La malaltia de Crohn és una malaltia inflamatòria intestinal en la qual s´ha observat que la densitat i la diversitat bacteriana difereixen dels subjectes sans. En els últims anys s’estan emprant tractaments biològics com els anti-TNFα (Adalimumab) de manera alternativa a tractaments ja existents. Els resultats obtinguts després de l’anàlisi de la composició bacteriana de la mucosa intestinal de pacients amb Malaltia de Crohn tractats amb Adalimumab, són esperançadors ja que apunten a una recuperació del patró normal a través de la recuperació d’espècies pròpies de persones sanes i la desaparició d’aquelles més prevalents en malalts de Crohn.

La citologia anal és un mètode sensible en el despistatge de la neoplàsia de canal anal i similar a la biòpsia dirigida mitjançant rectoscopi i tinció amb àcid acètic al 5%, sobretot en lesions d’alt grau de malignitat; a més de ser una prova adequada en el seguiment d’aquestes lesions. Els pacients VIH homes que tenen sexe amb altres homes tenen major incidència i prevalència de virus papiloma humà i major nombre d’anormalitats en la mucosa del canal anal que els VIH negatius.

Se presenta un estudio descriptivo de prevalencia de patología de la mucosa bucal, en una muestra de 1491 pacientes, visitados en la Clínica Odontológica Universitaria de la Facultad de Odontología de la Universidad de Barcelona. Entre los objetivos planteados se persigue conocer la asociación de las lesiones más prevalentes con la patología sistémica y con los hábitos tóxicos. Las lesiones más prevalentes fueron las clasificadas en el grupo de lesiones blancas. No se pudo confirmar una mayor asociación de la patología bucal en general con la patología sistémica, pero sí una mayor prevalencia de patología bucal en pacientes con hábitos tóxicos, particularmente tabaquismo.

Currently a growing interest to improve the pharmacological therapy exists, not only by the production and the appearance of new drugs, but guaranteeing that the uses of those which already exist, become more effective. In fact, the conventional pharmaceutical formulations of different drugs present a few secondary effects due to oral administration. In order to avoid these undesired side effects, the purpose of current therapeutic is the development and research of formulations as an alternative to others routes of administration. Therefore, in spite of the undoubtedly complete parenteral absorption, the transdermal and transbuccal routes appear to be a rather attractive alternative to provide an efficient absorption. In this chapter a new technological, biopharmaceutical and pharmacokinetic approach of strategies for application on skin and buccal mucosa are reported. In the future new transdermal drug delivery systems will emerge to be more effective, equipped with an improved aesthetic appearance, better adherence and greater diffusion. But to reach these aims, it is necessary previous knowledge of histology and physiology of skin, and factors involved in the penetration of drugs through it.

Proteasome inhibitors, used in cancer treatment for their proapoptotic effects, have anti-inflammatory and antifibrotic effects on animal models of various inflammatory and fibrotic diseases. Their effects in cells from patients affected by either inflammatory or fibrotic diseases have been poorly investigated. Nasal polyposis is a chronic inflammatory disease of the sinus mucosa characterized by tissue inflammation and remodeling. We tested the hypothesis that proteasome inhibition of nasal polyp fibroblasts might reduce their proliferation and inflammatory and fibrotic response. Accordingly, we investigated the effect of the proteasome inhibitor Z-Leu-Leu-Leu-B(OH)2 (MG262) on cell viability and proliferation and on the production of collagen and inflammatory cytokines in nasal polyp and nasal mucosa fibroblasts obtained from surgery specimens. MG262 reduced the viability of nasal mucosa and polyp fibroblasts concentration- and time-dependently, with marked effects after 48 h of treatment. The proteasome inhibitor bortezomib provoked a similar effect. MG262-induced cell death involved loss of mitochondrial membrane potential, caspase-3 and poly(ADP-ribose) polymerase activation, induction of c-Jun phosphorylation, and mitogen-activated protein kinase phosphatase-1 expression. Low concentrations of MG262 provoked growth arrest, inhibited DNA replication and retinoblastoma phosphorylation, and increased expression of the cell cycle inhibitors p21 and p27. MG262 concentration-dependently inhibited basal and transforming growth factor-β-induced collagen mRNA expression and interleukin (IL)-1β-induced production of IL-6, IL-8, monocyte chemoattractant protein-1, regulated on activation normal T cell expressed and secreted, and granulocyte/macrophage colony-stimulating factor in both fibroblast types. MG262 inhibited IL-1β/tumor necrosis factor-α-induced activation of nuclear factor-κB. We conclude that noncytotoxic treatment with MG262 reduces the proliferative, fibrotic, and inflammatory response of nasal fibroblasts, whereas high MG262 concentrations induce apoptosis.

Introducció: El Stromal Derived Factor 1 (SDF-1) és una quimioquina que compta amb la capacitat de modular en la proliferació, supervivència, angiogènesi, quimiotaxi i metàstasi de les cèl•lules tumorals actuant a través del seu receptor: CXCR4. L'objectiu d'aquest estudi és valorar la relació en l'expressió dels gens de SDF-1 i CXCR4 comparant-los amb l'expressió en mucosa sana i determinar l'impacte en la supervivència en pacients amb Carcinoma escamós de cap i coll. Material i mètodes: Es va dur a terme una determinació de l'expressió de SDF-1 i CXCR4 en mostres de biòpsies prèvies al tractament i de mucosa sana en 76 pacients amb carcinoma escamós de cap i coll mitjançant una tècnica de PCR quantitativa. Es va determinar la supervivència ajustada mitjançant una tècnica d'arbres de classificació. Resultats: Van existir diferències significatives en la supervivència en funció dels nivells d'expressió de SDF-1 i CXCR4 (p = 0,004). Conclusions: L'expressió dels gens que codifiquen l'eix SDF-1 / CXCR4 té capacitat pronòstica significativa en pacients amb Carcinoma escatós de cap i coll.

Objectius: Avaluar la retirada d’infliximab en pacients amb MII, en remissió mantinguda i curació mucosa sota tractament combinat. Mètodes: Pacients amb MC luminal o CU sota tractament combinat (infliximab i azatioprina) durant mínim un any, als quals se’ls retira l’infliximab per remissió clínica, biològica i endoscòpica. Resultats: S’inclogueren 26 pacients. Un 73% es van mantenir en remissió. La recidiva no s’associà al fracàs previ d’immunomoduladors. A les recidives se’ls va reiniciar infliximab amb bona resposta. Cap factor va associar-se a la recidiva. Conclusions: La retirada de l’infliximab en aquests pacients, s’acompanya d’una baixa probabilitat de recidiva a mig termini.

La colitis ulcerosa (CU) es caracteritza per una afectació contínua de la mucosa rectal en sentit proximal cap a altres zones del còlon. Hi ha formes de CU amb afectació distal i periapendicular (CU-PA). Objectius: avaluar la prevalença de la CU-PA, i comparar les seves característiques clíniques, terapèutiques i evolutives. Mètodes: 14 pacients amb CU-PA van ser comparats en termes d'evolució clínica amb 25 pacients amb CU distal sense afectació periapendicular. Resultats: Es va trobar una major freqüència de CU-PA en homes (p = 0,047), sense diferències en la resta de les variables comparades entre els dos grups de pacients.

La utilización de un implante orbitario es imprescindible para restaurar el volumen tras la evisceración o enucleación del globo ocular. Dicho volumen en la cavidad anoftálmica permitirá la utilización de una cascarilla que dará al paciente una mejor apariencia y simetría tras la pérdida del globo ocular. La exposición del implante orbitario es una de las complicaciones más frecuentes que nos encontramos tras evisceración o enucleación con la utilización de implante orbitario. Hay diferentes factores reconocidos que propiciarán dicha exposición, entre los que se encuentran la colonización del implante, escasez de mucosa conjuntival, tamaño y composición del implante, etc... Hay diferentes opciones de tratamiento de la exposición del implante orbitario que incluyen la utilización de tejidos por medio de injerto o flap o bien la extracción del propio implante con la consecuente afectación en la apariencia del paciente. El uso de injerto dermo-graso ha sido descrito con anterioridad, pero no hemos encontrado un estudio de una serie de casos que demuestre su utilidad. Nuestro objetivo es determinar la efectividad del injerto dermo-graso en el tratamiento de la exposición del implante orbitario en la cavidad anoftálmica. Para ello estudiamos la evolución de 15 pacientes que presentaron exposición de implante orbitario, tras enucleación o evisceración, y a los que se les ha tratado dicha exposición con un injerto dermo-graso. Se evaluan clínicamente los resultados de dicha técnica en estos pacientes, en algún caso hasta 22 meses tras la intervención.
Measurement of cell microrheology by magnetic twisting cytometry with frequency domain demodulation.

Although antihistamines and topical corticosteroids are used in combination to treat allergic rhinitis, their additive effect has not been yet demonstrated. The aim was investigate the antiinflammatory additive effect of mometasone and desloratadine on cytokine and sICAM-1 secretion by epithelial cells, and on eosinophil survival stimulated by human epithelial cells secretions from nasal mucosa and polyps. Methods Epithelial cells obtained from nasal mucosa or polyps were stimulated with 10% fetal bovine serum in presence of mometasone (10-11M-10-5M) with/without desloratadine (10-5M). Cytokine and sICAM-1 concentrations in supernatants were measured by ELISA. Peripheral blood eosinophils were incubated during 4 days with epithelial cell secretions with (10-11M-10-5M) and/or desloratadine (10-5M) and survival assessed by Trypan blue. Results are expressed as percentage (mean ± SEM) compared to control. Results Fetal bovine serum stimulated IL-6, IL-8, GM-CSF and sICAM-1 secretion. In mucosa and polyp epithelial cells, mometasone inhibited this induced secretion while desloratadine inhibited IL-6 and IL-8. The combination of 10-5M desloratadine and 10-9M mometasone reduced IL-6 secretion (48 ± 11%, p < 0.05) greater extent than mometasone alone (68 ± 10%) compared to control (100%). Epithelial cell secretions induced eosinophil survival from day 1 to 4, this effect being inhibited by mometasone. At day 4, the combination of mometasone (10-11M) and desloratadine (10-5M) provoked an increased inhibition of eosinophil survival induced by cell secretions (27 ± 5%, p < 0.01) than mometasone (44 ± 7%) or desloratadine (46 ± 7%) alone. Conclusions These results suggest that the combination of desloratadine and mometasone furoate have a greater antinflammatory effect in an in vitro model of eosinophil inflammation than those drugs administered alone.

Despite data favouring a role of dietary fat in colonic carcinogenesis, no study has focused on tissue n3 and n6 fatty acid (FA) status in human colon adenoma-carcinoma sequence. Thus, FA profile was measured in plasma phospholipids of patients with colorectal cancer (n = 22), sporadic adenoma (n = 27), and normal colon (n = 12) (control group). Additionally, mucosal FAs were assessed in both diseased and normal mucosa of cancer (n = 15) and adenoma (n = 21) patients, and from normal mucosa of controls (n = 8). There were no differences in FA profile of both plasma phospholipids and normal mucosa, between adenoma and control patients. There were considerable differences, however, in FAs between diseased and paired normal mucosa of adenoma patients, with increases of linoleic (p = 0.02), dihomogammalinolenic (p = 0.014), and eicosapentaenoic (p = 0.012) acids, and decreases of alpha linolenic (p = 0.001) and arachidonic (p = 0.02) acids in diseased mucosa. A stepwise reduction of eicosapentaenoic acid concentrations in diseased mucosa from benign adenoma to the most advanced colon cancer was seen (p = 0.009). Cancer patients showed lower alpha linolenate (p = 0.002) and higher dihomogammalinolenate (p = 0.003) in diseased than in paired normal mucosa. In conclusion changes in tissue n3 and n6 FA status might participate in the early phases of the human colorectal carcinogenesis.

Background: Limited data on a short series of patients suggest that lymphocytic enteritis (classically considered as latent coeliac disease) may produce symptoms of malabsorption, although the true prevalence of this situation is unknown. Serological markers of coeliac disease are of little diagnostic value in identifying these patients. Aims: To evaluate the usefulness of human leucocyte antigen-DQ2 genotyping followed by duodenal biopsy for the detection of gluten-sensitive enteropathy in first-degree relatives of patients with coeliac disease and to assess the clinical relevance of lymphocytic enteritis diagnosed with this screening strategy. Patients and methods: 221 first-degree relatives of 82 DQ2+ patients with coeliac disease were consecutively included. Duodenal biopsy (for histological examination and tissue transglutaminase antibody assay in culture supernatant) was carried out on all DQ2+ relatives. Clinical features, biochemical parameters and bone mineral density were recorded. Results: 130 relatives (58.8%) were DQ2+, showing the following histological stages: 64 (49.2%) Marsh 0; 32 (24.6%) Marsh I; 1 (0.8%) Marsh II; 13 (10.0%) Marsh III; 15.4% refused the biopsy. 49 relatives showed gluten sensitive enteropathy, 46 with histological abnormalities and 3 with Marsh 0 but positive tissue transglutaminase antibody in culture supernatant. Only 17 of 221 relatives had positive serological markers. Differences in the diagnostic yield between the proposed strategy and serology were significant (22.2% v 7.2%, p<0.001). Relatives with Marsh I and Marsh II¿III were more often symptomatic (56.3% and 53.8%, respectively) than relatives with normal mucosa (21.1%; p=0.002). Marsh I relatives had more severe abdominal pain (p=0.006), severe distension (p=0.047) and anaemia (p=0.038) than those with Marsh 0. The prevalence of abnormal bone mineral density was similar in relatives with Marsh I (37%) and Marsh III (44.4%). Conclusions: The high number of symptomatic patients with lymphocytic enteritis (Marsh I) supports the need for a strategy based on human leucocyte antigen-DQ2 genotyping followed by duodenal biopsy in relatives of patients with coeliac disease and modifies the current concept that villous atrophy is required to prescribe a gluten-free diet.
