Lotes de Biomphalaria glabrata (controle), B. tenagophila e B. straminea (com respectivamente 139, 77 e 149 exemplares) criados em laboratório a partir de espécimes coletados na região metropolitana de Belo Horizonte, MG (Brasil), foram infectados experimentalmente com larvas L1 de Angiostrongylus costaricensis. Decorridos aproximadamente 25 dias, os moluscos foram digeridos individual e artificialmente para exame. De 87 B. glabrata examinadas, 62 (71,3%) estavam positivas e apresentaram de uma a 61 larvas L3; de 42 B. tenagophila, 21 (50,0%) possuíam de uma a cinco L3; e de 89 B. straminea, 69 (77,5%), de uma a 72 L3. As três espécies de planorbídeos mostraram-se suscetíveis à infecção pelo A. costaricensis, sendo a B. glabrata e a B. straminea as mais eficientes para manutenção do ciclo do nematódeo em laboratório.

Se estudió la respuesta clínica y serológica a la infección chagásica de 937 embarazadas y sus 929 recién nacidos (RN) vivos, grupo I; 4 RN de origen diverso, grupo II y 35 RN derivados de otros centros, grupo III. Las embarazadas se estudiaron con 3 reacciones serológicas; se definió infección cuando 2 o más reacciones eran positivas. En los RN el diagnóstico se confirmó por observación directa del T. cruzi en una muestra de sangre. Los RN con Chagas congénita (RN-ChC) fueron tratados y seguidos con estudios clínicos y de laboratorio. Se detectaron 149 embarazadas chagásicas (15.9%), de las cuales se diagnosticaron 6 RN-ChC (4%). En el total de 968 RN estudiados se detectaron 12 RN infectados. El micro-hematócrito fue el método parasitológico de lectura rápida más efectivo para el diagnóstico de infección en nuestra serie. El par de reacciones serológicas específicas constituyó un criterio de mayor seguridad para el control y seguimiento de la infección congénita. Las expresiones clínicas más comunes de infección fueron hepatomegalia, esplenomegalia, ictericia, anemia y prematurez, con distintos grados de asociación. Se concluye que dadas las características clínicas de la enfermedad de Chagas congénita en nuestro medio, se impone como estrategia el diagnóstico serológico para la enfermedad de Chagas en todas las embarazadas y el control y seguimiento de sus RN hasta descartar o confirmar infección congénita.

The aims of this study were to verify the presence of Toxoplasma gondii cysts in fresh pork sausage and the presence of antibodies against T. gondii in serum of workers from factories with Municipal Inspection Service, in Londrina, PR, Brazil. 149 samples of sausage were collected from eight factories and blood samples from 47 workers. We also took information about the practices that were adopted in the factories and the workers' habits that could influence the prevalence of toxoplasmosis. After bioassay in mice, 13 (8.7%) sausage samples were positive, in one of them T. gondii was isolated and in the other 12 the mice seroconverted. Of 47 workers, 36 (76.6%) worked in sausage production and 11 (23.4%) were involved in other functions; 59.5% (28/47), 55.5% (20/36) and 72.7% (8/11), respectively, had T. gondii antibodies. There were no significant differences in the variables of industries' practices and workers' habits related to T. gondii infection. We concluded that fresh pork sausage could be important in the transmission of toxoplasmosis.

The present study intended to analyze the seroprevalence of Helicobacter pylori, IgG, and its relation to dyspepsia in a population from the western Amazon region. During the "Projeto Bandeira Científica", a University of São Paulo Medical School program, in Monte Negro's rural areas, state of Rondônia, 266 blood samples were collected from volunteers. The material was tested for IgG antibodies anti-Helicobacter pylori by ELISA method and the participants were also interviewed on dyspepsia, hygiene and social aspects. Participants aged between five and 81 years old (34 years on average), 149 (56%) were female and 117 (44%) male. We found 210 (78.9%) positive, 50 (18.8%) negative and six (2.3%) undetermined samples. Dyspeptic complaints were found in 226 cases (85.2%). There was no statistical association between dyspepsia and positive serology for H. pylori. We concluded that the seroprevalence in all age categories is similar to results found in other studies conducted in developing countries, including those from Brazil. On the other hand, the seroprevalence found in Monte Negro was higher than that reported in developed countries. As expected, there was a progressive increase in the positivity for H. pylori in older age groups.

The diagnosis of asymptomatic infection with Leishmania (Leishmania) chagasi has become more important over recent years. Expansion of visceral leishmaniasis might be associated with other routes of transmission such as transfusion, congenital or even vector transmission, and subjects with asymptomatic infection are potential reservoirs. Moreover, the identification of infection may contribute to the management of patients with immunosuppressive conditions (HIV, transplants, use of immunomodulators) and to the assessment of the effectiveness of control measures. In this study, 149 subjects living in a visceral leishmaniasis endemic area were evaluated clinically and submitted to genus-specific polymerase chain reaction (PCR), serological testing, and the Montenegro skin test. Forty-nine (32.9%) of the subjects had a positive PCR result and none of them developed the disease within a follow-up period of three years. No association was observed between the results of PCR, serological and skin tests. A positive PCR result in subjects from the endemic area did not indicate a risk of progression to visceral leishmaniasis and was not associated with a positive result in the serological tests.

In Colombia, pentavalent antimonials and miltefosine are the drugs of choice for the treatment of cutaneous leishmaniasis; however, their toxicity, treatment duration, (treatment adherence problems), cost, and decreased parasite sensitivity make the search for alternative treatments of American cutaneous leishmaniasis necessary. Based on the results found in a controlled, open, randomized, phase III clinical trial, the efficacy and safety of miltefosine was compared to that of thermotherapy for the treatment of cutaneous leishmaniasis in Colombia. Adult patients from the Colombian army participated in the study; they received either 50 mg of miltefosine three times per day for 28 days by the oral route (n = 145) or a thermotherapy (Thermomed®) application of 50 °C for 30 seconds over the lesion and surrounding area (n = 149). Both groups were comparable with respect to their sociodemographic, clinical, and parasitological characteristics. The efficacy of miltefosine by protocol and by intention to treat was 70% (85/122 patients) and 69% (85/145 patients), respectively. The adverse effects were primarily gastrointestinal for miltefosine and pain at the lesion site after treatment for thermotherapy. No statistically significant difference was found in the efficacy analysis (intention to treat and protocol) between the two treatments. ClinicalTrials.gov: NCT00471705.

Objective: This study was designed to determine the frequency and causative agent(s) of urinary tract infections (UTIs) in individuals with symptoms of urinary tract infections in Enugu State of Southeast Nigeria, and to determine the antibiotic susceptibility pattern of microbial agents isolated from urine culture.Methods: The study involved 211 individuals (149 females and 62 males) clinically suspected for UTI. Urine samples were collected by the mid-stream ‘clean catch’ method and tested using standard procedures. Antibiotic susceptibility of the isolated pathogens was tested using the Kirby-Bauer technique according to the Clinical and Laboratory Standards Institute (CLSI) guidelines.Results: Microscopy of centrifuged urine samples showed 16 patients had pyuria while 54 had pus cells. Calcium oxalate crystals were found in 14 samples. Urinalysis performed with urine samples showed 17 had protein; seven were nitrite positive and three had moderate to high glucose concentration. Fifty-four urine samples (36.2%) from females and 12 (19.4%) from males showed significant growth upon culture. Gram stain and biochemical tests identified nine different organisms with Escherichia coli as the most common isolated species. Forty three randomly selected strains were further tested for their susceptibility against a panel of antibiotics. Thirty isolates (81.08%) were resistant to four or more antibiotics with the highest resistance shown by E. coli (76.67%). All the Gram- negative isolates were resistant to Ampicilox, Cefuroxime and Amoxicillin.Conclusion: Urinary tract infections were found more in females in the area under study. As found in other studies, E. coli was the most predominant isolate, although other organisms seem to be on the increase.

A cross-sectional study was conducted to assess the frequencies and characteristics of occupational exposures among medical and nursing students at a Brazilian public university, in addition to their prevention and post-exposure behavior. During the second semester of 2010, a self-administered semi-structured questionnaire was completed by 253/320 (79.1%) medical students of the clinical course and 149/200 (74.5%) nursing students who were already performing practical activities. Among medical students, 53 (20.9%) suffered 73 injuries, which mainly occurred while performing extra-curricular activities (32.9%), with cutting and piercing objects (56.2%), in the emergency room (39.7%), and as a result of lack of technical preparation or distraction (54.8%). Among nursing students, 27 (18.1%) suffered 37 injuries, which mainly occurred with hollow needles (67.6%) in the operating room or wards (72.2%), and as a result of lack of technical preparation or distraction (62.1%). Among medical and nursing students, respectively, 96.4% and 48% were dissatisfied with the instructions on previously received exposure prevention; 48% and 18% did not always use personal protective equipment; 67.6% and 16.8% recapped used needles; 49.3% and 35.1% did not bother to find out the source patient's serological results post-exposure; and 1.4% and 18.9% officially reported injuries. In conclusion, this study found high frequencies of exposures among the assessed students, inadequate practices in prevention and post-exposure, and, consequently, the need for training in “standard precautions” to prevent such exposures.

From March 1991 to April 1992, 250 measles suspected cases were studied in the Municipality of Niterói, State of Rio de Janeiro. The median age found was 11 years and 76.0% of the cases were in school age children. Exposure histories were present in 149 patients and schools were the most frequent sites of transmission (45.0%). Vaccination status was known for 127 studied cases and 76.4% of them had received measles vaccine before their first birthday. One or more complications were reported for 68 cases aitd in 8.9% of the studied cases hospitalization was required. Frequency of complications varied according to each age group studied and were more commonly encountered among children < 1 year of age (55.6%). The history of previous vaccination did not diminish the number of complications of the cases studied. The results of this work show changes in age distribution of measles leading to sizeable outbreaks among teenagers and young adults.

Neste estudo reporta-se a identificação de rotavirus sorotipo G2 proveniente de amostras fecais de crianças com gastroenterite, da Cidade de Mérida, Yucatán, México. O diagnóstico virológico foi feito através da eletroforese em gel de poliacrilamidia e ensaio imunoenzimático. Das 149 amostras estudadas, 25 (16,7%) foram positivas para rotavirus do grupo A; dessas 23 (92%) foram sorotipo G2, subgrupo I e padrão eletroforético curto e 2 (8%) subgrupo II e padrão eletroforético longo, porém, não foi possível determinar o sorotipo G. Desde 1985, até hoje, foi a única vez que o sorotipo G2 foi identificado, em mais de 90% das amostras analisadas. Acredita-se que, provavelmente, nos últimos anos tem acumulado na população, um grupo de pessoas suscetíveis ao sorotipo de RV, e num futuro poderia ter um surto importante de gastroenterite, associado ao sorotipo G2 de rotavirus.

Este trabalho objetivou investigar a prevalência de infecção pelo vírus da hepatite C e identificar possíveis fatores de risco para sua transmissão, em 406 indivíduos portadores do vírus da imunodeficiência humana, maiores de dezoito anos de idade, atendidos na rede pública de saúde da cidade de Belém, Pará, situada na Amazônia brasileira. Os exames referentes ao anti-VHC foram realizados pelo método de Elisa e a pesquisa do VHC RNA através da reação de polimerase em cadeia. A prevalência de infecção, atual ou pregressa, pelo vírus da hepatite C foi de 16% (IC: 12,4 - 19,6). A análise multivariada mostrou associação do vírus C com as variáveis idade, cujo risco significante recaiu no grupo com cinqüenta ou mais anos (OR=9,75), antecedente de transfusão de sangue (OR=4,74) e uso de droga ilícita injetável (OR=149,28). A prevalência do vírus da hepatite C entre os usuários de drogas injetáveis foi de 83,7% e de 22,1% na população de transfundidos. Estes resultados indicam a efetiva transmissão do vírus C através da exposição percutânea e reafirmam o grande potencial de risco para hepatite C contido no uso injetável de drogas ilícitas.

Este trabalho, realizado a partir de outubro de 2000 até janeiro de 2003, descreve as características clínico-epidemiológicas da leishmaniose visceral em Três Lagoas, Mato Grosso do Sul. Foram confirmados 149 casos da doença, com predominância do sexo masculino (71,1%). A faixa etária mais acometida foi de 0 a 4 anos (42%). Quanto ao quadro clínico, estiveram presente febre (97,3%), esplenomegalia (85,9%) e anemia (75,8%). Em 32 (21,5%) pacientes ocorreu infecção associada, predominando a pneumonia. Sobre as alterações laboratoriais observou-se mediana de hemoglobina de 8mg/dl e de leucócitos de 3.100 cel/mm³. O esfregaço de medula foi positivo em 90,6%. Dos 97,9% de pacientes tratados, 78,2% utilizaram antimoniato pentavalente. Ocorreram 8% de óbitos, metade deles portadores de infecção associada. O conjunto destes dados sugere mudanças no padrão fisiográfico de ocorrência da leishmaniose visceral na localidade, com expansão e urbanização da doença, necessitando atenção para o diagnóstico e tratamento precoces.

Um estudo seccional foi realizado nas Vilas Waimiri e Atroari em Balbina, entre julho e outubro de 2006, com o objetivo de estimar a freqüência de anticorpo antiToxocara canis da classe IgG e avaliar as variáveis epidemiológicas e socioculturais. Foram estudadas 34 famílias e incluídos 100 indivíduos, o que correspondeu a 5% (100/2.000) da população das vilas. A idade variou de zero a 76 anos (M=22,9 Dp=18). Quanto ao gênero, 53% eram femininos e 47% masculino; 52% das amostras foram positivas para Toxocara canis, 44,5% negativas e 3,2% inconclusivas. Observou-se menor número de indivíduos com sorologia negativa na Vila Atroari 29,5% (13/44) em comparação com a Waimiri 46,4% (26/56). Com relação ao contato com cães, dos 55 indivíduos com contato domiciliar 60% (33/55) foram positivos para anticorpo antiToxocara canis Apresentaram sorologia positiva 66,6% (10/15) dos indivíduos que tinham contato domiciliar com filhotes de cão (chi²22,149 p=0,008). A existência de contato domiciliar com cães e filhotes mostrou associação com a presença de anticorpo anti-Toxocara canis na população estudada.

Introduction A few older publications describe leprosy associated with pregnancy, a situation that has been linked to leprosy exacerbation. This study aimed to describe the detection rate of this association in the State of Pará by county and Integration Region (IR) from 2007 to 2009 via an analysis of sociodemographic, epidemiological and operational indices. Methods This was a descriptive study using information generated by the SINAN. The Detection Coefficient of the Leprosy and Pregnancy Association (DCLP) epidemiological index was constructed to help interpret the endemicity parameters. The disease was considered hyperendemic when greater than two cases per 10,000 inhabitants were identified. Results During the study period, 149 associations were detected, with 14 hyperendemic counties: seven in 2007, five in 2008 and two in 2009. The Carajás Integrated Region displayed the highest DCLP index in the period. Eldorado dos Carajás had the single highest DCLP index (5.7/10,000 inhabitants, 2008), whereas the DCLP index in Conceição do Araguaia was very high in all three years. However, most counties displayed low or medium DCLP indices. The annual averages were 0.31 DCLP (2007), bass; 0.30 (2008), bass and 0.19 (2009), bass. The average DCLP index was 0.26, which is considered low. Three clusters of medium endemicity were identified by the average DCLP in the study period. Conclusions The analyses indicated that the surveillance program is still unsatisfactory in Pará. The interpretation of the endemicity parameters enabled qualitative and quantitative analyses to determine the epidemiological panorama of this association. The identification of high endemicity requires further clarification.

Introduction Knowledge of the prevalence and risk factors for Toxoplasma gondii dissemination among pregnant women is relevant because the parasite can be spread from mother to infant. The objective of this study was to assess the epidemiology and risk factors of toxoplasmosis in pregnant women from Gurupi, State of Tocantins, Brazil, from February 2012 to June 2013. Methods The study population included 487 pregnant women. Sociodemographic, dietary and cultural data were collected using a standardized and validated form. Peripheral blood was collected for serologic testing using the ELISA test (IgM/IgG antibodies). The data were analyzed by comparing seropositivity with risk factors using crude and adjusted odds ratios. Results The prevalence rate for IgG and IgM antibodies was 68.7% and 5.7%, respectively. Sociodemographic characteristics associated with toxoplasmosis risk included the following: education level ≤ 8 years (OR: 6.612; CI: 1.450-30.144), age ≥ 30 years (OR: 5.273; CI: 1.166-23.844), working outside the home (OR: 1.604; CI: 1.015-2.536), and family income of two minimum wages or lower (OR: 2.700; CI: 1.891-8.182). Regarding dietary habits, there was a significant association of seropositivity with meat intake (OR: 1.78; CI: 1.149-4.080), cutting vegetables without washing the cutting board beforehand (OR: 2.051; CI: 1.165-3.614), frequent intake of vegetables (OR: 2.051; CI: 1.368-3.006) and in natura milk intake (OR: 2.422; CI: 1.014-5.785). Conclusions The high prevalence rates of toxoplasmosis in Gurupi are related to age, raw meat and in natura milk intake, as well as education level, working outside the home, and poor hygienic habits during meal preparation.
