We performed a histopathologic analysis to assess the extent of the extracapsular extension (ECE) beyond the capsule of metastatic lymph nodes (LN) in head and neck cancer to determine appropriate clinical target volume (CTV) expansions.

Background To determine the outcome and patterns of failure in oral cavity cancer (OCC) patients after postoperative intensity modulated radiotherapy (IMRT) with concomitant systemic therapy. Methods All patients with locally advanced (AJCC stage III/IV) or high-risk OCC (AJCC stage II) who underwent postoperative IMRT at our institution between December 2006 and July 2010 were retrospectively analyzed. The primary endpoint was locoregional recurrence-free survival (LRRFS). Secondary endpoints included distant metastasis-free survival (DMFS), overall survival (OS), acute and late toxicities. Results Overall 53 patients were analyzed. Twenty-three patients (43%) underwent concomitant chemotherapy with cisplatin, two patients with carboplatin (4%) and four patients were treated with the monoclonal antibody cetuximab (8%). At a median follow-up of 2.3 (range, 1.1–4.6) years the 3-year LRRFS, DMFS and OS estimates were 79%, 90%, and 73% respectively. Twelve patients experienced a locoregional recurrence. Eight patients, 5 of which had both a flap reconstruction and extracapsular extension (ECE), showed an unusual multifocal pattern of recurrence. Ten locoregional recurrences occurred marginally or outside of the high-risk target volumes. Acute toxicity grades of 2 (27%) and 3 (66%) and late toxicity grades of 2 (34%) and 3 (11%) were observed. Conclusion LRRFS after postoperative IMRT is satisfying and toxicity is acceptable. The majority of locoregional recurrences occurred marginally or outside of the high-risk target volumes. Improvement of high-risk target volume definition especially in patients with flap reconstruction and ECE might transfer into better locoregional control.

Familial hypomagnesemia with hypercalciuria and nephrocalcinosis is an autosomal recessive renal tubular disorder characterized by renal magnesium wasting, hypercalciuria, advanced nephrocalcinosis and progressive renal failure. Mutations in the paracellin-1 (CLDN16) gene have been defined as the underlying genetic defect. The tubular disorders and progression in renal failure are usually resistant to magnesium substitution and hydrochlorothiazide therapy, but hypomagnesemia may improve with advanced renal insufficiency. We present a patient with a homozygous truncating CLDN16 gene mutation (W237X) who had early onset of renal insufficiency despite early diagnosis at 2 months. He also had additional abnormalities including horseshoe kidney, neonatal teeth, atypical face, cardiac abnormalities including coarctation of the aorta associated with atrial and ventricular septal defects, umbilical hernia and hypertrichosis. To the best of our knowledge, this is the youngest case diagnosed as familial hypomagnesemia with hypercalciuria and nephrocalcinosis and the first case having such additional congenital abnormalities independent of the disease itself.

Endothelin regulates cytokine expression in vitro and in vivo. This study investigated the effects of chronic allograft rejection on hepatic endothelin-converting enzyme-1 (ECE-1) gene expression and endothelin-1 (ET-1) plasma clearance. Using the Lewis-F344 minor histocompatibility mismatch model of heterotopic cardiac transplantation, hepatic ECE-1 gene expression was measured by real-time polymerase chain reaction and host plasma clearance of ET-1 was measured 8 weeks after transplantation in the absence of immunosuppression. In animals undergoing allograft rejection, hepatic ECE-1 gene expression increased 2-fold (P < 0.05), whereas no effect of rejection on ET-1 clearance from plasma was observed. In summary, upregulation of ECE-1 gene expression occurs in the liver of the host during chronic allograft rejection. Because the liver represents both a key organ for cytokine production and for endothelin metabolism, increased hepatic ECE-1-mediated ET-1 synthesis may contribute to host responses and cytokine production during allograft rejection.

The understanding of the continental carbon budget is essential to predict future climate change. In order to quantify CO₂ and CH₄ fluxes at the regional scale, a measurement system was installed at the former radio tower in Beromünster as part of the Swiss greenhouse gas monitoring network (CarboCount CH). We have been measuring the mixing ratios of CO₂, CH₄ and CO on this tower with sample inlets at 12.5, 44.6, 71.5, 131.6 and 212.5 m above ground level using a cavity ring down spectroscopy (CRDS) analyzer. The first 2-year (December 2012–December 2014) continuous atmospheric record was analyzed for seasonal and diurnal variations and interspecies correlations. In addition, storage fluxes were calculated from the hourly profiles along the tower. The atmospheric growth rates from 2013 to 2014 determined from this 2-year data set were 1.78 ppm yr⁻¹, 9.66 ppb yr⁻¹ and and -1.27 ppb yr⁻¹ for CO₂, CH₄ and CO, respectively. After detrending, clear seasonal cycles were detected for CO₂ and CO, whereas CH₄ showed a stable baseline suggesting a net balance between sources and sinks over the course of the year. CO and CO₂ were strongly correlated (r² > 0.75) in winter (DJF), but almost uncorrelated in summer. In winter, anthropogenic emissions dominate the biospheric CO₂ fluxes and the variations in mixing ratios are large due to reduced vertical mixing. The diurnal variations of all species showed distinct cycles in spring and summer, with the lowest sampling level showing the most pronounced diurnal amplitudes. The storage flux estimates exhibited reasonable diurnal shapes for CO₂, but underestimated the strength of the surface sinks during daytime. This seems plausible, keeping in mind that we were only able to calculate the storage fluxes along the profile of the tower but not the flux into or out of this profile, since no Eddy covariance flux measurements were taken at the top of the tower.
