En l'actualitat hi ha controvèrsia sobre el tractament del coll negatiu en el carcinoma faringolaríngeo. En el nostre estudi hem inclòs 11 pacients afectats de carcinoma escamós faringolaríngeo N0, i després de realitzar una injecció amb Tc99m-nanocoloide hem identificat els possibles ganglis sentinella mitjançant limfogammagrafia, SPECT-CT i sonda gamma. S'han detectat de mitjana 2,0 possibles ganglis sentinella. En tres casos no s'ha detectat gangli sentinella. En un futur la realització d'aquesta tècnica, que està en fase de desenvolupament, pot evitar el sobretractament cervical.

Hi ha evidències a la literatura obtingudes de registres poblacionals que senyalen canvis en les característiques epidemiològiques, de tractament i supervivència dels pacients amb carcinomes de la laringe al llarg de les últimes dècades. Hem dut a terme un estudi d’aquestes característiques a partir d’una base de dades hospitalària. D’acord amb els nostres resultats, durant els últims 25 anys s’ha produït un increment en el percentatge de pacients amb carcinoma de laringe del sexe femení, un menor ús de la cirurgia, i una tendència significativa cap a la millora en la supervivència.

Introducció: El Stromal Derived Factor 1 (SDF-1) és una quimioquina que compta amb la capacitat de modular en la proliferació, supervivència, angiogènesi, quimiotaxi i metàstasi de les cèl•lules tumorals actuant a través del seu receptor: CXCR4. L'objectiu d'aquest estudi és valorar la relació en l'expressió dels gens de SDF-1 i CXCR4 comparant-los amb l'expressió en mucosa sana i determinar l'impacte en la supervivència en pacients amb Carcinoma escamós de cap i coll. Material i mètodes: Es va dur a terme una determinació de l'expressió de SDF-1 i CXCR4 en mostres de biòpsies prèvies al tractament i de mucosa sana en 76 pacients amb carcinoma escamós de cap i coll mitjançant una tècnica de PCR quantitativa. Es va determinar la supervivència ajustada mitjançant una tècnica d'arbres de classificació. Resultats: Van existir diferències significatives en la supervivència en funció dels nivells d'expressió de SDF-1 i CXCR4 (p = 0,004). Conclusions: L'expressió dels gens que codifiquen l'eix SDF-1 / CXCR4 té capacitat pronòstica significativa en pacients amb Carcinoma escatós de cap i coll.

Papillary cystadenoma lymphomatosum is a benign salivary gland tumor most frequently located in the parotid gland (Warthin"s tumor). Its presentation in other major, or in minor, salivary glands is rare. Clinically, it manifests as a slow growing tumor, fluctuant on palpation due to its cystic morphology. The treatment of choice is complete excision with wide tumor-free margins. We present a 73-year-old female patient with an asymptomatic tumor of 8 years evolution in the right posterior area of the hard palate. We performed surgical excision and a biopsy, which was reported as papillary cystadenoma lymphomatosum. During the post-operative examination carried out after 3 weeks, it was observed that the lesion had recurred. The lesion was re-operated, performing the excision with CO2 laser and including the periosteum to ensure complete resection of the tumor. At 10 months follow-up, there was no recurrence of the lesion. This article includes a review of this condition and discusses its most important clinical and pathologic features and therapeutic approaches.

Introducció: Els factors predictius patològics vigents per al carcinoma de cèl•lules renals cromòfob (CCRC) mantenen gran controvèrsia en la actualitat, sent la utilitat de la classificació del grau nuclear Fuhrman per a aquest subtipus histològic un dels factors més qüestionats. Objectiu: Avaluar la utilitat pronòstica dels actuals factors patològics predictius a la nostra sèrie de carcinomes de cèl•lules renals tipus cromòfob. Materials i metodologia: Realitzarem una revisió retrospectiva dels paràmetres clínico-patològics de pacients portats a nefrectomia radical per CCRC. Les característiques clínico-patològiques analitzades van ser l’estadi TNM, grau Fuhrman, invasió microvascular, necrosi tumoral, trombus tumoral, marges quirúrgics positius, invasió del greix perinefrític i compromís del sistema col•lector. Resultats: En l’anàlisi multivariant només l’estadi tumoral es va confirmar com a factor predictiu independent de recurrència. Conclusions: l’estadi tumoral prediu agressivitat en el CCR cromòfob. La classificació de grau nuclear de Fuhrman no és útil en aquest subtipus histològic.

Els carcinomes de cèl•lules escamoses de cap i coll (CCECC) estan relacionats en un 80% dels casos amb el tabac i/o l’alcohol. Per altra banda, la infecció per virus papil•loma humà (VPH), la malaltia de transmissió sexual més freqüent als Estats Units actualment, ha resultat ser oncogènica no només a nivell genital sinó també a nivell del tracte aero-digestiu superior, sobretot a oro-faringe. Els estudis realitzats a posteriori han demostrat que els pacients amb CCECC VPH+ no fumadors ni bebedors, solen ser més joves, presentar diferents marcadors tumorals, respondre més al tractament i tenir millor pronòstic.

El objetivo de este trabajo fue describir las características de los pacientes tratados con cirugía micrográfica de Mohs entre 1999-2011 y evaluar la eficacia de esta cirugía para el tratamiento de los carcinomas basocelulares. Durante este periodo se realizaron 560 cirugías de Mohs, el 95.36% de las cuales fueron de carcinomas basocelulares. Se observó una tasa de recidiva del 1.40% para los carcinomas basocelulares primarios y del 13.62% para los recurrentes, siendo la diferencia estadísticamente significativa (p <0.0001).

BACKGROUND Animal model studies have shown that the colon tumour promoting effect of dietary fat depends not only on the amount but on its fatty acid composition. With respect to this, the effect of n9 fatty acids, present in olive oil, on colon carcinogenesis has been scarcely investigated. AIMS To assess the effect of an n9 fat diet on precancer events, carcinoma development, and changes in mucosal fatty acid composition and prostaglandin (PG)E2 formation in male Sprague-Dawley rats with azoxymethane induced colon cancer. METHODS Rats were divided into three groups to receive isocaloric diets (5% of the energy as fat) rich in n9, n3, or n6 fat, and were administered azoxymethane subcutaneously once a week for 11 weeks at a dose rate of 7.4 mg/kg body weight. Vehicle treated groups received an equal volume of normal saline. Groups of animals were colectomised at weeks 12 and 19 after the first dose of azoxymethane or saline. Mucosal fatty acids were assessed at 12 and 19 weeks. Aberrant crypt foci and the in vivo intracolonic release of PGE2 were assessed at week 12, and tumour formation at week 19. RESULTS Rats on the n6 diet were found to have colonic aberrant crypt foci and adenocarcinomas more often than those consuming either the n9 or n3 diet. There were no differences between the rats on the n9 and n3 diets. On the other hand, administration of both n9 and n3 diets was associated with a decrease in mucosal arachidonate concentrations as compared with the n6 diet. Carcinogen treatment induced an appreciable increase in PGE2 formation in rats fed the n6 diet, but not in those fed the n3 and n9 diets. CONCLUSIONS Dietary olive oil prevented the development of aberrant crypt foci and colon carcinomas in rats, suggesting that olive oil may have chemopreventive activity against colon carcinogenesis. These effects may be partly due to modulation of arachidonic acid metabolism and local PGE2synthesis.

Background Fatty acid synthase (FASN) is overexpressed and hyperactivated in several human carcinomas, including lung cancer. We characterize and compare the anti-cancer effects of the FASN inhibitors C75 and (−)-epigallocatechin-3-gallate (EGCG) in a lung cancer model. Methods We evaluated in vitro the effects of C75 and EGCG on fatty acid metabolism (FASN and CPT enzymes), cellular proliferation, apoptosis and cell signaling (EGFR, ERK1/2, AKT and mTOR) in human A549 lung carcinoma cells. In vivo, we evaluated their anti-tumour activity and their effect on body weight in a mice model of human adenocarcinoma xenograft. Results C75 and EGCG had comparable effects in blocking FASN activity (96,9% and 89,3% of inhibition, respectively). In contrast, EGCG had either no significant effect in CPT activity, the rate-limiting enzyme of fatty acid β-oxidation, while C75 stimulated CPT up to 130%. Treating lung cancer cells with EGCG or C75 induced apoptosis and affected EGFR-signaling. While EGCG abolished p-EGFR, p-AKT, p-ERK1/2 and p-mTOR, C75 was less active in decreasing the levels of EGFR and p-AKT. In vivo, EGCG and C75 blocked the growth of lung cancer xenografts but C75 treatment, not EGCG, caused a marked animal weight loss. Conclusions In lung cancer, inhibition of FASN using EGCG can be achieved without parallel stimulation of fatty acid oxidation and this effect is related mainly to EGFR signaling pathway. EGCG reduce the growth of adenocarcinoma human lung cancer xenografts without inducing body weight loss. Taken together, EGCG may be a candidate for future pre-clinical development.

Intrinsic resistance to the epidermal growth factor receptor (EGFR; HER1) tyrosine kinase inhibitor (TKI) gefitinib, and more generally to EGFR TKIs, is a common phenomenon in breast cancer. The availability of molecular criteria for predicting sensitivity to EGFR-TKIs is, therefore, the most relevant issue for their correct use and for planning future research. Though it appears that in non-small-cell lung cancer (NSCLC) response to gefitinib is directly related to the occurrence of specific mutations in the EGFR TK domain, breast cancer patients cannot be selected for treatment with gefitinib on the same basis as such EGFR mutations have beenreported neither in primary breast carcinomas nor in several breast cancer cell lines. Alternatively, there is a generalagreement on the hypothesis that the occurrence of molecular alterations that activate transduction pathways downstreamof EGFR (i.e., MEK1/MEK2 - ERK1/2 MAPK and PI-3'K - AKT growth/survival signaling cascades) significantly affect the response to EGFR TKIs in breast carcinomas. However,there are no studies so far addressing a role of EGF-related ligands as intrinsic breast cancer cell modulators of EGFR TKIefficacy. We recently monitored gene expression profiles andsub-cellular localization of HER-1/-2/-3/-4 related ligands (i.e., EGF, amphiregulin, transforming growth factor-α, ß-cellulin,epiregulin and neuregulins) prior to and after gefitinib treatment in a panel of human breast cancer cell lines. First, gefitinibinduced changes in the endogenous levels of EGF-related ligands correlated with the natural degree of breast cancer cellsensitivity to gefitinib. While breast cancer cells intrinsically resistant to gefitinib (IC50 ≥15 μM) markedly up-regulated(up to 600 times) the expression of genes codifying for HERspecific ligands, a significant down-regulation (up to 106 times)of HER ligand gene transcription was found in breast cancer cells intrinsically sensitive to gefitinib (IC50 ≤1 μM). Second,loss of HER1 function differentially regulated the nuclear trafficking of HER-related ligands. While gefitinib treatment induced an active import and nuclear accumulation of the HER ligand NRG in intrinsically gefitinib-resistant breastcancer cells, an active export and nuclear loss of NRG was observed in intrinsically gefitinib-sensitive breast cancer cells.In summary, through in vitro and pharmacodynamic studies we have learned that, besides mutations in the HER1 gene,oncogenic changes downstream of HER1 are the key players regulating gefitinib efficacy in breast cancer cells. It now appears that pharmacological inhibition of HER1 functionalso leads to striking changes in both the gene expression and the nucleo-cytoplasmic trafficking of HER-specific ligands,and that this response correlates with the intrinsic degree of breast cancer sensitivity to the EGFR TKI gefitinib. Therelevance of this previously unrecognized intracrine feedback to gefitinib warrants further studies as cancer cells could bypassthe antiproliferative effects of HER1-targeted therapeutics without a need for the overexpression and/or activation of other HER family members and/or the activation of HER-driven downstream signaling cascades

Recently, three new polyomaviruses (KI, WU and Merkel cell polyomavirus) have been reported to infect humans. It has also been suggested that lymphotropic polyomavirus, a virus of simian origin, infects humans. KI and WU polyomaviruses have been detected mainly in specimens from the respiratory tract while Merkel cell polyomavirus has been described in a very high percentage of Merkel cell carcinomas. The distribution, excretion level and transmission routes of these viruses remain unknown. Here we analyzed the presence and characteristics of newly described human polyomaviruses in urban sewage and river water in order to assess the excretion level and the potential role of water as a route of transmission of these viruses. Nested-PCR assays were designed for the sensitive detection of the viruses studied and the amplicons obtained were confirmed by sequencing analysis. The viruses were concentrated following a methodology previously developed for the detection of JC and BK human polyomaviruses in environmental samples. JC polyomavirus and human adenoviruses were used as markers of human contamination in the samples. Merkel cell polyomavirus was detected in 7/8 urban sewage samples collected and in 2/7 river water samples. Also one urine sample from a pregnant woman, out of 4 samples analyzed, was positive for this virus. KI and WU polyomaviruses were identified in 1/8 and 2/8 sewage samples respectively. The viral strains detected were highly homologous with other strains reported from several other geographical areas. Lymphotropic polyomavirus was not detected in any of the 13 sewage neither in 9 biosolid/sludge samples analyzed. This is the first description of a virus isolated from sewage and river water with a strong association with cancer. Our data indicate that the Merkel cell polyomavirus is prevalent in the population and that it may be disseminated through the fecal/urine contamination of water. The procedure developed may constitute a useful tool for studying the excreted strains, prevalence and transmission of these recently described polyomaviruses.

Con esta revisión se pretende remarcar al odontoestomatólogo los aspectos clínicos que con mayor frecuencia se asocian a la infección producida por el VIH. Entre ellos cabe resaltar las infecciones producidas por virus (herpes simple, herpes varicela-zoster, citomegalovirus. papilomavirus), hongos (sobre todo candidiasis) y bacterias (caries y patología periodontal); la patología tumoral asociada (sarcoma de Kaposi, linfoma no Hodgkin, carcinomas); así como otras manifestaciones patológicas que tienen su asiento en el territorio del aparato estomatognático (ulceraciones atípicas, alteraciones neurológicas, patología glandular. etc.). No puede concluirse el trabajo sin recordar la importancia que tiene seguir un protocolo en la atención odontológica de estos pacientes seropositivos, que debería hacerse extensivo al resto de nuestros pacientes.

Inverted ductal papilloma of the oral cavity is an infrequent benign neoplasm of papillary appearance that originates in the secretory duct of a salivary gland. The etiology is unknown, though some authors have related it to human papillomavirus (HPV) infection. We present the case of a 40-year-old woman with a tumor of the lower lip mucosa. Histopathological study of the lesion diagnosed inverted ductal papilloma of the oral cavity. Human papillomavirus DNA detection and typing based on tumor lesion DNA amplification and posterior hybridization, revealed no presence of viral DNA. The antecedents of trauma reported by the patient could have played an important role in the development of this tumor

The dose makes the poison, the common motto of toxicology first expressed by Paracelsus more than 400 years ago, may effectively serve to guide potential applications for metformin and related biguanides in oncology. While Paracelsus' law for the dose-response effect has been commonly exploited for the use of some anti-cancer drugs at lower doses in non-neoplastic diseases (e.g., methotrexate), the opposite scenario also holds true; in other words, higher doses of non-oncology drugs, such as anti-diabetic biguanides, might exert direct anti-neoplastic effects. Here, we propose that, as for any drug, there is a dose range for biguanides that is without any effect, one corresponding to"diabetobiguanides" with a pharmacological effect (e.g., insulin sensitization in type 2 diabetes, prevention of insulin-dependent carcinogenesis, indirect inhibition of insulin and growth factor-dependent cancer growth) but with minimal toxicity and another corresponding to 'oncobiguanides' with pharmacological (i.e., direct and strong anticancer activity against cancer cells) as well as toxic effects. Considering that biguanides demonstrate a better safety profile than most oncology drugs in current use, we should contemplate the possibility of administering biguanides through non-conventional routes (e.g., inhaled for carcinomas of the lung, topical for skin cancers, intravenous as an adjunctive therapy, rectal suppositories for rectal cancer) to unambiguously investigate the therapeutic value of high-dose transient biguanide exposure in cancer. Perhaps then, the oncobiguanides, as we call them here, could be viewed as a mechanistically different type of anti-cancer drugs employed at doses notably higher than those used chronically when functioning as diabetobiguanides
