rivolta. Nos 43 casos de parasitismo por I. rivolta foi encontrada nas fezes uma alta percentagem de esporocistos livres com 4 esporozoitos (35 casos, representando 81,4%). As fezes foram coletadas diretamente do reto e a técnica empregada para o exame do material foi a centrífugo-flutuação em sulfato de zinco. Entre os 25 cães de estimação não foi encontrado nenhum caso de parasitismo por Isospora sp. Os valores médios para os diâmetros maior e menor das formas de Isospora sp. encontradas foram de 38,63μ x 31,93μ. para oocistos de I. canis; 23μ x 18,94μ. para oocistos de I. rivolta e, 19,94μ x 10,05μ para os esporocistos de I. rivolta, encontrados livres nas fezes. Não foi diagnosticado nenhum caso de I. bigemina. Entre todos os casos positivos foram encontrados apenas 3 casos de infecção maciça nos animais. Nos 3 casos, tratavam-se de animais jovens parasitados por I. rivolta. O teste estatístico de diferença de proporções demonstrou que o sexo e a idade dos animais não tiveram influência na maior ou menor percentagem de cães positivos para Isospora sp, na presente casuística.

Em Santa Catarina, o Inquérito Sorológico Nacional para doença de Chagas (CNPq-SUCAM) revelou positividade de 1,3% em cerca de 74.000 amostras de soro processadas pela reação de imunofluorescência indireta em papel de filtro, com 15 municípios apresentando prevalências de 5,4% a 41,3%. Na presente investigação, em 9 dos municípios com alta prevalência, foram obtidas amostras de sangue, por punção venosa, de 222 indivíduos dos quais 140 haviam sido sorologicamente positivos e 58 negativos no Inquérito Nacional. Em 24 outros indivíduos a reação foi executada pela primeira vez. Os testes sorológicos (imunofluorescência indireta, hemaglutinação indireta, aglutinação direta com e sem 2-mercaptoetanol e fixação do complemento) realizados em 3 diferentes laboratórios evidenciaram 220 soros negativos e apenas 2 positivos. Dados epidenúológicos obtidos nas áreas trabalhadas confirmaram estes resultados negativos. Os resultados discordam daqueles encontrados pelo Inquérito Sorológico Nacional e confirmam a inexistência de focos domiciliares de transmissão da doença de Chagas em Santa Catarina.

Estudou-se a distribuição do sistema ABO em 222 controles e em 148 chagásicos crônicos, assintomáticos e sintomáticos, com diferentes formas anatomoclinicas (insuficiência cardíaca congestiva, visceromegalias e morte súbita). O foi calculado a partir de tabelas de contingência e pelo método de Woolf. OX², obtido por estes dois métodos, demonstra que a freqüência do grupo OX² foi significativamente menor nos chagásicos sintomáticos considerados como um todo e naqueles com "megas", em relação aos controles. Nos chagásicos falecidos subitamente observou-se maior freqüência de grupos B que nos controles, sendo o X² significativo apenas quando calculado por tabelas de contingência. A relativa proteção aparentemente conferida aos chagásicos do grupo O, no que se refere à evolução para formas sintomáticas da doença, se explicaria por antigenicidade cruzada entre populações do Trypanosoma cruzi e o sistema ABO ou por outros fatores que se deixam influenciar ou influenciam a distribuição dos grupos sangüíneos.

O Parque Indígena do Xingu (PIX) está localizado no estado do Mato Grosso, na região de transição de cenado ao sul e da floresta Amazônica ao norte. Dados de literatura mostram que a população adulta apresenta elevada prevalência de marcadores do vírus da hepatite B (HBV). O presente estudo visa determinar a prevalência dos marcadores do HBV e do HDV na população indígena do PIX de zero a 14 anos, e investigar a forma de transmissão do HBV na região. Entre as 17 tribos existentes no PIX escolheu-se os Caiabi e os Txucairamãe que diferem em seus hábitos de vida e habitam a região Norte do parque com características de clima, vegetação e fauna semelhantes ás da região Amazônica. Avaliaram-se 222 crianças (116 Txucarramãe e 106 Caiabi) e 33 mulheres em idade fértil. A pesquisa de marcadores sorológicos para HBV e HDV foi feita por técnica imunoenzimãtica. A prevalência global dos marcadores sorológicos nas crianças foi: HBsAg 4,5%; anti-HBs 39,6%; anti- HBc 44,1%; presença de algum marcador do HBV 47,3% e anti-HDV 0,0%, enquanto que nas mulheres em idade fértil foi: HBsAg 12%, todas anti-HBe positivas. A infecção pelo HBV ocoireu mais precocemente entre os Txucarramãe, quando se considerou algum marcador do HBV (p < 0,001). No PIX, área de alta endemicidadepara infecção pelo HBV, a transmissão do vírus ocorre provavelmente de maneira horizontal e não vertical como seria de se esperar, características culturais, condições de habitação e presença de insetos hematófagos são importantes determinantes na sua transmissão. Apesar de não ter sido detectado nenhum caso com hepatite Delta, o PIX é uma área propícia para a sua disseminação.

Foi estudada a distribuição dos enteroparasitos mais freqüentes na população de Assis, São Paulo, de 1990 a 1992. Foram analisados 18.366 exames oriundos de seis postos de atendimento sanitário (PAS): Vila Marialves, Vila Progresso, Centro, Vila Xavier, Vila Fiúza e Vila Bonfim. A prevalência de enteroparasitoses geral foi 23,3%. Os enteroparasitos mais encontrados foram: Giardia intestinalis (8,7%), Ascaris lumbricoides (5,5%), Trichuris trichiura (2,4%) e Hymenolepis nana (1,9%). Na Vila Marialves, região de baixo nível sócio-econômico essas freqüências foram, respectivamente: 17%; 13,1%; 5,9% e 4,2%. A faixa etária 3 a 12 anos apresentou maior número de indivíduos parasitados. Estabeleceu-se uma correlação entre as condições de saneamento básico, expressos pelo número de ligações de água e esgoto, e a freqüência de parasitoses. Houve queda na freqüência de parasitoses nos PAS entre 1990 e 1992, coincidindo com o aumento do número de ligações de água e esgoto nestas regiões.

The cardiac effects of experimentally induced myocarditis, when the parasite is obtained from mouse blood, are well known. However, the consequences of the infection when the parasites are obtained from bug faeces are less well defined. In the present investigation, we have used the "Y" strain of Trypanosoma cruzi, which was maintained in Rhodnius prolixus by repeated passages in mice. The faeces of 30 infected bugs were collected, the number of parasites counted and 4,000 parasites inoculated by the conjunctival route in 60 rats. Twenty-nine other rats received faeces from noninfected bugs (sham-inoculated controls) and 40 were used as normal controls. The heart rate of the three groups of animals was recorded under general anesthesia with ether. The heart rate, at day 0 pre-inoculation, was similar in the three groups of animals (Controls: 379 ± 27 beats/min Mean ± SD; Sham-inoculated: 366 ± 31; Infected: 351 ± 29) (p> 0.05). In the infected animals, the mean heart rate began to increase significantly by day 12 following infection (375 ± 31), reaching the highest values between days 18 (390 ± 33) and 21 (403 ± 33) and returned to baseline by day 30 (359 ± 28) (p< 0.05). The heart rate changes were statistically different from those observed in the sham-inoculated controls and in the control animals. Therefore, these heart rate changes were provoked by the Trypanosoma cruzi-induced infection. Thus, it appears that irrespective of the source of the parasite and route of inoculation Trypanosoma cruziacute infection provokes a transient sinus tachycardia.

Estudo retrospectivo descritivo dos 3.314 casos de malária notificados na área de abrangência da Superintendência de Controle de Endemias, Campinas (88 municípios, 5.366.081 habitantes), no período de 1980 a 2000. Foram considerados elementos da história da expansão da malária na região. Houve queda dos casos diagnosticados mesmo em períodos de recrudescimento da malária na Amazônia. Predominaram homens (83%), em idade produtiva (20 a 49 anos), vindos principalmente de Rondônia, Pará e Mato Grosso; 59% foram diagnosticados nos 3 primeiros dias dos sintomas. Considerou-se o possível impacto positivo de campanhas educativas endereçadas às populações de risco e aos profissionais de saúde na região. Em áreas não endêmicas, a assistência oportuna ao paciente com malária, a vigilância epidemiológica/entomológica e a ações educativas podem diminuir a gravidade dos casos e impedem o estabelecimento de focos de transmissão.

Este trabalho teve como objetivo detectar os casos novos de hanseníase do período de 1998 a 2005 no município de Prudentópolis, PR. Assim como, verificar a faixa etária e a forma clínica predominante entre os casos de hanseníase. Os dados foram obtidos através das fichas clínico-epidemiológicas de casos com diagnóstico definitivo de hanseníase notificado no Sistema de Informação sobre Agravos de Notificação, de 1998 a 2005. Neste período, foram relatados 222 casos de hanseníase, dos quais 63% apresentavam a forma multibacilar, com predominância da forma clínica virchowiana, indicando um processo de intensa transmissão da doença. Dos casos de hanseníase, 35% dos pacientes se encontravam na faixa economicamente ativa, 31 a 45 anos. Esses indicadores apontam para uma elevada circulação do bacilo na comunidade, já que um fator de risco para infecção pelo bacilo Mycobacterium Leprae é o contato com pacientes com a forma multibacilar, sem tratamento, embora nas duas últimas décadas, tenha havido redução drástica da prevalência da hanseníase pela implementação da poliquimioterapia e outras medidas preventivas.
