The primary complex like Ghon was observed in a child's clinical roentgenographic study. C.S., white, male, 6 years old, was born in Curitiba (PR), Brazil and living in Guaratingueta (SP), Brazil, developed common cold, bimodal diary fever, chills, shake and sweats. Dyspnea, cough with general lymphadenopathy. Foot and right shoulder arthralgias. Six months ago visited a cave, equitation practice, dog and cat contacts and no transfusion, frontal sweats, fever (38.4 degrees C). T.A. was 8/6, tachycardia in generalized lymphadenopathy. Cardiopulmonary system was normal, mesogastric tumoral mass, hepatosplenomegaly and no ascites. Bone marrow with eosinophilia; nodule demonstrated presence of P. brasiliensis, hypoalbuminemia; hyperglobulinemia; anemia; leukocytosis with eosinophilia. Immunodiffusion with exoantigen 43 kd of P. brasiliensis was 1/32. Primary complex like Ghon was observed in interstitial pneumonia followed by mediastinal and mesogastric mass (35 to 40 days). Clavicular osteolytic lesions (45 to 60 days) appeared during paracoccidioidomycosis therapy. Recovery was observed 2 months after treatment of acute infantile paracoccidioidomycosis.

Scorpionism is a common problem that occurs in tropical and subtropical countries and assumes great medical-sanitary importance due to its fatal effect on sensitive individuals, being able to lead children and aged people to death. The envenomation lethal potential is responsible for the serious cardiopulmonary alterations the scorpion toxin produces in its victims. The present research evaluated the effects of Tityus serrulatus venom on dogs, using two distinct doses: a dose that simulates natural envenomation (0.4 mg/total dose), and an experimental dose (0.25 mg/kg). General clinical signs were observed at different moments after envenomation, and specific data related to the cardiopulmonary system were evaluated by systemic arterial pressure measurement, CK-MB enzymatic activity dosage, and radiographic, electrocardiographic and echocardiographic examinations. Results demonstrated that the scorpion venom, in experimental doses, was able to cause acute and reversible cardiac injury in few days, and, in the dose that simulated natural accident, it produced clinical signs of light envenomation, such as local pain, hyperesthesia, sialorrhea, vomiting, diarrhea, sneeze and prostration.

Em condições de hipóxia (redução da pressão parcial de oxigênio), respostas reflexas de aumento da ventilação pulmonar, aumento da pressão arterial e diminuição da frequência cardíaca são observadas com o objetivo de aumentar os processos de trocas gasosas e perfusão tecidual. Para que essas respostas reflexas à hipóxia ocorram, quimiorreceptores periféricos são ativados pela redução da pressão parcial de oxigênio arterial e enviam sinais excitatórios para o sistema nervoso central, promovendo a excitação de importantes núcleos cerebrais envolvidos com o controle das atividades respiratória, simpática e parassimpática. Evidências experimentais sugerem que os neurônios do complexo parabraquial/Kölliker-Fuse (PBN/KF), importante região respiratória localizada na porção dorsolateral da ponte, contribuem para o processamento das respostas cardiorrespiratórias do quimiorreflexo. Contudo, os neurotransmissores envolvidos ainda não foram explorados. No presente projeto, exploraremos a hipótese de que a neurotransmissão glutamatérgica da região PBN/KF é importante para a manutenção da ventilação basal e para o processamento das respostas ventilatórias à hipóxia. Para tanto, utilizamos a técnica de microinjeção de agonistas e antagonistas dos receptores de L-glutamato no PBN/KF de animais anestesiados e não anestesiados, avaliando parâmetros ventilatórios (eletromiografia dos músculos diafragma e abdominal em animais anestesiados; e frequência respiratória, volume corrente e ventilação minuto em animais não anestesiados) e parâmetros cardiovasculares (pressão arterial e frequência cardíaca em ambas condições experimentais) em condições de repouso ou em situações de estimulação dos quimiorreceptores periféricos (com cianeto de potássio). Objective - To investigate the effects of buprenorphine on cardiopulmonary variables and on abdominal auscultation scores in horses. Animals - 6 healthy adult horses. Procedures - Horses were restrained in stocks and allocated to 2 treatments in a randomized crossover design, with 1-week intervals between each treatment. Saline (0.9% NaCl) solution was administered IV as a control, whereas buprenorphine (10 μg/kg, IV) was administered to the experimental group. Cardiopulmonary data were collected for 120 minutes after buprenorphine or saline solution administration. Abdominal auscultation scores were monitored for 2 and 12 hours after drug administration in the control and experimental groups, respectively. Results - Following control treatment, horses remained calm while restrained in the stocks and no significant changes in cardiopulmonary variables were observed throughout the study. Buprenorphine administration caused excitatory phenomena (restlessness and head shaking). Heart rate, cardiac index, and arterial blood pressure were significantly increased after buprenorphine administration until the end of the observational period (120 minutes). Minimal changes were found in arterial blood gas tensions. Abdominal auscultation scores decreased significantly from baseline for 4 hours after buprenorphine administration. Conclusions and Clinical Relevance - Buprenorphine induced excitement and hemodynamic stimulation with minimal changes in arterial blood gas tensions. These effects may impact the clinical use of buprenorphine in horses. Further studies are indicated to investigate the effects of buprenorphine on gastrointestinal motility and fecal output.
