Mitjançant les tècniques de visió per computador aquest projecte pretén desenvolupar una aplicació capaç de segmentar la pell, detectar nevus (pigues i altres taques) i poder comparar imatges de pacients amb risc de contreure melanoma preses en moments diferents. Aquest projecte pretén oferir diferents eines informàtiques als dermatòlegs per a propòsits relacionats amb la investigació. L’ objectiu principal d’ aquest projecte és desenvolupar un sistema informàtic que proporcioni als dermatòlegs agilitat a l’hora de gestionar les dades dels pacients amb les sevesimatges corresponents, ajudar-los en la realització de deteccions dels nevus d’aquestes imatges, i ajudar-los en la comparació d’exploracions (amb les deteccions realitzades)de diferents èpoques d’un mateix pacient

Un dels camps que aquest projecte ha estat treballant és el de la telemedicina. Aquest projecte presenta una aplicació per assistir al personal mèdic rehabilitador en tasques de telerehabilitació (i teleassistència) fent servir una connexió bàsica ADSL i hardware comú. És la continuació del projecte TeleRehabilitació i Esclerosi Múltiple (TRiEM), un esforç conjunt entre el grup de recerca Comunicacions i Sistemes Distribuïts (BCDS en anglès) de la Universitat de Girona (UdG) i de la Fundació Esclerosi Múltiple (FEM) per desenvolupar una eina per ajudar a tasques de rehabilitació de l’Esclerosi Múltiple. L’aplicació AXARM és una iniciativa del grup de Comunicacions i Sistemes Distribuïts de la Universitat de Girona per impulsar una eina d’assistència telemàtica entre doctors i pacients. Facilita una eina útil als especialistes d’un centre per realitzar tasques de rehabilitació, assistència remota o monitorització (sanitària, assistencial o d’una altra mena) amb pacients que es trobin en un altre punt físic a través d’Internet

El objeto de estudio del presente trabajo era: analizar la ética comunicativa en un hospital terciario de 1200 camas ubicado en una ciudad de la Comunidad Autónoma Vasca.Los objetivos marcados eran: - Conseguir información de cómo se realizaba esa comunicación entre pacientes-usuarios/as yfacultativos/as. - Clarificar los posibles problemas existentes en torno a la comunicación.- Conocer cuál era la percepción de los pacientes y sus familias, en relación a la comunicación médico-paciente-familia.- Conocer las circunstancias en las que sedesarrollaba la comunicación médico-pacientefamilia.

Scopolamine is an alkaloid widely used in medicine for its anticholinergic activity. The aim of this review is to show that metabolic engineering techniques constitute a suitable tool to improve the production of tropane alkaloids, focusing in particular on scopolamine. We present an overview of results obtained by various research groups, including our own, who have studied the overexpression of genes involved in the biosynthesis of scopolamine in different plant species that produce tropane alkaloids. Experiments carried out to improve production in hairy root cultures will also be described, as well as those attempting to biotransform hyoscyamine into scopolamine in roots and transgenic tobacco cells.

Sessió clínica a un EAP amb dos objectius: proporcionar als professionals sanitaris de l'EAP orientacions bàsiques per ajudar els pacients a avaluar la informació sobre temes de salut localitzada a Internet, i promoure entre els professionals sanitaris l'ús de la prescripció web per formentar l'autonomia dels pacients. El contingut de la sessió va ser: breu introducció a Internet, tendències actuals en e-salut, criteris d'avaluació de pàgines web, segells de qualitat i prescripció web.

Purpose: To assess the feasibility of a method based on microwave spectrometry to detect structural distortions of metallic stents in open air conditions and envisage the prospects of this approach toward possible medical applicability for the evaluation of implanted stents. Methods: Microwave absorbance spectra between 2.0 and 18.0 GHz were acquired in open air for the characterization of a set of commercial stents using a specifically design setup. Rotating each sample over 360º, 2D absorbance diagrams were generated as a function of frequency and rotation angle. To check our approach for detecting changes in stent length (fracture) and diameter (recoil), two specific tests were performed in open air. Finally, with a few adjustments, this same system provides 2D absorbance diagrams of stents immersed in a water-based phantom, this time over a bandwidth ranging from 0.2 to 1.8 GHz. Results: The authors show that metallic stents exhibit characteristic resonant frequencies in their microwave absorbance spectra in open air which depend on their length and, as a result, may reflect the occurrence of structural distortions. These resonances can be understood considering that such devices behave like dipole antennas in terms of microwave scattering. From fracture tests, the authors infer that microwave spectrometry provides signs of presence of Type I to Type IV stent fractures and allows in particular a quantitative evaluation of Type III and Type IV fractures. Recoil tests show that microwave spectrometry seems able to provide some quantitative assessment of diametrical shrinkage, but only if it involves longitudinal shortening. Finally, the authors observe that the resonant frequencies of stents placed inside the phantom shift down with respect to the corresponding open air frequencies, as it should be expected considering the increase of dielectric permittivity from air to water. Conclusions: The evaluation of stent resonant frequencies provided by microwave spectrometry allows detection and some quantitative assessment of stent fracture and recoil in open air conditions. Resonances of stents immersed in water can be also detected and their characteristic frequencies are in good agreement with theoretical estimates. Although these are promising results, further verifica tion in a more relevant phantom is required in order to foresee the real potential of this approach.

The control of the right application of medical protocols is a key issue in hospital environments. For the automated monitoring of medical protocols, we need a domain-independent language for their representation and a fully, or semi, autonomous system that understands the protocols and supervises their application. In this paper we describe a specification language and a multi-agent system architecture for monitoring medical protocols. We model medical services in hospital environments as specialized domain agents and interpret a medical protocol as a negotiation process between agents. A medical service can be involved in multiple medical protocols, and so specialized domain agents are independent of negotiation processes and autonomous system agents perform monitoring tasks. We present the detailed architecture of the system agents and of an important domain agent, the database broker agent, that is responsible of obtaining relevant information about the clinical history of patients. We also describe how we tackle the problems of privacy, integrity and authentication during the process of exchanging information between agents.

Diferentes estudios han demostrado que, en ocasiones, los pacientes tienen dificultades a la hora de entender lo que el médico quiere comunicarles. El presente artículo intenta reflexionar sobre cuáles son los determinantes que pueden influir en el proceso de comunicación del riesgo en pacientes que tienen antecedentes familiares de cáncer y acuden a las Unidades de Consejo Genético en busca de asesoramiento. Existen estudios que señalan que los pacientes con antecedentes familiares de cáncer se preocupan por padecer dicha enfermedad. Sólo algunos de ellos han explorado cómo los pacientes entienden y perciben el concepto de riesgo y los posibles sesgos asociados a él. Como primeras aproximaciones, creemos que es necesario utilizar un lenguaje sencillo, claro y fácil de entender por parte de los pacientes, huyendo del lenguaje epidemiológico o médico. Del mismo modo es importante diseñar programas de formación psicoeducativos y material de soporte adecuado, así como el correcto manejo del counselling como herramienta principal de trabajo por parte de los profesionales y fomentar futuras investigaciones en este área determinada.

This paper describes an evaluation framework that allows a standardized and quantitative comparison of IVUS lumen and media segmentation algorithms. This framework has been introduced at the MICCAI 2011 Computing and Visualization for (Intra)Vascular Imaging (CVII) workshop, comparing the results of eight teams that participated. We describe the available data-base comprising of multi-center, multi-vendor and multi-frequency IVUS datasets, their acquisition, the creation of the reference standard and the evaluation measures. The approaches address segmentation of the lumen, the media, or both borders; semi- or fully-automatic operation; and 2-D vs. 3-D methodology. Three performance measures for quantitative analysis have been proposed. The results of the evaluation indicate that segmentation of the vessel lumen and media is possible with an accuracy that is comparable to manual annotation when semi-automatic methods are used, as well as encouraging results can be obtained also in case of fully-automatic segmentation. The analysis performed in this paper also highlights the challenges in IVUS segmentation that remains to be solved.

Background: Antiretroviral therapy has changed the natural history of human immunodeficiency virus (HIV) infection in developed countries, where it has become a chronic disease. This clinical scenario requires a new approach to simplify follow-up appointments and facilitate access to healthcare professionals. Methodology: We developed a new internet-based home care model covering the entire management of chronic HIV-infected patients. This was called Virtual Hospital. We report the results of a prospective randomised study performed over two years, comparing standard care received by HIV-infected patients with Virtual Hospital care. HIV-infected patients with access to a computer and broadband were randomised to be monitored either through Virtual Hospital (Arm I) or through standard care at the day hospital (Arm II). After one year of follow up, patients switched their care to the other arm. Virtual Hospital offered four main services: Virtual Consultations, Telepharmacy, Virtual Library and Virtual Community. A technical and clinical evaluation of Virtual Hospital was carried out. Findings: Of the 83 randomised patients, 42 were monitored during the first year through Virtual Hospital (Arm I) and 41 through standard care (Arm II). Baseline characteristics of patients were similar in the two arms. The level of technical satisfaction with the virtual system was high: 85% of patients considered that Virtual Hospital improved their access to clinical data and they felt comfortable with the videoconference system. Neither clinical parameters [level of CD4 + T lymphocytes, proportion of patients with an undetectable level of viral load (p = 0.21) and compliance levels 90% (p = 0.58)] nor the evaluation of quality of life or psychological questionnaires changed significantly between the two types of care. Conclusions: Virtual Hospital is a feasible and safe tool for the multidisciplinary home care of chronic HIV patients. Telemedicine should be considered as an appropriate support service for the management of chronic HIV infection.

La parada cardiorrespiratoria (PCR) dentro del hospital es considerada una emergencia vital y está demostrado que existe una relación directa entre la respuesta asistencial y la mortalidad asociada a este evento. Los resultados del tratamiento de la PCR son un indicador de calidad de los centros sanitarios. En el caso en concreto de España, la mayoría de hospitales no disponen de un sistema organizado e integral de atención a la PCR. A pesar de los avances en medicina y tecnológicos, la tasa de supervivencia no ha variado significativamente en los últimos 30 años por lo que se ha de considerar como un problema social, económico y sanitario de gran magnitud que cabe abordar con todas las herramientas disponibles. Objetivos: Conocer cómo se organiza la reanimación cardiopulmonar (RCP) dentro del hospital y conocer los planes integrales de RCP publicados por los hospitales españoles. Metodología: Se realiza una revisión de la literatura a través de las bibliotecas indexadas Pubmed y Web of Science mediante criterios de inclusión/exclusión, uso de operadores booleanos y búsqueda bibliográfica manual. Además se realiza una entrevista a un médico referente nacional en la investigación sobre RCP. Resultados: Se han encontrado 7 planes integrales para la atención a la PCR y solo 2 de ellos están avalados por el Plan Nacional de RCP (PNRCP), miembro del Consejo Español de RCP (CERCP). Conclusiones: Existe una falta de publicaciones por parte de los hospitales españoles sobre planes integrales para la organización de la RCP que cumplan los criterios de calidad necesarios. Si bien todos los hospitales disponen de protocolos propios dirigidos a sus profesionales para la actuación en caso de PCR, estos son de uso interno y solo tienen en común entre ellos el hecho de seguir las recomendaciones de las guías del European Resuscitation Council (ERC). III Es necesario que los centros sanitarios fomenten y compartan su actividad investigadora sobre el tema. A new approach to mammographic mass detection is presented in this paper. Although different algorithms have been proposed for such a task, most of them are application dependent. In contrast, our approach makes use of a kindred topic in computer vision adapted to our particular problem. In this sense, we translate the eigenfaces approach for face detection/classification problems to a mass detection. Two different databases were used to show the robustness of the approach. The first one consisted on a set of 160 regions of interest (RoIs) extracted from the MIAS database, being 40 of them with confirmed masses and the rest normal tissue. The second set of RoIs was extracted from the DDSM database, and contained 196 RoIs containing masses and 392 with normal, but suspicious regions. Initial results demonstrate the feasibility of using such approach with performances comparable to other algorithms, with the advantage of being a more general, simple and cost-effective approach

El càncer de mama és una de les causes de més mortalitat entreles dones dels països desenvolupats. És tractat d'una maneramés eficient quan es fa una detecció precoç, on les tècniques d'imatge són molt importants. Una de les tècniques d'imatge més utilitzades després dels raigs-X són els ultrasons. A l'hora de fer un processat d'imatges d'ultrasò, els experts en aquest camp es troben amb una sèrie de limitacions en el moment d'utilitzar uns filtrats per les imatges, quan es fa ús de determinades eines. Una d'aquestes limitacions consisteix en la falta d'interactivitat que aquestes ens ofereixen. Per tal de solventar aquestes limitacions, s'ha desenvolupat una eina interactiva que permet explorar el mapa de paràmetres visualitzant el resultat del filtrat en temps real, d'una manera dinàmica i intuïtiva. Aquesta eina s'ha desenvolupat dins l'entorn de visualització d'imatge mèdica MeVisLab. El MeVisLab és un entorn molt potent i modular pel desenvolupament d'algorismes de processat d'imatges, visualització i mètodes d'interacció, especialment enfocats a la imatge mèdica. A més del processament bàsic d'imatges i de mòduls de visualització, inclou algorismes avançats de segmentació, registre i moltes análisis morfològiques i funcionals de les imatges.S'ha dut a terme un experiment amb quatre experts que, utilitzantl'eina desenvolupada, han escollit els paràmetres que creien adientsper al filtrat d'una sèrie d'imatges d'ultrasò. En aquest experiments'han utilitzat uns filtres que l'entorn MeVisLab ja té implementats:el Bilateral Filter, l'Anisotropic Difusion i una combinació d'un filtrede Mediana i un de Mitjana.Amb l'experiment realitzat, s'ha fet un estudi dels paràmetres capturats i s'han proposat una sèrie d'estimadors que seran favorables en la majoria dels casos per dur a terme el preprocessat d'imatges d'ultrasò

This study sought to assess the impact of health care professional (HCP) communication on breast cancer patients across the acute care process as perceived by patients. Methodological approach was based on eight focus groups conducted with a sample of patients (n ¼ 37) drawn from 15 Spanish Regions; thematic analysis was undertaken using the National Cancer Institute (NCI) framework of HCP communication as the theoretical basis. Relevant results of this study were the identification of four main communication components: (1) reassurance in coping with uncertainty after symptom detection and prompt access until confirmed diagnosis; (2) fostering involvement before delivering treatments, by anticipating information on practical and emotional illness-related issues; (3) guidance on the different therapeutic options, through use of clinical scenarios; and, (4) eliciting the feeling of emotional exhaustion after ending treatments and addressing the management of potential treatment-related effects. These communication-related components highlighted the need for a comprehensive approach in this area of cancer care
