Ce texte est une prise de position claire face à la pluralité des pratiques psychothérapeutiques : se référant à l'anthropologie clinique, présentée ici selon ses trois visées (pratique, épistémologique et herméneutique), l'auteur rejette les positions extrêmes de l'éclectisme et du dogmatisme liée à la pensée d'école. Chaque thérapeute est invité à se situer dans le champ de la psychothérapie en explicitant sa "philosophie", c'est-à-dire les croyances qui animent ses gestes professionnels.

Introduction :¦Le syndrome de Brugada (SB) se définit par une élévation du segment ST dans les dérivations précordiales droites (V1-V3) associée à une incidence élevée de mort subite par arythmies chez des patients sans cardiopathie structurelle. La prévention de la mort subite repose sur la pose d'un défibrillateur automatique implantable (DAI).¦Matériel et méthode :¦Cette étude est une analyse rétrospective de tous les patients avec SB implantés d'un DAI au CHUV entre 1998 et 2010. Le profil des patients, les complications, ainsi que les thérapies appropriées et inappropriées délivrées par le défibrillateur ont été évaluées. Une thérapie appropriée est définie par l'action appropriée du DAI (choc ou stimulation anti-tachycardie = ATP) en réponse à une TV ou une FV correctement détectée. La thérapie inappropriée est définie comme une thérapie délivrée par l'appareil en l'absence d'une arythmie ventriculaire.¦Résultats :¦Durant la période étudiée (01.01.1998 - 31.12.2010) nous avons collecté au total 23 patients soit 19 hommes (83%) et 4 femmes (17%) âgés de 12 à 60 ans (41 13 ans). Le rapport hommes/femmes est de 4.75. Le suivi quant à lui porte sur 21 patients: un patient, après échec de primo-implantation au CHUV, a été implanté d'un défibrillateur dans un autre canton et un second patient a été perdu de vue entre l'implantation et le premier contrôle prévu. Le suivi moyen des 21 patients restants est de 8432 mois (37-149 mois, médiane : 78 mois). Il y a eu au total 45 thérapies délivrées : 12 appropriées (8 chocs; 4 ATP) et 33 inappropriées. 18% des patients ont eu ≥ 1 thérapie appropriée alors que 43% ont eu ≥ 1 choc inapproprié. 14 des 23 patients (61%) ont eu ≥ 1 complication (9 avec chocs inappropriés, 7 avec complications opératoires après primo-implantations ou ré-interventions, 6 avec dysfonction de sonde ventriculaire et 1 avec explantation).¦Conclusion :¦Le profil de ce petit collectif de patients avec SB implantés d'un DAI au CHUV est comparable à celui décrit dans la littérature. Durant un suivi moyen de 7 ans après la primo-implantation, l'incidence d'arythmies ventriculaires malignes est faible, avec un taux annuel de 2.6%. Le taux de complication est quant à lui élevé (8.7%/an). Près des trois-quarts des thérapies délivrées par le DAI sont inappropriées et les patients avec thérapies inappropriées sont 2.4x plus fréquents que les patients avec thérapies appropriées.

RESUME L'angiogénèse tumorale est un processus essentiel au développement des tumeurs. Les intégrines, molécules d'adhésions transmembranaires, sont d'importants effecteurs de l'angiogenèse. En permettant l'adhésion à la matrice extra-cellulaire, les intégrines transmettant des signaux de survie, de migration, et de prolifération. Le facteur de nécrose tumorale α (TNFα) est utilisé pour le traitement régional de cancers chez l'homme. II agit en détruisant sélectivement les vaisseaux angiogéniques. Cependant, son administration systémique chez l'homme est limitée par les réactions de vaso-dilatation sévères qu'il provoque. Le but de mon travail fut de rechercher des conditions permettant la sensibilisation des cellules endothéliales au TNFα et qui pourraient être applicables en clinique, ceci afin d'accroître l'efficacité de cette molécule. Nous avons testé la possibilité d'interférer avec les signaux de survie provenant des intégrines. Pour cela, des cellules endothéliales furent cultivées dans des conditions d'adhésion ou en suspension, ou alors exposées dans des conditions d'adhésion au zoledronate (biphosphonate contenant du nitrogène). Dans ces conditions, les effets du TNFα sur les cellules endothéliales furent étudiés, en particulier l'induction de la mort cellulaire. Dans ce travail, nous montrons que le zoledronate sensibilise les cellules endothéliales à la nécrose induite par TNFα. Cet effet s'accompagne de l'inhibition de la phosphorylation de FAK, PKB, et JNK, ainsi que de l'inhibition de la prénylation des protéines. En revanche, l'activation de NF-kB et p38 n'est pas perturbée. La restoration de la prénylation des protéines empêche la mort des HUVEC traitées par zoledronate et TNFα, et rétablit la phosphorylation de FAK, PKB, et JNK. Des essais d'angiogénèse in vivo montrent que le zoledronate inhibe l'angiogénèse induite par FGF-2. Le zoledronate encapsulé dans des liposomes permet de ralentir la croissance tumorale et synergise avec le TNFα en l'inhibant. L'inihibtion de la prénylation des protéines est un des mécanismes de sensibilisation du zoledronate au TNFα. In vivo, la synergie de leur association sur la croissance tumorale est efficace. Ces résultats encouragent la poursuite de l'étude des effets de ces deux drogues sur la croissance tumorale. SUMMARY The formation of tumor-associated vessels is essential for tumor progression. Cell adhesion molecules of the integrin family are important mediators of angiogenesis, by providing adhesive signals necessary for endothelial cell migration, proliferation and survival. Anti-angiogenic therapies are currently considered as highly promising in the treatment of human cancer. Tumor Necrosis Factor α (TNFα) is used for the regional treatment of human cancer, whose mechanisms of action involved selective disruption of angiogenic tumor vessels. Systemic administration of TNFα in humans, however, induces a severe inflammatory condition that prevents its use far the treatments of tumors localized outside of limbs. The aim of my work was to find strategies to sensitize angiogenic endothelial cells to TNFα-induced death, which could be potentially translated into clinical setting to improve the therapeutic efficacy of TNFα. We specifically tested the hypothesis whether interference with integrin-mediated adhesion and signaling may sensitize endothelial cells to TNFα-induced death. To test this hypothesis we cultured endothelial cells (EC) under conditions of cell-matrix or cell-cell adhesion or exposed matrix-adherent EC to the nitrogen-containing bisphosphonate zoledronate, and characterized the effect on TNFα-mediated signaling events and cell death. We show that zoledronate sensitizes HUVEC to TNFα-induced necrosis-like programmed cell death. This effect was associated with suppression of sustained phosphorylation of PKB and JNK and decreased protein prenylation, whereas TNFα-induced activation of NF-kB and p38 were not inhibited. Restoration of protein prenylation rescued HUVEC from zoledronate and TNFα-induced death, and restored FAK, PKB and JNK phosphorylation. By using in vivo angiogenesis assay we showed that zoledronate suppressed FGF-2-induced angiogenesis. Liposome-encapulated zoledronate partially inhibited tumor growth and synergized with TNFα to fully suppress tumor growth. Taken together, this work has identified protein prenylation as a mechanisms by which zoledronate sensitizes endothelial cells to TNFα-induced death in vitro and provides initial evidence that zoledronate synergizes with TNFα in vivo resulting in improved anti-tumor activity. These results warrant further study of the anti-tumor effects of zoledronate and TNFα and should be further studies in view of their clinical relevance.

Neuroblastoma (NB) is a typical childhood and heterogeneous neoplasm for which efficient targeted therapies for high-risk tumors are not yet identified. The chemokine CXCL12, and its receptors CXCR4 and CXCR7 have been involved in tumor progression and dissemination. While CXCR4 expression is associated to undifferentiated tumors and poor prognosis, the role of CXCR7, the recently identified second CXCL12 receptor, has not yet been elucidated in NB. In this report, CXCR7 and CXCL12 expressions were evaluated using a tissue micro-array including 156 primary and 56 metastatic NB tissues. CXCL12 was found to be highly associated to NB vascular and stromal structures. In contrast to CXCR4, CXCR7 expression was low in undifferentiated tumors, while its expression was stronger in matured tissues and specifically associated to differentiated neural tumor cells. As determined by RT-PCR, CXCR7 expression was mainly detected in N-and S-type NB cell lines, and was slightly induced upon NB cell differentiation in vitro. The relative roles of the two CXCL12 receptors were further assessed by overexpressing CXCR7 or CXCR4 receptor alone, or in combination, in the IGR-NB8 and the SH-SY5Y NB cell lines. In vitro functional analyses indicated that, in response to their common ligand, both receptors induced activation of ERK1/2 cascade, but not Akt pathway. CXCR7 strongly reduced in vitro growth, in contrast to CXCR4, and impaired CXCR4/CXCL12-mediated chemotaxis. Subcutaneous implantation of CXCR7-expressing NB cells showed that CXCR7 also significantly reduced in vivo growth. Moreover, CXCR7 affected CXCR4-mediated orthotopic growth in a CXCL12-producing environment. In such model, CXCR7, in association with CXCR4, did not induce NB cell metastatic dissemination. In conclusion, the CXCR7 and CXCR4 receptors revealed specific expression patterns and distinct functional roles in NB. Our data suggest that CXCR7 elicits anti-tumorigenic functions, and may act as a regulator of CXCR4/CXCL12-mediated signaling in NB.

Diabetic nephropathy is the first cause of endstage renal disease. The demographic expansion, the increase in the incidence of diabetes and the prolonged survival rates explain the steep increase observed these last 30 years. In the United States, improved treatment has brought to a decline in the incidence of end-stage renal disease in the diabetic population since the mid nineties. We examined the change in prevalence of diabetics on dialysis from 2001 and 2009 in the Canton de Vaud, Switzerland. The prevalence of diabetics on dialysis increased from 18% to 31% in dialysis centers and increased from 1.1/1000 to 1.9/1000 in the diabetic population. These are strong indicators that efforts are needed to improve the renal outcome of patients with diabetic nephropathy.

From practice to research in a couple and family therapy consultation - This article presents a study that took place at a Couples and Family outpatient clinic. Multiple levels are involved in couples and family therapy. The definition of relevant variables allowing the evaluation of a systemic therapeutic intervention's efficiency is thus particularly difficult. The aim of our study is to assess the short- and medium-term evolution on four different levels: (1) individual symptomatology; (2) conjugal satisfaction; (3) parental and co-parental relationship quality, and (4) global family functioning for a brief systemic intervention (BSI). BSI consists in a therapeutic treatment of a maximum of six sessions. The different levels are assessed by validated self-reported questionnaires before and after BSI as well as at a three-month follow-up. The therapeutic alliance is also measured by a self-reported questionnaire (WAI) after each session. The results of a pilot sample of N = 10 couples and families showed the efficiency of BSI on most of the variables measured and a greater therapeutic change for women than for men. No therapeutic effect was observed for men's individual symptomatology or for global family functioning. These results suggest that BSI has a different impact on the four levels measured. The importance of the therapeutic alliance on the therapeutic success is also confirmed.

The non-invasive evaluation of myocardial ischemia is a priority in cardiology. The preferred initial non-invasive test is exercise ECG, because of its high accessibility and its low cost. Stress radionuclide myocardial perfusion imaging or stress echocardiography are now routinely performed, and new non-invasive techniques such as perfusion-MRI, dobutamine stress-MRI or 82rubidium perfusion PET have recently gained acceptance in clinical practice. In the same time, an increasing attention has been accorded to the concept of myocardial viability in the decisional processes in case of ischemic heart failure. In this indication, MRI with late enhancement after intravenous injection of gadolinium and 18F-FDG PET showed an excellent diagnostic accuracy. This article will present these new imaging modalities and their accepted indications.

A cutaneous horn was observed close to the free margin of the inferior right eyelid in a 26-year-old-male patient. A minimal resection was primarily performed. Histopathologic study disclosed a precancerous keratosis. As the tumor had not been entirely excised, a complementary resection was performed secondarily to obtain the entire resection of the tumor.

OBJECTIVE: Best long-term practice in primary HIV-1 infection (PHI) remains unknown for the individual. A risk-based scoring system associated with surrogate markers of HIV-1 disease progression could be helpful to stratify patients with PHI at highest risk for HIV-1 disease progression. METHODS: We prospectively enrolled 290 individuals with well-documented PHI in the Zurich Primary HIV-1 Infection Study, an open-label, non-randomized, observational, single-center study. Patients could choose to undergo early antiretroviral treatment (eART) and stop it after one year of undetectable viremia, to go on with treatment indefinitely, or to defer treatment. For each patient we calculated an a priori defined "Acute Retroviral Syndrome Severity Score" (ARSSS), consisting of clinical and basic laboratory variables, ranging from zero to ten points. We used linear regression models to assess the association between ARSSS and log baseline viral load (VL), baseline CD4+ cell count, and log viral setpoint (sVL) (i.e. VL measured ≥90 days after infection or treatment interruption). RESULTS: Mean ARSSS was 2.89. CD4+ cell count at baseline was negatively correlated with ARSSS (p = 0.03, n = 289), whereas HIV-RNA levels at baseline showed a strong positive correlation with ARSSS (p<0.001, n = 290). In the regression models, a 1-point increase in the score corresponded to a 0.10 log increase in baseline VL and a CD4+cell count decline of 12/µl, respectively. In patients with PHI and not undergoing eART, higher ARSSS were significantly associated with higher sVL (p = 0.029, n = 64). In contrast, in patients undergoing eART with subsequent structured treatment interruption, no correlation was found between sVL and ARSSS (p = 0.28, n = 40). CONCLUSION: The ARSSS is a simple clinical score that correlates with the best-validated surrogate markers of HIV-1 disease progression. In regions where ART is not universally available and eART is not standard this score may help identifying patients who will profit the most from early antiretroviral therapy.

The goal of this paper is to provide clinicians and researchers, who may not be experts in psychometrics, with a guide for the selection and adaptation of an instrument for clinical research. Issues related to the concept to be measured, the targeted clientele, the selection criteria for the instrument (algorithm), the strategies for translation and adaptation, as well as potential bias related to the administration of an instrument are reviewed and discussed.
