Background: Despite their relevance to the prevention of sexually transmitted infections, there are few data on the frequency of recourse to prostitution in the male population in Switzerland. Using data gathered for the evaluation of the Swiss AIDS prevention strategy, we analysed net aggregate change and cohort-based change in lifetime prevalence of recourse to prostitution. Methods: Seven repeated cross-sectional telephone surveys of the general population aged 17-45 years (17-30 years only for the 1987 and 1988 surveys) were undertaken from 1987 to 2000 providing information on sexual behaviour including men's recourse to prostitution (total n¼9318). Age categories were: 17-20, 21-25, 26-30, 31-35, 36-40 and 41-45 years. Prevalence at 17-30 years was available in all surveys and prevalence at 41-45 was available for 1989-2000, though not for the same cohorts. Intra-cohort increase in prevalence over 10 years was analysed using truncated information for cohorts aged 21-25 and 26-30 years in 1987 and 1990. Population estimates were computed with 95% confidence intervals (CI). Results: No net change occurred in the 17-45 years male population prevalence between 1989 (17.6%, CI ¼ 15.4; 20.0) and 2000 (17.7%, CI ¼ 15.6; 20.0). The median starting prevalence of recourse to prostitution at age 17-20 was 4.8% (in 1989, CI ¼ 2.0; 9.7) and the range was from 1.8 (in 1994) to 10.4% (in 1990). The median ending prevalence at age 41-45 was 21.9% (in 1994, CI 16.7; 27.9) and the range was from 17.9 (in 2000) to 26.1% (in 1992). No clear trend was observed in either starting or ending prevalence. Intra-cohort evolution of the 1997 and 1990 cohorts was very similar. Conclusions: Based on available data, there was no net (aggregate) change in the prevalence of recourse to prostitution by males in Switzerland between 1989 and 2000. Within the time frame available, intra-cohort evolution was also very similar.

Cette étude est destinée à évaluer les effets des campagnes de prévention du SIDA en Suisse chez les homosexuels, en termes de modifications des représentations, attitudes et comportements face à cette maladie et aux manières de s'en protéger. Elle s'intéresse aussi aux modalités d'acquisition d'attitudes et comportements nouveaux, ainsi qu'à la manière dont les campagnes ont été accueillies et répercutées. (Questionnaire en annexe).

OBJECTIVES: To analyse the prevalence of lifetime recourse to prostitution (LRP) among men in the general population of Switzerland from a trend and cohort perspective. METHODS: Using nine repeated representative cross-sectional surveys from 1987 to 2000, age-specific estimates of LRP were computed. Trends and period effect were analysed as the evolution of cross-sectional population estimates within age groups and overall. Cohort analysis relied on cohorts constructed from the 1989 survey and followed in subsequent waves. Age and cohort effects were modelled using logistic regression and non-parametric monotone regression. RESULTS: Whereas prevalence for the younger groups was found to be logically lower, there was no consistent increasing or decreasing trend over the years; there was no significant period effect. For the 17-30 year age group, the mean estimate over 1987-2000 was 11.5% (range 8.3 to 12.7%); for the 31-45 year group, the mean was 21.5% (range over 1989-2000 20.3 to 23.0%). Regarding cohort analysis, the prevalence of LRP was found to increase steeply in the youngest ages before reaching a plateau near the age of 40 years. At the age of 43 years, the prevalence was estimated to be 22.6% (95% CI 21.1% to 24.1%). CONCLUSIONS: The steep increase in the cohort-wise prevalence of LRP in younger ages calls for a concentration of prevention activities in young people. If the plateauing at approximately 40 years of age is not followed by a further increase later in life, which is not known, then consumers of paid sex would be repeat buyers only, a fact that should be taken into account by prevention.

Die Untersuchung verfolgt mehrere Ziele: -> erstens soll der Informationsstand über die Aktivitäten zur Aidsbekämpfung in den Kantonen erhöht werden, -> zweitens sollen Lösungsmodelle bezüglich des Umgangs mit dem Problem vergleichen dargestellt werden, -> drittens sollen Erkenntnisse darüber gesammelt werden, welche Individuen sich als besonders gute Vermittler präventiver Botschaften eignen.

Die Analyse der Medienreporte untersucht vor allem Prozesse wie die Verbreitung der Botschaft der Kampagne STOP AIDS durch die Medien, die Verbreitung von informellen und institutionellen Aktionen (zum Beispiel die Aktivitäten zur Aufklärung in den Schulen), die Rolle der Multiplikatoren (Aids-Hilfe, Informanten an öffentlichen Veranstaltungen), den Meinungsbildungsprozess zur Aidsproblematik im allgemeinen und zur Präventionsstrategie im besonderen, soweit dieser in den Medien abgebildet, beziehungsweise von den Medien getragen wird. Im weiteren beachtet die Analyse Rückmeldungen über die Effekte (Resultate) der Kampagne (Beurteilung der Wirkung der Kampagne, zum Beispiel auf das Verhalten von Jugendlichen, Fixern). Estimates have recently been made of the incidence of cancers in the countries of the European Community. Similar estimates are given for Switzerland, based on data from the six Swiss cantonal cancer registries, all of which have been operating for at least 12 years. These registries cover Basel, Geneva, Neuchatel, St Gall and Appenzell, Vaud and Zurich, which account for about 50% of the Swiss population as a whole. Two different methods were used to extrapolate from the incidences observed in the regions covered by cancer registration to the entire country. The first method is based solely on the distribution of populations according to the country's main linguistic groups, whereas the second relies on mortality data. Estimates obtained by the second approach are presented and their reliability is discussed. Comparison of the age incidence curve with that of Denmark tends to confirm the validity of the estimations. Estimated standardised rates (world population) for all sites except nonmelanomatous skin cancer are 294.3 for males and 214.2 for females. Comparisons with other European countries show that in males, lung cancer is relatively less common in Switzerland, whereas in females, breast cancer is relatively more frequent.

Die vorliegende Studie untersucht den Stand der Anpassung Adoleszenter and die Gefährdung durch Aids im Frühsommer 1987 anhand einer Befragung von Berufschülern. Die Gefährdung durch Fixen mit gebrauchten Nadeln wurde in dieser Untersuchung nicht berücksichtigt. Im ersten Teil des Schlussberichtes werden die Methoden vorgestellt. Im zweiten Teil berichten wir, wie die befragten Berufschüler auf die Kampagne STOP AIDS reagiert haben. Im dritten Teil werden Einstellungen in Form von Aussagen zur Relevanz und Akzeptanz der Botschaften und die daraus resultierende Beweise entsprechend der Gefährdung durch Aids zu gestalten, besschrieben. Auf der Einstellungs- und Verhaltensebene werden ausserdem der Gebrauch des Präservativs und des HIV-Tests untersucht. Die Analyse im vierten Teil wertet die Ergebnisse unter den Gesichtspunkten "anomische Situation", "individuelle Handlungskonzepte" und "Störfaktoren" aus. Den Abschluss bildet eine Zusammenfassung und Empfehlungen an die Initianten und Verantwortlichen der Kampagne.
