Early identification of beginning readers at risk of developing reading and writing difficulties plays an important role in the prevention and provision of appropriate intervention. In Tanzania, as in other countries, there are children in schools who are at risk of developing reading and writing difficulties. Many of these children complete school without being identified and without proper and relevant support. The main language in Tanzania is Kiswahili, a transparent language. Contextually relevant, reliable and valid instruments of identification are needed in Tanzanian schools. This study aimed at the construction and validation of a group-based screening instrument in the Kiswahili language for identifying beginning readers at risk of reading and writing difficulties. In studying the function of the test there was special interest in analyzing the explanatory power of certain contextual factors related to the home and school. Halfway through grade one, 337 children from four purposively selected primary schools in Morogoro municipality were screened with a group test consisting of 7 subscales measuring phonological awareness, word and letter knowledge and spelling. A questionnaire about background factors and the home and school environments related to literacy was also used. The schools were chosen based on performance status (i.e. high, good, average and low performing schools) in order to include variation. For validation, 64 children were chosen from the original sample to take an individual test measuring nonsense word reading, word reading, actual text reading, one-minute reading and writing. School marks from grade one and a follow-up test half way through grade two were also used for validation. The correlations between the results from the group test and the three measures used for validation were very high (.83-.95). Content validity of the group test was established by using items drawn from authorized text books for reading in grade one. Construct validity was analyzed through item analysis and principal component analysis. The difficulty level of most items in both the group test and the follow-up test was good. The items also discriminated well. Principal component analysis revealed one powerful latent dimension (initial literacy factor), accounting for 93% of the variance. This implies that it could be possible to use any set of the subtests of the group test for screening and prediction. The K-Means cluster analysis revealed four clusters: at-risk children, strugglers, readers and good readers. The main concern in this study was with the groups of at-risk children (24%) and strugglers (22%), who need the most assistance. The predictive validity of the group test was analyzed by correlating the measures from the two school years and by cross tabulating grade one and grade two clusters. All the correlations were positive and very high, and 94% of the at-risk children in grade two were already identified in the group test in grade one. The explanatory power of some of the home and school factors was very strong. The number of books at home accounted for 38% of the variance in reading and writing ability measured by the group test. Parents´ reading ability and the support children received at home for schoolwork were also influential factors. Among the studied school factors school attendance had the strongest explanatory power, accounting for 21% of the variance in reading and writing ability. Having been in nursery school was also of importance. Based on the findings in the study a short version of the group test was created. It is suggested for use in the screening processes in grade one aiming at identifying children at risk of reading and writing difficulties in the Tanzanian context. Suggestions for further research as well as for actions for improving the literacy skills of Tanzanian children are presented.

Drug discovery is a continuous process where researchers are constantly trying to find new and better drugs for the treatment of various conditions. Alzheimer’s disease, a neurodegenerative disease mostly affecting the elderly, has a complex etiology with several possible drug targets. Some of these targets have been known for years while other new targets and theories have emerged more recently. Cholinesterase inhibitors are the major class of drugs currently used for the symptomatic treatment of Alzheimer’s disease. In the Alzheimer’s disease brain there is a deficit of acetylcholine and an impairment in signal transmission. Acetylcholinesterase has therefore been the main target as this is the main enzyme hydrolysing acetylcholine and ending neurotransmission. It is believed that by inhibiting acetylcholinesterase the cholinergic signalling can be enhanced and the cognitive symptoms that arise in Alzheimer’s disease can be improved. Butyrylcholinesterase, the second enzyme of the cholinesterase family, has more recently attracted interest among researchers. Its function is still not fully known, but it is believed to play a role in several diseases, one of them being Alzheimer’s disease. In this contribution the aim has primarily been to identify butyrylcholinesterase inhibitors to be used as drug molecules or molecular probes in the future. Both synthetic and natural compounds in diverse and targeted screening libraries have been used for this purpose. The active compounds have been further characterized regarding their potencies, cytotoxicity, and furthermore, in two of the publications, the inhibitors ability to also inhibit Aβ aggregation in an attempt to discover bifunctional compounds. Further, in silico methods were used to evaluate the binding position of the active compounds with the enzyme targets. Mostly to differentiate between the selectivity towards acetylcholinesterase and butyrylcholinesterase, but also to assess the structural features required for enzyme inhibition. We also evaluated the compounds, active and non-active, in chemical space using the web-based tool ChemGPS-NP to try and determine the relevant chemical space occupied by cholinesterase inhibitors. In this study, we have succeeded in finding potent butyrylcholinesterase inhibitors with a diverse set of structures, nine chemical classes in total. In addition, some of the compounds are bifunctional as they also inhibit Aβ aggregation. The data gathered from all publications regarding the chemical space occupied by butyrylcholinesterase inhibitors we believe will give an insight into the chemically active space occupied by this type of inhibitors and will hopefully facilitate future screening and result in an even deeper knowledge of butyrylcholinesterase inhibitors.

The purpose of this study was to gather information on hearing impairment and related factors among elderly people. The HHIE-S questionnaire (Hearing Handicap Inventory for Elderly-Screening) and a single hearing question (”Do you feel you have a hearing loss”) were compared to audiometric hearing thresholds (N=164). HHIE-S was reliable for detecting moderate or worse hearing impairment. The single question was equally sensitive and more specific in identifying mild hearing impairment. The prevalence of hearing impairment was evaluated in four age cohorts (70, 75, 80 and 85 years, N=4067) in Turku, Finland. The HHIE-S cut-off score >8 as an indicator of at least mild hearing impairment yielded prevalence values of 37.7% - 54.1%, and a score >18 (moderate or more severe hearing impairment) was 21.1% - 38.9%. The single question test was positive in 25.5% - 46.2%. Hearing aid compliance and problems experienced by hearing aid users were recorded as informed by the participants in a mailed interview (N=249/4067). The hearing aids were used daily by 55.4%, and never by 10.7%. Use sank with advancing age. The disturbance caused by tinnitus among 583 subjects was compared to their level of alexithymia (TAS-20) and depressiveness (BDI). Depressiveness was weakly associated with annoying tinnitus, but not alexithymia. The prevalence of hearing impairment can be measured by enquiry. Hearing aid compliance should be improved by technical means and better counseling. The factors affecting the distress experienced by tinnitus patients need further study.

Methyl chloride is an important chemical intermediate with a variety of applications. It is produced today in large units and shipped to the endusers. Most of the derived products are harmless, as silicones, butyl rubber and methyl cellulose. However, methyl chloride is highly toxic and flammable. On-site production in the required quantities is desirable to reduce the risks involved in transportation and storage. Ethyl chloride is a smaller-scale chemical intermediate that is mainly used in the production of cellulose derivatives. Thus, the combination of onsite production of methyl and ethyl chloride is attractive for the cellulose processing industry, e.g. current and future biorefineries. Both alkyl chlorides can be produced by hydrochlorination of the corresponding alcohol, ethanol or methanol. Microreactors are attractive for the on-site production as the reactions are very fast and involve toxic chemicals. In microreactors, the diffusion limitations can be suppressed and the process safety can be improved. The modular setup of microreactors is flexible to adjust the production capacity as needed. Although methyl and ethyl chloride are important chemical intermediates, the literature available on potential catalysts and reaction kinetics is limited. Thus the thesis includes an extensive catalyst screening and characterization, along with kinetic studies and engineering the hydrochlorination process in microreactors. A range of zeolite and alumina based catalysts, neat and impregnated with ZnCl2, were screened for the methanol hydrochlorination. The influence of zinc loading, support, zinc precursor and pH was investigated. The catalysts were characterized with FTIR, TEM, XPS, nitrogen physisorption, XRD and EDX to identify the relationship between the catalyst characteristics and the activity and selectivity in the methyl chloride synthesis. The acidic properties of the catalyst were strongly influenced upon the ZnCl2 modification. In both cases, alumina and zeolite supports, zinc reacted to a certain amount with specific surface sites, which resulted in a decrease of strong and medium Brønsted and Lewis acid sites and the formation of zinc-based weak Lewis acid sites. The latter are highly active and selective in methanol hydrochlorination. Along with the molecular zinc sites, bulk zinc species are present on the support material. Zinc modified zeolite catalysts exhibited the highest activity also at low temperatures (ca 200 °C), however, showing deactivation with time-onstream. Zn/H-ZSM-5 zeolite catalysts had a higher stability than ZnCl2 modified H-Beta and they could be regenerated by burning the coke in air at 400 °C. Neat alumina and zinc modified alumina catalysts were active and selective at 300 °C and higher temperatures. However, zeolite catalysts can be suitable for methyl chloride synthesis at lower temperatures, i.e. 200 °C. Neat γ-alumina was found to be the most stable catalyst when coated in a microreactor channel and it was thus used as the catalyst for systematic kinetic studies in the microreactor. A binder-free and reproducible catalyst coating technique was developed. The uniformity, thickness and stability of the coatings were extensively characterized by SEM, confocal microscopy and EDX analysis. A stable coating could be obtained by thermally pretreating the microreactor platelets and ball milling the alumina to obtain a small particle size. Slurry aging and slow drying improved the coating uniformity. Methyl chloride synthesis from methanol and hydrochloric acid was performed in an alumina-coated microreactor. Conversions from 4% to 83% were achieved in the investigated temperature range of 280-340 °C. This demonstrated that the reaction is fast enough to be successfully performed in a microreactor system. The performance of the microreactor was compared with a tubular fixed bed reactor. The results obtained with both reactors were comparable, but the microreactor allows a rapid catalytic screening with low consumption of chemicals. As a complete conversion of methanol could not be reached in a single microreactor, a second microreactor was coupled in series. A maximum conversion of 97.6 % and a selectivity of 98.8 % were reached at 340°C, which is close to the calculated values at a thermodynamic equilibrium. A kinetic model based on kinetic experiments and thermodynamic calculations was developed. The model was based on a Langmuir Hinshelwood-type mechanism and a plug flow model for the microreactor. The influence of the reactant adsorption on the catalyst surface was investigated by performing transient experiments and comparing different kinetic models. The obtained activation energy for methyl chloride was ca. two fold higher than the previously published, indicating diffusion limitations in the previous studies. A detailed modeling of the diffusion in the porous catalyst layer revealed that severe diffusion limitations occur starting from catalyst coating thicknesses of 50 μm. At a catalyst coating thickness of ca 15 μm as in the microreactor, the conditions of intrinsic kinetics prevail. Ethanol hydrochlorination was performed successfully in the microreactor system. The reaction temperature was 240-340°C. An almost complete conversion of ethanol was achieved at 340°C. The product distribution was broader than for methanol hydrochlorination. Ethylene, diethyl ether and acetaldehyde were detected as by-products, ethylene being the most dominant by-product. A kinetic model including a thorough thermodynamic analysis was developed and the influence of adsorbed HCl on the reaction rate of ethanol dehydration reactions was demonstrated. The separation of methyl chloride using condensers was investigated. The proposed microreactor-condenser concept enables the production of methyl chloride with a high purity of 99%.

The objective of this thesis was to evaluate whether a more extensive mammography screening programme (TurkuMSP) conducted by the city of Turku, had an effect on breast cancer (BC) incidence, survival, or mortality in years 1987 to 2009. Despite the fact that some studies have suggested a 20 percent reduction in BC mortality due to mammography screening, there are findings of harm to subjects, which are claimed to negate the benefits of screening. Thus, the aims of this study are most pertinent. A total of 176 908 screening examinations were performed in 36 000 women aged 40−74 during the years 1987−1997. In all, 685 primary BCs were found in the screened women, either screen-detected (n=531) or during screening intervals (n=154). Survival and BC recurrence rate of women with screen-detected BC was compared to 184 women with clinical BCs detected among individuals who did not take part in the screening. The invitation interval, which may influence the outcome, was studied in the age group 40 to 49 by inviting those born in even calendar years annually for mammography screening and those born in odd years, triennially. In addition, BC incidence and mortality in the total female population of Turku aged 40 to 84 years was compared with the respective figures of Helsinki and the rest of Finland, both during the pre-screening era (1976-1986) and the screening era (1987-2009). The study was designed to compare women by age groups, because women aged 50 to 59 were generally screened in all of Finland, whereas only in Turku women aged 40 to 49 and 60 to 74 were screened in addition. Data regarding cancer recurrence were derived from the Finnish Cancer Registry and data on deaths were collected from Statistics Finland. In survival analyses, screened women with invasive BC had a significantly higher survival rate than the women with clinical BC. The survival benefit started to appear already during the first follow-up years and was evident in all age groups. A marginal survival extension was also seen in screened women when BC had spread to ipsilateral axillary nodes already at diagnosis. Recurrence-free survival rate after BC treatment was significantly more favorable among the screened women compared with women with BC found clinically. The screening invitation interval did not significantly influence BC mortality in the subset of women aged 40 to 49 years. There were no consistent differences in the changes of BC incidence between Turku and the comparison areas during the screening era. In Turku, the BC mortality incidence in women aged 55−69 years was significantly lower during the screening era (from 1987 to 1997) compared with the pre-screening era, whereas no such change was found in the city of Helsinki or Tampere. When comparing the changes in incidence-based BC mortality during years 1987 to 2009 in Turku to those of Helsinki and the rest of Finland, there was a suggestion of more than 20 percent lower mortality in Turku among oldest age group (75-84 years) compared with the reference residential areas, but the differences were not consistently significant. Interpretation of the study results should be made with caution because there were no random control groups, and on the other hand, the number of cases in subgroups was fairly low to yield definite conclusions. Also due to the many statistical analyses, some of the findings may be due to chance. The results are, however, suggestive for a decrease of BC mortality in the elderly age groups due to wide mammography screening. This finding needs confirmation in further studies before recommending an expansion of mammography screening to women up to the age of 74 years

Malaria continues to infect millions and kill hundreds of thousands of people worldwide each year, despite over a century of research and attempts to control and eliminate this infectious disease. Challenges such as the development and spread of drug resistant malaria parasites, insecticide resistance to mosquitoes, climate change, the presence of individuals with subpatent malaria infections which normally are asymptomatic and behavioral plasticity in the mosquito hinder the prospects of malaria control and elimination. In this thesis, mathematical models of malaria transmission and control that address the role of drug resistance, immunity, iron supplementation and anemia, immigration and visitation, and the presence of asymptomatic carriers in malaria transmission are developed. A within-host mathematical model of severe Plasmodium falciparum malaria is also developed. First, a deterministic mathematical model for transmission of antimalarial drug resistance parasites with superinfection is developed and analyzed. The possibility of increase in the risk of superinfection due to iron supplementation and fortification in malaria endemic areas is discussed. The model results calls upon stakeholders to weigh the pros and cons of iron supplementation to individuals living in malaria endemic regions. Second, a deterministic model of transmission of drug resistant malaria parasites, including the inflow of infective immigrants, is presented and analyzed. The optimal control theory is applied to this model to study the impact of various malaria and vector control strategies, such as screening of immigrants, treatment of drug-sensitive infections, treatment of drug-resistant infections, and the use of insecticide-treated bed nets and indoor spraying of mosquitoes. The results of the model emphasize the importance of using a combination of all four controls tools for effective malaria intervention. Next, a two-age-class mathematical model for malaria transmission with asymptomatic carriers is developed and analyzed. In development of this model, four possible control measures are analyzed: the use of long-lasting treated mosquito nets, indoor residual spraying, screening and treatment of symptomatic, and screening and treatment of asymptomatic individuals. The numerical results show that a disease-free equilibrium can be attained if all four control measures are used. A common pitfall for most epidemiological models is the absence of real data; model-based conclusions have to be drawn based on uncertain parameter values. In this thesis, an approach to study the robustness of optimal control solutions under such parameter uncertainty is presented. Numerical analysis of the optimal control problem in the presence of parameter uncertainty demonstrate the robustness of the optimal control approach that: when a comprehensive control strategy is used the main conclusions of the optimal control remain unchanged, even if inevitable variability remains in the control profiles. The results provide a promising framework for the design of cost-effective strategies for disease control with multiple interventions, even under considerable uncertainty of model parameters. Finally, a separate work modeling the within-host Plasmodium falciparum infection in humans is presented. The developed model allows re-infection of already-infected red blood cells. The model hypothesizes that in severe malaria due to parasite quest for survival and rapid multiplication, the Plasmodium falciparum can be absorbed in the already-infected red blood cells which accelerates the rupture rate and consequently cause anemia. Analysis of the model and parameter identifiability using Markov chain Monte Carlo methods is presented.

Older age increases the risk of developing a chronic atherosclerotic cardiovascular disease (CVD), such as coronary heart disease. Complications of CVDs, myocardial infarction or stroke often lead to loss of functional capacity or premature death. Dyslipidemia, high serum levels of total or low-density lipoprotein cholesterol (LDL-c) and low levels of high-density lipoprotein cholesterol (HDL-c), is among the most important modifiable risk factors for CVDs; it can be treated with lifestyle modifications, and with lipid-lowering drugs, primarily statins. In older persons, however, the association of cholesterol levels with cardiovascular and all-cause mortality has been inconsistent in previous studies. Furthermore, the beneficial effects of statins in older persons without previous CVD are still somewhat unclear, and older persons are more prone to adverse effects from statins. This thesis presents a prospective cohort study (TUVA), exploring associations of cholesterol levels with mortality and the changes in cholesterol levels of a 70-year-old population in long-term follow-up. Further, prevalence of CVDs, risk factors and preventive medication use in the TUVA cohort is compared with respective prevalences in another age-matched cohort (UTUVA) 20 years later in order to examine the changes in cardiovascular risk over time. Additionally, to evaluate statin use patterns among older persons, an observational register study was conducted covering the total Finnish population aged 70 and older during 2000-2008. Based on individual-level data retrieved from national health registries, the population was classified into low, moderate and high risk groups according to estimated CVD risk. The prevalence, incidence and persistence of statin use among the risk groups was then evaluated based upon yearly statin purchases tracked from the Prescription Register. The prospective cohort study demonstrated that low total cholesterol, LDL-c and HDL-c were associated with higher mortality in a cohort of home-dwelling 70-year-olds. However, after adjusting for traditional cardiovascular risk factors and cancer this association disappeared. Further, low total cholesterol seemed to be protective, whereas low HDL-c strongly predicted increased risk of CVD death. Cholesterol levels of those elderly who remained available for follow-up and were still home-dwelling at the age of 85 seemed to improve with advancing age. Compared to the TUVA cohort, the later born UTUVA cohort had less CVDs and their risk factors were better controlled, which was reflected in the higher use of preventive medications such as statins and antihypertensives. The register studies confirmed that statin use has increased significantly during 2000-2008 among older persons, especially among the oldest (80+) age groups and among those at high risk for cardiovascular events. Two-thirds of new statin users persisted with their use during the four years of follow-up; the most discontinuations were made during the first year of use. In conclusion, statins are commonly used among older age groups in Finland. Most of the older statin users had a high cardiovascular event risk, indicating that the treatment is well directed towards those who are likely to benefit from it the most. No age-limits should be put on the screening and treatment of dyslipidemia in older persons, but the benefits and adverse effects of statin treatment should be carefully weighed based on an individual assessment of the person’s general health status and functional capacity. Physicians should pay more attention to medication adherence, especially when prescribing preventive medications.

Intimate partner violence is a growing problem in health care. The purpose of this thesis was to find ways of confronting a victim of intimate partner violence in nursing care and methods how nurses can encourage the victim to disclose the abuse. The aim of this thesis was to help the health care professionals to understand the issue and the need of the victims better, and that way provide a better care. This thesis is a literature review of previously made research articles about intimate partner violence. The research material consisted of 10 published articles, which were collected from different databases. The articles were published within 10 years. A content analysis method was used to examine the articles by making descriptive summary tables according to each questions. The results of this study showed multiple factors which the health care providers should take into consideration when caring the patients. Asking with a non-judgemental attitude, in a safe, confidential setting without the partner present and prioritizing the abuse was mentioned to be important for the victims. Routinely screening and different kind of brochures was considered as good methods to encourage the victims to disclose the abuse. The need for better training and counselling of the health care providers was also discovered. The results of this thesis, did answer to the chosen study questions and that way the purpose of the thesis was filled. Hopefully, in the future this problem can be minimized and prevented in advanced. Further studies are needed to examine whether these caring methods are actually being used in clinical settings and do they have any effect.

In this master's thesis, material characteristics which includes material database will be developed, through which will be managed process device deliveries device selections. The analysed bulk materials exact values serve as basic data when choosing glass- and dried product industry raw material equipment solutions. In this study, the material database function structure will be divided into classified bulk materials characteristics and process device guidelines and planning which they are based on. In order to achieve the best possible advantage in use of the material database, the raw material handling with the process equipment has to be controlled with the help of database bulk material information. With the help of material database, the personnel of the company will find the needed material and device information to implement the processes. The structure of the database enables adding of information, and hence the database will be always kept updated. The material database uses free PhpOpenDatabase software programme, which adapts itself flexibly with the software and requirements of the company. The material frame and considered features of the material database were determined by the requirements of the company's device technology. National and international classifications were used as help. The collected material information was analysed and the correspondence between internal study results and found classifications were determined. As presented before, on the grounds of chosen and specified material features, also the controllability of device selecting and solutions was improved with device-specific screening and planning guidelines. As material frame of the material database was chosen ANSI/CEMA Standard 550. Its almost 1000 bulk materials form an extensive database frame. Over 600 of these raw materials have been determined according to classification. However, the most important ones are the materials of glass- and dried material plants, which consist of the last part of material frame. The analysis will be sustained and material information will be completed primarily of material part with test run in one's own material laboratory.

Tutkimustyön tarkoituksena oli selvittää Tainionkosken sellutehtaan vuonna 2001 uudistetun kuitulinjan prosessin vaikutusta kuumajauhatukseen, kuumalajitteluun, pesutulokseen ja hienolajitteluun. Työn tavoitteena oli löytää ja todentaa optimaalinen ajotapa lopputuotteen eli Tainionkosken kartonkitehtaan kartongin kannalta pesussa, kuidutuksessa, kuumalajittelussa ja hienolajittelussa. Ennen kuitulinjan uudistusta kartoitettiin tilanne kuumajauhatuksessa ja kuumalajittelussa ennen prosessimuutoksia. Kartoituksessa havaittiin, että kuumalajittelu ja kuumakuidutus toimivat parhaiten puhtaan jauhatusenergian ollessa n. 18 kWh/ADT, sekä tilavuusmääräisen ja massavirtamääräisen rejektisuhteen ollessa n. 30 %.Uuden pesemön valmistumisen jälkeen tutkittiin kappatason ja energiatason vaikutusta kuumalajitteluun, kuumakuidutukseen ja pesutulokseen. Tutkimuksessa todettiin tikkupitoisuuksien kasvavan kappatason kohoamisen seurauksena. Kuumajauhimen syöttösakeus kasvoi kappatason noustua, koska prosessimittaus ei huomioinut sakeuden kasvua ja tällöin jauhimen laimennusveden määrä jäi liian vähäiseksi. Korkean kapan seurauksena keitosta tulevien suurien partikkelien määrä lisääntyi. Kuumajauhatuksessa korkeampi kappaluku merkitsi hienoaineen lisääntymistä jauhetussa massassa. Tämän seurauksena kuidutetun massan suotautumisvastus kasvoi kappatason kohottua. Uuden prosessin kuumajauhatukseen tuleva massa on jo pesty 4-vaiheisella DD-pesurilla. Puhtaampi massa kuumajauhatuksessa ja kuumalajittelussa on merkinnyt jauhimen tikkureduktion paranemista, mutta samalla kuumalajittimen tikkujen puhdistustehokkuus on heikentynyt. Laboratoriomittausten mukaan kappatason kasvu huonontaa pesutulosta uudessa pesemössä. Uuden hienolajittamon suorituskykyä määritettiin taseiden avulla. Vanhan lajittamon toiminnassa huomattiin puutteita siinä, että 2-portaan lajittimen 1-portaan syöttöön tulevan rejektivirran tikkupitoisuus oli suurempi mitä tikkupitoisuus 1-portaan syöttövirrassa. Uudessa lajittamossa molempien virtausten tikkupitoisuudet olivat lähes samat. Kokonaisuudessaan hienolajittamon toiminta havaittiin hyväksi. Pesemö ja hienolajittelu toimivat kokeiden perusteella parhaiten lähellä mitoitettua maksimituotantoa 700 ADT/d.

Työssä kartoitettiin sellulinjojen rejektinkäsittelyn toimintaa ja arvioitiin rejektinkäsittelyssä liikkuvan rejektin laatua. Tavoitteena oli arvioida rejektinkäsittelyn toiminnan hyvyyttä ja löytää mahdollisia ongelmakohtia. Toiminnan hyvyyttä arvioitiin rejektinkäsittelyn ajettavuuden, hallinnan ja käytettävyyden sekä rejektin laadun perusteella. Työn kirjallisuusosassa käsitellään yleisesti sellulinjoilla esiintyviä epäpuhtauksia ja kemiallisen massan lajittelua. Kirjallisuusosan pääpaino on sellulinjan ruskean massan lajittelun ja erityisesti lajittamon rejektinkäsittelyn esittelyssä. Rejektinkäsittelyosuus käsittelee nykyaikaiseen rejektinkäsittelyyn kuuluvia laitteita ja toimintoja sekä esittelee erilaisia rejektinkäsittelyjärjestelmiä ja –vaihtoehtoja. Työn kokeellisessa osassa arvioitiin kolmen sellutehtaan kuitulinjan rejektinkäsittelyn toiminnan hyvyyttä. Arvioitavat tehdaskohteet olivat Stora Enson Kaukopään sellutehdas, Botnian Joutsenon sellutehdas sekä UPM-Kymmenen Kaukaan sellutehdas. Tavoitteena oli selvittää ruskean massan lajittelun rejektinkäsittelyssä liikkuvan rejektin laatua, jonka perusteella toiminnan hyvyyttä voitiin arvioida. Tutkimus tehtiin siten, että tehdaskäyntien aikana otettiin arviointiin tarvittavat tehdasnäytteet sekä kerättin muu arviointiin tarvittava tieto. Tehdyn tutkimuksen perusteella saatiin kokonaiskuva kolmen nykyaikaisen sellutehtaan rejektinkäsittelyn toiminnasta. Työssä saatiin runsaasti tietoa rejektinkäsittelyyn ajettavan rejektin laadusta ja määristä sekä rejektinkäsittelylaitteiden toimivuudesta. Prosessista poistettavan rejektin määrän ja laadun selvittäminen oli tehtaille erityisen tärkeä asia selvittää. Kuitutappioiden muodossa prosessista poistettavan rejektin laadulla on myös jonkinlainen taloudellinen merkitys. Arvokasta tietoa saatiin myös prosessissa liikkuvista ja poistettavista hiekkamääristä. Yleisenä havaintona oli, että työhön kuuluneilla sellutehtailla rejektinkäsittely oli hyvin hallinnassa eikä suuria ongelmakohtia löytynyt.

Työn tavoitteena oli löytää painelajittimen roottorin keskeiset muuttujat ja ajoparametrit, kun pyritään jälkilajittelemaan valkaistua sellumassaa korkeassa sakeudessa, sekä näiden vaikutukset. Tavoitteena oli teknisesti onnistunut lajittelu korkeassa sakeudessa siten, että laitteen puhdistustehokkuus on hyvä, ominaisenergian kulutus on pieni ja kapasiteetti on korkea. Ensin tarkasteltiin kuitenkin teoreettisesti valkaistun sellumassan painelajittelun ongelmakenttää, keskeisten epäpuhtauksien poistoa jälkilajittelussa ja vertailtiin korkea- ja matalasakeuslajittelua. Lisäksi esiteltiin yleisellä tasolla koesuunnittelumenetelmien periaatteita ja menetelmiä sekä analyysiä so. mallinnus. Tätä taustaa vasten haluttiin myös kartoittaa ja esitellä painelajittimen keskeinen muuttujakenttä. Tämän jälkeen selvitettiin kokeellisesti miten ajettavan valkaistun mäntysellumassan sakeus, roottorin kehänopeus ja roottorin rakenteen muutokset vaikuttavat painelajittimen kapasiteettiin, rejektin sakeutumiskertoimeen, ominaisenergian kulutukseen ja puhdistustehokkuuteen. Vaikutusten ja parhaan roottorirakenteen sekä ajosuureiden määritys suoritettiin mallintamalla mittaustulokset lineaarisella regressioanalyysilla. Saatiin tärkeimmät vasteisiin vaikuttavat riippumattomat muuttujat ja niiden matemaattiset esityk-set, malliyhtälöt. Mallinnusta hyväksi käyttäen tarkasteltiin vielä erikseen yhden roottorin ajoa. Tärkeimpinä tuloksina saatiin selville, että puhdistustehokkuus on lähes vakio tietyllä roottorirakenteella riippumatta sakeudesta ja roottorin kehänopeudesta. Edullista puhdistustehokkuuden kannalta on käyttää ensisijaisesti isoa roottorin palaelementin korkeutta ja välystä suhteessa sihtirumpuun. Jälkilajittelu painelajittimella korkeassa sakeudessa kannattaa suorittaa pienellä roottorin kehänopeudella, runko- ja palaelementtivälyksellä suhteessa sihtirumpuun sekä suurella palaelementin korkeudella. Tällöin saavutetaan hyvä kompromissi ominaisenergian kulutuksen ja laitteen ajettavuuden osalta. Iso elementin korkeus ja pieni elementtivälys eivät aja massaa tehokkaasti laitteen alaosaan. Tällöin ei pyöritetä myöskään suurta massamäärää. Pieni elementtivälys mahdollistaa myös massan tehokkaamman leikkauksen, joka parantaa fluidisoitumista. Kitkavoimat ovat luonnollisesti ko. olosuhteissa suuremmat, joten ominais-energian kulutus kasvaa jonkin verran. Ratkaisevinta ominaisenergian kulutuksen kannalta on kuitenkin ajaa laitetta pienellä kehänopeudella.

Työssä tutkittiin uuden ShortFlowÔ-konseptin eri osa-alueiden toimivuutta paperikoneen lyhytkierrossa. Prosessin sekoituspumppuna perinteinen keskipakopumppu korvataan potkuri-pumpulla, joten työssä selvitettiin uudentyyppisen pumpun kavitointirajat, säädettävyys ja sen soveltuminen suunniteltuun tehtävään ja toiminta-alueeseen. Prosessiin kuuluu myös uudentyyppinen viirakaivo, josta selvitettiin sen kyky poistaa ilmaa ja virtauskäyttäytyminen kaivossa. Hienopaperikoneen osalta kartoitettiin prosessiin liittyviä riskejä ja ShortFlowÔ-lyhytkiertoprosessin kannattavuutta investointina. Työn kirjallisessa osassa on käsitelty lyhytkiertoa eri osa-alueittain. Aluksi on lyhytkierron yleisempi tarkastelu, jossa on selvitetty lajittelua, ilmanpoistoa ja pumppauksia. Erityisempää huomiota on kiinnitetty ShortFlowÔ-lyhytkiertoprosessiin liittyviin erityispiirteisiin joita ovat sakeamassalajittelu, täyteaineet paperitehtaalla, hylky ja virtausten sekoittuminen. Sekoituspumpun osalta on käsitelty myös kavitointia hieman tarkemmin. Työn kokeellisessa osassa suunniteltiin prosessin koelaitteisto, joka myös koeajettiin. Potkuripumpun koeajossa määritettiin potkuripumpun toiminta-alue ja säädettävyys. Pumpun todettiin sekoittavan ja ilman vaikutus potkuripumpun ominaisuuksiin todettiin merkityksettömäksi pumpun toiminta-alueella. Viirakaivon koeajossa selvisi, että viirakaivo poistaa ilmaa suunnittelualueella ja kaivolle löytyi muoto, jossa virtauskäyttäytyminen viirakaivossa eri laskeutumisnopeuksilla on mahdollisimman häiriötöntä. Hienopaperikoneen osalta riskien kartoituksessa selvisi, ettei täyteaine aiheuta ongelmia ja että päällystelaattaa sisältävä hylky on lajiteltava ennen lyhytkiertoa. ShortFlowÔ-lyhytkiertoprosessi oli edullisin sekä käyttö- että investointikustannuksiltaan.

Tutkimuksen tavoitteena oli soveltaa innovaatioprosessiteorioita käytäntöön konteksti huomioonottaen, koska langattoman sähköisen liiketoiminnan nopeat muutokset, metsäteollisuuden erityispiirteet ja ei-kaupallisuus asettavat erityistarpeita innovoinnille. Painopiste on prosessin aikaisissa vaiheissa, koska ne ovat kriittisiä koko innovaatioprosessin menestymisen kannalta. Tähtäimenä oli löytää yrityssektorille uusia langattomia sähköisen liiketoiminnan mahdollisuuksia, jotka hyödyttävät tutkimuksessa mukanaolevaa yritystä. Tutkimuksessa käytettiin konstruktiivista tutkimusotetta ja metodeina sähköistä aivomyrskyä, tarkistuslistaa sekä GDSS- ja postikyselyä.Tutkimuksen tuloksena on esitetty innovaatioprosessin aikaiset vaiheet kirjallisuudesta, metsäteollisuuden erityispiirteet ja innovaatioprosessisovellus. Lisäksi tämä tutkimus tuotti uusia sähköisen liiketoiminnan ideoita, suoritti niille seulonnan ja kävi läpi alustavan tutkimuksen.

The aims of this study were 1) to clarify the factors associated with family functioning in cancer patient’s families with dependant children, 2) to examine children’ mental health when they are exposed to parental cancer, 3) to explore the subjective experience of having cancer during pregnancy, and finally, 4) to describe the implementation of a childcentred family intervention for cancer patients’ families with dependant children in an adult oncology setting. The study groups were collected between May 1st 2002 and April 30th 2004. They consisted of one European group collected from six different countries (N = 381) and two Finnish clinical groups (N = 85 and N = 2). The first Finnish clinical group of 85 cancer patient families with dependant children included a sub-sample of 54 families with children aged 11-17 years. The second Finnish clinical group consisted of two pregnant cancer patients. Additionally, a control group (N = 59) consisting of a sub-sample of 49 families with children aged 11-17 years was used. Quantitative methods (FAD, BDI, YSR, SOC, SF-8) and qualitative methods (observation, interviews, diaries, videotapes) were used exclusively and/or in combination. The results can be summarised as follows: 1) cancer “per se “ did not impair family functioning, children’s mental health, early interaction between ill mothers and their infant, 2) maternal depression or the ill parent’s depression were significantly associated with impairment in family functioning, 3) the individual’s good sense of coherence was associated with improvement in family functioning, and 4) a child-centred family intervention, which aims to give space for elaborating on cancer in the family, validates the sense of coherence and children’s feelings, and promotes open communication was welcomed. It is important to note that in the European study group, the prevalence of depression was 35 % (BDI > 16) among ill mothers, and 28% among healthy mothers, 28% among ill fathers, and 13% among healthy fathers. Early screening and effective treatment of depression in cancer patients and their partners is of paramount importance for the mental health of children and the well-being of the family. Pregnant cancer patients are in need of psychosocial support.
