La neurocirurgia és el tractament definitiu més utilitzat pels tumors hipofisaris. Objectiu: valorar les complicacions postquirúrgiques immediates(1º mes) i durant el 1º any dels adenomes hipofisaris secretors de GH, ACTH i no funcionants(NF) operats des del 2001. Metodologia: estudi observacional restrospectiu de 94 pacients (39H, 55D) amb edat a la cirurgia de 46,9±15,5 anys, intervinguts pels 2 mateixos neurocirurgians. Resultats: 40 pacients tenen alguna complicació immediata(42,5% dels NF, 37% GH i 48,5% ACTH) sense diferències en la freqüència de complicacions entre els 3 grups. Les complicacions més freqüents són: diabetis insípida transitòria(23,4%), fístula LCR(6,7%), sinusitis i meningitis(2,2%). Els secretors d'ACTH tenen una tendència a tenir més DI transitòria i sinusitis respecte els NF(p=0,071), mentre que els NF tendeixen a presentar més fístules LCR, meningitis i convulsions(p=0,08). En els GH, existeix major incidència de fístules LCR comparat amb els ACTH(p&0,05), sense diferències amb els NF. 10 pacients tenen complicacions al 1º any postquirúrgic(7,5% dels NF, 11,1% GH i 14,8% ACTH), destacant major incidència d'artromiàlgies i síndrome del túnel carpià en els ACTH comparat amb els altres 2 grups (p&0,05). Les variables més importants quan fem una predicció d'aparició de complicacions són: tipus de cirurgia utilitzada (més a craniotomies que als abordatges transesfenoidals) i presència d'extensió extraselar tumoral, sense ser significatiu(p=0,091). Conclusions: malgrat que els tumors d'ACTH són majoritàriament microdenomes(77,7%), i es presenten en pacients més joves, tendeixen a associar-se a major nombre de complicacions immediates i durant el primer any en comparació amb els NF i GH(97,5%, 81,4% macroadenomes respectivament).

The pituitary adenylate cyclase activating polypeptide (PACAP) type I receptor (PAC1) is a G-protein-coupled receptor binding the strongly conserved neuropeptide PACAP with 1000-fold higher affinity than the related peptide vasoactive intestinal peptide. PAC1-mediated signaling has been implicated in neuronal differentiation and synaptic plasticity. To gain further insight into the biological significance of PAC1-mediated signaling in vivo, we generated two different mutant mouse strains, harboring either a complete or a forebrain-specific inactivation of PAC1. Mutants from both strains show a deficit in contextual fear conditioning, a hippocampus-dependent associative learning paradigm. In sharp contrast, amygdala-dependent cued fear conditioning remains intact. Interestingly, no deficits in other hippocampus-dependent tasks modeling declarative learning such as the Morris water maze or the social transmission of food preference are observed. At the cellular level, the deficit in hippocampus-dependent associative learning is accompanied by an impairment of mossy fiber long-term potentiation (LTP). Because the hippocampal expression of PAC1 is restricted to mossy fiber terminals, we conclude that presynaptic PAC1-mediated signaling at the mossy fiber synapse is involved in both LTP and hippocampus-dependent associative learning.

Despite data favouring a role of dietary fat in colonic carcinogenesis, no study has focused on tissue n3 and n6 fatty acid (FA) status in human colon adenoma-carcinoma sequence. Thus, FA profile was measured in plasma phospholipids of patients with colorectal cancer (n = 22), sporadic adenoma (n = 27), and normal colon (n = 12) (control group). Additionally, mucosal FAs were assessed in both diseased and normal mucosa of cancer (n = 15) and adenoma (n = 21) patients, and from normal mucosa of controls (n = 8). There were no differences in FA profile of both plasma phospholipids and normal mucosa, between adenoma and control patients. There were considerable differences, however, in FAs between diseased and paired normal mucosa of adenoma patients, with increases of linoleic (p = 0.02), dihomogammalinolenic (p = 0.014), and eicosapentaenoic (p = 0.012) acids, and decreases of alpha linolenic (p = 0.001) and arachidonic (p = 0.02) acids in diseased mucosa. A stepwise reduction of eicosapentaenoic acid concentrations in diseased mucosa from benign adenoma to the most advanced colon cancer was seen (p = 0.009). Cancer patients showed lower alpha linolenate (p = 0.002) and higher dihomogammalinolenate (p = 0.003) in diseased than in paired normal mucosa. In conclusion changes in tissue n3 and n6 FA status might participate in the early phases of the human colorectal carcinogenesis.

Introduction: Minor salivary gland tumors (MSGTs) are infrequent, representing 10-15% of all salivary neoplasms. Despite this low frequency, MSGTs conform a heterogeneous group of neoplasms characterized by a broad range of histological types. Patients and method: We identified cases of MSGT in a retrospective study of the biopsies made in the period 1997-2007 in the Service of Oral Surgery (Dental Clinic of the University of Barcelona, Spain). The data collected comprised patient age and sex, the clinical characteristics and location of the tumor, the duration of the lesion, its size, the treatment provided, and the histopathological findings. Results: Of the 18 cases of MSGT studied, 12 corresponded to women (66.7%) and 6 to men (33.3%). The great majority (94.4%) were benign tumors. The preferential location was the posterior third of the hard palate (33.2%), followed by the soft palate (16.7%) and the mucosa of the upper lip (16.7%). The histopathological diagnoses of our MSGTs comprised 10 pleomorphic adenomas (55.3%), 2 cystadenomas (11.1%), 1 myoepithelioma (5.6%), 1 sialadenoma papilliferum (5.6%), 1 basal cell adenoma (5.6%), 1 Warthin"s tumor (5.6%), 1 canalicular adenoma (5.6%), and 1 low-grade polymorphic adenocarcinoma (5.6%). Discussion and conclusions: Coinciding with our own results, the literature describes a high recurrence rate for MSGTs (5-30%) when surgical removal is incomplete. Six percent of all benign minor salivary gland tumors are considered to relapse, versus 65% of all malignant lesions. Periodic clinical controls are required, since the possibility of malignant transformation must be taken into account

El cáncer colorrectal (CCR) es una de las neoplasias más frecuentes en nuestro medio. En la actualidad, constituye la segunda neoplasia tanto en varones como en mujeres, tras el cáncer de pulmón y de mama, respectivamente. Cuando se consideran ambos sexos conjuntamente, ocupa el primer lugar en incidencia y representa la segunda causa de muerte por cáncer. En los últimos años hemos asistido a un avance muy significativo en el conocimiento de los mecanismos que participan en el desarrollo y progresión del CCR. Este avance abarca desde la identificación de diversos factores genéticos o moleculares implicados en la fisiopatología de esta neoplasia, hasta la caracterización de múltiples aspectos epidemiológicos involucrados en su génesis. En concreto, la demostración del potencial premaligno del adenoma colorrectal y la identificación de los genes responsables de las formas hereditarias de CCR han dado pie a diversas estrategias preventivas que pueden contribuir significativamente a disminuir la incidencia y la morbimortalidad por CCR.
