As recomendações da Secção de Neonatologia da Sociedade Portuguesa de Pediatria (SNN-SPP) prevêem a profilaxia medicamentosa da infecção por vírus sincicial respiratório (VSR) com palivizumab em idades gestacionais (IG)inferiores a 30 semanas. Alguns Hospitais de Apoio Perinatal Diferenciado seguem práticas mais restritas, limitando o seu uso à extrema prematuridade e/ou a prematuros com doença pulmonar crónica da prematuridade. Objectivos. Estimar a relação custo-eficácia da profilaxia com palivizumab segundo as recomendações da SNN-SPP, através da aplicação de um modelo teórico a uma coorte real de prematuros. Metodologia. Estudo prospectivo histórico. Coorte de crianças nascidas num Hospital de Apoio Perinatal Diferenciado entre 1/10/2002 e 30/04/2005 com IG ² 35 semanas. Considerou- se caso o internamento no mesmo hospital por bronquiolite por VSR nas épocas 2003/04 e 2004/05. Baseando-nos nas recomendações SNN-SPP e no Number Needed to Treat dos estudos IMpact e IRIS, estimámos a redução prevista nas taxas de hospitalização caso a profilaxia fosse efectuada, comparando os seus custos com a redução de custos de hospitalização. Resultados. Dos 356 recém-nascidos elegíveis, nove foram excluídos por óbito e dois por administração de palivizumab. A taxa de hospitalização por bronquiolite por VSR nas 345 crianças incluídas foi 9,3%. No subgrupo com indicação para profilaxia (26 crianças) a taxa de hospitalização foi 15,4%, com uma estimativa de custo médio de hospitalização de Û 6.542,35. Não ocorreu nenhuma morte por infecção por VSR. A redução estimável no número de hospitalizações sob profilaxia seria de 1,5 (IMpact) ou 2,4 (IRIS). O custo necessário para prevenir um internamento seria de Û 26.263,11 na melhor estimativa e Û 57.716,26 na pior estimativa. Conclusão. Com o modelo desenvolvido, não conseguimos demonstrar nesta coorte uma estimativa de relação custo-eficácia favorável à administração de Palivizumab segundo recomendações da SNN-SPP.

Acquired factor X deficiency is an extremely rare situation. It has shown to be associated with systemic amyloidosis, respiratory mycoplasma infection, factor X inhibitors, antiphospholipid antibodies, vitamin K defi ciency/liver disease as well as the use of certain medications (meropenem, valproic acid). The pathogenesis and transient nature of this deficit remain poorly understood. The authors describe the case of a teenager hospitalised for extensive burns that developed active bleeding after removal of central venous catheter. He was diagnosed with transient factor X deficiency. Normalisation of coagulation status and factor X levels occurred spontaneously 10 days after the bleeding episode.

Primary angle closure occurs as a result of crowded anterior segment anatomy, causing appositional contact between the peripheral iris and trabecular meshwork, thereby obstructing aqueous outflow. Several studies highlight the role of the crystalline lens in its pathogenesis. The objective of this work is to compare the long-term efficacy of phacoemulsification versus laser peripheral iridotomy (LPI) in the management of chronic primary angle closure (CPAC). Prospective case-control study with 30 eyes of 30 patients randomly divided in two groups: 15 eyes in the LPI group and 15 eyes in the IOL group. Patients in the LPI group underwent LPI using argon and Nd:YAG laser. Patients in the IOL group underwent phacoemulsification with posterior chamber intraocular lens (IOL) implantation. Examinations before and after the procedure included gonioscopy, Goldmann applanation tonometry, and anterior chamber evaluation using the Pentacam rotating Scheimpflug camera. The mean follow-up time was 31.13 ± 4.97 months. There was a statistically significant reduction in the intraocular pressure (IOP) and number of anti-glaucoma medications (p < 0.01) only in the IOL group. Anterior chamber depth, angle, and volume were all higher in the IOL group (p < 0.01) at the end of the follow-up period. Phacoemulsification with posterior chamber IOL implantation results in a higher anterior chamber depth, angle, and volume, when compared to LPI. Consequently, phacoemulsification has greater efficacy in lowering IOP and preventing its long-term increase in patients with CPAC and cataract.

The ovarian cystic teratoma is a rare cause of autoimmune haemolytic anaemia by warm antibodies, resistant to corticotherapy, with few case reports published in the medical literature. We present a case of a 45-year-old woman admitted to hospital due to general weakness. Laboratory studies revealed macrocytic anaemia, biochemical parameters of haemolysis and peripheral spherocytosis. The direct Coombs test was positive. Viral serologies, anti-nuclear antibodies, anti-double-stranded DNA antibodies and β2-microglobulin were negative. CT scan of the thorax, abdomen and pelvis showed a heterogeneous right anexial lesion. The patient was treated with corticotherapy without improvement of anaemia. Regression of extra-vascular haemolysis and normalisation of haemoglobin was obtained only after laparoscopic splenectomy and right ooforectomy, and the histopathology of the right anexial mass revealed a cystic teratoma. Previously published cases controlled the haemolysis by surgically removing the lesion associated with splenectomy.
