Forty one cases of pemphigus vulgaris and thirty cases of pemphigus foliaceus were investigated at Hospital Universitário Clementino Fraga Filho from 1978 to 1999. They were divided into two treatment groups: one group received up to 100 mg of oral prednisone daily and the other group received >120 mg daily. The dose up to 100 mg provided good initial control of pemphigus vulgaris and pemphigus foliaceus and did not increase the mortality rate associated to disease. The dose >120 mg induced higher morbidity. These data allowed us to establish a regimen of oral prednisone (1-2 mg/kg/daily) with maximum of 120 mg daily in the treatment of pemphigus vulgaris and pemphigus foliaceus.

BACKGROUND: Pemphigus vulgaris (PV) is an autoimmune disease characterized by blistering of the skin and mucosa, which develops due to the interaction between predisposing genetic and environmental factors. Infections caused by members of the Herpesviridae family have been suggested as a possible triggering factor for PV. OBJECTIVE AND METHODS: In this report, we investigate the presence of herpesviruses in refractory lesions on the right upper eyelid. The lesion has persisted despite the treatment with corticosteroids. Polymerase chain reaction (PCR) and DNA sequence analysis have been used to detect the DNA of HSV 1/2, VZV, EBV, CMV, HHV-6, HHV-7, and HHV-8. RESULTS: The sample collected from the right upper eyelid has tested positive for HSV 1/2. Sequence analysis has confirmed the PCR results and allowed the identification of the HSV strain as belonging to type 1. After treatment with acyclovir, the lesion of the right upper eyelid has cleared and not relapsed. CONCLUSION: When patients present PV lesions which are refractory to corticosteroid therapy, herpetic infection should be considered.

Nail involvement in pemphigus vulgaris is rare. We describe 5 patients with pemphigus vulgaris presenting nail involvement. In this disease, nail manifestations present, by order of frequency, as chronic paronychia, onychomadesis, onycholysis, Beau's lines and trachyonychia. All our 5 cases presented with paronychia, and 1 of them also had Beau's lines. Treatment with prednisone and/or cyclophosphamide controlled mucocutaneous and nail manifestations in all cases.

Pemphigus are autoimmune intraepidermal blistering diseases in which immunoglobulin G (IgG) autoantibodies are directed against desmosomal glycoproteins. The aim of this study was to determine the IgG subclass profile of endemic pemphigus foliaceus (fogo selvagem) and pemphigus vulgaris utilizing indirect immunofluorescence. PATIENTS AND METHODS: Twenty-five patients with pemphigus vulgaris, 25 with endemic pemphigus foliaceus (fogo selvagem), and 25 healthy controls were analyzed by indirect immunofluorescence for circulating autoantibodies (total IgG and its subclasses). RESULTS: Our data revealed a significant correlation (P <.05) of disease activity and autoantibody levels in both forms of pemphigus, i.e., negative titers related to clinical remission, whereas positive results related to active disease. Immunoglobulin G subclass analysis in fogo selvagem demonstrated that in patients in remission, 56% showed positive immunoglobulin G4; in active disease, immunoglobulin G4 was the predominant subclass (100% positive in all cases). The IgG subclass profile in pemphigus vulgaris showed that in patients in remission, only 10% were positive for immunoglobulin G4; in active disease, positivity for immunoglobulin G4 was present in 78% to 88% of the cases. CONCLUSION: Subclass characterization of immunoglobulin G autoantibodies is a useful tool for pemphigus follow-up, since immunoglobulin G4 (IgG4) is the subclass that is closely related to recognition of pathogenic epitopes, and consequently with disease activity. Careful monitoring should be performed for fogo selvagem in clinical remission with a homogeneous IgG4 response, since this may indicate more frequent relapses.

Neste estudo objetivou-se avaliar e selecionar variedades de feijoeiro (Phaseolus vulgaris L.) quanto à tolerância ao estresse salino e identificar, usando variedades de feijoeiro com diferentes graus de tolerância, variáveis que auxiliem na discriminagão de variedades de feijoeiro quanto à tolerância a esse tipo de estresse, independentemente do mecanismo apresentado pela planta. Os experimentos foram realizados em casa de vegetagão. Inicialmente foram avaliadas 48 variedades de P. vulgaris (alocadas nas subparcelas) em dois níveis de salinidade (distribuídos nas parcelas): solução nutritiva normal (SNN) a 0,81 dS m-1 e a teste (SNT) a 5,6 dS m-1, obtida pela adigão de NaCl no delineamento experimental em blocos casualizados, com quatro repetigões. A partir da massa seca da parte aérea calculou-se a relagão percentual de crescimento alcançada na SNT relativo à SNN das variedades, que variou de 138,7 a 54,1 %, discriminando-as de acordo com o critério de Scott-Knott em duas populagões: uma com 14 variedades mais "tolerantes" e outra com 34 variedades, onde ficaram agrupadas variedades "moderadamente tolerantes" e "sensíveis". Para identificar variáveis de crescimento que permitam selecionar feijoeiros quanto à tolerância ao estresse salino duas variedades tolerantes (Vermelho e CNF 5574), uma medianamente tolerante (FT 83-86) e uma sensível (LM 30074), classificadas no experimento anterior, foram cultivadas em solução nutritiva com cinco níveis de salinidade (0,81; 2,7; 4,6; 6,5; e 8,4 dS m-1). Analisando-se a massa seca da raiz, do caule, do pecíolo, das folhas e da parte aérea, a área foliar e a área foliar específica, concluiu-se que a área foliar específica foi o índice que efetivamente mais contribuiu para a discriminação das variedades de feijoeiro quanto à tolerância à salinidade.

Este trabalho teve como objetivo avaliar a aceitabilidade de feijão preto (Phaseolus vulgaris L.), fortificado com micropartículas de ferro, para que seja posteriormente utilizado na prevenção e controle da anemia ferropriva. Foram preparadas três amostras de feijão preto cozido, sendo uma sem adição de micropartículas de ferro (controle); à segunda foi adicionada quantidade equivalente a 5 mg de ferro, para cada concha média de feijão, e, à última amostra, 10 mg de ferro para cada concha média. Utilizou-se escala hedônica de cinco pontos para avaliação dos atributos. Com base nos resultados obtidos, as amostras controle e 5 mg/porção foram as mais aceitas; houve, porém, pouca diferença em relação à amostra com 10 mg/porção. A boa aceitação do alimento pelos provadores mostra ser viável a fortificação de feijão preto com ferro microencapsulado, podendo ser usado na prevenção e controle da anemia ferropriva.

A study was conducted on 16 patients with pemphigus foliaceus, ten of them with the localized form (group G1) and six with the disseminated form (group G2). These patients were submitted to full blood counts, quantitation of mononuclear cell subpopulations by monoclonal antibodies, study of blastic lymphocyte transformation, and quantitation of circulating antibodies by the indirect immunofluorescence test, in order to correlate their clinical signs and symptoms and laboratory data with their immunological profile, and to determine the relationship between circulating autoantibody titers and lesion intensity and course of lesions under treatment. Leucocytosis was observed especially in group G2. All patients showed decreased relative CD3+ and CD4+ values and a tendency to decreased relative values of the CD8+ subpopulation. Blastic lymphocyte transformation indices in the presence of phytohemagglutinin were higher in patients (group G1+G2) than in controls. The indirect immunofluorescence test was positive in 100% of G2 patients and in 80% of G1 patients. The median value for the titers was higher in group G2 than in group G1. Analysis of the results as a whole permits us to conclude that cell immunity was preserved and that there was a relationship between antibody titers detected by the direct immunofluorescence test and extent of skin lesions.

Sixteen patients with endemic pemphigus foliaceus were submitted to nutritional evaluation. Ten had the localized form of the disease (Group G1) and six the disseminated form (Group G2). The patients were submitted to anthropometric measurements (weight, height, Quetelét index, tricipital skin fold, subscapular skin fold, arm circumference, arm muscle circumference, arm area, arm muscle area, and arm adipose area) and to laboratory evaluation by protein electrophoresis. Arm circumference, arm area and arm muscle area showed lower values in G2 than in G1. Weight and arm muscle circumference tended to the lower in G2 than in G1. Protein electrophoresis showed decreased albumin levels in both groups, with lower values in G2. Overall analysis of the results permits us to conclude that patients with endemic pemphigus foliaceus present signs and symptoms of protein, but not calorie, malnutrition and that this malnutrition is more marked in patients with disseminated pemphigus foliaceus.

Our objective was to compare food intake and nutritional status of Pemphigus Foliaceus patients (PG) on long term glucocorticoid therapy to a Control Group (CG). Fourteen PG female inpatients receiving prednisone (0.33 ± 0.22mg/kg) for at least 12 months and twelve CG subjects were submitted to nutritional evaluation, including anthropometry, urinary creatinine determination and serum biochemical measurements, besides 48-h-based food intake records. Groups were compared by Chi-square, Mann-Whitney and "t" tests. PG patients and CG were paired, respectively, in relation to age (24.7 ± 14.1 vs. 22.0 ± 12.0 years), body mass index (25.8 ± 6.4 vs. 24.0 ± 5.6kg/m2), daily protein intake (132.9 ± 49.8 vs. 95.2 ± 58.9g), and serum albumin (median; range) (3.8; 3.5-4.1 vs. 3.8; 3.6-5.0g/dl). However, PG patients had lower height-creatinine index (64.8 ± 17.6 vs. 90.1 ± 33.4%), and higher daily energy (3080 ± 1099 vs. 2187 ± 702kcal) and carbohydrate (376.8 ± 135.8 vs. 242.0 ± 80.7g) intakes. Despite high food, protein and energy consumption, PG patients on long term glucocorticoid therapy had lower body muscle mass than controls, while showing high body fat stores. These findings are possibly related to combined metabolic effects of long term corticotherapy and inflammatory disease plus corticosteroid-induced increased appetite.

Endemic Pemphigus Foliaceus (EPF) is a bullous autoimmune skin disease whose incidence used to be high in the State of São Paulo (SP), Brazil, during the forties, but has declined thereafter. OBJECTIVES: to report a series of EPF patients from the northeastern region of SP. METHODS: a retrospective study concerning demographic and epidemiological data of patients seen from 1973 to 1998 was conducted at the University Hospital, Faculty of Medicine of Ribeirão Preto, SP. RESULTS: bullous disease was diagnosed in 340 patients, 245 with EPF (72.1%), 9.4 cases per year, 60.4% females, and 70.2% white, 7 to 82 year-old (29.4% in their teens); 46.9% lived in the rural zone. Concerning profession, housewives predominated among women (67.6%) and agricultural workers among men (40.2%). The time of disease was less than 1 year in 62.0% of cases, followed by 1 and 5 years (27%), and more than 5 years for the remaining patients (11%). 36.7% of patients were referred by the Direção Regional de Saúde (DIR) XVIII of Ribeirão Preto, with the largest number of cases being from Ribeirão Preto and Batatais: 33.3% and 23.3%, respectively; 22% from DIR XIII (Franca); 13.5% from DIR VII (Araraquara); 2.9% from DIR IX (Barretos); 4.1% from other DIRs of SP, and 20.8% from other States (16.7% from Minas Gerais). Thirteen (5.3%) patients reported occurrence of the disease in some relative, and 4 (1.6%) in neighbors. CONCLUSIONS: the present data characterize the northeastern region of the state of São Paulo as a remaining endemic focus of EPF.

Endemic pemphigus foliaceus, and long-term corticotherapy may affect serum lipid levels. The aim of this study was to compare serum lipids of pemphigus foliaceus patients on glucocorticoid therapy to a healthy control group. Fifteen patients receiving prednisone (0.33 ± 0.22mg/kg) for at least 12 months and 15 controls were submitted to 48-h food intake records, anthropometry, and biochemical measurements. Data were compared by chi2, Mann-Whitney and Student "t" tests. The groups were matched for gender, age, weight, body mass index, arm circumference and triceps skin fold. No differences were observed in relation to energy, fat, protein and carbohydrate daily intakes, total cholesterol, HDL, LDL, uric acid, and serum creatinine levels. Pemphigus foliaceus patients had higher triglyceride [159 (64-371) vs. 100 (45-133) mg/dl], VLDL [32 (13-74) vs. 20 (9-114) mg/dl] and ESR [44 (9-87) vs. 7 (1-30) mm/h] levels than controls, probably due to metabolic effects of inflammatory disease and corticotherapy.

The effects of tangerine (Phaseolus vulgaris Blanco), lemon (Citrus medica limonum Lush), pear orange (Citrus sinensis L. Osbeck), red copaiba (Copaifera langsdorffii Desf.), rosemary (Baccharis dracunculifolia De Candole), Eucalyptus (Eucalyptus globulus Labillardière and E. citriodora Hook), lemongrass (Cymbopogon citratus Stapf.) and citronella (Cimbopogon nardus Linnaeus) oils at several concentrations on Zabrotes subfasciatus (Boheman) were studied. In toxicity tests, grains of Phaseolus vulgaris L. cv. Rajadinho were impregnated with oils and infested with adults of Z. subfasciatus up to 24 hours old. All tested oils were effective in reducing the viable egg-laying and adult emergence of this pest, in function of the concentrations used, highlighting E. citriodora and E. globulus oils which caused 100% effectiveness from 0.5 mL Kg-1 concentration. In repellency tests, two arenas consisting of plastic containers, connected symmetrically to a central box by two plastic tubes were used. In one of the boxes, untreated beans were placed and on the other ones beans treated with each oil concentration were used. In the central box, five couples of Z. subfasciatus were released. Grains of P. vulgaris treated with oils of E. citriodora, C. citratus and C. oleifera reduced the attraction percentage of Z. subfasciatus adults, while the E. globulus increased this percentage. The percentages of reduced viable eggs ranged from 17.9% (C. medica limonum) to 93.3% (C. nardus), while the reduction on the number of emerged insects was 23.9% and 95.9%, respectively for these same oils.

According previous studies about longevity in maize by ACCORSI e ADÂMOLI DE BARROS, (1961) the authors presents in this paper the results of work on longevity of seedlings of beans. Seeds were separated in three groups according their weight, as followings: small 80-120 mg; medium 130-140 mg and big 150-200 mg. The sowing of the seeds was made in pure sand and the seedlings were distributed in distil. water and in complete solution of Arnon and Hoagland. Each treatment was made in two replications with eight seedlings by treatment. At present time the following conclusions can be related: 1.°) - Eight days after germination, the cotiledones of all the seedlings started to fell down, fourteen days after, all cotiledones had fell down. 2.°) - Fifteen days after germination, the seedlings in nutritive solution showed better development than those in distil. water. Table I e II gives results. 3.°) - All seedlings in distil. water showed symptoms of N, Ca, Fe deficiencies. 4.°) - Twenty nine days after germination the seedlings in distil. water manifested exhaust trace, by falling of the leaves and death of some plants although the aplicai buds keep green. 5.°) - After thirty-one days the plants in nutritives solution was in better condition than those in distil. water, although some alteration aboved mentioned was observed. The causes of this alteration are being studied. 6.°) - In many plants in complet solution the seminal leaves showed clorosis initial and some with necrosis, although apical buds keeps in ativity. 7.°) - Symptoms of clorosis and necrosis in diferents stages were observed in all leaflet; these symptoms were more strong in the groups of little seed and medium seeds.

Os aminoácidos livres da nova variedade de feijão (Phaseolus vulgaris L.) de 60 Dias, foram estudados por croma tografia em papel utilizando-se o seguinte par de solventes: 1) fenol-água, 8:2 (p/v); 2) butanol-ácido acético-água, 4:1:1 (v/v). Os aminoácidos identificados e dosados nos extratos foram: ácido aspártico, ácido glutãmico, lisina, serina, asparagina, glicina, treonina, alanina, glutamina, histidina, acido gama-aminobutírico, arginina, metionina, valina e leucina. Ás determinações quantitativas foram feitas por eluição das manchas dos cromatogramas seguidas de colorimetria. Os aminoácidos mais abundantes foram lisina, ácido glutâmico e ácido aspártico.
