RESUMO: O relatório anual de 2001 da Organização Mundial de Saúde (OMS), anunciou o interesse social dos problemas crescentes em matéria de saúde mental e relativo à necessidade urgente de uma extensão, como também, para uma melhor organização da oferta de tratamento. Sob proposta da Bélgica, esta observação foi traduzida em uma resolução « Mental health :responding to the call for action » (dclaração de intenção do 18 de maio de 2002). Nós notamos que certos países, como os Estados Unidos ou a Grã Bretanha transformam sistematicamente o tratamento residencial em um tratamento mais dirigido para a comunidade. Eles também se concentram na descoberta prematura dos problemas psiquiátricos. Este fenômeno de dsinstitutionalisação obteve resultados concretos e traz um ganho certo, tanto para os atores do campo da saúde mental, como também para os candidatos a um tratamento. O tratamento das pessoas dentro do seu meio de vida é muito claramente reconhecido. As evoluções que marcaram os tratamentos em saúde mental na Bélgica durante estes utimos 40 anos mostram que vários paços importantes já foram feitos. A reforma tem como objetivo converter a oferta hospitalar e montar tratamentos dirigidos para e dentro da comunidade pela criação de equipas móveis que se inscrevem no dispositivo comunitário, historicamente muito activo bem que sub-financiado. A reorganização dos sistemas de tratamento está baseada na criação de redes, para construir um dispositivo flexível e contínuo que considera as necessidades dos pacientes. Esta reforma é, ao mesmo tempo, ambiciosa e complexa,. É uma visão nova, uma mudança de cultura, não só, para todos os atores da saúde mental, mas também para os pacientes e suas famílias. A reforma está baseada numa visão global e integrada que associa no mesmo movimento todas as autoridades competentes relativamente a saúde que elas sejam federais, regionais ou comunitárias.-----------ABSTRACT: The World Health Organization’s (WHO) annual report of 2001 identified a social interest for the increasing problems related to mental health and the urgent need of an extension and a better organization of mental health care. On a proposal of Belgium this statement was transformed into a resolution « Mental health: responding to the call for action» (Declaration of Intent, May 18th, 2002). Some countries such as the USA or the UK systematically dismantled in-patient residential care in favor of more community-based care and a focus on early detection of psychiatric problems. This de -institutionalization has clearly and concretely paid off and the value of bringing mental health care to claimants and treating them in their own residence was acknowledged. In Belgium, the evolutions which have shaped mental health care in the last forty years indicate that a number of important steps are already taken. The Reform aims to convert the supply of hospital care into community-based mental health care services through the creation of mobile teams which offer services in everyday life of the user. These teams take place in the community, historically highly active howbeit underfunded. The reorganization of health care system relies upon the creation of networks in order to build a flexible and continuous device which take into account user’s needs. This Reform can be seen as both ambitious and complex. It is a completely new vision, a major cultural shift for all mental health care stakeholders, but also for users and their relatives. The Reform is based on a global and integrated approach which links I, a same movement all relevant health authorities whether they be federal, regional or community. de-institutionalization – community mental health – recovery - users and families involvement - networking.

This paper is a summary of the results of the authors recent researches about the Western Iberian continental margin. During the Mesozoic, the margin is affected by two consecutive extensional phases interpreted as the result from two episodes of rifting in the Atlantic. Then during Cenozoic, subsidence was interrupted by compression and related deformation, specially during Eocene time. Ante-mesozoic basement controls the structural and sedimentary evolution of the margin.

The structural and sedimentary evolution of the portuguese continental margin South of Setúbal Canyon is outlined from the study of many seismic reflection profiles and rocks samples. During Triassic and Early Liassic time, a distension affects the Algarve margin that belongs to the Mesogean area. Off Baixo Alentejo rifting phases at Late Jurassic and Early Cretaceous times induced opening or widening of the adjacent part of the Atlantic ocean. Alpine orogeny is inferred to explain the Eocene and Miocene deformation of the margin specially along the main NE-SW fractures.

Examination of samples from eight outcrops from Albian and Cenomanian of Estremadura, induced to take a census of sixty-two species and subspecies of Ostracodes among which fifteen are new and described here. Their associations pennited - first, to characterize three faunistic sets: a lower and middle Albian set with a mediolittoral and infralittoral (shallow marine} sedimentation; an upper Albian s.l. (near formations of Rudists}; a lagoonal lower Cenomanian; - on the other hand, to state local comparisons with the middle Cretaceous of Southern France, of the South-pyrenean Zone (Sierra d' Aulet: district of Sopeira), of the district of Oviedo (Northwestern Spain), and of the Aragonese Iberian Range (Aragon and Maestrazgo), placing in a prominent position faunistic exchanges of Ostracodes between the above-mentioned regions and the Estremadura, during the lower Cenomanian.

This paper presents a resume of the results achieved by researchers of the Centro de Estratigrafia e Paleobiologia da U. N. L. on the Neogene of Algarve, since 1977. The detailed study of several profiles as well as that af calcareous nannoplanton, planktonic foraminifera, ostracoda, fishes and mammals allowed to obtain data and correlation elements leading to a new interpretation of the Miocene of Algarve. It was possible to date and to characterize the following units: a) Carbonate formation of Lagos-Portimão, of marine facies, ascribed to the Lower Miocene (Aquitanian? and mainly Burdigalian), possibly attaining the Lower Langhian. b) Essentially arenaceous series of continental facies with a marine intercalation of Arrifão, Olhos de Água and Auramar Hotel beach, middle Miocene (Langhian-Serravallian) in age. c) Marine (tripoli, conglomerates, sands and limestones) deposits of Tunes-Mem Moniz, Ponte das Lavadeiras (Faro), Arroteia (Fuzeta) and Luz de Tavira, corresponding, at least partially, to the first part of the upper Miocene (Lower Tortonian). d) Cacela formation with three members: The lower member (conglomerates and sands), the middle (yellow silts) and the upper ones (gray silts), uppermost Tortonian and mainly Messinian in age. An interpretation of the tectonic and paleogeographic evolution of the portuguese littoral during the Miocene is also presented considering its insertion in the meridional part of the Peninsula (Guadalquivir depression, Betic massif basins and in the spanish Levant in general). Comparisons among the Neogene vulcanism of this region and similar manifestations documented in Algarve (basanite of Figueira-Portimão, etc) are established.

The presence of Craspedochelys (Plesiochelys s.l.) at Romão, Portugal, indicates that, in that country, the Thalassemydid dispersal began with the first occurrences of the group in Europe, in the late Kimmeridgian: communications existed between septentrional, atlantic and mesogean seas. The review of the family is considered.

Ramsey pricing has been proposed in the pharmaceutical industry as a principle to price discriminate among markets while allowing to recover the (fixed) R&D cost. However, such analyses neglect the presence of insurance or the fund raising costs for most of drug reimbursement. By incorporating these new elements, we aim at providing some building blocks towards an economic theory incorporating Ramsey pricing and insurance coverage. We show how coinsurance affects the optimal prices to pay for the R&D investment. We also show that under certain conditions, there is no strategic incentive by governments to set coinsurance rates in order to shift the financial burden of R&D. This will have important implications to the application of Ramsey pricing principles to pharmaceutical products across countries.
