JUSTIFICATIVA E OBJETIVOS: Dentre as indicações mais freqüentes de sedação em pacientes internados em Unidades de Terapia Intensiva (UTI) estão a instituição e a manutenção de ventilação artificial, a ansiedade e procedimentos desconfortáveis ou dolorosos. O objetivo deste estudo retrospectivo foi verificar as indicações e as técnicas mais comuns de sedação em pacientes graves internados na Unidade de Terapia Intensiva Cirúrgica da Escola Paulista de Medicina (EPM/UNIFESP) durante um período de 11 meses. MÉTODO: Após terem sido excluídos os pacientes que permaneceram na UTI menos de 24 horas ou estavam sem exames indispensáveis para o cálculo do índice de gravidade (APACHE II), a amostra ficou reduzida a 307 pacientes. Foram analisadas as técnicas mais utilizadas, as indicações de sedação e a associação de bloqueadores neuromusculares. RESULTADOS: A sedação foi administrada em 37,4% dos pacientes. Entre as indicações de sedação estão os distúrbios de natureza psiquiátrica, como delírio, agitação, medo e ansiedade. Estas corresponderam a 25,77% de todas as indicações. A maioria dos pacientes ventilados artificialmente também necessita de sedativos. Instalação e manutenção de ventilação mecânica corresponderam à maioria das indicações, em torno de 57,73% dentre os pacientes sedados. Procedimentos como intubação traqueal e broncoscopia tiveram 11,34% das indicações e controle do metabolismo (coma barbitúrico e tétano), 5,15%. As técnicas mais comumente utilizadas incluíram opióides isolados ou associados a benzodiazepínicos. Neste estudo, o fentanil foi utilizado em 58% das técnicas, isoladamente, e em 21,64% associado ao midazolam. Haloperidol, diazepam, propofol e tiopental somaram 19,5%. Bloqueadores neuromusculares foram utilizados em 22,7% dos casos em pacientes ventilados artificialmente. CONCLUSÕES: A sedação é recurso terapêutico freqüente em Terapia Intensiva, comumente utilizada para facilitar a ventilação artificial e tratar os problemas de natureza psiquiátrica. Fentanil, isoladamente ou em associação com midazolam, foi o agente mais utilizado.

Genetic and phylogenetic analyses of the region containing the glycoprotein (G) gene, which is related to pathogenicity and antigenicity, and the G-L intergenic region were carried out in 14 Brazilian rabies virus isolates. The isolates were classified as dog-related rabies virus (DRRV) or vampire bat-related rabies virus (VRRV), by nucleoprotein (N) analysis. The nucleotide and amino acid (AA) homologies of the area containing the G protein gene and G-L intergenic region were generally lower than those of the ectodomain. In both regions, nucleotide and deduced AA homologies were lower among VRRVs than among DRRVs. There were AA differences between DRRV and VRRV at 3 antigenic sites and epitopes (IIa, WB+ and III), suggesting that DRRV and VRRV can be distinguished by differences of antigenicity. In a comparison of phylogenetic trees between the ectodomain and the area containing the G protein gene and G-L intergenic region, the branching patterns of the chiropteran and carnivoran rabies virus groups differed, whereas there were clear similarities in patterns within the DRRV and VRRV groups. Additionally, the VRRV isolates were more closely related to chiropteran strains isolated from Latin America than to Brazilian DRRV. These results indicate that Brazilian rabies virus isolates can be classified as DRRV or VRRV by analysis of the G gene and the G-L intergenic region, as well as by N gene analysis.

Foi conduzido, em casa de vegetação, um experimento em solução nutritiva de Steimberg modificada, cultivando-se o girassol (Helianthus annuus L.), cv. Anhandy, com cinco níveis de cálcio e cinco de boro combinados, em esquema fatorial completo 5 x 5, visando-se induzir sintomas tardios, no início do florescimento, de deficiência desses elementos. A deficiência isolada de cálcio induziu o sintoma de colapso do capítulo quando o nível inicial de cálcio na solução foi tal (15,24 ppm de Ca) que permitiu a manifestação do sintoma no início do florescimento. A deficiência de boro manifestou-se antes do florescimento, para todos os níveis iniciais do micronutriente em solução, e isto impediu sua avaliação, em relação ao capítulo.

Human monocytes lacked fungicidal activity against high virulence strain of Paracoccidioides brasiliensis, even after IFN-gamma activation. However, monocytes treated with indomethacin exhibited an effective killing against this fungus, suggesting a role of prostaglandin E-2 (PGE(2)) in the inhibition process. Thus, the purpose of this work was to determine whether the effect of PGE2 in fungicidal activity was related with decrease on H2O2 release, the metabolite involved in P. brasiliensis killing, and changes in the levels of TNF-alpha, IL-6 and IL-10. Human monocytes challenged with the fungus produced high PGE(2) levels, which in turn inhibited the fungicidal activity of these cells by reducing H2O2 and TNF-alpha production. We report a study of residual stress in PbTiO3 (PT) thin films prepared on Si substrates by a polymeric chemical method. The E(1TO) frequency was used to evaluate the residual stress through an empirical equation available for bulk PT. We find that the residual stress in PT films increases as the film thickness decreases and conclude that it originates essentially from the contributions of extrinsic and intrinsic factors. Polarized Raman experiments showed that the PT films prepared by a polymeric chemical method are somewhat a-domain (polar axis c parallel to the substrate) oriented.

Bioactivity-directed fractionation of the CH2Cl2/MeOH (2:1, v/v) extract of the roots of Petiveria alliacea, using mutant yeast strains of Saccharomyces cerevisiae and fungi Cladosporium cladosporioides and C. sphaerospermum led to the isolation of dipropyl disulphide (1), dibenzyl sulphide (2), dibenzyl disulphide (3), dibenzyl trisulphide (4), dibenzyl tetrasulphide (5), benzylhydroxymethyl sulphide (6) and di(benzyltrithio) methane (7). of these, 5-7 are new compounds and this is the first report of the natural occurrence of 2 and 3. The aboveground biomass content of a region can be estimated by either direct or indirect methods. Direct methods correspond to the biomass content determination with scales and extrapolation of results to larger areas. It is a destructive and very laborious procedure. Indirect methods utilize formulas whose entrance parameters are obtained from forest inventories. Forest inventories are made with the purpose to plan exploration and land use and the inventory data are frequently not suitable for biomass estimation. Problems with both methods increase in the Amazon region, where little information is available on forest biomass. The objective of this paper is to establish, by comparing the application of the indirect and direct methods in the determination of the biomass, the more appropriate indirect formulation to represent the characteristic vegetation of a region in the amazonian forest. A 0.2 hectare area was chosen, which was part of a major forest clearing experiment conducted in Tomé Açu, a town located 250 km south of Belém, the capital of the Brazilian state of Pará. The entire biomass in the area was weighted with scales during the three weeks that followed the cut of the forest in July 1994. A detailed inventory was carried out in the area and then the indirect method was applied in the data. Seven different formulas for determining biomass were used. Comparison of the data of real mass and the mass obtained through the application of the seven formulas indicated that the more suitable for the region is given by FW = α · φβ · Hγ, where FW is total fresh weight (kg), φ is the diameter at breast height (cm), H is the total height of the tree and α, β and γ are regression coefficients (equal to 0.026, 1.529 and 1.747, respectively).

Studies were carried out to natural papain inhibitor from papaya latex. Fresh latex from green fruits of Carica papaya was collected and immediately transported in ice bath to the lab, from which three fractions with inhibitor effect of esterase papain activity were isolated by latex dialysis, Sephadex G-25 gel filtration and ionic exchange chromatography in SP-Sephadex C-25. The isolated fractions, identified as inhibitors I and II, showed a negative reaction with ninhydrin; however, the fraction identified as P-III showed positive reaction with ninhydrin. Kinetics data showed non-competitive inhibition (inhibitor I) and uncompetitive (inhibitors II and P -III).

The objective of this study was to evaluate the effect of beta-lactoglobulin polymorphism, breed and seasonality on physico-chemical characteristics and stability of bovine milk. Six Holstein-Zebu (H-Z) and five Holstein dairy herds were selected, of which 660 Holstein and 293 crossbred Holstein-Zebu milk and blood samples were collected, adding up 953 samples, distributed in two collecting periods at rainy and two at dry seasons. Each milk sample was analyzed for titratable acidity, pH, freezing point and milk ethanol stability, at the following ethanol concentrations: 70, 76, 80 and 84GL. Individual blood samples were submitted to polymerase chain reaction (PCR) for the determination of beta-lactoglobulin polymorphism. No effect of beta-lactoglobulin polymorphism was observed in physico-chemical characteristics of milk, in both studied breed. Breed effect for Holstein and H-Z, respectively, was observed on titratable acidity (16.16 and 17.07°D, while effect of seasonality (for rainy and dry seasons, respectively) was also observed on freezing point (-0.5411 and -0.5376°H). Effects of breed and seasonality on milk stability were observed (Holstein-Zebu milk was less stable on dry season), however, no effect of beta-lactoglobulin on milk stability was observed.

The aim of the current work was to evaluate the occurrence of Cryptosporidium sp in AIDS patients in a region of São Paulo State, Brazil. Patients were divided into groups according to CD4+ T lymphocyte count and use of potent antiretroviral treatment. Two hundred and ten fecal samples from 105 patients were fixed in 10% formalin and subjected to centrifuge formol-ether sedimentation. Slides were stained with auramine and confirmed by modified Ziehl-Neelsen. Cryptosporidiosis occurrence was 10.5% with no relationship among gender, age or the presence of diarrhea. The number of oocysts in all samples was small, independent of CD4+ T lymphocyte count, HIV plasma viral load, and presence of diarrhea. These results may be due to the reduced prevalence of opportunistic infections in AIDS individuals after the advent of highly active antiretroviral therapy.

This study sought to investigate the surface roughness and the adherence of Streptococcus mutans (in the presence and absence of saliva) to ceramics and composites. The early dental biofilms formed in situ on the materials were illustrated, using scanning electron microscopy (SEM). Feldspathic and leucite/feldspathic ceramics and microhybrid and microfilled composites were evaluated. Human dental enamel was used as the control. Standardized specimens of the materials were produced and surface roughness was analyzed. The adhesion tests were carried out in 24-well plates and colony forming units (CFU/mL) were evaluated. Values of roughness (μm) and adherence (CFU/mL) were analyzed statistically. Of all the surfaces tested, enamel was the roughest. Leucite/feldspathic ceramics were rougher than the feldspathic ceramic, while composites were similar statistically. Enamel offered the highest level of adherence to uncoated and saliva-coated specimens, while the leucite/feldspathic ceramic demonstrated greater adherence than the feldspathic ceramic and the composites were similar statically. The rougher restorative materials increased the adherence of S, mutans on the material surfaces.
