Background: Nursing home short stays (NHSS) in the canton of Vaud have been introduced for respite care purpose. However, a growing number of older patients are urgently admitted from home (within 24h) or directly after hospital discharge (58% of all admissions in 2010). NHSS appears therefore as an increasingly important component of the health care system, but the characteristics of admitted patients have not been previously described. A better knowledge would contribute to identify specific care needs and enhance their care. Objectives: 1) To describe the characteristics of patients admitted in unplanned NHSS ( after hospital stay or urgently from home); 2) To determine living disposition 3-month after NHSS discharge. Method: Over a 18-month period, elderly patients with unplanned NHSS admission to 2 facilities in Lausanne were identified. Demographic, social, health, and functional data, as well as main reason for admission were collected. Death and place of living at 3-months were collected using the administrative database. Results: Overall, 114 patients (mean age 83.1 ± 6.2 years, 77% women, 84% living alone) were assessed, 80% being admitted from hospital. Mean score in Lawton's instrumental ADL before NHSS admission was 4.6 ± 2.5 and 69% of the patients were home care recipients (median number of weekly visits: 5 ± 3). Patients reported going out 4.2 ± 1.3 times/week and 56% reported at least one fall over the past year. Among the 91 patients coming from the hospital, main reason for admission was injury/limb immobilization (58%), recuperation (13%) and functional impairment in basic ADL (10%). Mean score at Katz's Basic ADL at admission was 3.7 ± 1.9. Overall, 90% of patients were identified with gait and balance impairment, 78% with cognitive impairment and 70% with polypharmacy (>6 different drugs). At 3-month after NHSS discharge (N = 92), 72% patients were living at home, 16% had been admitted to long term care, and 6% died. Among patients living at home at follow-up, 11% had been readmitted to hospital during the follow-up period. Conclusion: Older patients with unplanned NHSS admission show a high prevalence of functional, mobility, and cognitive impairments, as well as other geriatric syndromes. Specific measures should be considered during these stays to prevent further functional decline and, possibly, hospital readmission. Patients admitted with basic ADLs impairment might be candidate for higher levels of care (rehabilitation).

L'image que l'on associe traditionnellement au Valais ressort plutôt de ses paysages alpins ou de ses agriculteurs de montagne que de ses industries. Celles-ci forment pourtant un maillon essentiel dans l'histoire du développement du canton.Au Moyen Age déjà, la prospection du fer, du plomb et surtout de l'argent donnait lieu a de sévères concurrences. Mais c'est l'arrivée des grandes industries chimiques et métallurgiques, vers 1900, qui a provoqué la reconversion en masse de centaines de paysans en ouvriers d'usines.Dans la complexité d'un marché devenu mondial, ces industries ont dû évoluer, certaines ont disparu, d'autres sont en passe de l'être.C'est ce regard sur un Valais méconnu qu'une dizaine d'historiens, avec l'oeil complice de plusieurs photographes, proposent à votre lecture.

Excess risk of subsequent cancers has been documented in women diagnosed with ovarian cancer. We updated to 2006 data on second cancers in women diagnosed with invasive and borderline ovarian cancer in the Swiss canton of Vaud. Between 1974 and 2006, 304 borderline and 1530 invasive first ovarian tumours were abstracted from the Vaud Cancer Registry database and followed up till the end of 2006. Calculation of expected numbers of tumours in the cohorts was based on site-specific, age-specific and calendar-year-specific incidence rates. We computed the standardized incidence ratios (SIRs) of second cancers, and the corresponding 95% confidence intervals (CI). There was no change in the incidence of malignant cancers, but that of borderline tumours increased over more recent years. Overall, 110 second neoplasms were observed versus 49.7 expected after invasive ovarian cancer (SIR 2.21; 95% CI: 1.82-2.67). Significant excess risks were observed for cancers of the breast, corpus uteri and leukaemias. When synchronous cancers were excluded, the overall SIR for all sites declined to 1.05. Thirty-one second neoplasms were observed after borderline tumours compared with 21.1 expected (SIR=1.47; 95% CI: 1.00-2.09). SIRs were above unity for ovary, colorectum and uterus. After exclusion of synchronous neoplasms, SIR for all neoplasms declined to 1.09, and remained significant only for second ovarian cancers (SIR=4.93). The present record linkage cohort study shows an excess risk for selected synchronous neoplasms in women diagnosed with both borderline and invasive ovarian cancer, likely because of shared genetic and perhaps environmental factors.

OBJECTIVE: To evaluate the effects of nutrient intake and vitamin D status on markers of type I collagen formation and degradation in adolescent boys and girls. DESIGN: Cross-sectional study. SETTING: Canton of Vaud, West Switzerland. SUBJECTS: A total of 92 boys and 104 girls, aged 11-16 y. Data were collected on height, weight, pubertal status (self-assessment of Tanner stage), nutrient intake (3-day dietary record) and fasting serum concentration of 25-hydroxyvitamin D (25OHD), and markers of collagen formation (P1NP) and degradation (serum C-terminal telopeptides: S-CTX). RESULTS: Tanner stage was a significant determinant of P1NP in boys and girls and S-CTX in girls. Of the nutrients examined, only the ratio of calcium to phosphorus (Ca/P) was positively associated with P1NP in boys, after adjustment for pubertal status. 25OHD decreased significantly at each Tanner stage in boys. Overall, 15% of boys and 17% of girls were identified as being vitamin D insufficient (serum 25OHD <30 nmol/l), with the highest proportion of insufficiency at Tanner stage 4-5 (29%) in boys and at Tanner stage 3 (24%) in girls. A significant association was not found between 25OHD and either bone turnover marker, nor was 25OHD insufficiency associated with higher concentrations of the bone turnover markers. CONCLUSIONS: The marked effects of puberty on bone metabolism may have obscured any possible effects of diet and vitamin D status on markers of bone metabolism. The mechanistic basis for the positive association between dietary Ca/P ratio and P1NP in boys is not clear and may be attributable to a higher Ca intake per se, a critical balance between Ca and P intake or higher dairy product consumption. A higher incidence of vitamin D insufficiency in older adolescents may reflect a more sedentary lifestyle or increased utilisation of 25OHD, and suggests that further research is needed to define their requirements. Objective: To assess the importance of hospital transfers between Swiss cantons and the factors associated with them. Methods: Hospital discharge data, mean number of hospitals, hospital beds and corresponding density per 100.000 inhabitants for period 1998-2008 were provided by the Federal Office of Statistics. Inclusion criteria were: >=18 years; transferred from one hospital to another and living in a canton different from the one they were transferred to. Cumulative data for the study period was used. Results: Between 1998 and 2008, 247.355 hospital transfers occurred between cantons, representing 17.7% of all hospital transfers. This value ranged between 2.7% (Geneva) and 81.6% (Appenzell Innerrhoden). The main diagnoses were: circulatory system (20.1%); musculoskeletal system (9.3%); mental and behavioural disorders (6.5%); oncology (4.6%) and repertories diseases (2.6%). Factors influencing health status (28.7%) and injuries and external causes (12%) were not included in the analysis. Cantons with a university hospital received more patients than they transferred to other cantons. The other cantons were either "senders" or "receivers" according to the disease considered. German-speaking cantons transferred patients more easily than the others. Most transfers were made between geographically or linguistically close cantons. The number of patients received was associated with the number of hospitals (r = 0.82, p <0.001) or beds (r = 0.85, p <0.001)in the canton; bed density (r = 0.51, p <0.01), but not with hospital density(r = 0.08, p <0.7). Conclusions: in Switzerland, over one-sixth of hospital transfers occur between cantons. Several cantons are dependent of others for the treatment of specific diseases. Cantons with a university hospital attract more patients, suggesting an optimisation of health resources.

An analysis of perinatal mortality by hour of birth among 10,059 births in Canton Ticino (Switzerland) during the years 1979-1982 showed that fewer births occurred at night than during the day. The variations in number of births by hour of birth were attributed to obstetric practices. The perinatal mortality rate for night-time births was more than twice as high as that for the daytime births (+127%, P less than 0.001) and the rates for night-time births exceeded those for daytime births for 13 of the 19 causes of death examined. A higher proportion of the low and very-low-birthweight babies (less than 2500 g and less than 1500 g) were born at night between 19.00 and 06.59 hours.

OBJECTIVE: To assess whether problematic internet use is associated with somatic complaints and whether this association remains when checking for internet activity among a random sample of adolescents living in the canton of Vaud, Switzerland. METHODS: Cross-sectional survey of 3,067 8th graders (50.3% females) divided into average (n = 2,708) and problematic (n = 359) Internet users and compared for somatic complaints (backache, overweight, headaches, musculoskeletal pain, sleep problems and sight problems) controlling for sociodemographic and internet-related variables. Logistic regressions were performed for each complaint and for all of them simultaneously controlling variables significant at the bivariate level. RESULTS: At the multivariate level, when taken separately, problematic internet users were more likely to have a chronic condition (adjusted odds ratio [aOR] with 95% CI: 1.58 [1.11:2.23]) and to report back pain (aOR: 1.46 [1.04:2.05]), overweight (aOR: 1.74 [1.03:2.93]), musculoskeletal pain (aOR: 1.36 [1.00:1.84]) and sleep problems (aOR: 2.16 [1.62:2.88]). When considered in the full model, only sleep problems remained significant (aOR: 2.03 [1.50:2.74]). CONCLUSIONS: Our results confirm that problematic internet users report health problems more frequently, with lack of sleep being the most strongly associated and seeming to act as mediator regarding the other ones. Clinicians should remember to screen for excessive internet use their patients complaining of sleep-related problems, back or musculoskeletal pain or overweight. Clinicians should advise parents to limit the amount of time their adolescent children can spend online for leisure activities. Furthermore, limiting the number of devices used to connect to the internet could help warrant enough sleeping time.

Sustainable resource use is one of the most important environmental issues of our times. It is closely related to discussions on the 'peaking' of various natural resources serving as energy sources, agricultural nutrients, or metals indispensable in high-technology applications. Although the peaking theory remains controversial, it is commonly recognized that a more sustainable use of resources would alleviate negative environmental impacts related to resource use. In this thesis, sustainable resource use is analysed from a practical standpoint, through several different case studies. Four of these case studies relate to resource metabolism in the Canton of Geneva in Switzerland: the aim was to model the evolution of chosen resource stocks and flows in the coming decades. The studied resources were copper (a bulk metal), phosphorus (a vital agricultural nutrient), and wood (a renewable resource). In addition, the case of lithium (a critical metal) was analysed briefly in a qualitative manner and in an electric mobility perspective. In addition to the Geneva case studies, this thesis includes a case study on the sustainability of space life support systems. Space life support systems are systems whose aim is to provide the crew of a spacecraft with the necessary metabolic consumables over the course of a mission. Sustainability was again analysed from a resource use perspective. In this case study, the functioning of two different types of life support systems, ARES and BIORAT, were evaluated and compared; these systems represent, respectively, physico-chemical and biological life support systems. Space life support systems could in fact be used as a kind of 'laboratory of sustainability' given that they represent closed and relatively simple systems compared to complex and open terrestrial systems such as the Canton of Geneva. The chosen analysis method used in the Geneva case studies was dynamic material flow analysis: dynamic material flow models were constructed for the resources copper, phosphorus, and wood. Besides a baseline scenario, various alternative scenarios (notably involving increased recycling) were also examined. In the case of space life support systems, the methodology of material flow analysis was also employed, but as the data available on the dynamic behaviour of the systems was insufficient, only static simulations could be performed. The results of the case studies in the Canton of Geneva show the following: were resource use to follow population growth, resource consumption would be multiplied by nearly 1.2 by 2030 and by 1.5 by 2080. A complete transition to electric mobility would be expected to only slightly (+5%) increase the copper consumption per capita while the lithium demand in cars would increase 350 fold. For example, phosphorus imports could be decreased by recycling sewage sludge or human urine; however, the health and environmental impacts of these options have yet to be studied. Increasing the wood production in the Canton would not significantly decrease the dependence on wood imports as the Canton's production represents only 5% of total consumption. In the comparison of space life support systems ARES and BIORAT, BIORAT outperforms ARES in resource use but not in energy use. However, as the systems are dimensioned very differently, it remains questionable whether they can be compared outright. In conclusion, the use of dynamic material flow analysis can provide useful information for policy makers and strategic decision-making; however, uncertainty in reference data greatly influences the precision of the results. Space life support systems constitute an extreme case of resource-using systems; nevertheless, it is not clear how their example could be of immediate use to terrestrial systems.

Contexte: L'évolution démographique est un facteur essentiel pour la planification hospitalière. Le scenario démographique moyen de l'OFS ne répond que partiellement aux exigences de la planification pour le canton du Valais ; il ne permet pas de faire des projections par région hospitalière. De plus, la croissance en 2010 et 2011 selon le scenario OFS est inferieure a celle observée. Nous avons développe un scenario ajuste en tenant compte de l'évolution récente de la population valaisanne, et ce par région hospitalière. Méthode: La population de départ et les taux d'accroissement du au solde naturel et migratoire 2012 ont été déterminés à partir des données réelles 2011. Entre 2002 et 2011, le taux d'accroissement migratoire a fortement varie. Les taux 2011 ont donc été ajustés en fonction des taux moyens d'accroissement migratoire 2002-2011 calcules pour chaque région hospitalière. Pour les années 2013-2025, les écarts de taux d'accroissement naturel et migratoire du scenario OFS ont été repris tel quel. Résultats: Entre 2000 et 2010, la population valaisanne est passée de 276'170 a 312'684 habitants (+13.2%). Entre 2012 et 2025, les deux scenarios prévoient une poursuite de la croissance de la population, mais de manière moins soutenue, particulièrement selon le scenario OFS. Ainsi, en 2020, la population devrait atteindre 348'783 habitants (+11.5%) selon le scenario ajuste et 330'616 (+5.7%) selon le scenario OFS. La part de la population âgée de 65 ans et plus passera de 17.1% en 2010 a 20.5% en 2020 selon le scenario ajuste et a 21.4% selon le scenario OFS. Conclusion: Au vu des limites du scenario moyen de l'OFS, il est pertinent de développer des alternatives tenant compte au mieux des spécificités démographiques cantonales. Le scenario ajuste prédit une croissance plus forte de la population, ce qui a un impact important sur les besoins en soins hospitaliers et la planification hospitalière.

Over the last few years, most OECD countries have extended their activation policy to new groups of non-working people, including the long-term unemployed (LTU). However, it is widely known that employers tend to regard LTU people as potentially problematic persons. This is likely to constitute a major obstacle for long-term unemployed jobseekers. On the basis of a survey among employers in a Swiss canton (N = 722), this article aims to shed light on the perception employers have of the long-term unemployed and whether this may matter for their recruitment practices. It also asks what, from the employer point of view, may facilitate access to employment for an LTU person. A key finding is that large companies have a worse image of the long-term unemployed and are less likely to hire them. Furthermore, independent of company size, a test period or the recommendation of a trustworthy person is seen as the factors most likely to facilitate access to jobs for LTU people.

A la veille de l'introduction de la réforme pénale et civile fédérale dans le canton de Vaud en 2011, la Revue historique vaudoise s'est intéressée à l'histoire de la justice et de la criminalité. Le présent numéro réunit des contributions d'historiens, historiens du droit et historiens de l'art, jeunes ou confirmés, qui livrent ici les résultats de leurs recherches en cours ou récentes, ainsi que des témoignages de magistrats sur l'exercice de la justice et sur les conséquences des changements à venir. Pour cette thématique qui est un champ dynamique de la recherche historiques en Suisse et en Europe depuis la fin des années 1970, deux axes principaux ont été privilégies pour interroger l'exercice de la justice et les interactions entre l'Etat et les justiciables. Le premier traite de la figure du criminel et de la représentation du crime par un examen des pratiques et des discours judiciaires. Le second se concentre sur les acteurs de la justice et sur leurs modes d'actions et leurs motivations. En privilégiant un traitement sur la longue durée, du Moyen Âge à nos jours, ainsi que des études de cas, ce dossier thématique propose des éclairages successifs sur la justice et la criminalité en terres vaudoises. Ils mettent en évidence des transitions, des ruptures et des innovations révélatrices des institutions, de leur fonctionnement et du rôle social de leurs agents

Purpose: To assess the relation between cannabis and tobacco consumption among adolescents in Switzerland and whether cannabis and tobacco co-users can quit cigarette smoking. Methods: Based on individual interviews and focus groups, 22 youths aged 15-20 discussed cannabis consumption behaviours. Twenty (14 males) were cannabis consumers - of which 18 also smoked tobacco and 2 quit tobacco smoking - and 2 were former cannabis consumers (both females and daily smokers). Data were transcribed verbatim and analyzed using Atlas.ti qualitative analysis software. Results: Among the co-consumers, 9 started with tobacco, 7 with cannabis, and 2 with both. The main consumption mode among all cannabis consumers is joints, while other ways of consuming such as food preparations and water pipes are rare and experimental. Joints always mix cannabis with tobacco for 3 reasons: to burn correctly, pure cannabis is too strong, and smoking cannabis alone is too expensive. Two cannabis consumers - one former tobacco smoker and one occasional tobacco smoker - consider rolling tobacco less addictive than cigarette tobacco alone, and hence use it in their joints. Overall cannabis is considered 'natural' and less harmful to health than tobacco. Thus, many users describe their wish, in the longer term, to quit tobacco consumption without excluding occasional cannabis consumption. Nonetheless, all coconsumers declare that they smoke cigarettes as a substitute for cannabis: For example, "If I don't have a joint, I need fags; if I don't have fags, I need joints; and if I don't have anything, I go crazy!" or "About 20 minutes after smoking a joint we feel like smoking something again, because in the joint there is pure tobacco without a filter as in cigarettes, and that creates a crazy dependency!". Finally, all co-consumers state that the consumption of one of the substances increases when trying to diminish the other: "A few months ago I stopped smoking joints for a month. Well I was smoking more than a pack [of cigarettes] a day." Similarly, the 2 former cannabis consumers increased their cigarette use since quitting cannabis. Conclusions: The majority of cannabis users co-consume tobacco as a way of compensating for one substance or the other. Using tobacco within joints implies that there is a risk that even occasional joints can revive nicotine addiction. Consequently, health professionals wishing to help adolescents in substance use cessation and prevention efforts should consider both substances in a global perspective. Während im Kanton Freiburg durchschnittlich 2'500 Lottos pro Jahr veranstaltet werden, sind es im Kanton Zürich gerade einmal 150. Der Kanton Schaffhausen kennt keine gesetzlichen Bestimmungen zu Kleinlotterien, Tombolas oder Lottos, der Kanton Waadt dagegen regelt sogar die Farbe der Lottokarten. Obwohl der Bund umfassende Gesetzgebungskompetenz im Bereich der Glücksspiele innehat, trifft er im Lotteriegesetz keine abschliessenden Regelungen für gemeinnützige und wohltätige Lotterien. Für Unterhaltungslotterien sieht er sogar einen absoluten Vorbehalt des kantonalen Rechts vor. Die Folge davon ist eine fast unüberschaubare Vielzahl von Regelungen, die sich von Kanton zu Kanton unterscheiden. Eine syste¬matische Über¬sicht fehlt bis anhin ebenso wie eine Gesamterhebung der in den letzten Jahren durchgeführten Tombolas und Lottos. Die vorliegende Arbeit will diese Lücke schliessen. Sie untersucht, welche gesetzlichen Regelungen es für Kleinlotterien, Tombolas und Lottos in den Kantonen gibt und wie oft diese veranstaltet werden. Die dabei umgesetzten Geld¬summen werden miteinander verglichen. Alors qu'en moyenne, 2500 lotos sont organisés chaque année dans le canton de Fribourg, seuls 150 le sont sur une même période dans le canton de Zürich. Le canton de Schaffhouse ne dispose pas de loi concernant les petites loteries, les tombolas ou les lotos, alors que le canton de Vaud va jusqu'à définir la couleur des cartons pour les lotos. On constate que même si la Confédération possède une compétence législative globale en matière de jeux de hasard, elle n'a pas légiféré exhaustivement la matière des loteries servant à des fins d'utilité publique ou de bienfaisance. De plus, la compétence législative en matière de loterie organisée à l'occasion d'une réunion récréative est déléguée au niveau cantonal. Par conséquent, on observe une multitude presque confuse de différentes régulations cantonales. Actuellement, il est impossible d'avoir une vue d'ensemble systématique ainsi qu'une statistique globale contenant des données des tombolas et lotos organisés dans les différents cantons. Ce travail vise à combler cette lacune en analysant quelques régulations cantonales pour les petites loteries, tombolas et lotos, en comparant le nombre de fois que ces loteries sont organisées ainsi que les sommes dépensées pour ces mêmes loteries.
