In this thesis we implement estimating procedures in order to estimate threshold parameters for the continuous time threshold models driven by stochastic di®erential equations. The ¯rst procedure is based on the EM (expectation-maximization) algorithm applied to the threshold model built from the Brownian motion with drift process. The second procedure mimics one of the fundamental ideas in the estimation of the thresholds in time series context, that is, conditional least squares estimation. We implement this procedure not only for the threshold model built from the Brownian motion with drift process but also for more generic models as the ones built from the geometric Brownian motion or the Ornstein-Uhlenbeck process. Both procedures are implemented for simu- lated data and the least squares estimation procedure is also implemented for real data of daily prices from a set of international funds. The ¯rst fund is the PF-European Sus- tainable Equities-R fund from the Pictet Funds company and the second is the Parvest Europe Dynamic Growth fund from the BNP Paribas company. The data for both funds are daily prices from the year 2004. The last fund to be considered is the Converging Europe Bond fund from the Schroder company and the data are daily prices from the year 2005.

Embora não tenham sido estatisticamente evidenciadas as relações entre o sentido de coerência e a ligação materno-fetal e entre estes e os comportamentos de saúde, o carácter preliminar destes resultados, devido à subjectividade do processo de selecção não probabilístico da amostra estudada e à reduzida dimensão desta amostra, e a escassez de estudos descritos na literatura fazem com que seja prudente a realização de estudos de follow-up, com amostras de maiores dimensões, aleatórias e representativas da população, para que sejam estabelecidas quaisquer conclusões acerca destas questões.-----------------------------ABSTRACT - One of the greatest challenges faced by Public Health in nowadays is the burden represented by chronic diseases as co-responsible for the majority of deaths that occurs in the world, for the meaningful and progressive reduction of quality of life and increase of disabilities in affected individuals and for an important part of health expenses. However, the complexity of the genetic, biological, psychological, emotional, social, cultural and environmental dynamics that involves human behaviours related to some of these diseases – cardiovascular diseases, some kind of cancers, obesity, hypertension, diabetes and joint and bone diseases – poses the continuous challenge of searching for new and effective interventions of health promotion that positively influence individuals, groups and community lifestyles. Due to the fact that sense of coherence is an individual personality trace directly or indirectly influenced by the same factors listed above, the discovery of a relationship between this construct and human behaviours might be promising to the creation of new health promotion interventions. On the other hand, pregnancy may represent a turn point to the mother’s life and a good start in the baby’s life in relation to sense of coherence and health behaviours and It might occur because of the influence of maternal-fetal attachment. With the purpose of contributing with the creation of effective health promotion interventions through the discovery of probable salutogenic potentials in the referred constructs – sense of coherence and maternal-fetal attachment – , it was developed a quantitative observational study with the following objectives: to increase knowledge about changes in sense of coherence throughout life, specifically during pregnancy; to increase knowledge about the relationship between sense of coherence and maternal-fetal attachment and some social, demographical, psychological, emotional and obstetric factors of pregnant women; to identify probable relationships between sense of coherence, maternal-fetal attachment and pregnant women’s lifestyles, represented by diet habits, caffeine consumption, alcohol consumption, smoking habits, physical activity habits and weigh gain during pregnancy. This report describes the structure and the findings of this study involving a sample of 61 pregnant women who had been followed by health professionals in the mother health services of Carnaxide (Linda-a-Velha unity) and Oeiras Health Centres, in Lisbon, Portugal, between February and July of 2005. The results show that, for the 61 pregnant women who filled the self-reported questionnaire, it was found a statistically significant association between sense of coherence and education level. It was also found some evidence of the associations between sense of coherence and age, marital status and mensal household income and a trend toward pregnant women with higher levels of sense of coherence to drink less alcoholic beverages than pregnant women with lower levels of sense of coherence. However, the others associations tested were not confirmed. Regarding maternal-fetal attachment, it was found, for the sample of 41 women who participated in the second moment of data collection, between the 20th and the 24th week of pregnancy, statistically significant associations with education level and mensal household income. The others associations tested were not confirmed. Although the associations between sense of coherence and maternal-fetal attachment and between these constructs and health behaviours were not confirmed, all findings presented here are considered preliminary because of small dimension of sample and non-probabilistic criteria used for sample selection. What’s more, there are almost no studies described in the literature which could confirm or contradict these findings. Therefore, it is better to be careful and develop follow-up studies, with bigger and representative of population samples, before draw any conclusions about these theme.

Impaired dorsiflexion range of motion has been shown to be one of the consequences of this injury, as well as one of the contributing factors to recurrence. Several studies have shown the presence of an anterior positional fault of the fibula in injuried ankles. A cadaveric study revealed that a posterosuperior glide of the distal tibiofibular may contribute to improve dorsiflexion. There is a manual therapy technique which provides a posterosuperior glide of the lateral malleolus combined with dorsiflexion active movement (MWM). However, there was no study, until now, that investigated the effectiveness of this MWM in individuals with impaired dorsiflexion and history of unilateral ankle sprain. Design: Double-blind randomized placebo controlled trial. Objectives: To determine the immediate effects of a distal tibiofibular MWM in ankle dorsiflexion and talar posterior glide in patients with history of unilateral ankle sprain and limitation of dorsiflexion. The treatment technique was used as a single treatment against a placebo group. Methods: A sample of 30 subjects with a history of unilateral ankle sprain and limitation of dorsiflexion were randomized into two groups: distal tibiofibular MWM or a placebo group. The outcome measures used in this study were the posterior talar glide and weight-bearing (WB) ankle dorsiflexion range of motion. The measures were taken before and immediately after the intervention. Results: No significant differences were found in baseline measures between groups. A one-way ANCOVA revealed that, immediately after the intervention, there was an improvement in ankle dorsiflexion in the MWM group (increase of 1.37 cm (SD, 0.97) significantly superior to the placebo group (decrease of 0.15cm (SD, 0.63) (P<.001). Posterior talar glide increased by 1.51º (SD, 1.77) for the MWM group, which was more than 0.76º (SD, 1.25) for the placebo intervention although there wasn’t a significant difference between groups (P=.113). Conclusion: This investigation’s findings suggest that an inferior tibio-fibular MWM produced a significant effect on WB dorsiflexion range of motion and posterior talar glide. These results provide preliminary evidence for the efficacy of mobilisations with movement in the management of individuals with history of unilateral ankle sprain and limitation of dorsiflexion.

The use of exercises under the premises of dynamic stability (DS), with the aid of electromyographic biofeedback (EMGBF) has been mentioned as a way to increase the effectiveness of the intervention in disorders of the shoulder joint complex (DSJC) Main Objective: To evaluate the results of an intervention protocol, with EMGBF whose aims were to abolish the pain, increase functionality and DS of the scapula in patients with DSJC. Objectives: To compare the subgroups and the characteristics, outcomes, and total intervention times to reach discharge criteria; to compare the results at the beginning and at the end. Methods: We conducted a clinical cohort, longitudinal, retrospective, observational analysis. The sample consisted of 82 subjects, divided into two subgroups (n = 53 shoulder impingement syndrome SIS n = 29 shoulder joint instability SJI). The measurement instruments used were the VAS, DASH, SPADI, and EMGBF. It was followed the DS protocol proposed by Santos and Matias (2007), according to the three phases of intervention, described by performing a weekly session, monitoring exercises that follow these principles, with EMGBF. The variables were pain, posture, recruitment pattern, motor control, the initial position of the scapula (IPS), range of motion (ROM), muscular strength (MS) and posture. To analyze the data, we used the descriptive and inferential statistics. Results: The intervention was effective in abolishing the pain at the time (0.43 to 0.00/10 VAS in the SIS subgroup and 0.66 to 0.00/10 VAS in the SJI subgroup) in worst pain (5.47 to 0,06/10 VAS in the SIS subgroup and 5.28/ to 0.14/10 VAS in the SJI subgroup), increasing the function (28,57 to 0,66/100 DASH and 39,00 to 0,63/100 SAPDI in the SIS subgroup and 25,80 to 0,38/100 DASH and 28,19 to 0,39/100 SPADI in the SJI subgroup) and the increase in DS of the scapula with normalization of the pattern recruitment, motor control, IPS within the spectrum of normalcy and good ability to control its position along the movement of the upper limb (UL). The intervention provided, although the normalization of ROM, MS and self-correcting posture. The average length of intervention was 6.45 weeks in the SIS subgroup and 5.83 in the IS subgroup. These results are comparable with similar studies that used an intervention based on the same principles (Matias e Cruz, 2004; Cunha e Matias, 2006; Santos e Matias, 2007; Rodrigues e Matias, 2009). Conclusion: The intervention protocol implemented with the use of exercises based on the principles of DS and the use of EMGBF was effective, allowing the accomplishment of goals, in patients with DSJC (SIS and SJI). It was found that, regardless of condition, the results were identical in both groups, showing no significant differences between the variables of the final measure, the total time of intervention and the times to reach discharge criteria, except for pain, which was higher in the SIS subgroup.

----------- ABSTRACT:This study was conducted to assess the effects of deinstitutionalization of “long-stay” patients during the process of closing Hospital Miguel Bombarda (2007-2011). This process included the fusion, in 2008, of the two main psychiatric hospitals in Lisbon- Hospital Miguel Bombarda (HMB) and Hospital Júlio de Matos (HJM), into Centro Psiquiátrico Hospitalar de Lisboa (CHPL). A control group of still institutionalized patients in CHPL (n=166) was used as a comparison with the deinstitutionalized population (n=146). Of this 312 initial sample only 142 (76 cases and 66 controls) were included, the main causes of exclusion being diagnoses (organic disease, dementia and mental retardation- as first diagnoses) and transference between hospitals. Deinstitutionalization is mainly evaluated in terms of psychopathology, use of services, satisfaction, crime, vagrancy and deaths. The results show that most long-stay patients can successfully leave psychiatric hospitals and be relocated in the community without an increase in psychopathology, crime or vagrancy. Satisfaction seems to be improved in those patients. On the other hand, mortality remains an issue of concern: Although there was no possibility of comparing it between cases and controls, the Standard Mortality Rate (SMR) in our study was found to be much higher than expected judging by other studies results. A longitudinal further study of this same population will be the matter for a future investigation, possibily compared with another similar population from a desinstitutionalization programme in another country.

The osteoarthritis of the knee is a clinical condition that has been increasing among the elderly population over the past few years. It’s a condition that can be fairly disabling to the elderly individual, and which generates a strong social and economic impact. Aquatic exercise has been named as a promotional factor in the improvement of the clinical picture in this population, but its effectiveness is yet to be determined. Objective: To investigate the effectiveness, in a short and medium term, of an aquatic exercise program undertaken by a group over eight weeks, in the joint amplitude, pain and stiffness of the knee, and in the physical function and quality of life in elderly people who suffer from knee osteoarthritis. Methodology: was used a double-blind, quasi-experimental study with no randomization and its 43 subjects were selected as a matter of convenience. The subjects of the experimental group (n=22) were subjected to an aquatic exercise program and those of the control group (n=21) were subjected to a land-based exercise program. Both programs have followed international recommendations on the practice of physical exercise in elderly people who suffer from osteoarthritis (Arthritis Foundation, 2009a, 2009b). The entire group was evaluated in the beginning and at the end of the program (8 weeks later), and after a 6 week follow-up, in these parameters: knee range of motion (through a goniometer), pain, stiffness, function and quality of life (through KOOS). Results: This sample was predominantly female (76,7%) with a mean age of 72,7 years (SD=5,49). Aquatic exercise resulted in increase of passive joint flexion (p<0,0001) and knee extension (p=0,016), in the decline of pain and stiffness (p<0,0001), in the increase of function in the ADLs (p<0,0001). However, only the knee extension amplitude (p=0,083) and the function in sports/leisure (p=0,244) were kept through the six-week follow-up. Conclusions: Despite some limitations, this study is of great importance to the community and to the physiotherapists concerned with the growing prevalence of osteoarthritis and the associated costs. Aquatic exercise appears to be an important strategy in the improvement of the health condition of patients who suffer from knee osteoarthritis.

The prediction of asymmetric equilibria with Stackelberg outcomes is clearly the most frequent result in the endogenous timing literature. Several experiments have tried to validate this prediction empirically, but failed to find support for it. By contrast, the experiments find that simultaneous-move outcomes are modal and that behavior in endogenous timing games is quite heterogeneous. This paper generalizes Saloner’s (1987) and Hamilton and Slutsky’s (1990) endogenous timing games by assuming that players are averse to inequality in payoffs. We explore the theoretical implications of inequity aversion and compare them to the empirical evidence. We find that this explanation is able to organize most of the experimental evidence on endogenous timing games. However, inequity aversion is not able to explain delay in Hamilton and Slutsky’s endogenous timing games.

With the recent advances in technology and miniaturization of devices such as GPS or IMU, Unmanned Aerial Vehicles became a feasible platform for a Remote Sensing applications. The use of UAVs compared to the conventional aerial platforms provides a set of advantages such as higher spatial resolution of the derived products. UAV - based imagery obtained by a user grade cameras introduces a set of problems which have to be solved, e. g. rotational or angular differences or unknown or insufficiently precise IO and EO camera parameters. In this work, UAV - based imagery of RGB and CIR type was processed using two different workflows based on PhotoScan and VisualSfM software solutions resulting in the DSM and orthophoto products. Feature detection and matching parameters influence on the result quality as well as a processing time was examined and the optimal parameter setup was presented. Products of the both workflows were compared in terms of a quality and a spatial accuracy. Both workflows were compared by presenting the processing times and quality of the results. RESUMO: Enthesitis is the hallmark of spondyloarthritis (SpA), and is observed in all subtypes. Wide information on SpA abnormalities, including synovitis, tendinitis and enthesitis, can be efficiently perceived by Doppler ultrasound. Furthermore, several studies on imaging of enthesis showed that imaging techniques are better than clinical examination to detect enthesis alterations; and vascularized enthesitis detected by Doppler ultrasound appears to be a valuable diagnostic tool to confirm SpA diagnosis. However, data published until now concerning entheseal elementary alterations that characterize SpA enthesitis (enthesis inflammatory activity) or enthesopathy (permanent structural changes) reflect rather the authors’ empiric opinion than a methodological validation process. In this sense it seems crucial to identify elementary entheseal lesions associated with activity or damage, in order to improve monitoring and treatment response in SpA patients. The development of better assessment tools is today a challenge and a need in SpA. The first study of this thesis focused on the analysis of the reliability of inter-lector and inter-ultrasonography equipment of Madrid sonography enthesitis index (MASEI). Fundamental data for the remaining unrolling project validity. In the second and third studies we concerned about two entheseal elemental lesions: erosions and bursa. In literature erosions represent a permanent structural damage, being useful for monitoring joint injury, disease activity and therapeutic response in many rheumatic diseases; and to date, this concept has been mostly applied in rheumatoid arthritis (RA). Unquestionably, erosion is a tissue-related damage and a structural change. However, the hypothesis that we decided to test was if erosions represent a permanent structural change that can only grow and worsen over time, as occurs in RA, or a transitory alteration. A longitudinal study of early SpA patients was undertaken, and the Achilles enthesis was used as a model. Our results strongly suggested that previously detected erosions could disappear during the course of the disease, being consistent with the dynamic behavior of erosion over time. Based on these striking results it seems reasonable to suggest that the new-bone formation process in SpA could be associated with the resolution of cortical entheseal erosion over time. These results could also be in agreement with the apparent failure of anti-tumor necrosis factor (TNF) therapies to control bone proliferation in SpA; and with the relation of TNF-α, Dickkopf-related protein 1 (Dkk-1) and the regulatory molecule of the Wnt signaling pathway in the bone proliferation in SpA. In the same model, we then proceeded to study the enthesis bursa. Interestingly, the Outcome Measures in Rheumatology Clinical Trials (OMERACT) enthesopathy definition does not include bursa as an elementary entheseal lesion. Nonetheless, bursa was included in 46% of the enthesis studies in a recently systematic literature review, being in agreement with the concept of “synovio-entheseal complex” that includes the link between enthesitis and osteitis in SpA. It has been clarified in recent data that there is not only a close functional integration of the enthesis with the neighboring bone, but also a connection between enthesitis and synovitis. Therefore, we tried to assess the prevalence and relevance of the bursa-synovial lesion in SpA. Our findings showed a significant increase of Achilles bursa presence and thickness in SpA patients compared to controls (healthy/mechanical controls and RA controls). These results raise awareness to the need to improve the enthesopathy ultrasonographic definition. In the final work of this thesis, we have explored new perspectives, not previously reported, about construct validity of enthesis ultrasound as a possible activity outcome in SpA. We performed a longitudinal Achilles enthesis ultrasound study in patients with early SpA. Achilles ultrasound examinations were performed at baseline, six- and twelve-month time periods and compared with clinical outcome measures collected at basal visit. Our results showed that basal erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) are higher in patients with Doppler signal in enthesis, and even that higher basal ESR, CRP and Ankylosing Spondylitis Disease Activity Score (ASDAS) predicted a higher Doppler signal (an ultrasound alteration accepted as representative of inflammation) six months later. Patients with very high disease activity assessed by ASDAS (>3.5) at baseline had significantly higher Achilles total ultrasound score verified at the same time; and ASDAS <1.3 predicted no Doppler signal at six and twelve months. This seems to represent a connection between classical biomarkers and clinical outcomes associated with SpA activity and Doppler signal, not only at the same time, but also for the following months. Remarkably, patients with inactive disease (ASDAS < 1.3) at baseline had no Doppler signal at six and twelve months. These findings reinforce the potential use of ultrasound related techniques for disease progression assessment and prognosis purposes. Intriguingly, Ankylosing Spondylitis Disease Activity Index (BASDAI) didn’t show significant differences between different cut-offs concerning ultrasound lesions or Doppler signal, while verified with ASDAS. These results seem to indicate that ASDAS reflects better than BASDAI what happens in the enthesis. The work herein discussed clearly shows the potential utility of ultrasound in enthesis assessment in SpA patients, and can be important for the development of ultrasound activity and structural damage scores for diagnosis and monitoring purposes. Therefore, local promotion of this technique constitutes a medical intervention that is worth being tested in SpA patients for diagnosis, monitoring and prognosis purposes.

In couples, caregiving experiences in dementia usually occur within long-standing relationships preceding the onset of the illness and evolving as it progresses. The quality of relationship (QR) and Antonovsky’s salutogenic construct of sense of coherence (SOC) are assumed to promote and protect health in stressful situations, such as dementia. SOC refers to the extent to which a person considers his/her life as comprehensible, manageable and meaningful. We aim to uncover the role of QR and SOC as moderator or mediator in the ability of patients and their partners to cope with the impact of dementia We studied six couples in which one spouse had been diagnosed with dementia. We used a qualitative research approach (combining in-depth individual and joint interviews), triangulated with the measures Orientation to Life Questionnaire - SOC scale (Antonovsky, 1987) and the Quality of Carer-Patient Relationships – QCPR (Spruytte et al. 2000). The method was informed by a literature review on the role of RQ and SOC within dementia. Results highlight the importance of listening to both spouses, in a dyadic perspective, while acknowledging the voice of PwD themselves despite methodological challenges. Aspects of prior relationship impact on caregiving dynamics and on how the care-giver and care-recipient roles are experienced. At the same time, the experience of living with dementia impacts on the relationship, often causing change or loss which is difficult to accept or adjust to. Diagnosis played an important role on the SOC’s comprehensibility component (life makes sense). Good pre-caregiving and current relationship was associated with carers’ ability to cope, namely on SOC’s manageability (problems will be bearable) and meaningfulness (life is viewed as a challenge). Our findings suggest that an understanding of QR and SOC prior to diagnosis may encourage positive patterns of care, foster successful adaptation to changing needs, and support in-home arrangements as long as possible.
