Objectives: The objective of this study was to apply low-level laser therapy (LLLT) to accelerate the recovery process of a child patient with Bell's palsy (BP). Design: This was a prospective study. Subject: The subject was a three-year-old boy with a sudden onset of facial asymmetry due to an unknown cause. Materials and methods: The low-level laser source used was a gallium aluminum arsenide semiconductor diode laser device (660 nm and 780 nm). No steroids or other medications were given to the child. The laser beam with a 0.04-cm2 spot area, and an aperture with approximately 1-mm diameter, was applied in a continuous emission mode in direct contact with the facial area. The duration of a laser session was between 15 and 30 minutes, depending on the chosen points and the area being treated. Light was applied 10 seconds per point on a maximum number of 80 points, when the entire affected (right) side of the face was irradiated, based on the small laser beam spot size. According to the acupuncture literature, this treatment could also be carried out using 10-20 Chinese acupuncture points, located unilaterally on the face. In this case study, more points were used because the entire affected side of the face (a large area) was irradiated instead of using acupuncture points. Outcome measures: The House-Brackmann grading system was used to monitor the evolution of facial nerve motor function. Photographs were taken after every session, always using the same camera and the same magnitude. The three-year-old boy recovered completely from BP after 11 sessions of LLLT. There were 4 sessions a week for the first 2 weeks, and the total treatment time was 3 weeks. Results: The result of this study was the improvement of facial movement and facial symmetry, with complete reestablishment to normality. Introduction. Cerebral Venous Thrombosis has a highly variable clinical presentation. Four major syndromes had been described in patients with cerebral venous thrombosis: isolated intracranial hypertension, focal neurological deficits, focal or generalized seizures and disturbances of consciousness and cognitive dysfunction. Method. We describe five consecutive patients admitted to our service with a diagnosis of cerebral venous thrombosis, highlighting the different possibilities of clinical presentation and prognosis. Discussion. The diagnosis of cerebral venous thrombosis should be considered in patients with acute, subacute or chronic headache, with or without signs of intracranial hypertension or focal deficits, even in the absence of cerebrovascular risk factors. Treatment should be started as soon as the diagnosis is confirmed and consists of reversal of the underlying cause when known, control of seizures and intracranial hypertension, and antithrombotic therapy.

Background: The high prevalence of dysphagia after stroke leads to increased mortality, and cerebral reperfusion therapy has been effective in reducing neurologic deficits. The aim of this study was to investigate the severity and evolution of dysphagia and the occurrence of pneumonia in patients submitted to cerebral reperfusion therapy. Methods: Seventy ischemic stroke patients were evaluated. Of these, 35 patients (group 1) were submitted to cerebral reperfusion therapy and 35 (group 2) did not receive thrombolytic treatment. The following were evaluated: severity of dysphagia by means of videofluoroscopy, evolution of oral intake rate by means of the Functional Oral Intake Scale, and the occurrence of pneumonia by international protocol. The relation between the severity of dysphagia and the occurrence of pneumonia with the treatment was evaluated through the chi-square test; the daily oral intake rate and its relation to the treatment were assessed by the Mann-Whitney test and considered significant if P is less than .05. Results: The moderate and severe degrees of dysphagia were more frequent (P = .013) among the patients who were not submitted to cerebral reperfusion therapy. The daily oral intake evolved independently of the treatment type, without statistical significance when compared between the groups, whereas pneumonia occurred more frequently in group 2 (28%) in relation to group 1 (11%) and was associated with the worst degrees of dysphagia (P = .045). Conclusions: We can conclude that there is improvement in the oral intake rate in both groups, with lower severity of dysphagia and occurrence of pneumonia in ischemic stroke patients submitted to cerebral reperfusion therapy. Este trabalho configura-se numa proposição de estudo de caso, que procurou discutir e orientar a prática pedagógica de alunos da educação especial, com vistas ao seu desenvolvimento acadêmico. Foi realizado no decorrer de um ano letivo, numa unidade escolar de uma rede municipal de ensino do oeste paulista. Contou com a participação de professores do ensino comum, da educação especial, gestores da escola e fez parte de um conjunto de ações desenvolvidas no projeto de extensão universitária, numa proposição conjunta entre universidade e escola. Num primeiro momento foram realizadas observações do cotidiano escolar de um aluno com transtorno global do desenvolvimento, autismo, e de uma aluna com duplo diagnóstico, paralisia cerebral e autismo, associada à coleta de informações sobre as trajetórias educacionais e os diagnósticos clínicos. Com base nessas informações efetivaram-se intervenções psicoeducacionais com os alunos e professoras especialistas e aplicou-se a Escala de Comportamento Adaptativo Vineland. De modo geral, identificou-se um padrão restrito de aprendizagens funcionais e comportamentos interacionas nos casos observados. Ao final da proposta questionou-se qual a contribuição das práticas pedagógicas, ofertadas na sala de aula comum, para o desenvolvimento educacional dos alunos em questão. As análises foram compartilhadas com os profissionais da escola e com os respectivos familiares, e direcionamentos foram apontados a favor da manutenção no ensino comum ou ao encaminhamento à escola especial.

Indivíduos com Paralisia Cerebral apresentam desenvolvimento motor atípico, caracterizado por alterações posturais, de coordenação motora e de tônus muscular, que resulta em limitações no desempenho de atividades funcionais. Neste contexto destaca-se a prescrição e confecção de recursos de tecnologia assistiva com objetivo de maximizar as habilidades funcionais destas pessoas e prover sua inclusão social. Os indivíduos com paralisia cerebral, muitas vezes, têm dificuldade para a manutenção da dinâmica corporal, principalmente na postura sentada. O objetivo deste estudo foi o de confeccionar uma cadeira e mesa escolar adaptadas com material de baixo custo e verificar sua eficácia no desempenho grafomotor de uma criança com paralisia cerebral. O participante foi uma criança com diagnóstico de paralisia cerebral diplégica, do gênero masculino. A coleta e registro dos dados foram feitos em três momentos, com a criança posicionada no mobiliário adaptado, no mobiliário escolar comum e novamente no mobiliário adaptado, para se estabelecer a comparação. A análise de dados se deu por meio de testes estatísticos não paramétricos. Não houve significância estatística e foi verificada uma inconstância nos dados apresentados, já que não se pode afirmar ao certo qual mobiliário foi mais eficaz na realização da atividade proposta e nem se houve aprendizado motor com a sua repetição. Esse fato não invalida a adequação de mobiliário escolar ao aluno com paralisia cerebral, visto que este é um fator importante para favorecer o controle e estabilidade postural ao individuo, o que interfere na coordenação motora fina destes indivíduos, influenciando no seu desempenho em atividades escolares.

Objectives: This experimental study aimed to evaluate the effects of central catalase inhibition on cardiovascular responses in rats exposed to sidestream cigarette smoke (SSCS) for 3 weeks. Methodology: A total of 20 males Wistar rats (320–370g) were implanted with a stainless steel guide cannula into the fourth cerebral ventricle (4thV). Femoral artery and vein were cannulated for mean arterial pressure (MAP) and heart rate (HR) measurement and drug infusion, respectively. Rats were exposed to SSCS for three weeks, 180 minutes per day, 5 days/week [carbon monoxide (CO): 100–300 ppm)]. Baroreflex was tested with one pressor dose of phenylephrine (PHE, 8 μg/kg, bolus) and one depressor dose of sodium nitroprusside (SNP, 50 μg/kg, bolus). Cardiovascular responses were evaluated before and 15 minutes after 3-amino-1, 2, 4-triazole (ATZ, catalase inhibitor, 0.001g/100μL) injection into the 4th V. Results: Vehicle treatment into the 4th V did not change cardiovascular responses. Central catalase inhibition increased tachycardic peak, attenuated bradycardic peak and reduced HR range at 15 minutes, increased MAP at 5, 15 and 30 min and increased HR at 5 and 15 min. In rats exposed to SSCS, central ATZ increased basal MAP after 5 min and increased HR at 5, 15 and 30 minutes, respectively, and attenuated bradycardic peak at 15 minutes. Conclusion: This study suggests that brain oxidative stress caused by SSCS influences autonomic regulation of the cardiovascular system.
