Au Centre Hospitalier Universitaire Vaudois, l'Institut Universitaire de Médecine Légale de Lausanne a ouvert en 2006 l'Unité de Médecine des Violences qui offre une consultation médico-légale spécialisée dans la prise en charge des adultes victimes de violence. L'ouverture de cette unité a vu l'entrée de la profession infirmière dans le champ de la médecine légale clinique. Les infirmières assurent en effet de manière autonome les consultations sous la supervision d'un médecin légiste.

Le but de cette étude était d'investiguer l'association entre l'alliance thérapeutique et le contrôle de substances, comme il est vécu par le patient. La théorie de King et le concept de l'alliance thérapeutique figuraient comme cadres théoriques. L'investigation se faisait à l'aide d'un questionnaire auto-administré, incluant l'instrument working alliance inventory (WAI) dans sa forme abréviée, traduit et validé en français, visant un échantillon de convenance (n=28). L'alliance thérapeutique était évaluée sur une échelle Likert à 12 questions, et les alliances thérapeutiques se sont révélées comme bonnes (x=61.68, max=84). Mais il y a une relation linéaire négative significative entre le degré à quel point les contrôles sont perçus comme pesants, évalués par une question supplémentaire sur une visual analog scale (VAS), et l'établissement d'une alliance thérapeutique (r =-0.67, p-value>0.001). De plus, la forme d'admission joue un rôle significatif, car dans cette étude, les patients admis sous pression de l'extérieur (n=7) ont de moins bonnes alliances (t, p-value=0.018) et vivent les contrôles comme plus pesants (Z, p-value=0.004) que les patients admis de leur propre gré (n=21). Malgré que ces résultats ne sont, vu le petit échantillon (n=28), pas généralisables, ils pourraient impliquer pour la pratique de repenser la prise d'urine et d'être sensibilisés aux conséquences possibles. Des options, comme l'introduction d'une supervision par caméra ou le retrait complet de l'infirmière de référence de toute sorte de contrôles, seraient imaginables. Au moins pour les personnes hospitalisées sous pression de l'extérieur et qui sont probablement, pour cette raison, plus susceptibles à tout dérangement de la relation.

Communication between trainer and trainee plays a central role in teaching and learning in the clinical environment. There are various strategies to frame the dialogue between trainee and trainer. These strategies allow trainers to be more effective in their supervision, which is important in our busy clinical environment. Communication strategies are well adapted to both in- and out-patient settings, to both under- and postgraduate contexts. This article presents three strategies that we think are particularly useful. They are meant to give feedback, to ask questions and to present a case.

The increasing prevalence of obesity and its associated complications requires specialized care to improve outcomes and control health care costs. Obesity is associated with numerous serious and costly medical problems requiring specialized care in managing health. The economic burden of obesity includes increased inpatient and outpatient medical expenditures as well as employer-related issues of absenteeism and associate costs. The objectives of this study are: - To describe the health consequences and the economic burden of obesity, - To review the existing treatment - To argue in favor of a specialized nutritional intervention that has shown to improve health and reduce obesity related health care costs. Therefore, expose the possibility of introducing the specialized nutrition in Switzerland and the feasibility of this project considering the medical trends and reimbursement system in Switzerland The benefits and outcomes for the patients will be the significant weight loss which reduces the severity and risk factors for complications and the improved health and quality of life. Weight loss will be a combination of a diet, exercise and behavioral interventions which are the basic recommendations for obesity treatment in addition to the specialized nutritional support. By nutritional support, we mean products that are intended to provide nutritional support in the dietary management of people with specific diseases and conditions when adequate intake of regular foods is compromised. These products are called, Food for special medical purposes FSMP. They are not intended to treat, cure, prevent, mitigate or have a direct impact on disease in a manner similar to drugs or other medical treatments and should be used under medical supervision. They also provide a low cost alternative to surgery. From a health care system perspective, the specialized nutrition will drive its advantage by reducing the utilization of medical services for obesity associated complications like medication, physician's consultations and surgical interventions arriving to a cost effective care for the hospitals, the health care organizations and the third party payers which are the health insurances. OBJECTIVE: Client change talk has been proposed as a mechanism of change in motivational interviewing (MI) by mediating the link between therapist MI-consistent behaviors (MICO) and client behavioral outcomes. We tested under what circumstances this mechanism was supported in the context of a clinical trial of brief MI for heavy drinking among nontreatment seeking young men. METHOD: We conducted psycholinguistic coding of 174 sessions using the MI Skill Code 2.1 and derived the frequency of MICO and the strength of change talk (CTS) averaged over the session. CTS was examined as a mediator of the relationship between MICO and a drinking composite score measured at 3-month follow-up, controlling for the composite measure at baseline. Finally, we tested therapist gender and MI experience as well as client readiness to change and alcohol problem severity as moderators of this mediation model. RESULTS: CTS significantly predicted outcome (higher strength related to less drinking), but MICO did not predict CTS. However, CTS mediated the relationship between MICO and drinking outcomes when therapists had more experience in MI and when clients had more severe alcohol problems (i.e., significant conditional indirect effects). CONCLUSIONS: The mechanism hypothesized by MI theory was operative in our brief MI with heavy drinking young men, but only under particular conditions. Our results suggest that attention should be paid to therapist selection, training, and/or supervision until they reach a certain level of competence, and that MI might not be appropriate for nontreatment seeking clients drinking at a lower level of risk.
