Aquest projecte presenta, per una banda, una revisió del Lean Management i de l'estat actual de la seva aplicació al sector sanitari. Es descriuen els principis i la filosofia en què es basa aquest sistema, les eines i tècniques que li són pròpies, així com les avantatges que aporta cadascuna d’elles a l'empresa en general i, més concretament, al sector sanitari. D'altra banda, també descriu una petita aplicació pràctica de la metodologia Lean per al control de fluxos de pacients al servei d’atenció ambulatòria de l’Hospital Sant Rafael de Barcelona, amb l’objectiu de disminuir el temps total que els pacients tarden en ser atesos pel metge, a través d’eines que alhora aporten valor afegit a tant a pacients com a professionals.

Given positive integers n and m, we consider dynamical systems in which n copies of a topological space is homeomorphic to m copies of that same space. The universal such system is shown to arise naturally from the study of a C*-algebra we denote by Om;n, which in turn is obtained as a quotient of the well known Leavitt C*-algebra Lm;n, a process meant to transform the generating set of partial isometries of Lm;n into a tame set. Describing Om;n as the crossed-product of the universal (m; n) -dynamical system by a partial action of the free group Fm+n, we show that Om;n is not exact when n and m are both greater than or equal to 2, but the corresponding reduced crossed-product, denoted Or m;n, is shown to be exact and non-nuclear. Still under the assumption that m; n &= 2, we prove that the partial action of Fm+n is topologically free and that Or m;n satisfies property (SP) (small projections). We also show that Or m;n admits no finite dimensional representations. The techniques developed to treat this system include several new results pertaining to the theory of Fell bundles over discrete groups.

This paper discusses the role of deterministic components in the DGP and in the auxiliary regression model which underlies the implementation of the Fractional Dickey-Fuller (FDF) test for I(1) against I(d) processes with d ∈ [0, 1). This is an important test in many economic applications because I(d) processess with d & 1 are mean-reverting although, when 0.5 ≤ d & 1,, like I(1) processes, they are nonstationary. We show how simple is the implementation of the FDF in these situations, and argue that it has better properties than LM tests. A simple testing strategy entailing only asymptotically normally distributed tests is also proposed. Finally, an empirical application is provided where the FDF test allowing for deterministic components is used to test for long-memory in the per capita GDP of several OECD countries, an issue that has important consequences to discriminate between growth theories, and on which there is some controversy.

The potential for application of silicon nitride-based light sources to general lighting is reported. The mechanism of current injection and transport in silicon nitride layers and silicon oxide tunnel layers is determined by electro-optical characterization of both bi- and tri-layers. It is shown that red luminescence is due to bipolar injection by direct tunneling, whereas Poole-Frenkel ionization is responsible for blue-green emission. The emission appears warm white to the eye, and the technology has potential for large-area lighting devices. A photometric study, including color rendering, color quality and luminous efficacy of radiation, measured under various AC excitation conditions, is given for a spectrum deemed promising for lighting. A correlated color temperature of 4800K was obtained using a 35% duty cycle of the AC excitation signal. Under these conditions, values for general color rendering index of 93 and luminous efficacy of radiation of 112 lm/W are demonstrated. This proof of concept demonstrates that mature silicon technology, which is extendable to lowcost, large-area lamps, can be used for general lighting purposes. Once the external quantum efficiency is improved to exceed 10%, this technique could be competitive with other energy-efficient solid-state lighting options. ©2011 Optical Society of America OCIS codes: (230.2090) Electro-optical devices; (150.2950) Illumination.

En los primeros días de octubre de 1973, al tener noticia del derribo de la casa nº19 de la calle Gobernador González, de ,Tarragona, recabamos la oportuna autorización al nuevo propietario del inmueble, don Angel Molinero Sisamón, para poder recuperar el mosaico allí existente. El mosaico estaba visible en parte, ya de antiguo, en un pequeño patio de luces de la casa derribada, por lo que se presentaba ahora la ocasión de excavarlo y extraerlo en su totalidad (lám. J, a). La extracción había que realizarla con relativa rapidez para no entorpecer los trabajos preparatorios de la nueva construcción, por lo que pedimos la colaboración de parte de la brigada del Patrimonio Artístico Nacional, que en esas fechas trabajaban en la Plaza del Rey, además del capataz de las excavaciones de Els Munts, práctico ya en el arranque de mosaicos. Nuestra labor no se limitó a salvar el mosaico sino que aprovecharnos la oportunidad para estudiar el solar resultante del derribo de la casa y del jardín situado al norte¡ de la misma. A este fin realizamos una zanja de 5 metros al norte del mosaico hasta llegar a un pozo negro y efectuamos cuatro catas de sondeo de 1,50 por 1,50 metros dentro del área del antiguo jardín. La zanja dio unos resultados positivos, aunque modestos, y no así las catas que nos patentizaron la falta absoluta de restos arqueológicos de alguna importancia en toda aquella zona. La parte situada al sur del mosaico ya había sido rebajada antiguamente hasta el nivel de la calle.

Abstract. The ability of 2 Rapid Bioassessment Protocols (RBPs) to assess stream water quality was compared in 2 Mediterranean-climate regions. The most commonly used RBPs in South Africa (SAprotocol) and the Iberian Peninsula (IB-protocol) are both multihabitat, field-based methods that use macroinvertebrates. Both methods use preassigned sensitivity weightings to calculate metrics and biotic indices. The SA- and IB-protocols differ with respect to sampling equipment (mesh size: 1000 lm vs 250 300 lm, respectively), segregation of habitats (substrate vs flow-type), and sampling and sorting procedures (variable time and intensity). Sampling was undertaken at 6 sites in South Africa and 5 sites in the Iberian Peninsula. Forty-four and 51 macroinvertebrate families were recorded in South Africa and the Iberian Peninsula, respectively; 77.3% of South African families and 74.5% of Iberian Peninsula families were found using both protocols. Estimates of community similarity compared between the 2 protocols were .60% similar among sites in South Africa and .54% similar among sites in the Iberian Peninsula (BrayCurtis similarity), and no significant differences were found between protocols (Multiresponse Permutation Procedure). Ordination based on Non-metric Multidimensional Scaling grouped macroinvertebrate samples on the basis of site rather than protocol. Biotic indices generated with the 2 protocols at each site did not differ. Thus, both RBPs produced equivalent results, and both were able to distinguish between biotic communities (mountain streams vs foothills) and detect water-quality impairment, regardless of differences in sampling equipment, segregation of habitats, and sampling and sorting procedures. Our results indicate that sampling a single habitat may be sufficient for assessing water quality, but a multihabitat approach to sampling is recommended where intrinsic variability of macroinvertebrate assemblages is high (e.g., in undisturbed sites in regions with Mediterranean climates). The RBP of choice should depend on whether the objective is routine biomonitoring of water quality or autecological or faunistic studies.

Phototransduction in vertebrate photoreceptor cells represents a paradigm of signaling pathways mediated by G-protein-coupled receptors (GPCRs), which share common modules linking the initiation of the cascade to the final response of the cell. In this work, we focused on the recovery phase of the visual photoresponse, which is comprised of several interacting mechanisms. We employed current biochemical knowledge to investigate the response mechanisms of a comprehensive model of the visual phototransduction pathway. In particular, we have improved the model by implementing a more detailed representation of the recoverin (Rec)-mediated calcium feedback on rhodopsin kinase and including a dynamic arrestin (Arr) oligomerization mechanism. The model was successfully employed to investigate the rate limiting steps in the recovery of the rod photoreceptor cell after illumination. Simulation of experimental conditions in which the expression levels of rhodospin kinase (RK), of the regulator of the G-protein signaling (RGS), of Arr and of Rec were altered individually or in combination revealed severe kinetic constraints to the dynamics of the overall network. Our simulations confirm that RGS-mediated effector shutdown is the rate-limiting step in the recovery of the photoreceptor and show that the dynamic formation and dissociation of Arr homodimers and homotetramers at different light intensities significantly affect the timing of rhodopsin shutdown. The transition of Arr from its oligomeric storage forms to its monomeric form serves to temper its availability in the functional state. Our results may explain the puzzling evidence that overexpressing RK does not influence the saturation time of rod cells at bright light stimuli. The approach presented here could be extended to the study of other GPCR signaling pathways.

Visual perception is initiated in the photoreceptor cells of the retina via the phototransduction system.This system has shown marked evolution during mammalian divergence in such complex attributes as activation time and recovery time. We have performed a molecular evolutionary analysis of proteins involved in mammalianphototransduction in order to unravel how the action of natural selection has been distributed throughout thesystem to evolve such traits. We found selective pressures to be non-randomly distributed according to both a simple protein classification scheme and a protein-interaction network representation of the signaling pathway. Proteins which are topologically central in the signaling pathway, such as the G proteins, as well as retinoid cycle chaperones and proteins involved in photoreceptor cell-type determination, were found to be more constrained in their evolution. Proteins peripheral to the pathway, such as ion channels and exchangers, as well as the retinoid cycle enzymes, have experienced a relaxation of selective pressures. Furthermore, signals of positive selection were detected in two genes: the short-wave (blue) opsin (OPN1SW) in hominids and the rod-specific Na+/Ca2+,K+ ion exchanger (SLC24A1) in rodents. The functions of the proteins involved in phototransduction and the topology of the interactions between them have imposed non-random constraints on their evolution. Thus, in shaping or conserving system-level phototransduction traits, natural selection has targeted the underlying proteins in a concerted manner.

Asparagine N-Glycosylation is one of the most important forms of protein post-translational modification in eukaryotes. This metabolic pathway can be subdivided into two parts: an upstream sub-pathway required for achieving proper folding for most of the proteins synthesized in the secretory pathway, and a downstream sub-pathway required to give variability to trans-membrane proteins, and involved in adaptation to the environment and innate immunity. Here we analyze the nucleotide variability of the genes of this pathway in human populations, identifying which genes show greater population differentiation and which genes show signatures of recent positive selection. We also compare how these signals are distributed between the upstream and the downstream parts of the pathway, with the aim of exploring how forces of population differentiation and positive selection vary among genes involved in the same metabolic pathway but subject to different functional constraints. Our results show that genes in the downstream part of the pathway are more likely to show a signature of population differentiation, while events of positive selection are equally distributed among the two parts of the pathway. Moreover, events of positive selection are frequent on genes that are known to be at bifurcation points, and that are identified as being in key position by a network-level analysis such as MGAT3 and GCS1. These findings indicate that the upstream part of the Asparagine N-Glycosylation pathway has lower diversity among populations, while the downstream part is freer to tolerate diversity among populations. Moreover, the distribution of signatures of population differentiation and positive selection can change between parts of a pathway, especially between parts that are exposed to different functional constraints. Our results support the hypothesis that genes involved in constitutive processes can be expected to show lower population differentiation, while genes involved in traits related to the environment should show higher variability. Taken together, this work broadens our knowledge on how events of population differentiation and of positive selection are distributed among different parts of a metabolic pathway.

Para la realización de este estudio se ha dividido la España Peninsular en tres grandes regiones (agrupaciones de provincias): Noroeste (NW), Litoral Mediterráneo (LM) e Interior Peninsular (IP), con objeto de dilucidar si sus regímenes actuales (década 1986-95) de incendios son diferentes. Estas regiones difieren en sus condiciones meteorológicas, densidad demográfica y usos del territorio. Hemos calculado el índice de frecuencia (IF = número de incendios por cada 10000 ha forestales) y el índice de gravedad (IG = superficie quemada ( 100 / superficie forestal). Este último se ha desglosado también para la superficie quemada arbolada y la no arbolada. La región Noroeste es la que presenta más siniestros por hectárea forestal (IF = 27,9). Aunque los índices de frecuencias de las regiones Litoral Mediterráneo (6,1) e Interior (4,1) sean menores, el ratio superficie recorrida por el fuego respecto a la superficie forestal total no es diferente a la afectada en la región Noroeste. Esta región se diferencia de las otras dos por la importante extensión de superficie desarbolada que se quema (IG de superficies desarbolada es 2,0 en NW, frente a 0,4 y 0,8 para IP y LM respectivamente). La caracterización de las regiones en cuanto a factores meteorológicos y sociales puede ayudar a explicar dichas diferencias, muy especialmente el estudio de las causas de incendio.

Objetivo general: Determinar los indicadores de incidencia y prevalencia de úlceras por presión en tobillo-pie en lesionados medulares ingresados. Material y métodos: Ensayo clínico controlado de 549 lesionados medulares, durante 48 meses. Resultados: La incidencia promedio de pacientes con UPP-TP de todo el periodo estudiado fue de: 1"178 pac./mes. La incidencia promedio de UPP-TP de todo el periodo estudiado fue de: 1"789 UPP-TP/mes. La prevalencia promedio de pacientes con UPP-TP de todo el periodo estudiado, respecto al total de pacientes fue de: 7"75%. La prevalencia promedio de todo el periodo de las UPP-TP, respecto al total de UPP fue de: 39"28%. La prevalencia promedio de todo el periodo estudiado de las UPP-TP, respecto al total de heridas (UPP+Traumáticas) fue de: 24"39%. Todos los pacientes crónicos con UPP-TP presentaban LM completa, por tanto ningún pac. crónico incompleto presento UPP-TP. Un 39"66% (23 pac.) presentaron más de 1 lesión. El 47"72% de las UPP-TP se presentaron en talón y un 19"32 en maléolo ext, siendo respectivamente las zonas de mayor localización de las lesiones. Un 13"63% de las UPP-TP se presentaron en dedos a pesar de la no deambulación de estos y de los 6 pac. que tuvieron la lesión en 1º dedo 4 pac. padecieron onicocriptosis. En el 19"23% (5 pac.) de los pac. crónicos sus UPPTP presentaron estadio IV, 3 pac. la presentaron en su domicilio y 2 pac. en ULM. El 25"86% de los pac. cicatrizaron sus lesiones dentro del 1º mes. Conclusiones: Teniendo en cuenta que el 29"30% de los pacientes, la incidencia de su UPP-TP, tuvo origen en el domicilio habitual, que de estos el 94% eran crónicos con LM completa y que el 34"48% del total de pacientes afectos de UPP-TP del estudio fueron dados de alta a su domicilio sin la curación de su úlcera, es notoria la importancia del seguimiento domiciliario de proximidad, la educación sanitaria de pacientes y familiarescuidadores, así como el uso de guías clínicas preventivas en nuestra asistencia podológica, especialmente en pacientes con lesión medular completa.

Voltage-dependent K+ channels (Kv) are involved in the proliferation and differentiation of mammalian cells, since Kv antagonists impair cell cycle progression. Although myofibers are terminally differentiated, some myoblasts may re-enter the cell cycle and proliferate. Since Kv1.3 and Kv1.5 expression is remodeled during tumorigenesis and is involved in smooth muscle proliferation, the purpose of this study was to analyze the expression of Kv1.3 and Kv1.5 in smooth muscle neoplasms. In the present study, we examined human samples of smooth muscle tumors together with healthy speci­mens. Thus, leiomyoma (LM) and leiomyosarcoma (LMS) tumors were analyzed. Results showed that Kv1.3 was poorly expressed in the healthy muscle and indolent LM specimens, whereas aggressive LMS showed high levels of Kv1.3 expression. Kv1.5 staining was correlated with malignancy. The findings show a remodeling of Kv1.3 and Kv1.5 in human smooth muscle sarcoma. A correlation of Kv1.3 and Kv1.5 expression with tumor aggressiveness was observed. Thus, our results indicate Kv1.5 and Kv1.3 as potential tumorigenic targets for aggressive human LMS.
