PURPOSE: to evaluate the perinatal outcome of fetuses with congenital anomalies of the urinary tract. METHODS: we reviewed the perinatal outcome of 35 fetuses with congenital anomalies of the urinary tract. The following characteristics related to the uropathy were analyzed: type (hydronephrosis, dysplasia and renal agenesis), side of lesion (bilateral or unilateral), and level of the obstruction (high or low, in hydronephrosis). The perinatal outcome was evaluated according to these characteristics. The data were analyzed by the c² test and by the exact Fisher test. The level of significance was 0.05. RESULTS: the incidence of hydronephrosis was 68.6%. Half of the fetuses had unilateral hydronephrosis. Renal dysplasia occurred in 17.1% of the cases; 83.3% of these were bilateral and 16.7%, unilateral. The incidence of renal agenesis was 14.3%, all bilateral. The fetuses with dysplasia/agenesis had a 91% incidence of oligohydramnios, preterm birth, low birth weight, and death. In the group with bilateral disease the presence of oligohydramnios, preterm birth, low birth weight, death, urinary tract infections, and the need of hospitalization for a period greater than 7 days was significant when compared to the group with unilateral disease. The need of hospitalization for a period greater than 7 days in patients with low obstruction was significantly higher when compared to the patients with high obstruction. CONCLUSIONS: hydronephrosis, bilateral disease, and lower obstruction were the most frequent uropathies. The dysplasia/agenesis group had a worse prognosis when compared with the hydronephrosis group. Bilateral disease had a worse prognosis when compared with the unilateral disease group. In the low obstruction group, the need for a period of hospitalization greater than seven days was higher than in the high obstruction group.

OBJETIVOS: verificar a freqüência de infecção urinária recorrente (ITU) e avaliar os fatores associados à recorrência da ITU em crianças. MÉTODOS: estudo retrospectivo de 95 pacientes com seguimento de um ano (68 meninas e 27 meninos, mediana de idade três anos). As variáveis estudadas foram: sexo, idade, febre, constipação, tipo de bactéria, refluxo vésico-ureteral (RVU), anormalidades na cintilografia renal com ácido dimercaptosuccínico (DMSA). RESULTADOS: infecção urinária recorrente ocorreu em 49,5% crianças (19 com trato urinário normal, 19 com RVU e 9 com estenose da junção pielocalicial). Comparando o grupo com ITU recorrente com o grupo sem ITU recorrente não se encontrou diferença significativa entre sexos, presença de febre, constipação e anormalidades na cintilografia renal com DMSA. A ITU recorrente foi significativamente maior nas crianças com um ano ou menos, naquelas menores de dois anos com RVU, nas com bactéria diferente da Escherichia coli e sem profilaxia antibacteriana. Os fatores de risco significativos para a recorrência ITU foram idade < 2 anos (OR = 3,83) e refluxo vésico-ureteral (OR = 4,95). CONCLUSÕES: por causa da elevada freqüência de ITU recorrente é importante o seguimento regular de grupo de crianças com fatores de risco para ITU recorrente.

Susceptibility profile and multiple drug resistance in 158 E. coli strains isolated from urinary tract infection (UTI), pyometra, and feces of dogs were studied. Norfloxacin, ciprofloxacin, and enrofloxacin were the most-effective drugs (>60%) for E. coli strains. High rates of resistance to antimicrobials were observed in 60% or more of the isolated strains using sulfametoxazole/trimetoprim. Multiple drug resistance for three or more antimicrobials was observed in two (47.1%) strains isolated from UTI, seven (13.5%) from pyometra, and four (7.3%) from feces. From these, 17 (33.3%), one (1.9%), and three (5.5%), respectively, showed multiple resistance to five or more drugs.

Baccharis trimera (Less.) DC. Asteraceae e Casearia sylvestris Swartz Salicaceae são plantas nativas do Brasil e de uso tradicional em doenças do trato digestório. Seus extratos e substâncias purificadas apresentaram diversas atividades farmacológicas (ex.antiulcerogênica, antimicrobiana) e baixa toxicidade. O Helicobacter pylori (bacilo gram-negativo) coloniza o estômago humano, associada ao maior risco de incidência de várias doenças (úlceras pépticas, linfomas e adenocarcinomas gástricos). Com o objetivo de avaliar a atividade antibacteriana de diferentes extratos, frações e substâncias de ambas as plantas medicinais, extratos de partes aéreas de B. trimera e de folhas de C. sylvestris foram preparados utilizando-se técnicas de maceração, obtendo-se o extrato acetato de etila e etanólico de C. sylvestris e acetato de etila, etanólico, etanol 70% e hexânico de B. trimera. Estes foram fracionados e analisados por técnicas cromatográficas. Os extratos e suas frações e substâncias foram submetidos a ensaios para avaliação da atividade antibacteriana para a determinação da concentração inibitória mínima (CIM). O método utilizado para os ensaios foi a diluição em microplacas usando as bactérias Helicobacter pylori ATCC 43504, Staphylococcus aureus ATCC 25923 e Escherichia coli ATCC 25922. A partir do fracionamento do extrato acetato de etila de B. trimera e do extrato etanólico de C. sylvestris foram obtidas oito frações distintas do EAcBt e a fração de diterpenos do EEtCs. A análise em CCD demonstrou a presença de casearina X, B e caseargrewiina F na fração de diterpenos (EFSCs2). Os extratos da B. trimera EAcBt, EEtBt e EEt70Bt e da C. sylvestris EAcCs e EEtCs apresentaram atividade anti-H. pylori, com valores de CIM de 250, 500, 500, 125 e 750 g/mL, respectivamente... Introduction: Hospital-acquired urinary tract infection (HAUTI) is an important cause of morbidity in the elderly population. Objective: Evaluate the occurrence of HAUTI and risk factors associated with it. Method: This is a prospective study of a sample of 332 elderly people, interned in a university hospital. Criteria for defining infection were established by the Center for Diseases and Prevention Control. Statistical analysis of data used calculation of frequencies, odds ratio and logistic regression. The rate of hospital infection was 23.6%. The prevalent topographies of infection were respiratory infections (27.6%), urinary tract infections (26.4%) and surgical wound infections (23.6%, with 21, 20 and 19 episodes, respectively. The HAUTI incidence density associated with urinary catheterization was 24.2 infections by 1,000 catheter-days. The length of hospital stay of patients without nosocomial infection was 6.9 days and with HAUTI was increased in 10.4 (p<0.05).The rate of mortality of patients with HAUTI was 20%. Pathogens were isolated in 75% of episodes of HAUTI and the prevalent were: Escherichia coli (33%) and Pseudomonas aeruginosa (20%). Risk factors found for HAUTI were urinary catheterization implementation (odds ratio (OR) = 43.1; 95% confidence interval (95 CI%) = 3.9 – 311.1), hospitalization with community infection (OR= 21.9; 95% CI = 4.9 – 97.9); vascular diseases (OR=14; 95% CI = 2 – 98); diabetes mellitus (OR= 5.5; 95% CI = 1.4 – 21) and urinary catheterization by more than three days (OR=3.7; 95% CI = 1 – 13.8). Conclusions: HAUTI presented elevated incidence and it increased the length of hospital stay.

Lower urinary tract disease (LUTD) is a multifactorial disease, wich may be caused by uroliths and calculations, bacterial and viral infections and anatomical abnormalities, among others. The disease can also be of unknown origin in wich case it is called intersticial cystitis. Knowing that the disease may have many causes, it is necessary to know the exact etiology and the best method of diagnosis, to establish specific therapeutic measures for each case. The diagnostic methods most commonly used are: urinalysis, radiography and ultrassonography, but the blood count and biochemistry can be used, trying to assess the overall status of the animal. The terapy used depends on the causative agent of disease, wich may include changes in supply until emergency treatment, azotemia in cats with post-renal. The present study aims to present a complete study of a disease, addressing it’s etiology, diagnosis and treatment, with particular attention to first item
