To evaluate the effect of a fluoride dentifrice containing sodium hexametaphosphate (HMP) on enamel demineralization in situ. This double-blind and cross-over study consisted of 3 phases (7 days each) in which 12 volunteers wore intraoral appliances containing four enamel bovine blocks. Specimens were treated (3×/day) with placebo (no F or HMP), 1100ppm F (1100F) and 1100F plus HMP1% (1100F-HMP1%) toothpastes, and the cariogenic challenge was performed using a 30% sucrose solution (6×/day). Final surface hardness, the percentage of surface hardness loss (%SH), the integrated loss of subsurface hardness (ΔKHN), as well as enamel calcium (Ca), phosphorus (P) and firmly-bound fluoride (F) were determined. Also, biofilm formed on the blocks were analyzed for F, Ca, P and insoluble extracellular polysaccharide (EPS) concentrations. Data were submitted 1-way ANOVA, followed by Student-Newman-Keuls' test (p<0.05). 1100F-HMP1% promoted the lowest %SH and ΔKHN among all groups (p<0.001). The addition of HMP1% to 1100F did not enhance enamel F uptake, but significantly increased enamel Ca concentrations (p<0.001). Similar EPS concentrations were seen for 1100F-HMP1% and 1100F groups (p>0.05). All the groups were supersaturated with respect to HA. However, only 1100F-HMP1% group was supersaturated with respect to CaF2 (p<0.05). The ionic activities of F(-), CaF(+) and HF(0) for the 1100F-HMP1% group were the highest among all groups (p<0.001). The addition of HMP1% to a conventional toothpaste significantly reduces enamel demineralization in situ when compared to 1100F. This dentifrice could be a viable alternative to patients at high risk of caries.

This study evaluated the effect of fluoride gels, supplemented or not with sodium hexametaphosphate (HMP), on enamel erosive wear in situ. Twelve healthy volunteers wore palatal appliances containing four bovine enamel discs. Subjects were randomly allocated into four experimental phases (double-blind, crossover protocol) according to the gels: Placebo (no fluoride or HMP), 1% NaF, 2% NaF, and 1% NaF+9% HMP. Enamel discs were selected after polishing and surface hardness analysis, and treated only once with the respective gels prior to each experimental phase. Erosion (ERO) was performed by extra-oral immersion of the appliance in 0.05M citric acid, pH 3.2 (four times/day, five minutes each, 5 days). Additional abrasion (ERO+ABR) was produced on only two discs by toothbrushing with fluoridated dentifrice after ERO (four times/day, 30s, 5 days). The specimens were submitted to profilometry and hardness analysis. The results were analyzed by two-way ANOVA and the Student-Newman-Keuls test (p<0.05). The 1% NaF+9% HMP gel promoted significantly lower enamel wear for ERO compared to the other groups, being statistically lower than 1% NaF and Placebo for ERO+ABR. Similarly, the lowest values of integrated lesion area were found for 1% NaF+9% HMP and 2% NaF, respectively, for ERO and ERO+ABR. The addition of HMP to the 1% NaF gel promoted greater protective effect against ERO and ERO+ABR compared to the 1% NaF gel, achieving similar protective levels to those seen for the 2% NaF gel. Gel containing 1% NaF+9% HMP showed a high anti-erosive potential, being a safer alternative when compared to a conventional 2% NaF gel.

The sol-gel process is a technique based on the hydrolysis and polycondensation of metal alkoxides have been investigated as an alternative for the preparation of vitreous or glassceramic materials, for allowing the obtaining of high-purity materials at low temperatures, and to obtain hybrid materials with different compositions. When ORMOSILs are doped with nanoparticles, are called nanocomposites, and its functionality arises when electrons confined in scale three-dimensional structures near zero are excited. In principle, such materials exhibit discrete energy level, with peaks in the absorption spectrum. Therefore, the glasses doped semiconductor nanocrystals are important candidates for the preparation of optical filters with sharp cut-off, and are being exploited commercially for coloring glasses. This study evaluates the optical properties presented by hybrid films of silica doped with copper nanoparticles. The matrix was prepared using the alkoxides 3-glycidoxypropyltrimethoxysilane (GPTS) and tetraethylorthosilicate (TEOS) doped with Cu2O and hydrolyzed under reflux conditions in two different acidic conditions (HCl and HBr). After thermal treatment at temperatures between 100 °C and 170 °C and/or under the action of commercial black light radiation, CuCl and CuBr present in the hybrid film are transformed into nanoparticles of CuCl or CuBr. The aim of the present study was to investigate the abrasive effect of CaCO3 and SiO2-based fluoride-free experimental toothpastes on eroded human permanent dental enamel and evaluate the effectiveness of waiting periods between acid exposure and tooth brushing. Twelve volunteers wore palatal appliances containing human enamel blocks for two periods of five days each. The appliances were immersed in a soft drink for five minutes four times a day (9:00 am, 11:00 am, 2:00 pm and 4:00 pm). On two occasions, two blocks were not submitted to additional treatment; two blocks were brushed (30 s) either with a CaCO3 or SiO2 toothpaste immediately after erosion and two blocks were brushed 1 h after erosion. Thus, the sample was divided into six groups: erosion alone (CaCO3 and SiO2 control); brushing with fluoride- free toothpaste (CaCO3 immediate and 1 h after erosion; SiO2 immediate and 1 h after erosion). Significant differences in wear depth were found between the enamel blocks in the CaCO3 immediate and 1 h after erosion groups and the blocks in the CaCO3 control group (p=0.001; p=0.022). No significant differences were found regarding the change in roughness and wear depth between blocks submitted to immediate abrasion and 1 h after erosion (CaCO3 and SiO2). The data revealed that surface roughness and wear depth is increased when erosion is combined with dental abrasion, regardless of the abrasive used. Waiting for 1 h to brush the eroded blocks offered no protective effect.

Melanoma subungueal in situ tratado com cirurgia funcionalHamilton Ometto StolfI, Hélio Amante MiotI, Nilton de Ávila ReisIIDepartamento de Dermatologia e Radioterapia, Universidade Estadual Paulista (Unesp)INTRODUÇÃOO melanoma subungueal representa aproximadamente 2% a 3% dos melanomas cutâneos em pacientes caucasianos1 e 20% em pacientes negros2 ou asiáticos.3A exposição solar, tida com principal fator de risco para o melanoma cutâneo,4 parece desempenhar papel secundário no desenvolvimento da variante subungueal,5 uma vez que a radiação ultravioleta dificilmente penetra no leito ungueal. Além disso, nevos subungueais como lesões precursoras são extremamente raros.6Os polegares e háluces são os mais acometidos, sendo o polegar responsável por 56% dos casos entre todos os dedos e o hálux por 86% dos dedos dos pés.7A confirmação do diagnóstico é feita a partir do exame anatomopatológico da lesão, geralmente localizada na matriz ou leito ungueal. A verificação histológica do melanoma subungueal é frequentemente postergada por conta do atraso no diagnóstico clínico,8-10 gerando piora do prognóstico.O prognóstico geralmente é ruim: as taxas de sobrevida real de cinco anos variam entre 16% a 20%, podendo atingir até 80% se consideradas taxas de sobrevida estimada.2,3,11-18RELATO DE CASOPaciente de 44 anos, do sexo feminino, professora, com queixa de aparecimento de mancha na unha do hálux direito há cinco anos. Relata aumento progressivo lento na largura da faixa. Ao exame dermatológico, apresentava faixa de melanoníquia extensa, irregular, bordas mal definidas e estria de pigmentação mais acentuada em uma das margens do leito ungueal do hálux direito (Figura 1). Exame dermatoscópico (aparelho Dermalite II Pro, aumento de 10 vezes) confirmou faixas irregulares e cores variadas de hiperpigmentação.Foi realizada a avulsão parcial da lâmina ungueal e incisão na prega ungueal lateral para visualização do local da origem da pigmentação no leito ungueal,

Background: More than half of women present nipple discharge during reproductive age. Case: This case is about a 36 years old woman with unilateral spontaneous hyaline nipple discharge associated with breast pain. The study of scintimammography was compatible with multifocal proliferative lesion in situ. It was performed cytological smear of nipple discharge. It was paucicellular smear represented by cluster of ductal cells in three-dimensional design with hyperchromatic nuclei in the presence of myoepithelial cells. Red cells and signs of necrosis were not observed. The diagnosis of in situ ductal carcinoma was confirmed in biopsy and mastectomy specimen through the expression of calponin in myoepithelial cells at immunohistochemistry. It is known that the cytological examination of nipple discharge has low sensitivity and specificity. However, it is an easy and inexpensive procedure. Suspicious or positive results, may be important for guidance workup of patients in order to perform earlier diagnosis of malignancy. Conclusion: this case demonstrates that in situ ductal carcinoma can be characterized by positive nipple discharge, and cytology sample is an important tool for the diagnosis of suspicion of malignancy and further diagnostic investigation.
