The results of several large multicenter CMR studies were reported in 2012, thus, constantly corroborating the evidence on CMR performance. In this review, we present results of the MR-IMPACT programme and the CE-MARC study, which demonstrated the superiority of perfusion-CMR over gated SPECT for the workup of suspected CAD, the currently available data from the European CMR registry, comprising almost 30,000 patients from 57 participating centers in 15 European countries, and finally, the results of the Advisa-MRI study, which documented the safety of a MRI-compatible pacemaker system. These large trials and others set the basis for the recommendations in the new European guidelines on heart failure to use CMR as a first line method if echocardiographic quality is inadequate or the etiology of heart failure is unclear.

Chronic atrial fibrillation affects millions of people worldwide. Its surgical treatment often fails to restore the transport function of the atrium. This study first introduces the concept of an atrial assist device (AAD) to restore the pump function of the atrium. The AAD is developed to be totally implantable in the human body with a transcutaneous energy transfer system to recharge the implanted battery. The ADD consists of a motorless pump based on artificial muscle technology, positioned on the external surface of the atrium to compress it and restore its muscular activity. A bench model reproduces the function of a fibrillating atrium to assess the circulatory support that this pump can provide. Atripump (Nanopowers SA, Switzerland) is a dome-shaped silicone-coated nitinol actuator 5 mm high, sutured on the external surface of the atrium. A pacemaker-like control unit drives the actuator that compresses the atrium, providing the mechanical support to the blood circulation. Electrical characteristics: the system is composed of one actuator that needs a minimal tension of 15 V and has a maximum current of 1.5 A with a 50% duty cycle. The implantable rechargeable battery is made of a cell having the following specifications: nominal tension of a cell: 4.1 V, tension after 90% of discharge: 3.5 V, nominal capacity of a cell: 163 mA h. The bench model consists of an open circuit made of latex bladder 60 mm in diameter filled with water. The bladder is connected to a vertically positioned tube that is filled to different levels, reproducing changes in cardiac preload. The Atripump is placed on the outer surface of the bladder. Pressure, volume and temperature changes were recorded. The contraction rate was 1 Hz with a power supply of 12 V, 400 mA for 200 ms. Preload ranged from 15 to 21 cm H(2)O. Maximal silicone membrane temperature was 55 degrees C and maximal temperature of the liquid environment was 35 degrees C. The pump produced a maximal work of 16 x 10(-3) J. Maximal volume pumped was 492 ml min(-1). This artificial muscle pump is compact, follows the Starling law and reproduces the hemodynamic performances of a normal atrium. It could represent a new tool to restore the atrial kick in persistent atrial fibrillation.

AIMS: To evaluate short-term clinical outcomes following transcatheter aortic valve implantation (TAVI) using CE-mark approved devices in Switzerland. METHODS AND RESULTS: The Swiss TAVI registry is a national, prospective, multicentre, monitored cohort study evaluating clinical outcomes in consecutive patients undergoing TAVI at cardiovascular centres in Switzerland. From February 2011 to March 2013, a total of 697 patients underwent TAVI for native aortic valve stenosis (98.1%), degenerative aortic bioprosthesis (1.6%) or severe aortic regurgitation (0.3%). Patients were elderly (82.4±6 years), 52% were females, and the majority highly symptomatic (73.1% NYHA III/IV). Patients with severe aortic stenosis (mean gradient 44.8±17 mmHg, aortic valve area 0.7±0.3 cm²) were either deemed inoperable or at high risk for conventional surgery (STS 8.2%±7). The transfemoral access was the most frequently used (79.1%), followed by transapical (18.1%), direct aortic (1.7%) and subclavian access (1.1%). At 30 days, rates of all-cause mortality, cerebrovascular events and myocardial infarction were 4.8%, 3.3% and 0.4%, respectively. The most frequently observed adverse events were access-related complications (11.8%), permanent pacemaker implantation (20.5%) and bleeding complications (16.6%). The Swiss TAVI registry is registered at ClinicalTrials.gov (NCT01368250). CONCLUSIONS: The Swiss TAVI registry is a national cohort study evaluating consecutive TAVI procedures in Switzerland. This first outcome report provides favourable short-term clinical outcomes in unselected TAVI patients.

We report on two patients with recurrent syncope secondary to ictal bradyarrhythmias, triggered by partial epileptic seizures with atypical, stereotyped auras. Ictal bradyarrhythmias are potentially lethal, and likely originate from the involvement of limbic autonomic regions. The appropriate treatment is double-headed, including an antiepileptic drug and the implantation of a pacemaker.

The hyperpolarization-activated cyclic nucleotide-gated (HCN) channels are expressed in pacemaker cells very early during cardiogenesis. This work aimed at determining to what extent these channels are implicated in the electromechanical disturbances induced by a transient oxygen lack which may occur in utero. Spontaneously beating hearts or isolated ventricles and outflow tracts dissected from 4-day-old chick embryos were exposed to a selective inhibitor of HCN channels (ivabradine 0.1-10microM) to establish a dose-response relationship. The effects of ivabradine on electrocardiogram, excitation-contraction coupling and contractility of hearts submitted to anoxia (30min) and reoxygenation (60min) were also determined. The distribution of the predominant channel isoform, HCN4, was established in atria, ventricle and outflow tract by immunoblotting. Intrinsic beating rate of atria, ventricle and outflow tract was 164+/-22 (n=10), 78+/-24 (n=8) and 40+/-12bpm (n=23, mean+/-SD), respectively. In the whole heart, ivabradine (0.3microM) slowed the firing rate of atria by 16% and stabilized PR interval. These effects persisted throughout anoxia-reoxygenation, whereas the variations of QT duration, excitation-contraction coupling and contractility, as well as the types and duration of arrhythmias were not altered. Ivabradine (10microM) reduced the intrinsic rate of atria and isolated ventricle by 27% and 52%, respectively, whereas it abolished activity of the isolated outflow tract. Protein expression of HCN4 channels was higher in atria and ventricle than in the outflow tract. Thus, HCN channels are specifically distributed and control finely atrial, ventricular and outflow tract pacemakers as well as conduction in the embryonic heart under normoxia and throughout anoxia-reoxygenation.

Introduction: We recently observed in a chronic ovine model that a shortening of action potential duration (APD) as assessed by the activation recovery interval (ARI) may be a mechanism whereby pacing-induced atrial tachycardia (PIAT) facilitates atrial fibrillation (AF), mediated by a return to 1:1 atrial capture after the effective refractory period has been reached. The aim of the present study is to evaluate the effect of long term intermittent burst pacing on ARI before induction of AF.Methods: We specifically developed a chronic ovine model of PIAT using two pacemakers (PM) each with a right atrial (RA) lead separated by ∼2cm. The 1st PM (Vitatron T70) was used to record a broadband unipolar RA EGM (800 Hz, 0.4 Hz high pass filter). The 2nd was used to deliver PIAT during electrophysiological protocols at decremental pacing CL (400 beats, from 400 to 110ms) and long term intermittent RA burst pacing to promote electrical remodeling (5s of burst followed by 2s of sinus rhythm) until onset of sustained AF. ARI was defined as the time difference between the peak of the atrial repolarization wave and the first atrial depolarization. The mean ARIs of paired sequences (before and after remodeling), each consisting of 20 beats were compared.Results: As shown in the figure, ARIs (n=4 sheep, 46 recordings) decreased post remodeling compared to baseline (86±19 vs 103±12 ms, p<0.05). There was no difference in atrial structure as assessed by light microscopy between control and remodeled sheep.Conclusions: Using standard pacemaker technology, atrial ARIs as a surrogate of APDs were successfully measured in vivo during the electrical remodeling process leading to AF. The facilitation of AF by PIAT mimicking salvos from pulmonary veins is heralded by a significant shortening of ARI.

OBJECTIVES: The aim of the study was to statistically model the relative increased risk of cardiovascular disease (CVD) per year older in Data collection on Adverse events of anti-HIV Drugs (D:A:D) and to compare this with the relative increased risk of CVD per year older in general population risk equations. METHODS: We analysed three endpoints: myocardial infarction (MI), coronary heart disease (CHD: MI or invasive coronary procedure) and CVD (CHD or stroke). We fitted a number of parametric age effects, adjusting for known risk factors and antiretroviral therapy (ART) use. The best-fitting age effect was determined using the Akaike information criterion. We compared the ageing effect from D:A:D with that from the general population risk equations: the Framingham Heart Study, CUORE and ASSIGN risk scores. RESULTS: A total of 24 323 men were included in analyses. Crude MI, CHD and CVD event rates per 1000 person-years increased from 2.29, 3.11 and 3.65 in those aged 40-45 years to 6.53, 11.91 and 15.89 in those aged 60-65 years, respectively. The best-fitting models included inverse age for MI and age + age(2) for CHD and CVD. In D:A:D there was a slowly accelerating increased risk of CHD and CVD per year older, which appeared to be only modest yet was consistently raised compared with the risk in the general population. The relative risk of MI with age was not different between D:A:D and the general population. CONCLUSIONS: We found only limited evidence of accelerating increased risk of CVD with age in D:A:D compared with the general population. The absolute risk of CVD associated with HIV infection remains uncertain.

The mammalian circadian timing system consists of a central pacemaker in the brain's suprachiasmatic nucleus (SCN) and subsidiary oscillators in nearly all body cells. The SCN clock, which is adjusted to geophysical time by the photoperiod, synchronizes peripheral clocks through a wide variety of systemic cues. The latter include signals depending on feeding cycles, glucocorticoid hormones, rhythmic blood-borne signals eliciting daily changes in actin dynamics and serum response factor (SRF) activity, and sensors of body temperature rhythms, such as heat shock transcription factors and the cold-inducible RNA-binding protein CIRP. To study these systemic signalling pathways, we designed and engineered a novel, highly photosensitive apparatus, dubbed RT-Biolumicorder. This device enables us to record circadian luciferase reporter gene expression in the liver and other organs of freely moving mice over months in real time. Owing to the multitude of systemic signalling pathway involved in the phase resetting of peripheral clocks the disruption of any particular one has only minor effects on the steady state phase of circadian gene expression in organs such as the liver. Nonetheless, the implication of specific pathways in the synchronization of clock gene expression can readily be assessed by monitoring the phase-shifting kinetics using the RT-Biolumicorder.

Mostrando qualche reticenza nel riferire a se stesso il termine di 'narratore', Bassani ama definirsi soprattutto 'poeta' e 'saggista'. Nei saggi del volume Di là dal cuore, la voce in prima persona costruisce il proprio statuto di legittimità di 'genere'. L'esperienza saggistica, ci fa capire Bassani, non è un settore marginale della sua produzione, ma un punto di vista privilegiato per osservare l'insieme della sua opera di scrittore. In questo senso, i saggi accuratamente riuniti dall'autore, nonostante l'apparente occasionalità, formano un percorso unitario che si pone 'alla pari' nei confronti della prosa narrativa. Tra gli elementi di coerenza della prosa saggistica di Bassani si rilevano, in particolare, il posizionamento esplicito dell'io e la necessità del riferimento al vissuto, biografico e storico.

NlmCategory="UNASSIGNED">Rapid deployment aortic valve replacement (RDAVR) with the use of rapid deployment valve systems represents a smart alternative to the use of standard aortic bioprosthesis for aortic valve replacement. Nevertheless, its use is still debatable in patients with pure aortic valve regurgitation or true bicuspid aortic valve because of the risk of postoperative paravalvular leak. To address this issue, an optimal annulus-valve size match seems to be the ideal surgical strategy. This article describes a new technique developed to stabilize the aortic annulus and prevent paravalvular leak after RDAVR. To confirm the feasibility, this technique was performed in six patients with severe symptomatic aortic stenosis who were scheduled to undergo aortic valve replacement at our center. All patients survived surgery and were discharged from the hospital. There were no new intracardiac conduction system disturbances observed, and a permanent pacemaker implantation was not required in any of the patients. The intraoperative and postoperative echocardiogram confirmed successful positioning of the valve, and no paravalvular leak was observed. In this preliminary experience, RDAVR through a full sternotomy or an upper hemisternotomy approach with the use of aortic annulus stabilization technique was safe, and no leak was observed. Future studies on large series of patients are necessary to confirm the safety and effectiveness of this technique in preventing paravalvular leak in patients with true bicuspid aortic valves or pure aortic regurgitation.

BACKGROUND: New generation transcatheter heart valves (THV) may improve clinical outcomes of transcatheter aortic valve implantation. METHODS AND RESULTS: In a nationwide, prospective, multicenter cohort study (Swiss Transcatheter Aortic Valve Implantation Registry, NCT01368250), outcomes of consecutive transfemoral transcatheter aortic valve implantation patients treated with the Sapien 3 THV (S3) versus the Sapien XT THV (XT) were investigated. An overall of 153 consecutive S3 patients were compared with 445 consecutive XT patients. Postprocedural mean transprosthetic gradient (6.5±3.0 versus 7.8±6.3 mm Hg, P=0.17) did not differ between S3 and XT patients, respectively. The rate of more than mild paravalvular regurgitation (1.3% versus 5.3%, P=0.04) and of vascular (5.3% versus 16.9%, P<0.01) complications were significantly lower in S3 patients. A higher rate of new permanent pacemaker implantations was observed in patients receiving the S3 valve (17.0% versus 11.0%, P=0.01). There were no significant differences for disabling stroke (S3 1.3% versus XT 3.1%, P=0.29) and all-cause mortality (S3 3.3% versus XT 4.5%, P=0.27). CONCLUSIONS: The use of the new generation S3 balloon-expandable THV reduced the risk of more than mild paravalvular regurgitation and vascular complications but was associated with an increased permanent pacemaker rate compared with the XT. Transcatheter aortic valve implantation using the newest generation balloon-expandable THV is associated with a low risk of stroke and favorable clinical outcomes. La cardiomyopathie hypertrophique (CMH) est la maladie cardiaque monogénique la plus fréquente, touchant environ 1 individu sur 500 dans la population (1,2). L'étiologie est familiale dans la majorité des cas avec une transmission de type autosomal dominant à pénétrance variable. Deux gènes principaux sont à l'origine de la maladie chez 70% des patients avec un test génétique positif. Il s'agit des gènes qui codent pour la chaine lourde β de la myosine (MYH7) et la protéine C liant la myosine (MYBPC3) (1-3). La CMH est définie par la présence d'une hypertrophie myocardique « inadéquate » car se développant en l'absence d'une cause d'augmentation de la post-charge (HTA, sténose aortique, membrane sous-aortique), d'une pathologie infiltrative ou d'un entrainement physique (4,5). Le plus souvent asymétrique et affectant préférentiellement le septum, cette hypertrophie s'accompagne dans près de deux tiers des cas d'une obstruction dynamique sous-aortique de la chambre de chasse du ventricule gauche par la valve mitrale (systolic anterior motion ou SAM). Cette obstruction est à la fois la conséquence du rétrécissement de la chambre de chasse par l'hypertrophie septale mais également d'un malpositionnement de la valve mitrale (6-8). On parle alors de Cardiomyopathie Hypertrophique et Obstructive (CMHO). L'obstruction - présente au repos dans 50% des cas et uniquement après manoeuvres de provocation dans l'autre moitié des cas (manoeuvre de Valsalva, test de vasodilatation par nitrite d'amyle) est à l'origine d'un gradient de pression entre le ventricule gauche et l'aorte, et donc d'une surcharge de pression pour le ventricule gauche. Cette surcharge de pression est à l'origine des symptômes classiquement rencontrés soit dyspnée et angor d'effort, présyncope voire syncopes à l'effort. Un gradient sous-aortique de plus de 50 mmHg (mesuré au repos ou après provocation) est considéré comme un gradient à valeur pronostique (6-8) et justifiant un traitement si associé à des symptômes. Le traitement médical des formes obstructives repose sur l'administration de substances inotropes négatives et/ou susceptibles de favoriser la relaxation myocardique tels que les béta-bloqueurs, les antagonistes du calcium et le disopyramide - pris isolément ou en association. Pour les nombreux patients qui deviennent réfractaires ou intolérants à ces traitements, deux interventions peuvent leur être proposées pour lever l'obstruction : une myotomie-myectomie chirurgicale du septum (9,10) ou une alcoolisation du septum par voie percutanée (7,8). Les indications à ces interventions sont les suivantes (7,8,11) : 1. Symptômes (dyspnée de classe fonctionnelle NYHA III ou IV, angor de classe fonctionnelle CCS III ou IV, syncope, ou présyncope) réfractaires au traitement médical ou intolérance du patient au traitement. Une dyspnée de classe II est considérée suffisante dans le cas de jeunes patients. 2. Obstruction sous-aortique avec gradient supérieur ou égal à 50 mmHg, au repos ou après manoeuvre de provocation, associée à une hypertrophie septale et à un mouvement systolique antérieur de la valve mitrale (effet SAM) 3. Anatomiecardiaquefavorableàuntraitementinvasif(épaisseurduseptumde plus de 16 mm) Si la myectomie chirurgicale reste la méthode de référence (12-18), l'alcoolisation septale du myocarde par voie percutanée est devenue un des traitements de choix dans la thérapie de la Cardiomyopathie Hypertrophique Obstructive réfractaire. Elle consiste à repérer par coronarographie l'artère septale nourrissant le septum basal hypertrophié, puis à y introduire un petit ballon pour isoler ce territoire du reste du lit coronaire avant d'y injecter une dose d'alcool à 95% comprise entre 1 et 5 cc. On crée ainsi un infarctus chimique, technique qui fut dans le passé utilisée pour le traitement de certaines tumeurs. Les effets ne sont pas immédiats et nécessitent généralement 2-3 semaines avant de se manifester. On assiste alors à une diminution progressive de l'épaisseur du myocarde nécrosé (7), à la disparition progressive de l'obstruction et à l'amélioration / disparition des symptômes. La question de savoir qui de la chirurgie ou de l'alcoolisation est le plus efficace a été source de nombreux débats (7,11-13,18). Par rapport à la chirurgie, les avantages de la méthode percutanée sont les suivants (11,14,15,18,19) : - Efficacités hémodynamique et fonctionnelle jugées comparable à la chirurgie selon les études - Taux de morbidité et de mortalité très faible et non supérieure à la chirurgie - Absence de sternotomie - Diminution de la durée de l'hospitalisation et surtout de la période de convalescence, le patient pouvant reprendre une activité dès son retour à domicile Certains experts émettent néanmoins des doutes quant à l'innocuité à long terme de la méthode, les zones nécrotiques pouvant servir de terrain arythmogène. Pour ces raisons, la méthode n'est pas recommandée chez les patients de moins de 40 ans (6,8). Le risque majeur de l'alcoolisation du septum proximal réside dans l'induction d'un bloc atrio-ventriculaire complet chimique, le noeud atrio-ventriculaire étant justement situé dans cette région. Ce risque augmente avec la quantité d'alcool administrée et nécessite, si persistance après trois jours, l'implantation d'un pacemaker à demeure. Selon les centres, le taux d'implantation d'un stimulateur varie ainsi entre 7% et 20% (7,14,20). L'efficacité clinique et l'incidence des complications est donc en partie liée à la compétence technique et à l'expérience de l'opérateur (7,14), mais aussi aux choix des patients. Il peut donc varier grandement selon les centres médicaux. L'étude proposée vise à analyser les résultats de l'alcoolisation obtenus à Lausanne, jusqu'à présent pas encore été étudiés, et à les comparer à ceux de la littérature.
