Tendo-se como objetivo conhecer o comportamento nutricional da cultura de Gergelim (Sesamum indicum L.), instalou-se um ensaio no campo experimental do Departamento de Agricultura e Horticultura, da Escola Superior de Agricultura "Luiz de Queiroz" da Universidade de São Paulo, visando determinar as concentrações e acúmulo de micronutrientes pelo cultivar Venezuela em diferentes estádios de desenvolvimento, e avaliar a exportação pela colheita. O experimento foi conduzido no decorrer do ano agrícola 1981/1982, num solo Terra Roxa Estruturada, Série Luiz de Queiroz. O delineamento estatístico foi inteiramente casualizado, com um cultivar, oito épocas de amostragem e quatro repetições. As primeiras amostras foram coletadas 28 dias após a emergência das plantulas (desbaste) e as demais em intervalos regulares de 12 dias, de tal maneira que sempre houvessem outras quatro competitivas na fileira. No material coletado (folha, caule e fruto), determinou-se os teores e acúmulo de micronutrientes, com exceção do molibdênio e cloro. Concluiu-se que: - a concentração dos micronutrientes foi sempre maior nas folhas do que nos caules; - a acumulação dos micronutrientes foi sempre maior nas folhas do que nos caules, com exceção do zinco; - a concentração dos micronutrientes nos órgãos amostrados, ocorreu na seguinte ordem: Folha: Fe(772,46ppm) > Mn(311,83ppm) > Zn(117,20ppm) > B(95,33ppm) > Cu(22,99 ppm). Caule: Fe(528,08ppm) > Mn(148,41ppm) > B (72,28ppm) > Zn(65,09ppm) > Cu(15,89 ppm). Fruto: Fe(282,24ppm) > B(4104ppm) > Mn (30,87ppm) > Zn(22,04ppm) > Cu(l6,33 ppm). - A acumulação total dos micronutrientes pela planta inteira foi crescente com a idade até os 112 dias, com exceção do B nas folhas e B e Fe nos caules; - a acumulação pela planta (mg/planta) foi em ordem decrescente: Fe(13M2) > Mn(21,64) > Zn(10,54) > B(8,01) > Cu(3,81); - a exportação através da colheita (mg/planta) foi em ordem decrescente: Fe(67J7) > B(3,92) > Mn(3,75) > Zn (2,67) > Cu(1,54).

To assess the prevalence of primary resistance of human immunodeficiency virus type 1 (HIV-1) to antiretrovirals, 84 patients chronically infected with HIV without prior antiretroviral treatment from Northeast Brazil were studied. Genotyping was performed using the ViroSeqTM Genotyping System. Thimidine analog mutations occurred in 3 (3.6%) patients. Accessory mutations related to NRTI occurred in 6 (7.1%) and related to PI in 67 (79.8%). Subtypes B (72.6%), F (22.6%), B/F 3 (3.6%), and C (1.2%) were detected. A low prevalence of major mutations related to NRTI in patients chronically infected by HIV was observed.

Polymorphisms of the p53 gene, which participates in DNA repair, can affect the functioning of the p53 protein. The Arg and Pro variants in p53 codon 72 were shown to have different regulation properties of p53-dependent DNA repair target genes that can affect various levels of cytogenetic aberrations in chronic hepatitis B patients. The present study aimed to examine the frequency of chromosomal aberrations and the mitotic index in patients with chronic hepatitis B and their possible association with p53 gene exon 4 codon 72 Arg72Pro (Ex4+119 G>C; rs1042522) polymorphism. Fifty-eight patients with chronic hepatitis B and 30 healthy individuals were genotyped in terms of the p53 gene codon 72 Arg72Pro polymorphism by PCR-RFLP. A 72-h cell culture was performed on the same individuals and evaluated in terms of chromosomal aberrations and mitotic index. A high frequency of chromosomal aberrations and low mitotic index were detected in the patient group compared to the control group. A higher frequency of chromosomal aberrations was detected in both the patient and the control groups with a homozygous proline genotype (13 patients, 3 control subjects) compared to patients and controls with other genotypes [Arg/Pro (38 patients, 20 control subjects) and Arg/Arg (7 patients, 7 control subjects)]. We observed an increased frequency of cytogenetic aberrations in patients with chronic hepatitis B. In addition, a higher frequency of cytogenetic aberrations was observed in p53 variants having the homozygous proline genotype compared to variants having other genotypes both in patients and healthy individuals.

OBJETIVO: Inquéritos sorológicos realizados em diversos países mostraram uma maior prevalência da infecção pelo vírus da hepatite B (VHB) em dentistas, especialmente entre os cirurgiões, do que na população geral. O estudo realizado objetivou determinar a prevalência e os fatores associados à vacinação contra hepatite B (HB) entre os dentistas e investigar as principais razões alegadas para a não vacinação e vacinação incompleta. MÉTODOS: Foi conduzido um inquérito entre 299 cirurgiões dentistas residentes em Montes Claros, MG, por meio de questionário auto-aplicável. Foi determinada a prevalência de vacinação segundo o número de doses e os fatores associados à não vacinação e à vacinação incompleta através de regressão logística multinomial. RESULTADOS: Dos 299 questionários distribuídos, 296 (99%) foram respondidos. Destes, 74,9% tomaram três doses; 14%, duas doses; 2%, uma dose e 10% não foram vacinados. A vacinação completa foi maior entre os que relataram fazer exclusivamente cirurgia e/ou periodontia (89%). A principal razão alegada para a não vacinação ou vacinação incompleta foi a necessitade de maiores informações. A não vacinação foi mais freqüente entre aqueles com mais de 40 anos (OR=8,62; IC 95%: 1,88-39,41) e os que não se reciclaram nos dois anos prévios ao inquérito (OR=2,72; IC 95%: 1,02-7,22). A vacinação incompleta foi maior entre os que não usam luva no trabalho (OR=2,32; IC 95%: 1,08-4,97). CONCLUSÃO: A falta de informação, possivelmente relacionada a menor reciclagem profissional, parece ser um dos principais fatores limitantes da vacinação.

280 students, between 6 and 14 years old, residents in the Iguape county, southern coast of the State of São Paulo, were studied in order to identify the existence of a possible association between the prevalence of specific antibodies to the hepatitis B virus and the exposure to haematophagous mosquitoes, evaluated indirectly through the prevalence of antibodies to 17 arboviruses isolated in Brazil. The children were from 4 areas with different topographical characteristics: 89 of the children were from the urban zone of the town of Iguape, 89 were from the periurban zone, 30 were from the rural area with extensive banana plantations, and 72 were from the jungle zone. Previous studies had shown significantly higher prevalence of antibodies to different arboviruses in the cultivated zone and the jungle zone, when compared to the urban and periurban zones of Iguape. The detection of antibodies to the HBV surface antigen (HBs Ag) was done through the radioimmunoassay (Ausab, Abbott Laboratory). The cases considered positive were confirmed through the presence of anti-core HBV antibodies (anti-HBc-EIA Roche). A significantly higher prevalence of anti-HBV antibodies was observed in children from the jungle zone (26/72 = 36,1% ) when compared to those from the urban zone (5/89 = 5,6%), peri-urban (6/89 = 6,7%) or from the cultivated zone (0/30 = 0%). The result suggest the existence of a common factor in the dissemination of the arboviruses and the hepatitis B virus, supporting the hypothesis that mosquitoes may play an important role in the HBV transmission in tropical forested region.

Group B Streptococcus is the most common pathogen found in neonatal sepsis in North America. OBJECTIVES: We describe 15 cases of neonatal infections by Group B Streptococcus (Streptococcus agalactiae) at a Neonatal Intensive Care Unit of a public and teaching hospital. METHODS: We conducted a study at Hospital de Clínicas de Porto Alegre, from January 1st, 1996 to June 30, 1999. Diagnosis of neonatal infection was established according to the findings of Group B Streptococcus in blood culture associated with alterations resembling sepsis on the basis of clinical picture and laboratory findings. RESULTS: Fifteen cases of neonatal infections by Group B Streptococcus were detected. Eleven cases consisted of early-onset sepsis, 2 cases of occult bacteremia and 2 cases of late-onset sepsis. Eight cases had septic shock (53%), 8 cases had pneumonia (53%), and 4 cases had meningitis (27%). Fourteen cases were diagnosed from a positive blood culture, and 1 case from evidence of these bacteria in pulmonary anatomopathological examination. Thirteen cases (87%) were diagnosed before 72 hours of life. We had 3 deaths (20%), and 3 cases of meningitis developing neurological deficits. CONCLUSIONS: Streptococcus Group B is one of the most important pathogens in the etiology of early-onset neonatal sepsis at our hospital, with high mortality and morbidity. However, we do not know the incidence of GBS neonatal infections at other hospitals. More data are needed to establish a basis for trials of different strategies to reduce these infections.
Marcadores sorológicos das hepatites B e C em doadores de sangue do Hemocentro de Ribeirão Preto, SP

Este estudo envolveu 25.891 doadores de sangue que compareceram pela primeira vez ao Hemocentro de Ribeirão Preto, entre 23/06/1996 e 22/06/2001. Seu objetivo foi estudar a positividade de marcadores sorológicos das hepatites B e C em testes de triagem e estimar a prevalência de infecção atual ou pregressa pelos vírus de ambas as hepatites, através da análise dos resultados de testes confirmatórios. Estudaram-se dados do Hemocentro e do Hospital das Clínicas da Faculdade de Medicina, coletando-se informações referentes ao doador e aos resultados dos testes sorológicos. A população foi composta majoritariamente por homens (83,6%) de 26 a 45 (64%) anos de idade. Os valores de positividade nos testes da triagem foram 0,6% (IC95%: 0,54 0,72) para o HBsAg e 1,2% (IC95%: 1,02 1,28), para o anti-HCV. Os valores da prevalência, nos testes confirmatórios, foram 0,2% (IC95%: 0,16 0,28), para a hepatite B, e 0,3% (IC95%: 0,24 0,38) para a hepatite C.

OBJECTIVE: To evaluate whether apolipoproteins A-I (Apo A-I) and B (Apo B) have, higher ensitivity (SN), specificity (SP) and positive predictive value (PPV) than lipoproteins (LP), total cholesterol (TC), high density lipoprotein (HDL), low density lipoprotein (LDL), very low density lipoprotein (VLDL), and triglycerides (TGL) in assessing the risk of coronary heart disease (CHD). METHODS: This is a transversal case-control study of 241 patients, who were divided into two groups: 1) 145 patients with CHD, and 2) 96 patients without coronary disease. A model of logistic regression to evaluate the relation between the LPs and CHD was developed in which variables with a p-alpha <0.1 were included. RESULTS: Apo A-I levels were higher in the patients without CHD, (OR 2.08, CI 1.20-3.57). There were no statistical differences between the values of Apo A-I and the remaining lipid fractions (Apo A-I: 67%; Apo B: 100%; PPV: TC= 71%; TGC=71%; HDL=71%; LDL=71%). The costs of the tests in Reais were as follows: Apo A-I: R$ 56.60; Apo B-100: R$ 56.60; TC: R$ 9.94; HDL: R$ 21.30; LDL: R$ 28.40; TGL: R$ 14.20. CONCLUSION: Levels of Apo A-I and Apo B have no advantage over conventional lipoproteins in predicting the risk of CHD, despite the statistical association between Apo A-I and CHD; in addition, their costs are higher than those of the conventional lipoproteins.

OBJECTIVE: To determine the utility of B-type natriuretic peptide (BNP) in the diagnosis of congestive heart failure (CHF) in patients presenting with dyspnea to an emergency department (ED). METHODS: Seventy patients presenting with dyspnea to an ED from April to July 2001 were included in the study. Mean age was 72±16 years and 33 (47%) were male. BNP was measured in all patients at the moment of admission to the ED. Emergency-care physicians, blinded to BNP values, were required to assign a probable initial diagnosis. A cardiologist retrospectively reviewed the data (blinded to BNP measurements) and assigned a definite diagnosis, which was considered the gold standard for assessing the diagnostic performance of BNP. RESULTS: The mean BNP concentration was higher in patients with CHF (n=36) than in those with other diagnoses (990±550 vs 80±67 pg/mL, p<0.0001). Patients with systolic dysfunction had higher BNP levels than those with preserved systolic function (1,180±641 vs 753±437 pg/mL, p=0.03). At a blood concentration of 200 pg/mL, BNP showed a sensitivity of 100%, specificity of 97.1%, positive predictive value of 97.3%, and negative predictive value of 100%. The application of BNP could have potentially corrected all 16 cases in which the diagnosis was missed by the emergency department physician. CONCLUSION: BNP measurement is a useful tool in the diagnosis of CHF in patients presenting to the ED with dyspnea.

FUNDAMENTO: A avaliação clínica nem sempre é suficiente para predizer complicações cardíacas pós-operatórias (PO). O peptídeo natriurético do tipo B (BNP) tem grande valor prognóstico em pacientes (pts) com insuficiência cardíaca. Seu valor como preditor de eventos em cirurgias ortopédicas ainda não foi testado. OBJETIVO: Avaliar o valor do BNP em predizer complicações cardíacas no PO de cirurgia ortopédica. MÉTODOS: Avaliados de modo prospectivo, 208 pts submetidos à cirurgia para correção de fratura de fêmur e artroplastia de quadril ou de joelho. Foram 149 (71,6%) mulheres e a idade média foi de 72,6 ± 8,8 anos. Os pacientes foram submetidos, no pré-operatório, à avaliação clínica convencional e estimativa do risco cirúrgico pela classificação da American Society of Anesthesiologists (ASA). O BNP foi dosado no pré-operatório e avaliou-se a sua capacidade de predizer eventos cardíacos (morte, infarto agudo do miocárdio, angina instável, fibrilação atrial, taquicardia ventricular ou insuficiência cardíaca) no PO, através de análise multivariada por regressão logística. RESULTADOS: Dezessete (8,0%) pacientes apresentaram eventos cardíacos. A mediana de BNP foi significativamente maior nesses pacientes quando comparada a dos sem eventos cardíacos (93 [variação interquartil 73-424] vs 26,6 [13,2-53,1], p = 0,0001). O BNP foi o principal preditor independente de eventos (p = 0,01). A classificação da ASA não foi preditor independente. A análise de curva ROC demonstrou que para um corte de 60 pg/ml, o BNP apresentou sensibilidade de 76,0% e especificidade de 79,0% para predizer eventos, com área sob a curva de 83,0%. CONCLUSÃO: O BNP é um preditor independente de eventos cardíacos no PO de cirurgias ortopédicas.

A total of 250 dentists (53.6% men and 46.4% women), with a mean age of 35.1 ± 9.8 years, were submitted to serological tests for the diagnosis of hepatitis B (HB) - HBsAg, anti-HBs, anti-HBc, HBeAg, and anti-HBe - using a radioimmunoassay. One or more of these markers were detected in 78 individuals (31.2%) who were excluded from the group to be vaccinated. Of the 172 HB-susceptible individuals, 135 (78.5%) responded to the call and were intradermally injected with three 2 µg doses of the Belgian HB recombinant vaccine, applied at an interval of one month between the 1st and 2nd dose and of five months between the 2nd and 3rd dose. A new determination of HB markers carried out 50 days after the 3rd dose showed that 110 (81.5%) individuals had become anti-HBs positive (65.5% good responders and 34.5% poor responders). Mean serum anti-HBs titer of these 110 dentists was 42.4 U S/N, similar in both sexes. The adverse effects analyzed in 106 dentists were: (a) local: pain (12.3%), burning sensation (14.1%), pruritus (25.5%), erythema (28.3%), local heat (18.9%), and a hypochromic spot (32.1%); (b) systemic (4.7%): discomfort in two patients, and fever, anorexia, and asthenia in one patient each. Intradermal administration of a fourth 2 µg vaccine dose to 39 dentists (poor or non-responders) increased the total number of anti-HBs-positive individuals from 110 (81.5%) to 114 (84.4%), with the number of good responders increasing from 72 (65.5%) to 85 (74.6%). We conclude that the Belgian recombinant vaccine applied in the scheme used here induces a high rate of seroconversion and causes only mild and transitory adverse effects.

There are about 350 million hepatitis B virus (HBV) carriers worldwide and chronic HBV is considered a major public health problem. The objective of the present study was to assess the effectiveness of the nucleos(t)ide analogues tenofovir (TDF) and entecavir (ETV) in the treatment of chronic HBV. A cross-sectional study was carried out from March-December 2013, including all patients with chronic HBV, over 18 years of age, undergoing therapy through the public health system in southern Brazil. Only the data relating to the first treatments performed with TDF or ETV were considered. Retreatment, co-infection, transplanted or immunosuppressed patients were excluded. Six hundred and forty patients were evaluated, of which 336 (52.5%) received TDF and 165 (25.8%) ETV. The other 139 (21.7%) used various combinations of nucleos(t)ide analogues and were excluded. The negativation of viral load was observed in 87.3% and 78.8% and the negativation of hepatitis B e antigen was achieved in 79% and 72% of those treated with ETV or TDF, respectively. Negativation of hepatitis B surface antigen was not observed. There was no occurrence of adverse effects. This is a real-life study demonstrating that long-term treatment with ETV and TDF is both safe and effective.

Entre os fatores que limitam a produtividade do mamoeiro, destacam-se a disponibilidade de água e nutrientes minerais. Assim, o ajuste de uma lâmina de irrigação adequada para a cultura exige também o monitoramento do estado nutricional das plantas. O objetivo desse trabalho foi avaliar a resposta das diferentes lâminas de irrigação (0; 40; 80; 120; 160; 200 e 240% da ETo) sobre os teores de nutrientes nas folhas do mamoeiro 'Improved Sunrise Solo 72/12'. As avaliações dos teores de nutrientes no mamoeiro foram realizadas em junho de 1999, retirando-se amostras de duas plantas de cada tratamento, das quais foram analisados, separadamente, o limbo e o pecíolo foliar. Os resultados indicam que os teores de N, P, K, Ca, Mg, B, Na, Fe e Cu, no limbo, e os teores de N, P, Ca, Mg, S, B, Na e Fe, no pecíolo, variaram significativamente com a lâmina de água aplicada. A concentração desses nutrientes em função das lâminas de irrigação foi explicada, na maioria dos casos, por regressões lineares, decrescentes para N, K e Fe, e crescente para os demais nutrientes. A variação significativa dos teores de nutrientes, em função da irrigação, mostra a necessidade de estudos que permitam o ajuste da adubação à lâmina de irrigação empregada.

Patients with diffuse large B-cell lymphoma treated in a University Hospital were studied from 1990 to 2001. Two treatment regimens were used: ProMACE-CytaBOM and then, from November 1996 on, the CHOP regimen. Complete remission (CR), disease-free survival (DFS), and overall survival (OS) rates were determined. Primary refractory patients and relapsed patients were also assessed. A total of 111 patients under 60 years of age were assessed and ranked according to the international prognostic index adjusted to age. Twenty (18%) of them were classified as low risk, 40 (36%) as intermediate risk, 33 (29.7%) as high intermediate risk, and 18 (16.3%) as high risk. Over a five-year period, OS and DFS rates were 71 and 59%, respectively, for all patients. For the same time period, OS and DFS rates were 72.8 and 61.3%, respectively, for 77 patients treated with CHOP chemotherapy and 71.3 and 60% for patients treated with the ProMACE-CytaBOM protocol. There was no significant difference in OS or DFS between the two groups. Eleven of 50 refractory and relapsed patients were consolidated with high doses of chemotherapy. Three received allogenic and 8 autologous bone marrow transplantation. For the latter, CR was 62.5% and mean OS was 41.1 months. The clinical behavior, CR, DFS, and OS of the present patients were similar to those reported in the literature. We conclude that both the CHOP and ProMACE-CytaBOM protocols can be used to treat diffuse large B-cell lymphoma patients, although the CHOP protocol is preferable because of its lower cost and lower toxicity.

The etiology of respiratory distress syndrome (RDS) is multifactorial and multigenic. Studies have suggested that polymorphisms and mutations in the surfactant protein B (SP-B) gene are associated with the pathogenesis of RDS. The objectives of this study were to determine and compare the frequencies of SP-B gene polymorphisms in preterm babies with and without RDS. We studied 151 neonates: 79 preterm babies without RDS and 72 preterm newborns with RDS. The following four SP-B gene polymorphisms were analyzed: A/C at -18, C/T at 1580, A/G at 9306, and G/C at nucleotide 8714. The polymorphisms were detected by PCR amplification of genomic DNA and genotyping. The genotypes were determined using PCR-based converted restriction fragment length polymorphisms. The control group consisted of 42 (53%) girls and 37 (47%) boys. Weight ranged from 1170 to 3260 g and mean gestational age (GA) was 33.9 weeks (range: 29 to 35 weeks and 6 days). The RDS group consisted of 31 (43%) girls and 41 (57%) boys. Weight ranged from 614 to 2410 g and mean GA was 32 weeks (range: 26 to 35 weeks). The logistic regression model showed that GA was the variable that most contributed to the occurrence of RDS. The AG genotype of the A/G polymorphism at position 9306 of the SP-B gene was a protective factor in this population (OR = 0.1681; 95%CI = 0.0426-0.6629). We did not detect differences in the frequencies of the other polymorphisms between the two groups of newborns.
