L'autor exposa el treball de camp dut a terme durant més de cent hores de pràctiques en un centre de salut mental, CSAM de Manresa. S'inicia amb una visió general del centre, la seva història i els seus òrgans de funcionament. Es fa una petita exposició dels motius de l'elecció per part de l'alumne, l'orientació teòrica del centre, així com la metodologia utilitzada i la temporalització amb la qual s'ha dut a terme la tasca i, finalment, un seguiment i alguns resultats obtinguts de les sessions que s'han dut a terme en les àrees infanto-juvenil i d'addicció i control d'impulsos en adults. Acaba el treball amb una reflexió per als futurs estudiants de psicologia, en l'àrea clínica: Lluiteu, lluitem tots junts "por un prácticum clínico que no convierta a los estudiantes de psicología en pedigüeños de las prácticas y los coloquen en la zozobra inquieta que les lleve a un peregrinaje absurdo por diversos centros públicos o privados, a la búsqueda de algún paciente al que puedan simplemente observar" (De Josep Mª Farré, Psiquiatre de l'Institut Dexeus, en el llibre de María Dolores Avia (dir), "Cartas a un joven psicólogo", en la seva segonda edició).

Background Depression is one of the more severe and serious health problems because of its morbidity, disabling effects and for its societal and economic burden. Despite the variety of existing pharmacological and psychological treatments, most of the cases evolve with only partial remission, relapse and recurrence. Cognitive models have contributed significantly to the understanding of unipolar depression and its psychological treatment. However, success is only partial and many authors affirm the need to improve those models and also the treatment programs derived from them. One of the issues that requires further elaboration is the difficulty these patients experience in responding to treatment and in maintaining therapeutic gains across time without relapse or recurrence. Our research group has been working on the notion of cognitive conflict viewed as personal dilemmas according to personal construct theory. We use a novel method for identifying those conflicts using the repertory grid technique (RGT). Preliminary results with depressive patients show that about 90% of them have one or more of those conflicts. This fact might explain the blockage and the difficult progress of these patients, especially the more severe and/or chronic. These results justify the need for specific interventions focused on the resolution of these internal conflicts. This study aims to empirically test the hypothesis that an intervention focused on the dilemma(s) specifically detected for each patient will enhance the efficacy of cognitive behavioral therapy (CBT) for depression. Design A therapy manual for a dilemma-focused intervention will be tested using a randomized clinical trial by comparing the outcome of two treatment conditions: combined group CBT (eight, 2-hour weekly sessions) plus individual dilemma-focused therapy (eight, 1-hour weekly sessions) and CBT alone (eight, 2-hour group weekly sessions plus eight, 1-hour individual weekly sessions). Method Participants are patients aged over 18 years meeting diagnostic criteria for major depressive disorder or dysthymic disorder, with a score of 19 or above on the Beck depression inventory, second edition (BDI-II) and presenting at least one cognitive conflict (implicative dilemma or dilemmatic construct) as assessed using the RGT. The BDI-II is the primary outcome measure, collected at baseline, at the end of therapy, and at 3- and 12-month follow-up; other secondary measures are also used. Discussion We expect that adding a dilemma-focused intervention to CBT will increase the efficacy of one of the more prestigious therapies for depression, thus resulting in a significant contribution to the psychological treatment of depression. En este estudio se compara la intervención psicológica por excelencia en casos de obesidad y sobrepeso, la intervención cognitivoconductual, con la intervención humanista. Mediante el seguimiento de tres grupos de investigación a los que se prescriben restricciones propias de una dieta hipocalórica, uno sometido a terapia humanista individual, otro sometido a terapia cognitivoconductual grupal y un tercero sometido a una formación nutricional (grupo de cuasicontrol), este estudio pretende determinar si un enfoque humanista y la atención pormenorizada a la idiosincrasia del cliente marcan la diferencia en el tratamiento de la obesidad y/o el sobrepeso.

Projecte de recerca elaborat a partir d’una estada al Pain Management Unit de la University of Bath-Royal National Hospital for Rheumatic Disease, a Gran Bretanya, entre juliol i setembre del 2006. El dolor crònic en pediatria es defineix com aquell que és persistent o recurrent durant tres o més mesos. Recentment la prevalença a la nostra població entre els escolars de 8 a 16 anys ha estat quantificada en el 37.3%. Davant d’aquestes dades i coneixent la magnitud de l’impacte que el dolor crònic té en aquestes edats, sorgeix la necessitat de desenvolupar programes d’intervenció per donar una resposta a aquesta problemàtica en la nostra població. Les investigacions realitzades assenyalen que els programes multidisciplinars són els que obtenen una major eficàcia. Aquests programes estan adreçats a minimitzar l’impacte dels diferents factors que conformen l’experiència de dolor: físics, emocionals, cognitius, conductuals i socials. A Europa només l’hospital on s’ha realitzat l’estada ofereix un programa d’aquestes característiques. El servei que ofereixen en aquesta Unitat de dolor pediàtric està sent el model de referència pel disseny d’un programa de tractament a la nostra població. Per aquest motiu, s’ha realitzat una estada d’un mes de durada a la PMU, amb l’objectiu d’aprendre els procediments terapèutics per adaptar-los i aplicar-los en el nostre context. Sis adolescents amb problemes de dolor crònic i discapacitat associada, acompanyats de les seves mares han participat en aquest programa de tractament grupal interdisciplinari d’orientació cognitiu-conductual de tres setmanes. Es realitzen aproximadament 110 hores de tractament, distribuïdes en sessions de 50 minuts, d’activitat física i ocupacional, teràpia cognitiu-conductual i educació. Aquesta estada ha permès d’una banda, la formació d’un psicòleg dintre d’un equip de dolor pediàtric interdisciplinar i de l’altra evidenciar l’efectivitat que aquest programa interdisciplinar de rehabilitació cognitiu-conductual té pel maneig del dolor crònic i la discapacitat associada.

Aquest projecte final de Pràcticum I de Psicologia de l'Educació explica totes les activitats que he dut a terme al llarg de la meva estada en practiques al centre Orienta. S'hi descriuen les característiques més rellevants del centre, així com els serveis que ofereix a la comunitat educativa, molt especialment els adreçats a ajudar els infants a millorar les seves capacitats d'aprenentatge, des d'un marc teòric cognitiu-conductual i sistèmic, tot emfatitzant el treball col·laboratiu que mantenen els professionals de l'educació amb les famílies dels nens. El projecte detalla les activitats de col·laboració en la intervenció que he realitzat des de l'àmbit educatiu de la Psicologia, la seva temporització i seguiment i finalment, les conclusions i prospectiva envers el treball realitzat, així com la valoració crítica que en faig de la meva estada en pràctiques.

La recerca presenta el resultat de la segona aplicació del Programa del Pensament Prosocial, Versió Curta per a Joves en una mostra de menors que han complert mesures judicials en un equip de Medi Obert de Lleida. Aquest programa cognitiu-conductual entrena en l’aprenentatge d’habilitats sociocognitives i valors que es requereixen per la competència social i s’ha aplicat amb èxit a joves infractors i adults de 14 països de tot el món a més de quaranta mil subjectes. La recerca aplica el programa utilitzant com a mètode un disseny pre-test i post-test amb grup de control. Dels resultats es destaca que l’aplicació del programa incorpora en la intervenció en medi obert un canvi important en el plantejament de treball, ja que els instruments estan específicament dissenyats per a menors que s’han mostrat més resistents a altres intervencions.

Objective: The aim of the current study was to investigate the long-term cognitive effects of electroconvulsive therapy (ECT) in a sample of adolescent patients in whom schizophrenia spectrum disorders were diagnosed. Methods: The sample was composed of nine adolescent subjects in whom schizophrenia or schizoaffective disorder was diagnosed according to DSM-IV-TR criteria on whom ECT was conducted (ECT group) and nine adolescent subjects matched by age, socioeconomic status, and diagnostic and Positive and Negative Syndrome Scale (PANSS) total score at baseline on whom ECT was not conducted (NECT group). Clinical and neuropsychological assessments were carried out at baseline before ECT treatment and at 2-year follow-up. Results: Significant differences were found between groups in the number of unsuccessful medication trials. No statistically significant differences were found between the ECT group and theNECT group in either severity as assessed by the PANSS, or in any cognitive variables at baseline.At follow-up, both groups showed significant improvement in clinical variables (subscales of positive, general, and total scores of PANSS and Clinical Global Impressions-Improvement). In the cognitive assessment at follow-up, significant improvement was found in both groups in the semantic category of verbal fluency task and digits forward. However, no significant differences were found between groups in any clinical or cognitive variable at follow-up. Repeated measures analysis found no significant interaction of time · group in any clinical or neuropsychological measures. Conclusions: The current study showed no significant differences in change over time in clinical or neuropsychological variables between the ECT group and the NECT group at 2-year follow-up. Thus, ECT did not show any negative influence on long-term neuropsychological variables in our sample.

Background: Atypical antipsychotics provide better control of the negative and affective symptoms of schizophrenia when compared with conventional neuroleptics; nevertheless, their heightened ability to improve cognitive dysfunction remains a matter of debate. This study aimed to examine the changes in cognition associated with long-term antipsychotic treatment and to evaluate the effect of the type of antipsychotic (conventional versus novel antipsychotic drugs) on cognitive performance over time. Methods: In this naturalistic study, we used a comprehensive neuropsychological battery of tests to assess a sample of schizophrenia patients taking either conventional (n = 13) or novel antipsychotics (n = 26) at baseline and at two years after. Results: Continuous antipsychotic treatment regardless of class was associated with improvement on verbal fluency, executive functions, and visual and verbal memory. Patients taking atypical antipsychotics did not show greater cognitive enhancement over two years than patients taking conventional antipsychotics. Conclusions Although long-term antipsychotic treatment slightly improved cognitive function, the switch from conventional to atypical antipsychotic treatment should not be based exclusively on the presence of these cognitive deficits.

The Cognitive Reflection Test (CRT) is a test introduced by S. Frederick (2005) Cognitive reflection and decision making, J Econ Perspect 19(4): 25-42. The task is designed to measure the tendency to override an intuitive response that is incorrect and to engage in further reflection that leads to the correct response. The consistent sex differences in CRT performance may suggest a role for gonadal hormones, particularly testosterone. A now widely studied putative marker for fetal testosterone is the second-to-fourth digit ratio (2D:4D). This paper tests to what extent 2D:4D, as a proxy for prenatal exposure to testosterone, can predict CRT scores in a sample of 623 students. After controlling for sex, we observe that a lower 2D:4D (reflecting a higher exposure to testosterone) is significantly associated with a higher number of correct answers. The result holds for both hands? 2D:4Ds. In addition, the effect appears to be sharper for females than for males. We also control for patience and math proficiency, which are significantly related to performance in the CRT. But the effect of 2D:4D on performance in CRT is not reduced with these controls, implying that these variables are not mediating the relationship between digit ratio and CRT.

Playing a musical instrument demands the engagement of different neural systems. Recent studies about the musician"s brain and musical training highlight that this activity requires the close interaction between motor and somatosensory systems. Moreover, neuroplastic changes have been reported in motor-related areas after short and long-term musical training. Because of its capacity to promote neuroplastic changes, music has been used in the context of stroke neurorehabilitation. The majority of patients suffering from a stroke have motor impairments, preventing them to live independently. Thus, there is an increasing demand for effective restorative interventions for neurological deficits. Music-supported Therapy (MST) has been recently developed to restore motor deficits. We report data of a selected sample of stroke patients who have been enrolled in a MST program (1 month intense music learning). Prior to and after the therapy, patients were evaluated with different behavioral motor tests. Transcranial Magnetic Stimulation (TMS) was applied to evaluate changes in the sensorimotor representations underlying the motor gains observed. Several parameters of excitability of the motor cortex were assessed as well as the cortical somatotopic representation of a muscle in the affected hand. Our results revealed that participants obtained significant motor improvements in the paretic hand and those changes were accompanied by changes in the excitability of the motor cortex. Thus, MST leads to neuroplastic changes in the motor cortex of stroke patients which may explain its efficacy.

Body percussion using to the BAPNE method is a means of cognitive stimulation with multiple applications. The aim of this research is to assess their full potential as a source of therapy. The methodology used is theoretical in nature and makes use of a wide bibliography to find evidence for its therapeutic effect. In essence, body percussion can be seen to lead to improvements in three areas. the Physical, as it stimulates awareness of the body, control of movement and muscular strength, coordination and balance; the Mental, as it improves concentration, memory and perception; and finally Socio-affective, as it helps to build egalitarian relationships and leads to a decrease in anxiety in social interactions. This means of therapy has several different uses and it is targeted at different groups. In the present investigation we categorise them into five main groups: individuals with neurodegenerative diseases like Alzheimer's or Parkinson's disease; individuals with learning disorders such as dyslexia or ADHD; patients affected by diseases of the spinal cord, cranial neuropathies and trauma (Neurorehabilitation); and for the treatment of addictive behavior (addiction); and depressive disorders or anxiety disorders.After thorough analysis, we have found scientific evidence that the therapeutic body percussion using the BAPNE method improves the quality of life of patients and it is an important factor in stabilizing the development of different diseases.In addition, evidence involving certain biological indicators (in control and experimental groups, and through a pre-test and post-test) show its effect on levels of stress and anxiety (reduction of cortisol), as well as improvement of social relations as a result of working as a group (increased levels of oxytocin), and improvements seen in self-esteem and in a variety of personal aspects through the Aspects of Identity questionnaire.

This paper intends to elaborate the relationship between Kelly's Personal Construct Theory (PCT) and the systemic therapies beyond their notable similarities. Kelly's constructive alternativism is situated in the context of the current constructivist orientation that the family therapy movement seems to be adopting. A model of change is presented based on PCT's experience cycle. From this cycle, the relationship between behaviors and constructions is elaborated incorporating Procter's (52, 53) notions of the Family Construct System (FCS) andposition. This model allows for interventions both at behavioral and construction levels, as well as allowing for a certain technical eclecticism while, at the same time, retaining a strong theoretical coherence. This approach is discussed in the context of the debate about strategizing, power, and control held by authors such as Golann, Hoffman, and Tomm. Finally, some implications for research are outlined.
