Background: In recent years, microRNA (miRNA) pathways have emerged as a crucial system for the regulation of tumorogenesis. miR-SNPs are a novel class of single nucleotide polymorphisms that can affect miRNA pathways. Design and Methods: We analyzed eight miR-SNPs by allelic discrimination in 141 patients with Hodgkin lymphoma and correlated the results with treatment-related toxicity, response, disease-free survival (DFS) and overall survival (OS). Results: The KRT81 (rs3660) GG genotype was associated with an increased risk of neurological toxicity (P=0.016), while patients with XPO5 (rs11077) AA or CC genotypes had a higher rate of bleomycin-associated pulmonary toxicity (P=0.048). Both miR-SNPs emerged as independent factors in the multivariate analysis. The XPO5 AA and CC genotypes were also associated with a lower response rate (P=0.036). XPO5 (P=0.039) and TRBP (rs784567) (P=0.022) genotypes emerged as prognostic markers for DFS, and XPO5 was also associated with OS (P=0.033). In the multivariate analysis, only XPO5 emerged as an independent prognostic factor for DFS (HR: 2.622; 95%CI 1.039-6.620; P=0.041). Given the influence of XPO5 and TRBP as individual markers, we then investigated the combined effect of these miR-SNPs. Patients with both the XPO5 AA/CC and TRBP TT/TC genotypes had the shortest DFS (P=0.008) and OS (P=0.008). Conclusion: miR-SNPs can add useful prognostic information on treatment-related toxicity and clinical outcome in Hodgkin lymphoma and can be used to identify patients likely to be chemoresistant or to relapse.

El paper del fenomen angiogènic en els diferents tumors sòlids ha estat l’objectiu de nombrosos estudis en els darrers anys. La importància d’aquest procés en les neoplàsies d’origen hematològic encara és bastant desconeguda. En aquest treball hem estudiat l’expressió immunohistoquímica del factor de creixement de l’endoteli vascular (VEGF) en els “tissue microarrays” (TMA) de 252 pacients diagnosticats de limfoma de Hodgkin tractats de forma homogènia (poliquimioteràpia esquema ADVD amb o sense radioteràpia) entre els anys 1978 i 2003. Els resultats s’han correlacionat amb l’evolució dels pacients i s’ha avaluat la seva importància en la supervivència lliure de malaltia i supervivencia global de la sèrie.

Background: Mantle cell lymphoma (MCL) is genetically characterized by the t(11;14)(q13;q32) translocation and a high number of secondary chromosomal alterations. The contribution of DNA methylation to MCL lymphomagenesis is not well known. We sought to identify epigenetically silenced genes in these tumours that might have clinical relevance. Methodology/Principal Findings: To identify potential methylated genes in MCL we initially investigated seven MCL cell lines treated with epigenetic drugs and gene expression microarray profiling. The methylation status of selected candidate genes was validated by a quantitative assay and subsequently analyzed in a series of primary MCL (n=38). After pharmacological reversion we identified 252 potentially methylated genes. The methylation analysis of a subset of these genes (n=25) in the MCL cell lines and normal B lymphocytes confirmed that 80% of them were methylated in the cell lines but not in normal lymphocytes. The subsequent analysis in primary MCL identified five genes (SOX9,HOXA9,AHR,NR2F2 ,and ROBO1) frequently methylated in these tumours. The gene methylation events tended to occur in the same primary neoplasms and correlated with higher proliferation, increased number of chromosomal abnormalities, and shorter survival of the patients. Conclusions: We have identified a set of genes whose methylation degree and gene expression levels correlate with aggressive clinicopathological features of MCL. Our findings also suggest that a subset of MCL might show a CpG island methylator phenotype (CIMP) that may influence the behaviour of the tumours.

El limfoma de cèl•lules de mantell (LCM) és un limfoma de cèl•lules B incurable que presenta sobreexpressió de ciclina D1. Això fa necessari el desenvolupament de noves teràpies. Els gens supressors de tumors estan alterats en càncer pel silenciament epigenètic aberrant, com a conseqüència de la desacetilació de les histones dels seus promotors. Els inhibidors de les desacetilases d'histones (HDACi) són nous compostos amb resultats prometedors per al tractament de tumors. L'objectiu principal, i que ha durat 7 mesos, va ser analitzar l'activitat antitumoral de l'àcid hidroxàmic suberoilanílid (SAHA, vorinostat), un HDACi en fase d'assajos clínics per al tractament de varis tumors, en cèl•lules de LCM. Es va analitzar la sensibilitat al SAHA (Merck Pharmaceuticals) en nou línies cel•lulars humanes de LCM, que es diferenciaven en les alteracions genètiques, les característiques replicatives i la sensibilitat als fàrmacs; i cèl•lules primàries de 6 pacients. El SAHA va presentar un efecte citotòxic heterogeni amb DL50 (Dosi Letal 50) de 3.25 μM a &25 μM amb 24 d'incubació. Aquest efecte citotòxic s'incrementava notablement després de 48 hores d'incubació assolint una DL50 de 0.34 a 5.69 μM. Cal destacar que 5 dels 6 casos de les mostres primàries de LCM van mostrar una elevada sensibilitat (DL50 & 8.07 μM). A nivell mecanistic, el SAHA va augmentar l'acetilació de les histones H3 i H4, i va disminuir els nivells de proteïna de la ciclina D1 i c-Flip. La citometria de flux i els anàlisis per Western Blot van posar de manifest que l'efecte citotòxic del SAHA es dóna a través de l'activació de la via mitocondrial de mort cel•lular i la cascada de caspases. El SAHA indueix l'expressió transcripcional de la proteïna proapoptòtica Bmf. Aquests resultats suggereixen que el SAHA podria ser una nova teràpia prometedora per al tractament del LCM.

Anaplastic lymphoma kinase (ALK) rearrangements represents a new driver oncogenic event in non-small cell lung cancer (NSCLC). ALK positive patients account for a 1-7% of NSCLC patients. The objective of this study is to know the prevalence and clinical characteristics of ALK positive patients in a cohort of NSCLC patients and to compare inmunohistochemistry with D5F3 monoclonal antibody with gold standard method fluorescence in situ hybridation

DNA methylation has an important impact on normal cell physiology, thus any defects in this mechanism may be related to the development of various diseases In this project we are interested in identifying epigeneticaliy modified genes, in general controlled by processes related to the DNA methylation, by means of a new strategy combining protomic and genomic analyses. First, the two Dimensional-Difference Gel Electrophoresis (2-DIGE) protein analyses of extracts obtained from HCT-116 wt and double knockout for DNMT1 and DNMT3b (DKO) cells revealed 34 proteins overexpressed in the condition of DNMTs depletion. From five genes with higher transcript lavels in DKO cells, comparing with HCT-116 wt. oniy AKR1B1, UCHLl and VIM are melhylated in HCT-116. As expected. the DNA methvlation 1s lost in DKO cells. The rneth,vl ation of VIM and UCHLl promoters in some cancer samples has already been repaired, thus further studies has been focused on AKRlBI. AKR1B1 expression due lo DNA methyiaton of promoter region seems to occur specilfically in the colon cancer cell Iines. which was confirmed in the DNA rnethylation status and expression analyses. performed on 32 different cancer cell lines (including colon, breast, lymphoma, leukemia, neuroblastoma, glioma and lung cancer cell Iines) as well as normal colon and normal lymphocytes samples. AKRIBI expression after treatments with DNA demethvlating agent (AZA) was rescued in 5 coloncancer cell lines (including genetic regulation of the candidate gene. The methylation status of the rest of the genes identified in proteomic analysis was checked by methylation specific PCR (MSP) experiment and all appeared to be unmethylated. The similar research has been done also bv means of Mecp2-null mouse model For 14 selected candidate genes the analyses of expression leveis, methylation Status and MeCP2 interaction with promoters are currently being performed.

Background: Hospitals in countries with public health systems have recently adopted organizational changes to improve efficiency and resource allocation, and reducing inappropriate hospitalizations has been established as an important goal. AIMS: Our goal was to describe the functioning of a Quick Diagnosis Unit in a Spanish public university hospital after evaluating 1,000 consecutive patients. We also aimed to ascertain the degree of satisfaction among Quick Diagnosis Unit patients and the costs of the model compared to conventional hospitalization practices. DESIGN: Observational, descriptive study. METHODS: Our sample comprised 1,000 patients evaluated between November 2008 and January 2010 in the Quick Diagnosis Unit of a tertiary university public hospital in Barcelona. Included patients were those who had potentially severe diseases and would normally require hospital admission for diagnosis but whose general condition allowed outpatient treatment. We analyzed several variables, including time to diagnosis, final diagnoses and hospitalizations avoided, and we also investigated the mean cost (as compared to conventional hospitalization) and the patients' satisfaction. RESULTS: In 88% of cases, the reasons for consultation were anemia, anorexia-cachexia syndrome, febrile syndrome, adenopathies, abdominal pain, chronic diarrhea and lung abnormalities. The most frequent diagnoses were cancer (18.8%; mainly colon cancer and lymphoma) and Iron-deficiency anemia (18%). The mean time to diagnosis was 9.2 days (range 1 to 19 days). An estimated 12.5 admissions/day in a one-year period (in the internal medicine department) were avoided. In a subgroup analysis, the mean cost per process (admission-discharge) for a conventional hospitalization was 3,416.13 Euros, while it was 735.65 Euros in the Quick Diagnosis Unit. Patients expressed a high degree of satisfaction with Quick Diagnosis Unit care. CONCLUSIONS: Quick Diagnosis Units represent a useful and cost-saving model for the diagnostic study of patients with potentially severe diseases. Future randomized study designs involving comparisons between controls and intervention groups would help elucidate the usefulness of Quick Diagnosis Units as an alternative to conventional hospitalization.

Con esta revisión se pretende remarcar al odontoestomatólogo los aspectos clínicos que con mayor frecuencia se asocian a la infección producida por el VIH. Entre ellos cabe resaltar las infecciones producidas por virus (herpes simple, herpes varicela-zoster, citomegalovirus. papilomavirus), hongos (sobre todo candidiasis) y bacterias (caries y patología periodontal); la patología tumoral asociada (sarcoma de Kaposi, linfoma no Hodgkin, carcinomas); así como otras manifestaciones patológicas que tienen su asiento en el territorio del aparato estomatognático (ulceraciones atípicas, alteraciones neurológicas, patología glandular. etc.). No puede concluirse el trabajo sin recordar la importancia que tiene seguir un protocolo en la atención odontológica de estos pacientes seropositivos, que debería hacerse extensivo al resto de nuestros pacientes.

Helicobacter pylori (H. pylori) is a gram negative bacteria that represents a considerable global burden in the world and is related to many gastrointestinal diseases (peptic ulcer, gastric MALT lymphoma or gastric cancer). Currently the triple standard therapy is less used as there is an increase of the clarithromycin resistance. Therefore patients have to receive several lines of treatment with the consequence of adverse events and the possibility to interrupt the treatment. This is why the main objective is to determine if making a culture and antibiogram to do a targeted treatment cause less adverse events with the same eradication than making an empirical treatment to eradicate H. pylori. The secondary objective is to determine the prevalence of resistance to clarithromycin in the province of GironaThis is a multicentre clinical trial without blinding; patients are selected by non-probabilistic sampling, with a total sample of 868 patients randomized in two equal groups of 434 patients in each group. The study will last 2 years. The endpoints will be to evaluate the adverse events and eradication of each group of patients. Also it will be evaluated the resistance to clarithromycin
