Endometriosis is a common hormone-dependent gynecological disease leading to severe menstrual and/or chronic pelvic pain with or without subfertility. The disease is defined by the presence of endometrium-like tissue outside the uterine cavity, primarily on the pelvic peritoneum, ovaries and infiltrating organs of the peritoneal cavity. The current tools for diagnosis and treatment of endometriosis need to be improved to ensure reliable diagnosis and effective treatment. In addition, endometriosis is associated with increased risk of ovarian cancer and, therefore, the differential diagnosis between the benign and malignant ovarian cysts is of importance. The long-term objective of the present study was to support the discovery of novel tools for diagnosis and treatment of endometriosis. This was approached by exploiting genome-wide expression analysis of endometriosis specimens. A novel expression profiling -based classification of endometriosis indicated specific subgroups of lesions partially consistent with the clinical appearance, but partially according to unknown factors. The peritoneum of women with endometriosis appeared to be altered in comparison to that of healthy control subjects, suggesting a novel aspect on the pathogenesis of the disease. The evaluation of action and metabolism of sex hormones in endometrium and endometriosis tissue indicated a novel role of androgens in regulation of the tissues. In addition, an enzyme involved in androgen and neurosteroid metabolism, hydroxysteroid (17beta) dehydrogenase 6, was found to be highly up-regulated in endometriosis tissue as compared to healthy endometrium. The enzyme may have a role in the pathogenesis of endometriosis or in the endometriosis associated pain generation. Finally, a new diagnostic biomarker, HE4, was discovered distinguishing patients with ovarian endometriotic cysts from those with malignant ovarian cancer. The information acquired in this study enables deeper understanding of endometriosis and facilitates the development of improved diagnostic tools and more specific treatments of the disease

Tässä pro gradu -tutkielmassa tarkastellaan henkilöstöresurssien organisointia suuren alusöljyvahingon torjuntatilanteessa. Tavoitteena on suunnitella Suomenlahden rannikon öljyntorjunnasta vastaavien viranomaisten käyttöön optimaalinen rekrytointistrategia pahimman todennäköisen alusöljyvahingon varalle. Tutkimuksessa selvitetään myös millainen työsopimus öljyntorjuntatyöntekijöiden kanssa voidaan sopia. Näiden lisäksi etsitään vastausta siihen, kuinka työvoima saadaan pidettyä. Tämän laadullisen tutkimuksen teoreettinen osuus toteutettiin kirjallisuuskatsauksena. Tutkimuksen empiirinen aineisto kerättiin haastattelemalla yhdeksää asiantuntijaa syksyn 2009 aikana. Haastattelut olivat muodoltaan puolistrukturoituja teemahaastatteluja. Tutkimustulosten mukaan merkittävin lisätyövoiman tarve ilmenee käsin tehtävässä rantapuhdistustyössä. Etenkin puhdistustyön pitkittyessä pelastusviranomaiset tarvitsevat avukseen ulkopuolista työvoimaa. Rekrytointi suoritetaan muutamien viikkojen kuluessa öljyvahingon aiheutumisen jälkeen. Alueellinen pelastuslaitos suorittaa rekrytoinnin käyttäen tehokkaita, laajan kohderyhmän tavoittavia rekrytointiviestinnän välineitä (esim. sanomalehdet, TV ja Internet). Työntekijöiden kanssa sovitaan määräaikainen, Kunnallista virkaja työehtosopimusta noudattava työsopimus. Tärkeimpinä puhdistustyöntekijöitä motivoivina tekijöinä nähdään työn merkitys yhteiskunnalle, selkeästi määritelty, saavutettavissa oleva tavoite sekä palaute tehdystä työstä.

Deregulation of the electricity sector liberated the electricity sale and production for competitive forces while in the network business, electricity transmission and distribution, natural monopoly positions were recognised. Deregulation was accompanied by efficiencyoriented thinking on the whole electricity supply industry. For electricity distribution this meant a transition from a public service towards profit-driven business guided by economic regulation. Regulation is the primary means to enforce societal and other goals in the regulated monopoly sector. The design of economic regulation is concerned with two main attributes; end-customer price and quality of electricity distribution services. Regulation limits the costs of the regulated company but also defines the desired quality of monopoly services. The characteristics of the regulatory framework and the incentives it provides are therefore decisive for the electricity distribution sector. Regulation is not a static factor; changes in the regulatory practices cause discontinuity points, which in turn generate risks. A variety of social and environmental concerns together with technological advancements have emphasised the relevance of quality regulation, which is expected to lead to the large-scale replacement of overhead lines with underground cables. The electricity network construction activity is therefore currently witnessing revolutionary changes in its competitive landscape. In a business characterised by high statutory involvement and a high level of sunk costs, recognising and understanding the regulatory risks becomes a key success factor. As a response, electricity distribution companies have turned into outsourcing to attain efficiency and quality goals. This doctoral thesis addresses the impacts of regulatory risks on electricity network construction, which is a commonly outsourced activity in the electricity distribution network sector. The chosen research approach is characterised as an action analytical research on account of the fact that regulatory risks are greatly dependent on the individual nature of the regulatory regime applied in the electricity distribution sector. The main contribution of this doctoral thesis is to develop a concept for recognising and managing the business risks stemming from economic regulation. The degree of outsourcing in the sector is expected to increase in years to come. The results of the research provide new knowledge to manage the regulatory risks when outsourcing services.
