The aim of this study was to compare the release of bone markers during osseointegration of immediately loaded and nonloaded implants. Forty patients who were indicated for rehabilitation with dental implants randomly received either implant and prosthesis placement within 72 hours (group IM) or implant insertion and no prosthesis placement (group NL). Peri-implant crevicular fluid was collected immediately after implant insertion and 7, 15, 30, 60, 90, and 120 days after surgery and levels of osteoprotegerin, transforming growth factors, osteocalcin, osteopontin, and parathyroid hormone were evaluated using Luminex assay. Bleeding index and peri-implantar sulcus depth were also evaluated. The data were compared using statistical tests ( = 5%). No statistical difference was found regarding demographic and clinical parameters (p > .05). Transforming growth factors, osteoprotegerin, osteopontin, and parathyroid hormone presented an earlier release peak in group IM than in NL group (p < .05). Osteocalcin achieved higher levels in group IM versus group NL between 7 and 30 days of evaluation (p < .05). It may be concluded that earlier loading positively modulates bone mediators release around immediately loaded implants when compared with nonloaded dental implants (ClinicalTrials.gov NCT01909999).

Moderate and controlled loading environments support or enhance osteogenesis, and, consequently, a high degree of bone-to-implant contact can be acquired. This is because when osteoprogenitor cells are exposed to limited physical deformation, their differentiation into osteoblasts is enhanced. Then, some range of microstrain is considered advantageous for bone ingrowth and osseointegration. The primary stability has been considered one of the main clinical means of controlling micromotion between the implant and the forming interfacial tissue, which helps to establish the proper mechanical environment for osteogenesis. Based on the biological aspects of immediate loading (IL), the objective of this study is to present a clinical case of maxillary arch rehabilitation using immediate loading with implant-supported fixed restoration after bone graft. Ten dental implants were placed in the maxilla 6 months after the autogenous bone graft, removed from the mandible (bilateral oblique line and chin), followed by the installation of an immediate-load fixed cross-arch implant-supported restoration because primary stability was reached for 8 implants. In addition, instructions about masticatory function and how it is related to interfacial micromotion were addressed and emphasized to the patient. The reasons for the IL were further avoidance of an interim healing phase, a potential reduction in the number of clinical interventions for the patient, and aesthetic reasons. After monitoring the rehabilitation for 8 years, the authors can conclude that maxillary IL can be performed followed by a well-established treatment planning based on computed tomography, providing immediate esthetics and function to the patient even when autogenous bone graft was previously performed in the maxilla.

Bone Morphogenetic Proteins (BMPs) are multifunctional secreted cytokines, which belong to the TGF-beta superfamily. These glycoproteins act as a disulfide-linked homo- or heterodimers, being potent regulators of bone and cartilage formation and repair, cell proliferation during embryonic development and bone homeostasis in the adult. BMPs are promising molecules for tissue engineering and bone therapy. The present review discusses this family of proteins, their structure and biological function, their therapeutic applications and drawbacks, their effects on mesenchymal stem cells differentiation, and the cell signaling pathways involved in this process. Beside aging process comes the incidence of dementia and, among them Alzheimer's disease (AD) accounts for approximately 60% of cases. This disease is characterized as a neuropathology with unknown etiology that causes cognitive deficits and behavioral disorders. Caring for patients with AD can cause an overload, both physical and psychological, which can cause high levels of stress on the primary caregiver. It is necessary that the caregiver also receives attention and develop activities that promote health benefits, while providing moments of distraction from the task of caring. Nonpharmacological interventions may be favorable for improving health with consequent decreased on the levels of stress. The objective of this study was to conduct a systematic review of scientific papers that aimed to verify the effect of nonpharmacological interventions on stress levels in caregivers of patients with AD. To contamplate this goal was accomplished a systematic search in the following databases: Biological Abstracts, PsycINFO, PubMed/Medline, Web of Science, LILACS and SciELO. The following keywords and Boolean operators was used: “caregivers” OR “family” and “nonpharmacological interventions” OR “support groups” OR “therapies” AND “Alzheimer's disease” OR “Alzheimer's dementia” OR “Alzheimer” OR “elderly” AND “stress”. There were found 3studies that met inclusion criteria adopted for the present work, and none showed significant results for the variable stress. It is not possible to affirm, according to the studies, that nonpharmacologial interventions programs for caregivers of patients with AD are effective to influence and to control the stress. A remarkable characteristic of the physics subject is the abstraction ability needed during the learning process, which means a non-trivial comprehension and understanding. If the students could not link the themes learned in this subject with observable effects, physics would tend to be a group of mathematical formulas to be memorized. In general, it creates a distance between the student and the subject, and also a prejudice vision of the physics, sometimes for the whole life. Beyond this particular characteristics of physics, the education deals with a source of distraction reinforced by the popularization of mobile access such as cellphones and tablets. The traditional role played by the teacher becomes weak to attract student’s attention. It is necessary that the classes could be planned in such way to achieve three goals: attract student attention, link analytical concepts with real effects and, the most important, gives to the student the ability to use the learned concepts as the key to develop new knowledge. In this work, we present a research based in some experiments used in a CBEF (Brazilian magazine about physical education) and develop an activity consistent with the aims to be reached. To prepare of the experimental activity, we based on Piaget and Vygotsky theories. This work aimed to developed a theoretical research to obtain a didactical activity that deals with the current challenges of the society

Immediate reutilization of the expanding action in a case of rapid maxillary expansion surgically assisted. The orthopedic rapid maxillary expansion (RME) and rapid maxillary expansion surgically assisted (RMESA) are conducted with the aim of giving an appropriate jaw, capable of providing a normal occlusion. In extreme cases, where there is a severe atresia, it is necessary to perform an expansion beyond that allowed by the expander, followed by another conventional device or a butterfly expander, when the atresia is in the anterior maxillary region. In this situation, there are two options: wait about 90 days to allow intermaxillary suture restructuring and perform a new RME / RMESA or proceed immediately to the expansion process. Considering the biological cost, financial and clinical time, the procedure of reusing the immediate expander action becomes the technique of choice in these cases, been the operational procedure performed simplified and in just four steps. This work will show a case report where, after accomplishing the RMESA was observed that even after changing the shape of the maxillary arch, the severity of atresia could not be corrected, especially in the anterior region, and more expansion was needed. Aiming to correct the atresia in the anterior maxilla, the technique used was to reuse the immediate expander action through the change of an intraoral screw expander for a conventional butterfly type screw expander.

The early tooth loss and periodontal disease often leave inadequate bone volume for installation of osseointegrated implants. The autogenous bone graft is considered the gold standard for reconstruction of residual bone defects. Some surgical techniques can be performed, including extra or intraoral donor sites depending on the degree of bone loss, depending on surgical-prosthetic planning and general condition of the patient. The intraoral bone grafts offer a safe option to rebuilt bone volume in smaller rehabilitations, with low morbidity and minimal postoperative discomfort. Among the possible donor sites, the mandibular ramus and body, which offer predominantly cortical bone, and the chin area, which offers corticomedullary bone tissue, can be harvested. The graft will be suitable both in quantity and quality, preserving the capacity of osteogenesis, osteoinduction and osteoconduction, which differentiate autogenous grafts from other biomaterials. The aim of this study was to report a clinical case in which the mandibular ramus graft was used for total reconstruction of an edentulous maxilla, showing that even large areas can be reconstructed with grafts from intraoral origin. All the steps that allowed the complete reconstruction of the maxilla done by an intraoral donor area are listed in the text, culminating in an extremely satisfactory clinical result.

Jaws atrophy represents a great challenge to Implantology regarding rehabilitation of edentulous areas. This paper reports an extreme mandibular atrophy with only 9mm of height between the mental foramina. In these situations, treatment options include bone augmentation techniques, transmandibular, or short dental implants. It is well accepted that bone grafts to improve height do not have good predictability. Besides, osteogenic distraction is associated with a great patient discomfort and transmandibular implants are related with high failure rates. Also, surgical-prosthetic rehabilitations with short dental implants had been associated to good success rates. The purpose of this clinical case is to report a severe mandibular atrophy, successfully treated with four short dental implants between mental foramina for implant-supported prosthesis.

Osteodistraction is a clinical reality, available in the last decades for the resolution of large bone deficiencies, in cases that there are pre-existing, but misplaced implants and teeth. The aims of thearticle is to present a case report in which a new possibility for bone distraction, based on tooth-implant bone distractors, made from standard orthodontic expansion-screws, was used in an area where there was an extensive need of alveolar bone and aesthetical recovery, allied to teeth and dental implant misplacement. This technique presented good clinical results, associated to effective simplicity and low cost, becoming viable clinical solution for bone tissue augmentation and aesthetical optimization. In resume, it is possible to say that the suggested technique achieved its objectives, improving the hard and soft tissue profile, whilst allowing completion of oral rehabilitation.

The surface of dental implants is an important factor for osseointegration process and different methods of surface treatment have been described. Objective: To investigate the bone apposition in implant surface treated with sandblasting and acid-etching. Material and methods: Ten rabbits were selected and received one implant treated with method I in the left tibia and one implant treated with method II in the right tibia. Then, twenty implants were divided in two groups, according to methods of sandblasting and acid-etching (method I and method II). After 7, 14, 30, 45 and 60 days, tibias were retrieved and submitted to histotechnical procedures. The percentages of bone–implant contact (BIC) and bone area between threads (BABT) were determined throughout histomorphometric analysis and bone apposition was detected in implants of both groups. Results: In BABT measurements, an increase was observed after 45 and 60 days in the method II, compared to method I and no differences were found after 7, 14 and 30 days. In BIC measurements, an increase was detected with method II at 45 days when compared to method I. No differences between groups in BIC values were observed after 7, 14, 30 and 60 days. Conclusion: Our data demonstrated that implants treated with the method II presented increase in the contact between bone and implant after 45 days compared to method I. Moreover, with concern to bone area between threads, it was observed an increased in the method II after 45 and 60 days. However, both groups can be successfully used as a therapeutic strategy to rehabilitation of edentulous patients. Then, further experiments are needed to evaluate, in depth, the putative differential role of each surface treatment.
