BACKGROUND AND PURPOSE: We compared among young patients with ischemic stroke the distribution of vascular risk factors among sex, age groups, and 3 distinct geographic regions in Europe. METHODS: We included patients with first-ever ischemic stroke aged 15 to 49 years from existing hospital- or population-based prospective or consecutive young stroke registries involving 15 cities in 12 countries. Geographic regions were defined as northern (Finland, Norway), central (Austria, Belgium, France, Germany, Hungary, The Netherlands, Switzerland), and southern (Greece, Italy, Turkey) Europe. Hierarchical regression models were used for comparisons. RESULTS: In the study cohort (n=3944), the 3 most frequent risk factors were current smoking (48.7%), dyslipidemia (45.8%), and hypertension (35.9%). Compared with central (n=1868; median age, 43 years) and northern (n=1330; median age, 44 years) European patients, southern Europeans (n=746; median age, 41 years) were younger. No sex difference emerged between the regions, male:female ratio being 0.7 in those aged <34 years and reaching 1.7 in those aged 45 to 49 years. After accounting for confounders, no risk-factor differences emerged at the region level. Compared with females, males were older and they more frequently had dyslipidemia or coronary heart disease, or were smokers, irrespective of region. In both sexes, prevalence of family history of stroke, dyslipidemia, smoking, hypertension, diabetes mellitus, coronary heart disease, peripheral arterial disease, and atrial fibrillation positively correlated with age across all regions. CONCLUSIONS: Primary preventive strategies for ischemic stroke in young adults-having high rate of modifiable risk factors-should be targeted according to sex and age at continental level.

Résumé Le mammifère adulte possède des capacités de régénération tissulaire beaucoup plus limitées que celles des mammifères à l'âge foetal, ou d'autres vertébrés adultes comme les amphibiens urodèles et anuriens. Le mode de réparation tissulaire généralement utilisé par le mammifère adulte est la cicatrisation. Celle-ci suit un déroulement physio-pathologique très reproductible, qui a été le mieux décrit dans la peau, mais est également applicable à d'autres tissus comme le coeur en cas d'infarctus. Toutefois, le coeur de mammifère adulte semble posséder un certain potentiel régénérateur, bien qu'insuffisant pour réparer une lésion d'infarctus; en particulier, il contient des populations de cellules exprimant des marqueurs de surface des cellules souches hématopoiétiques comme l'antigène de cellules souches (stem cell antigen; Sca-1) ou le récepteur pour le facteur de cellules souches (stem cell factor; SCF), c-kit. Le comportement de ces cellules ressemble à de nombreux égards à celui de cellules souches adultes résidentes. D'autre part, un modèle mammifère adulte de régénération tissulaire, la souris NIRL, a été décrit ,récemment ; si cette souris répare. l'infarctus ischémique du ventricule gauche par cicatrisation, elle est par contre capable de régénérer complètement le myocarde après cryoinfarctus du ventricule droit, sans former la moindre cicatrice. Le but de cette thèse a été l'exploration par différentes approches des potentiels régénérateurs cardiaques après infarctus chez le mammifère adulte. La première approche choisie a été l'étude de la régénération myocardique chez la souris MRL. Il s'agissait de comprendre pourquoi la souris MRL régénère le coeur après cryoinfarctus du ventricule droit, et pas après infarctus ischémique du ventricule gauche, ainsi que d'élucider les mécanismes à la base de la régénération cardiaque chez cette souris. En utilisant le protocole original d'infarctus cryogénique du ventricule droit, nous n'avons pas observé de régénération cardiaque chez la souris MRL, qui a réparé l'infarctus par cicatrisation.- Nous avons ensuite modifié la sévérité du stimulus cryogénique, la localisation de la lésion cardiaque, et le type de lésion lui-même (infarctus ischémique induit par ligature coronarienne). En théorie, ces aspects expérimentaux sont les principaux facteurs pouvant influencer la réparation tissulaire. En utilisant cinq protocoles expérimentaux différents, nous n'avons pas observé de régénération cardiaque chez la souris MRL. Nous avons également analysé la prolifération cellulaire dans trois régions différentes du coeur à 15 et 40 jours après infarctus, et n'avons pas observé de différence entre la souris MRL et la souris contrôle C57B1/6. Quant à la composition en collagène de la cicatrice, elle est la même chez les deux souches de souris. Nos résultats ne peuvent donc pas confirmer la validité de ce modèle marin de régénération cardiaque récemment publié. Nous nous sommes alors tournés vers une deuxième approche d'étude du potentiel régénérateur du coeur de mammifère adulte, celle des cellules souches adultes résidentes. Nous avons isolé et purifié la population de cellules cardiaques qui expriment le marqueur de surface Sca-1 ;nous les avons maintenues en cultures pendant plusieurs dizaines de passages, et les avons ré-injectées dans le myocarde. Cette deuxième approche .ouvre la voie à l'étude de cellules souches cardiaques adultes candidates, ainsi qu'à la thérapie cellulaire de l'infarctus du myocarde. Summary Adult mammals possess limited tissue regeneration capacities as compared to foetal mammals or other adult vertebrates such as anurian and urodele amphibians. Usually, adult mammals heal tissues by scarring. The process of scarring is characterized by physiopathological events which have been best studied in skin; but which also occur in other organs like the heart. Nevertheless, the adult mammalian heart seems to possess a certain regenerative potential, though insufficient to efficiently repair infarct lesions. It indeed contains cell populations expressing haematopoietic stem cell surface markers such as Scat or c-kit. These cells behave in many ways like resident adult. stem cells. On the other hand; an adult mammalian model of tissue regeneration, the MRL mouse, has been recently described; although this mouse repairs an ischemic infarct of the left ventricle by scarring, it is able of fully regenerating a cryoinfarction of the right ventricle without scanning . The goal of this thesis was to explore the regenerative potential of the adult mammalian heart after infarction by using different approaches. A first approach was to study the myocardial regeneration in the MRL mouse. It was about understanding why this mouse regenerates a right ventricular cryoinfarction and not an ischemic infarction of the left ventricle, as well as elucidating the mechanisms underlying myocardial regeneration in this model. By using the original protocol of right ventricular cryoinfarction, we did not observe any heart regeneration in the MRL mouse, which healed the infarct by scarring. We then modified the intensity of the cryogenic stimulus, the site of lesion, and -the type of lesion itself (ischemic infarction by coronary artery ligation). In theory, these experimental aspects are the main factors likely to influence tissue repair. Although. we used five different protocols, we did not observe any regeneration in the MRL mouse. We also analysed cell proliferation in three different regions of the heart, at 15 and 40 days after infarction, and did not see any difference between the MRL and C57B1/6 mouse. Collagen content of the scar was shown to be the same in both strains. Our results cannot confirm the validity of this recently published model. We then chose another way to study the adult mammalian heart regenerative potential, by taking the adult resident stem cells approach. We isolated and purified a cardiac cell population expressing the Sca-1 surface marker; we kept these cells in culture for over 30 passages, and re-injected them into the myocardium. This second approach opens the way to candidate adult cardiac stem cell study, as well as cell therapy.

In response to chronic stress the heart undergoes an adverse remodeling process associated with cardiomyocyte hypertrophy, increased cellular apoptosis and fibrosis, which ultimately causes cardiac dysfunction and heart failure. Increasing evidence suggest the role of scaffolding and anchoring proteins in coordinating different signaling pathways that mediate the hypertrophic response of the heart. In this context, the family of Α-kinase anchoring proteins (AKAPs) emerged as important regulators of the cardiac function. During my thesis work I have conducted two independent projects, both of them aiming at elucidating the role of AKAPs in the heart. It has been shown that AKAP-Lbc, an anchoring protein that possesses an intrinsic Rho- specific exchange factor activity, organizes a signaling complex that links AKAP-Lbc- dependent activation of RhoA with the mitogen activated protein kinase (MAPK) p38. The first aim of my thesis was to study the role of this novel transduction pathway in the context of cardiac hypertrophy. Here we show that transgenic mice overexpressing in cardiomyocytes a competitor fragment of AKAP-Lbc, which specifically disrupts endogenous AKAP-Lbc / p38 complexes, developed early dilated cardiomyopathy in response to two weeks of transverse aortic constriction (TAC) as compared to controls. Interestingly, inhibition of the AKAP-Lbc / p38 transduction pathway significantly reduced the hypertrophic growth of single cardiomyocytes induced by pressure overload. Therefore, it appears that the AKAP- Lbc / p38 complex is crucially involved in the regulation of stress-induced cardiomyocyte hypertrophy and that disruption of this signaling pathway is detrimental for the heart under conditions of sustained hemodynamic stress. Secondly, in order to identify new AKAPs involved in the regulation of cardiac function, we followed a proteomic approach which allowed us to characterize AKAP2 as a major AKAP in the heart. Importantly, here we show that AKAP2 interacts with several proteins known to be involved in the control of gene transcription, such as the nuclear receptor coactivator 3 (NCoA3) or the ATP-dependent SWI/SNF chromatin remodeling complex. Thus, we propose AKAP2 as a novel mediator of cardiac gene expression through its interaction with these transcriptional regulators.

Idiopathic premature ventricular complexes originating from the ventricular outflow tract: evaluation, prognosis and management The prognosis of ventricular premature complexes (VPC) in the absence of heart disease is considered benign. VPC usually originate from the right or, less commonly, left ventricular outflow tract. QRS complexes therefore usually assume a left bundle branch block and inferior axis morphology. These VPC, particularly if very frequent (> 20,000 per day), may adversely affect left ventricular function and their suppression can restore normal function. Moreover, there is a clinical overlap with arrhythmogenic right ventricular dysplasia and this diagnosis should be considered when facing a left bundle branch block shaped VPC. However, the prognosis of outflow tract VPC is good for appropriately selected patients with normal left ventricular function, absence of syncope or ventricular tachycardia, and no evidence of cardiac disease.

BACKGROUND: The race- and sex-specific epidemiology of incident heart failure (HF) among a contemporary elderly cohort are not well described. METHODS: We studied 2934 participants without HF enrolled in the Health, Aging, and Body Composition Study (mean [SD] age, 73.6 [2.9] years; 47.9% men; 58.6% white; and 41.4% black) and assessed the incidence of HF, population-attributable risk (PAR) of independent risk factors for HF, and outcomes of incident HF. RESULTS: During a median follow-up of 7.1 years, 258 participants (8.8%) developed HF (13.6 cases per 1000 person-years; 95% confidence interval, 12.1-15.4). Men and black participants were more likely to develop HF. No significant sex-based differences were observed in risk factors. Coronary heart disease (PAR, 23.9% for white participants and 29.5% for black participants) and uncontrolled blood pressure (PAR, 21.3% for white participants and 30.1% for black participants) carried the highest PAR in both races. Among black participants, 6 of 8 risk factors assessed (smoking, increased heart rate, coronary heart disease, left ventricular hypertrophy, uncontrolled blood pressure, and reduced glomerular filtration rate) had more than 5% higher PAR compared with that among white participants, leading to a higher overall proportion of HF attributable to modifiable risk factors in black participants vs white participants (67.8% vs 48.9%). Participants who developed HF had higher annual mortality (18.0% vs 2.7%). No racial difference in survival after HF was noted; however, rehospitalization rates were higher among black participants (62.1 vs 30.3 hospitalizations per 100 person-years, P < .001). CONCLUSIONS: Incident HF is common in older persons; a large proportion of HF risk is attributed to modifiable risk factors. Racial differences in risk factors for HF and in hospitalization rates after HF need to be considered in prevention and treatment efforts.

Heart transplantation (HTx) started in 1987 at two university hospitals (CHUV, HUG) in the western part of Switzerland, with 223 HTx performed at the CHUV until December 2010. Between 1987 and 2003, 106 HTx were realized at the HUG resulting in a total of 329 HTx in the western part of Switzerland. After the relocation of organ transplantation activity in the western part of Switzerland in 2003, the surgical part and the early postoperative care of HTx remained limited to the CHUV. However, every other HTx activity are pursued at the two university hospitals (CHUV, HUG). This article summarizes the actual protocols for selection and pre-transplant follow-up of HTx candidates in the western part of Switzerland, permitting a uniform structure of pretransplant follow-up in the western part of Switzerland.

The pathophysiological role of an increase in circulating vasopressin in sustaining global and regional vasoconstriction in patients with congestive heart failure has not been established, particularly in patients with hyponatraemia. To assess this further, 20 patients with congestive heart failure refractory to digoxin and diuretics were studied before and 60 minutes after the intravenous injection (5 micrograms/kg) of the vascular antagonist of vasopressin [1(beta-mercapto-beta,beta-cyclopentamethylene-propionic acid), 2-(0-methyl) tyrosine] arginine vasopressin. Ten patients were hyponatraemic (plasma sodium less than 135 mmol/l) and 10 were normonatraemic. In both groups of patients the vascular vasopressin antagonist did not alter systemic or pulmonary artery pressures, right atrial pressure, pulmonary capillary wedge pressure, cardiac index, or vascular resistances. Furthermore, there was no change in skin and hepatic blood flow in either group after the injection of the vascular antagonist. Only one patient in the hyponatraemic group showed considerable haemodynamic improvement. He had severe congestive heart failure and a high concentration of plasma vasopressin (51 pmol/l). Plasma renin activity, vasopressin, or catecholamine concentrations were not significantly changed in response to the administration of the vasopressin antagonist in either the hyponatraemic or the normonatraemic groups. Patients with hyponatraemia, however, had higher baseline plasma catecholamine concentrations, heart rate, pulmonary pressure and resistance, and lower hepatic blood flow than patients without hyponatraemia. Plasma vasopressin and plasma renin activity were slightly, though not significantly, higher in the hyponatraemic group. Thus the role of vasopressin in sustaining regional or global vasoconstriction seems limited in patients with congestive heart failure whether or not concomitant hyponatraemia is present. Vasopressin significantly increases the vascular tone only in rare patients with severe congestive heart failure and considerably increased vasopressin concentrations. Patients with hyponatraemia do, however, have raised baseline catecholamine concentrations, heart rate, pulmonary arterial pressure and resistance, and decreased hepatic blood flow.

Mon projet de recherche s'est articulé autour des arythmies supraventriculaires chez les patients adultes porteurs d'une cardiopathie congénitale (Adults with Congenital Heart Disease ou ACHD). Grâce aux progrès de la chirurgie cardiaque et de la cardiologie, la survie à l'âge adulte des enfants nés avec une malformation cardiaque est excellente et la population des ACHD ne cesse de croître, tout en vieillissant. Les arythmies cardiaques sont une complication à long terme bien connue chez ces patients et une des principales causes de mortalité et de morbidité. Mon travail de recherche comporte une perspective épidémiologique avec une étude de population et une perspective anatomique, avec une étude d'imagerie cardiaque par échographie. 1. Étude épidémiologique : Arythmies auriculaires (AA) chez les patients porteurs d'une cardiopathie congénitale. Sous le titre anglais Atrial Arrhythmias in Adults with Congenital Heart Disease, cette étude a été publiée dans le journal américain Circulation. A travers les résultats de cette étude, j'ai démontré, pour la première fois dans une aussi grande population, que les AA étaient fréquentes chez les patients ACHD, que le risque à long-terme pour ces patients de développer des AA était élevé, et que le développement d'AA avait un impact négatif en termes de mortalité, de morbidité et d'intervention. 2. Étude anatomique : Les paramètres échographiques associés à la fibrillation auriculaire (FA) chez les patients adultes porteurs d'une cardiopathie congénitale. Sous le titre anglais Mirror image atrial dilatation in Adult Patients with Atrial Fibrillation and Congenital Heart Disease, cette étude a été publiée dans le journal américain International Journal of Cardiology, j'ai démontré dans cette étude que les patients ACHD avec une pathologie isolée du coeur gauche présentaient une dilatation progressive de l'oreillette gauche (OG) lorsqu'ils développaient de la FA. Chez les patients avec une pathologie isolée du coeur droit, j'ai décrit une image « miroir » : l'oreillette droite (OD) était toujours plus grande que l'OG βί présentait une dilatation progressive lors de FA. Ces résultats soutiennent l'hypothèse que la FA pourrait provenir de l'OD dilatée chez les patients ACHD avec une pathologie du coeur droit et ouvrent de nouvelles perspectives diagnostiques et thérapeutiques chez ces patients:

In the developing heart, Notch signaling plays an essential role in several key developmental processes, such as epithelial-to-mesenchymal transition and myocyte proliferation and differentiation. The importance of Notch in cardiac development has been demonstrated in knockout mice carrying null mutations in genes encoding components of the Notch pathway. Furthermore, humans with inactivating mutations in the Notch ligand Jagged1 suffer from Alagille syndrome, a condition characterized by several cardiac defects. Notch1 receptor haploinsufficiency has also been involved in aortic valve disease in humans. In addition, accumulating evidence indicates that Notch may also regulate homeostasis in the adult heart. Notch may protect the heart from an excessive and detrimental hypertrophic response and increase cardiomyocyte survival. Emerging evidence also suggests that Notch could be important for cardiac tissue renewal by controlling the maintenance and commitment of a cardiac stem cell compartment.

OBJECTIVE: Prenatal diagnosis has been shown to decrease pre-operative acidosis and might prevent the occurrence of disturbed developmental outcome. The aim of this study is to evaluate parameters for acidosis and their predictive value on developmental outcome in newborns with congenital heart disease. METHODS: A total of 117 patients requiring surgery for structural heart disease in the first 31 days of life were included. Diagnosis was established either pre- or postnatally. Preoperative values of lactate, pH and base excess levels were compared to the occurrence of disturbed developmental outcome, i.e. an underperformance of more than 10% on the P90 of a standardized Dutch developmental scale. Patients were divided into groups according to blood levels of acidosis parameters, using receiver operating characteristics curves to determine cut-off values for pH, base excess and lactate. RESULTS: No significant difference in developmental outcome was found using values for pH or base excess as a cut-off level. Preoperative lactate values exceeding 6.1 mmol/l resulted in a significant increase in impaired development compared to infants with a pre-operative lactate lower than 6.1 mmol/l: 40.9% vs 15.1% in (p=0.03). CONCLUSIONS: Pre-operative lactate values might have a prognostic value on developmental outcome in newborns with congenital heart disease. The limited prognostic value of pH can be explained by the fact that pH can be easily corrected, while lactate better reflects the total oxygen debt experienced by these patients.

In heart transplantation (HTx), acute antibody-mediated rejection (AMR) is infrequent but carries high mortality and increased risk of graft vasculopathy. The diagnosis requires evidence of acute graft dysfunction, capillary lesions on endomyocardial biopsy (EMB), and immunopathological criteria of antibodymediated injury. Multiple markers of antibody-mediated injuries have been proposed, but there is ample debate on their usefulness. In kidney transplantation, C4d deposition in peritubular capillaries is a reliable marker of alloantibody-dependant graft injury. In this study, we prospectively screened all EMBs for C4d and CD68 in new HTx recipients, and correlated pathological fi ndings with immunological evidence of donor-specifi c antibodies (DSA) and graft dysfunction. Methods Between Nov 05 and Aug 08, we had 22 HTx, and 17 cases were analysed. All recipients received polyclonal rabbit anti-thymocytes globulin, calcineurin inhibitors, mycophenolate mofetil, and corticosteroids (weaning in 6 -12 months). They had EMB every 1-2 weeks in the fi rst 3 months, and then monthly for 9 months. C4d and CD 68 were assessed by immunochemistry. Echocardiography and DSA assessment or crossmatch (early phase) were realised if C4d or CD68 staining was positive. Results There was 1 early and 1 late AMR. Table 1 C4d and CD68 positive, at least 1 EMB 6 / 17; 35% 1 treated C4d and CD68 positive, at least 2 consecutive EMBs 3 / 17; 17.5% 1 treated C4d and CD68 positive, and graft dysfunction 1 / 17; 6% 1 treated C4d and CD68 positive, with DSA and crossmatch + 1 / 17; 6% 1 treated Table 2 C4d and CD68 positive, at least 1 EMB 1 / 17; 6% 1 treated C4d and CD68 positive, at least 2 consecutive EMBs 1 /17; 6% 1 treated C4d and CD68 positive and graft dysfunction 1 / 17; 6% 1 treated C4d and CD68 positive, and + DSA 1 / 17; 6% 1 treated Conclusion In this single-center experience, C4d / CD68 positive staining was frequent in the early phase and raised the question of false positive cases of AMR. However, these markers showed high specifi city for the diagnosis of AMR in the late phase. Of course these data need to be confi rmed in larger multi-center studies.
