Multiple primary tumors (MPT) are a major cause of mortality and morbidity among patients that have survived after the treatment of a first cancer. It has been proposed that after the first primary tumor, high risk of a subsequent tumor could be associated with radiotherapy used as treatment for the first cancer. Other potential risk factors include unhealthy lifestyle, genetic predisposition, aging, environmental determinants or an interaction between these factors. However, an association between the presence of MPT and family history of cancer in cases without clinical and molecular evidence of a known hereditary cancer syndrome is rarely described. Genomic DNA from 12 patients with at least two primary tumors and without mutations on TP53 was evaluated by CytoScan HD Array (Affymetrix). Chromosome Analysis Suite (ChAS) software v.2.0.1 was used considering at least 50 markers for gains; 25 for losses and a minimum of 5Mb for cnLOHs. Data from 1038 phenotypically healthy individuals (Affymetrix) and from Database of Genomic Variants were used as reference. Only alterations found in <1% (rare) or never described (new rare) in the reference population were considered. All cases, except one, presented a family history of cancer. Five cases developed MTP after radiotherapy and only one was located in the same treated area. It was detected 67 rare and 15 new rare genomic alterations encompassing 5.906 genes: 17 losses, 29 gains, and 36 cnLOH. X chromosome presented the higher number of alterations. Two patients with breast cancer presented a large deletion/cnLOH on 7q21. Enrichment analysis revealed 1275 genes associated with breast cancer (p= 0.001), which was diagnosed in 6 patients and their family members (all negative for BRCA1/2 or TP53 mutations). cnLOHs accounted for 44% of all the alterations. A significant proportion of cases (11/12) presented family history of cancer and the patients were not submitted to radiotherapy (7/12). We demonstrated the presence of rare genomic alterations in patients with MPT suggesting their involvement in the MPT development. cnLOH may arise as a new mechanism associated with the risk to develop MPT. Patients with chronic kidney disease (CKD) have a lower exercise tolerance and poor functional capacity, carry on a sedentary lifestyle. Another important change found in patients with CKD is cognitive dysfunction. Physical inactivity has been associated with cognitive dysfunction in the general population, but few studies have evaluated this association in CKD. To assess the association between physical activity and cognitive function in patients with CKD on hemodialysis (HD). We evaluated 102 patients undergoing HD. The participants completed the International Physical Activity Questionnaire, which assesses the level of physical activity and the Mini Mental State Examination, used for cognitive screening. Patients were divided into three groups according to their level of physical activity (GI: active/GII: irregularly active/GIII: sedentary). It was applied logistic regression analysis and adopted as outcome variable the presence of cognitive impairment and preserving as independent variables those with a probability of statistical difference between groups of less than 0.1. It was considered statistically significant when p less than 0.05. The groups were similar in age, duration of HD, and smoking. Statistically significant difference regarding race, body mass index, diabetes mellitus, underlying disease and degree of cognitive impairment. Regarding laboratory data, the groups differed in terms of creatinine, glucose, hemoglobin and hematocrit. There was significant association with better physical activity and cognitive function, even adjusting for confounding variables. the highest level of physical activity was associated with better cognitive function in CKD patients undergoing HD.

Indicine cattle have lower reproductive performance in comparison to taurine. A chromosomal anomaly characterized by the presence Y markers in females was reported and associated with infertility in cattle. The aim of this study was to investigate the occurrence of the anomaly in Brahman cows. Brahman cows (n = 929) were genotyped for a Y chromosome specific region using real time-PCR. Only six out of 929 cows had the anomaly (0.6%). The anomaly frequency was much lower in Brahman cows than in the crossbred population, in which it was first detected. It also seems that the anomaly doesn't affect pregnancy in the population. Due to the low frequency, association analyses couldn't be executed. Further, SNP signal of the pseudoautosomal boundary region of the Y chromosome was investigated using HD SNP chip. Pooled DNA of non-pregnant and pregnant cows were compared and no difference in SNP allele frequency was observed. Results suggest that the anomaly had a very low frequency in this Australian Brahman population and had no effect on reproduction. Further studies comparing pregnant cows and cows that failed to conceive should be executed after better assembly and annotation of the Y chromosome in cattle.

The impact of peritoneal dialysis modality on patient survival and peritonitis rates is not fully understood, and no large-scale randomized clinical trial (RCT) is available. In the absence of a RCT, the use of an advanced matching procedure to reduce selection bias in large cohort studies may be the best approach. The aim of this study is to compare automated peritoneal dialysis (APD) and continuous ambulatory peritoneal dialysis (CAPD) according to peritonitis risk, technique failure and patient survival in a large nation-wide PD cohort. This is a prospective cohort study that included all incident PD patients with at least 90 days of PD recruited in the BRAZPD study. All patients who were treated exclusively with either APD or CAPD were matched for 15 different covariates using a propensity score calculated with the nearest neighbor method. Clinical outcomes analyzed were overall mortality, technique failure and time to first peritonitis. For all analysis we also adjusted the curves for the presence of competing risks with the Fine and Gray analysis. After the matching procedure, 2,890 patients were included in the analysis (1,445 in each group). Baseline characteristics were similar for all covariates including: age, diabetes, BMI, Center-experience, coronary artery disease, cancer, literacy, hypertension, race, previous HD, gender, pre-dialysis care, family income, peripheral artery disease and year of starting PD. Mortality rate was higher in CAPD patients (SHR1.44 CI95%1.21-1.71) compared to APD, but no difference was observed for technique failure (SHR0.83 CI95%0.69-1.02) nor for time till the first peritonitis episode (SHR0.96 CI95%0.93-1.11). In the first large PD cohort study with groups balanced for several covariates using propensity score matching, PD modality was not associated with differences in neither time to first peritonitis nor in technique failure. Nevertheless, patient survival was significantly better in APD patients.

Esse trabalho teve como objetivo avaliar, clinicamente, a efetividade de dois agentes clareadores aplicados sobre a superfície dentária durante 45 minutos, sem troca do gel. Foram selecionados 10 pacientes de acordo com os critérios de inclusão e exclusão, e a arcada superior dos pacientes foi dividida em duas hemiarcadas (n = 10): na hemiarcada direita (HD), foi aplicado o peróxido de hidrogênio a 35% (PH) Clàriant Office (Angelus); e, na hemiarcada esquerda (HE), o PH 38% Opalescence Boost (Ultradent). Foram realizadas duas sessões clínicas, de 45 minutos cada, com intervalo de uma semana entre as sessões. Inicialmente, ao tratamento clareador, os pacientes receberam profilaxia, moldagem para guia de mensuração de cor com silicone de condensação e avaliação de cor por meio de um aparelho espectrofotômetro Vita Easyshade (Vita Zhanfabrik, Alemanha). Após 14 dias do término do tratamento clareador, foi realizada a mensuração final da cor dos dentes. Os dados foram submetidos ao teste t e de Tukey (p < 0,05). Os resultados demonstraram não haver diferença estatisticamente significativa entre as hemiarcadas quanto à efetividade do clareamento (p = 0,146) e nem quanto à sensibilidade. Pode-se concluir que a aplicação dos materiais avaliados sem troca do gel, por 45 minutos, foi eficaz em promover o clareamento de dentes vitais e com baixa sensibilidade dentária.

This study aimed to evaluate the clinical effectiveness of two bleaching agents without changing the whitening gel during the clinic session 1X45minutos. 10 patients were selected according to the criteria of inclusion and exclusion and upper arch of the patients were divided into two quadrants (n: 10), G1: gel clareador Clàriant Office (hidrogênio35 Angellus) peroxide% (PH) in superior right side (LD) and the gel WhitenessHp Blue (FGM) PH 35% on the superior left side (LE. 2 clinical sessions were accomplished, 45 minutes each, with an interval of one week between sessions. At first the bleaching treatment patients received prophylaxis, molding to guide measurement of color with condensation silicon and the color evaluation through the apparel spectrophotometer VITA Easyshade (Vita Zhanfabrik, Alemanhã). 14 days after the end of the bleaching treatment was performed the measurement final color of the teeth. The patients reported sensitivity in scale 0-4. The result showed that through T Test comparing quadrants presented no statistical differences (p>0,05) in relation to coloration and dental sensibility. It can be concluded that the materials evaluated are effective for bleaching vital teeth with low tooth sensibility.
